A novel technology is proposed for non-contact and real-time detection of atrial fibrillation using millimeter-wave radar.A 60 GHz PCR millimeter wave radar is used to continuously detect the chest echo signal of the subject. After signal acquisition,I-Q signal is generated through I-Q demodulation, and the signal phase information is extracted using effective points phase trend evaluation for obtaining the signals from oscillations in the chest wall, from which the respiratory signals and cardiac signals are extracted through digital filtering for the analysis of cardiac movement.Whether the atrial fibrillation occurs or not is determined by the characteristics of atrial fibrillation wave in the time domain. The effective points phase trend evaluation for extracting more accurate signal phase information and the time-domain method for real-time atrial fibrillation detection are the innovations of the study. The experimental results show that the proposed method achieves a detection accuracy of 99.2%in clinic.

OBJECTIVE To provide reference for Smilax glabra quality control. METHODS UPLC method was established to simultaneously determine the contents of 5-O-caffeoylshikimic acid, neoastilbin, astilbin, neoisoastilbin, isoastilbin, engelitin, resveratrol and isoengelitin in 20 batches of S. glabra from different areas (No. S1-S20). The quality evaluation of 20 batches of samples was performed by chemometrics; the differential biomarkers that affected the quality of S. glabra were screened. RESULTS The measured 8 components had good linear relationship within the range of measured concentration (r≥0.999 6). RSDs of precision, repeatability and stability tests (24 h) were all lower than 2.00% (n=6). The average recoveries varied between 97.60% and 106.40% (RSDs were all lower than 2.00%, n=6). Cluster analysis showed that the samples produced in Zhejiang (S1-S5) and Jiangxi (S6-S10) were clustered into one category; the samples produced in Hunan (S11-S15) were clustered into one category; the samples produced in Yunnan (S16-S20) were clustered into one category. Principal component analysis showed that the first two principal components could represent 85.60% information of 8 components in S. glabra. Partial least squares-discriminant analysis showed that variable importance projection values of 5-O-caffeoylshikimic acid, astilbin, isoastilbin and neoastilbin were all greater than 1. CONCLUSIONS There are differences in the contents of the above 8 components in S. glabra from different origins; 5-O-caffeoylshikimic acid, astilbin, isoastilbin and neoastilbin may be differential markers affecting the quality of S. glabra.

SGLT2 inhibitors currently have clear benefits in the treatment of heart failure whether combined with diabetes or not. Ventricular remodeling after myocardial infarction leads to the occurrence and development of heart failure, and eventually leads to death. There are relatively few studies on SGLT2 inhibitors in patients with myocardial infarction. The purpose of this article is to review the research progress of SGLT2 inhibitors application before and after myocardial infarction.

Objective:To explore the influence of lateral patellofemoral joint degeneration on the treatment of anteromedial osteoarthritis of knee joint by Oxford medial unicompartmental knee arthroplasty. Methods:The clinical data of 73 patients (73 knees) with knee osteoarthritis underwent unicompartmental knee arthroplasty at Department of Orthopaedic Surgery, Xuanwu Hospital, Capital Medical University from March 2016 to December 2017 were analysed respectively. There were 18 males and 55 females, aged (68.6±7.5) years(range: 53 to 89 years). The lateral patellofemoral joints of patients were evaluated by Ahlback grading system. Patients with Ahlback 0 andⅠ were in the non degenerative group (37 cases), and those with Ahlback Ⅱ and above were in the degenerative group (36 cases). Hospital for special surgery knee score(HSS) and the Western Ontario and McMaster Universities(WOMAC) osteoarthritis index, as well as the condition of kneeling, sit to stand movement, up stair and down stair were recorded. The data before and after operation were compared by paired sample t test, and the data between groups were compared by independent sample t test. χ 2 test was used for counting data. Pearson correlation analysis was used to compare the correlation between ahlback score, HSS and WOMAC osteoarthritis index. Results:The follow-up time was (35.1±6.6) months (range: 25 to 47 months). The knee function of the patients improved significantly after operation. The HSS score increased from 57.7±11.8 preoperative to 81.8±7.8 postoperative ( t=16.64, P=0.00) and WOMAC osteoarthritis index decreased from 48.9±13.4 preoperative to 15.6±8.8 postoperative ( t=20.48, P=0.00). There was no statistical difference in the change of HSS between the degenerative group and the non-degenerative group before and after surgery(27.5±12.2 vs. 22.5±12.3, t=-1.65, P=0.10) as well as the change of WOMAC osteoarthritis index(31.8±14.0 vs. 36.4±13.7, t=-1.35, P=0.18), but the lateral patellofemoral joint degeneration was related to inability to complete squats (χ 2=5.17, P=0.04) and sitting up (χ 2=7.22, P=0.01). Conclusion:The degeneration of lateral patellofemoral joint has no effect on the early functional recovery of patients with anteromedial knee osteoarthritis after Oxford medial unicompartmental knee arthroplasty.

OBJECTIVE: To investigate the effects of Buyang huanwu decoction on endothelial-mesenchymal transformation (EndMT) of lung tissue in idiopathic pulmonary fibrosis (IPF) model rats, and to explore its potential mechanism. METHODS: Male SD rats were randomly divided into normal group, model gorup, dexamethasone group [0.405 mg/(kg·d)], Buyang huanwu decoction low-dose, medium-dose and high-dose groups [6.435, 12.87, 25.74 g/(kg·d), by raw material], with 8 rats in each group. Except for normal group, other groups were given endotracheal injection of bleomycin to induce IPF model. On the second day after modeling, normal group and model group were given water intrgastrically [10 mL/(kg·d)]; administration groups were given relevant medicine intragastrically, once a day, for consecutive 28 days. 24 h after last medication, the expression of endothelial cell markers [platelet endothelial cell adhesion molecule 1, vascular endothelial cell cadherin] and interstitial cell markers [α-smooth muscle actin, fibroblast specific protein 1] were detected by immunohistochemistry method. The expression of Notch4 and DLL4 in lung tissue of rats were detected by Western blotting assay. RESULTS: Compared with normal group, the expression of endothelial cell markers were decreased significantly in lung tissue of model group, while the expressipon of interstitial cell markers, Notch4 and DLL4 were increased significantly (P<0.01). Compared with model group, the expression of endothelial cell markers in lung tissue of rats were increased significantly in administration groups, while Buyang huanwu decoction low-dose group was significantly lower than dexamethasone group; the expression of interstitial cell markers, Notch4 and DLL4 were decreased significantly, while Buyang huanwu decoction low-dose group was significantly higher than dexamethasone group (P<0.05 or P<0.01). CONCLUSIONS: Buyang huanwu decoction can relieve IPF of model rats by intervening in EndMT, the mechanism of which may be associated with inhibiting DLL4/Notch4 singaling pathway.

Objective? To explore the effects of transitional care on postoperative urinary control function and quality of life among prostatic cancer patients so as to provide a basis for improving patients' quality of life after discharge. Methods? From January 2013 to December 2014, we selected 120 prostatic cancer patients with postoperative urinary incontinence of the First Affiliated Hospital of Wenzhou Medical University as subjects. All of the patients were divided into observation group and control group with the method of random number table. Patients of control group received routine nursing instruction after discharge. On the basis of routine nursing, patients of observation group were treated with transitional care after discharge. The urinary control function and quality of life of patients between two groups were compared six months after discharge. Results? The total compliance rate of functional exercise for pelvic mucles in observation group was 91.7% statistically higher than that (71.7%) of control group (χ2=8.015, P< 0.01); the scores of self-care, self-responsibility, self-concept and health knowledge of observation group were statistically higher than those of control group (t=4.536, 11.425, 9.674, 2.942; P< 0.01). The recovery time of urinary control function of observation group was (1.21±0.72) months lower than that (3.03±0.84) months of control group (t=12.742,P<0.01); the total score of quality of life was (77.13±12.42) in observation group higher than that (59.76±13.21) in control group (t=6.574, P< 0.01); the differences were all statistical. Conclusions? The application of transitional care in postoperative prostatic cancer patients after discharge can improve patient compliance as well as self-care ability, shorten postoperative recovery time of urinary control function and improve patients' quality of life.

Objective To investigate the risk factors of bladder spasm after transurethral resection of the prostate (TURP) and the intervention effect of cluster nursing. Methods From January 2011 to December 2012, the clinical data of 120 cases of TURP patients were analyzed for the risk factors of bladder spasm after TURP. According to the results of the analysis, cluster nursing plan was constructed. The 150 TURP patients from January 2013 to December 2014 were selected as the control group and given routine nursing care. The 150 TURP patients from January 2015 to December 2016 were recruited as the observation group and given cluster nursing. The postoperative bladder spasm, catheter blockage and patients satisfaction were compared between the two groups. Results The results of the multivariate regression analysis indicated that the score of Self-rating Anxiety Scale, air bag water injection, unstable bladder or low compliance bladder, and postoperative constipation were independent factors of bladder spasm after TURP (OR=1.427, 0.092, 13.263, 27.437; P<0.01). After the implementation of cluster nursing, the postoperative bladder spasm rate was 6.0%, and the catheter blockage rate was 5.3%, which were both lower than those of the control group (28.4%,29.3%), and the differences were statistically significant (χ2=25.726, 30.149; P<0.01). The patient postoperative satisfaction of the observation group was (95.21±4.63), which was higher than that of the control group (85.76±4.23), and the difference was statistically significant (t=14.469, P<0.01). Conclusions Cluster nursing can reduce the rate of bladder spasm and catheter blockage after TURP, and increase patients' satisfaction after surgery.
