1.Analysis of the effects of propofol target-controlled infusion combined with alfentanil on the anesthetic effect and hemodynamics of patients undergoing vocal polypectomy

Objective To assess the safety and therapeutic impact of propofol target-controlled infusion combined with alfentanil for vocal polyp excision.Methods From January 2023 to January 2024,vocal polyp ectomy removal surgery patients 80 cases in Ganzhou People's Hospital's were selected and randomly assigned into group A(40 cases)and group F(40 cases).In the anesthetic induction sequence,the two subject groups in the clinical trial had their propofol plasma target concentration set at 3.5μg/ml.Rocuronium bromide was administered at a dosage of 0.6mg/kg,followed by a slow and careful injection.In group F,this was succeeded by an injection of fentanyl at 3μg/kg.In contrast,group A received an intravenous injection of alfentanil at a dosage of 20μg/kg.In both groups,tracheal intubation was executed when muscle relaxation had been achieved.Monitoring and documentation of hemodynamic fluctuations during the perioperative stages.Records were kept for the procedure's conclusion time,the time frame for regaining consciousness and extubation,and any adverse effects experienced.Results While the contrast of mean arterial pressure(MAP)and heart rate(HR)between two groups at each time point from To to T4 were statistically non-significant(P>0.05).Compared with preoperative baseline(To),MAP and HR were lower in both groups at all time points within group at T1,T2,T3 and T4(P<0.05),the within-group examination of these parameters across time points and their interactions did show quantifiable differences(P<0.05).The interval from regaining consciousness to extubation was shorter in group A than that in group F(P<0.05).Additionally,the quantity of propofol and the number of unanticipated effects were diminished in group A than those in group F(P<0.05).Conclusion In the context of vocal polypectemy,the administration of propofol via a targeted infusion combined with alfentanil maintains hemodynamic stability,minimizes propofol dosing in contrast to fentanyl,and is linked to faster awakening times and a lower occurrence of adverse effects.
2.Value of a combined ultrasound imaging radiomics model to predict progression-free survival in endocrine therapy for prostate cancer

Objective:To investigate the value of the combined ultrasound imaging radiomics model for predicting progression-free survival in endocrine therapy for prostate cancer.Methods:A total of 283 prostate cancer patients who received endocrine treatment at the Inner Mongolia Autonomous Region People′s Hospital and the First Hospital of Hohhot from July 2018 to January 2023 were retrospectively collected, of which 198 patients from the Inner Mongolia Autonomous Region People′s Hospital were randomly divided into the training set and the validation set according to the ratio of 7∶3, and 85 patients from the First Hospital of Hohhot served as an independent external test set. They were classified into a progression group and a non-progression group based on whether the patients progressed to desmoplasia-resistant prostate cancer 18 months after the start of endocrine treatment.Based on the two-dimensional ultrasound images, the imaging radiomics features were extracted and the imaging radiomics score (Rad-score) were constructed, the immunopathology and other clinical data were analysed, and three prediction models were constructed using logistic regression: the clinical model, the ultrasonography model, and the ultrasonography-clinical combined model, respectively. The predictive efficacy and clinical utility of the models were assessed by the ROC curves and clinical decision curves.Results:Five ultrasonographic features were included in the ultrasound model; the prostate-specific antigen nadir, the neutrophil-to-lymphocyte ratio before treatment, and the expression level of tumour proliferating cell nuclear antigen 67 (Ki-67) were incorporated into the clinical model; and the Rad score computed from the output of the ultrasound model for the screening features, together with the prostate-specific antigen nadir (PSA nadir), the neutrophil to lymphocyte ratio (NLR) before treatment, and the expression level of Ki-67 were used to construct the ultrasound-clinical joint model. The joint model achieved the highest predictive performance in both the training and validation sets of the three groups of models, with the area under the curve of 0.85 and 0.84, and the clinical decision curve showed good clinical benefit.Conclusions:The combined ultrasound-clinical model constructed in this study based on two-dimensional ultrasound images of prostate cancer before endocrine therapy can predict progression-free survival of endocrine therapy for prostate cancer more accurately.
3.Status and development of hospice care services

A rapid growth and steady expansion has been witnessed in hospice care with initial effects in recent years, however, there are significant differences in practice among different regions, and a systematic and comprehensive summary and analysis are needed. Based on the consolidated framework for implementation research, this paper comprehensively analyzes and summarizes the implementation status of hospice care services from five aspects, including innovation, external setting, internal setting, individual characteristics and implementation process. This paper finds that the main factors contributing to the implementation of this service are the varied and individualized services, active implementation of training, clear division of labor among multidisciplinary teams, diversified service models, and emphasis on service publicity. The main challenges faced by the service are insufficient internal and external incentives, insufficient resources available for manpower and medicines, the level of knowledge, attitude, and practice of service providers needs to be improved, the service process needs to be harmonized, and the quality evaluation system needs to be improved. Relevant institutions at all levels should work together to play a positive role in facilitating factors, overcome obstacles, and further promote the development of hospice care in China.
4.A case report of prostate mucinous adenocarcinoma

Mucinous adenocarcinoma of the prostate is a rare pathological type of prostate cancer. We reported one case. The patient went to see a doctor because of intermittent hematuria. He was diagnosed as prostate adenocarcinoma before operation. He underwent laparoscopic radical prostatectomy. Postoperative pathological examination showed mucinous adenocarcinoma of the prostate. The patient was followed up for six months. Imaging showed no signs of recurrence and metastasis with normal tPSA.
5.Research progress on enteric-soluble soft capsule

Enteric-soluble soft capsule is a kind of new preparation that does not disintegrate in the stomach ,but releases rapidly in the intestinal tract to play a pharmacodynamic role. It has the unique advantages of improving drug stability ,reducing drug irritation ,delivering drugs directionally to the intestinal tract ,and prolonging drug action time. In this paper ,the decomposition and release mechanism ,application advantages ,classification of enteric-soluble coating materials and preparation methods of enteric-soluble soft capsule are sorted and summarized ,in order to provide reference for further development of this type of preparation.
6.Risk factors of bone cement leakage after percutaneous vertebroplasty for osteoporotic vertebral compression fracture

Objective:To investigate the risk factors of bone cement leakage after percutaneous vertebroplasty (PVP) for osteoporotic vertebral compression fracture (OVCF).Methods:A multi-center, large-sample, case-control study was carried out to analyze the clinical data of 2 273 OVCF patients (2 689 vertebrae) undergone PVP at four hospitals between May 2018 and October 2021, including 994 males and 1 279 females, with the age of 52-91 years [(69.1±3.1)years]. Of all, 581 patients (604 vertebrae) were allocated to leakage group and 1 692 patients (2 085 vertebrae) to no leakage group according to the occurrence of bone cement leakage. The gender, age, fracture sites, vertebral compression degree, endplate integrity of fractured vertebrae, surgical segments, surgical approaches and bone cement injection volume were recorded. Univariate analysis was used to investigate the correlation between those indicators with bone cement leakage. Multivariate Logistic regression analysis was used to identify the independent risk factors for bone cement leakage.Results:Univariate analysis showed that gender, age, fracture sites, vertebral compression degree, bone cement injection volume were related to bone cement leakage after PVP ( P<0.05 or 0.01), but no correlation was found in the endplate integrity of fractured vertebrae, surgical segments and surgical approaches (all P>0.05). Multivariate Logistic regression analysis showed that fracture sites ( OR=1.68, 95% CI 1.11-2.55, P<0.05), vertebral compression degree more than 40% ( OR=1.98, 95% CI 1.29-3.02, P<0.01), bone cement injection volume greater than or equal to 5.5 ml ( OR=1.55, 95% CI 1.07-2.26, P<0.05) were significantly associated with bone cement leakage after PVP. Conclusion:Thoracic vertebral fracture, vertebral compression degree more than 40% and bone cement injection volume greater than or equal to 5.5 ml are independent risk factors for bone cement leakage after PVP in OVCF.
7.Construction of recombinant adenovirus expressing EGFRvIII extracellular domain gene and preparation of single domain antibody.

The aim of this study was to construct a recombinant adenovirus expressing extracellular domain gene of human epidermal growth factor receptor variant Ⅲ (EGFRvIII ECD), and to prepare single domain antibody targeting EGFRvIII ECD by immunizing camels and constructing phage display antibody library. Total RNA was extracted from human prostate cancer cell line PC-3 cells and reversely transcribed into cDNA. EGFRvIII ECD gene was amplified using cDNA as template, and ligated into pAdTrack-CMV plasmid vector and transformed into E. coli BJ5183 competent cells containing pAdEasy-1 plasmid for homologous recombination. The recombinant adenovirus expressing EGFRvIII ECD was obtained through transfecting the plasmid into HEK293A cells. The recombinant adenovirus was used to immunize Bactrian camel to construct EGFRvIII ECD specific single domain antibody library. The single domain antibody was obtained by screening the library with EGFRvIII protein and the antibody was expressed, purified and identified. The results showed that recombinant adenovirus expressing EGFRvIII ECD was obtained. The capacity of EGFRvIII specific phage single domain antibody library was 1.4×109. After three rounds of enrichment and screening, thirty-one positive clones binding to EGFRvIII ECD were obtained by phage-ELISA, and the recombinant single domain antibody E14 with highest OD450 value was expressed and purified. The recombinant E14 antibody can react with EGFRvIII ECD with high affinity in ELISA assessment. The results indicated that the EGFRvIII specific single domain antibody library with high capacity and diversity was constructed and the single domain antibody with binding activity to EGFRvIII was obtained by screening the library. This study may facilitate the diagnosis and treatment of EGFRvIII targeted malignant tumors in the future.

8. Nursing human resource management of infectious disease hospitals under novel coronavirus pneumonia threats

With the outbreak of novel coronavirus pneumonia, Beijing You'an Hospital has become one of the three infectious disease specialist hospitals designated to treat patients of such pneumonia. Under the premise of comprehensively implementing various emergency treatment tasks and ensuring the normal operation of other wards, the Nursing Department has put in place emergency plans and deployed due manpower for rapid response, timely personnel deployment, and reasonable reserve echelon structure. These measures have been taken as required by the patients’ numbers, critical conditions, disease diagnosis, and the guidelines of treatment and protection. While ensuring the completion of treatment work, we manage to leverage nursing human resources in a scientific, standardized and maximized efficiency manner, to ensure the quality of nursing, and the physical and mental health of nursing staff.
9.Molecular diagnosis and characterization analysis of the first case of human Brucella suis infection in Jiangsu Province

Objective:To identify human infection with Brucella suis, analyze its biological and molecular characteristics, and to provide basis for prevention and control of brucellosis. Methods:Brucella suis strains were isolated from the body of the first case of human Brucella suis infection in Jiangsu Province. Serum agglutination test was used for serotyping. The specific gene bcsp-31 of Brucella was detected by PCR. AMOS-PCR was used to identify IS-711. The species and biotypes were identified by multiplex PCR. The wboA gene products were sequenced and phylogenetic tree was constructed. Multilocus sequence analysis (MLSA) was used for molecular typing, and cluster analysis was performed with reference strains. Results:The strain was confirmed to be Brucella suis biotype 3 by serum agglutination test and PCR. After sequencing the wboA gene, cluster analysis of the reference sequence showed that the wboA gene was closest to the biotype 3 strain Brucella suis str. 686 (CP007719). MLSA was typed into ST17(1-6-4-1-5-3-5-2-4). Conclusions:Brucella suis biotype 3 is reported in Jiangsu Province for the first time. The MLSA type is ST17. In the future, the prevention and control of human brucellosis should be carried out. We should actively cooperate with the animal husbandry and veterinary department to increase the quarantine, immunization and other control measures.
10.Risk factors for fatal outcome in patients with severe COVID-19: an analysis of 107 cases in Wuhan

Objective:To analyze the risk factors of fatal outcome in patients with severe COVID-19.Methods:The clinical characteristics of 107 patients with severe COVID-19 admitted in Renmin Hospital of Wuhan University from February 12 to March 12, 2020 were retrospectively analyzed. During the hospitalization 49 patients died (fatal group) and 58 patients survived (survival group). The clinical characteristics, baseline laboratory findings were analyzed using R and Python statistical software. The risk factors of fatal outcome in patients with severe COVID-19 were analyzed with multivariate logistic regression.Results:Univariate analysis showed that the two groups had statistically significant differences in age, clinical classification, dry cough, dyspnea and laboratory test indicators ( P<0.05 or <0.01). The random forest model was used to rank the significance of the statistically significant variables in the univariate analysis, and the selected variables were included in the binary logistic regression model. After stepwise regression analysis, the patient’s clinical type, age, neutrophil count, and the proportion of CD3 cells are independent risk factors for death in severe COVID-19 patients. Dry cough is an independent protective factor for the death of severe COVID-19 patients. Conclusion:COVID-19 patients with fatal outcome are more likely to have suppressed immune function, secondary infection and inflammatory factor storm. These factors may work together in severe patients, leading to intractable hypoxemia and multiple organ dysfunction and resulting in fatal outcome of patients. The study indicates that timely intervention and treatment measures against above factors may be effective to save the lives of patients with severe COVID-19.
