1.Exploring the Mechanism of Pinggan Yishen Decoction Against Target Organ Damage in Spontaneously Hypertensive Rats Based on UPLC-Q-TOF/MS and Network Pharmacology

		                        			OBJECTIVE To investigate the mechanisms by which Pinggan Yishen Decoction(PGYSD)contributes to alleviating target organ damage in spontaneously hypertensive rats.METHODS The chemical components of PGYSD were determined by ultra-performance liquid chromatography-quadrupole time-of-flight mass spectrometry(UPLC-Q-TOF/MS)and were analyzed by target a-nalysis and functional enrichment combined with network pharmacology methods to predict the potential mechanism of PGYSD in trea-ting hypertension and its target organ damage.Spontaneously hypertensive rats were randomly divided into the model group,low-dose PGYSD group(2 g·kg-1),high-dose PGYSD group(5 g·kg-1),and valsartan group(7.2 mg·kg-1),with 6 rats in each group.Wistar-Kyoto rats were used as the control group,and the control group and the model group were gavaged with normal saline for 8 consecutive weeks.HE and Masson staining were used to observe the pathological damage and fibrosis degree of rat heart and tho-racic aorta.Immunohistochemical staining and Western blot were used to detect the expression level of EGFR in the heart,liver and kidney of rats.Immunofluorescence staining was used to detect the co-localization of EGFR and EEA1 in the heart,liver and kidney of rats.RESULTS Twenty-six components of PGYSD were detected by UPLC-Q-TOF/MS.Network pharmacology revealed that EG-FR,PIK3R1 and EP300 may be key therapeutic targets of action of PGYSD for the treatment of hypertension and its target organ dam-age,and that the treatment of hypertension and its target organ damage by PGYSD may be closely related to EGFR tyrosine kinase in-hibitor resistance,lipids and atherosclerosis and HIF-1 signaling pathway.The high-dose group of PGYSD significantly reduced sys-tolic blood pressure and mean blood pressure in rats(P<0.05,P<0.01),attenuated pathological damage and fibrosis in the heart and thoracic aorta(P<0.01,P<0.001),significantly reduced the expression level of EGFR in the liver and kidney of rats(P<0.01),and treated fibrosis in liver and kidney,reduced the co-localization of EGFR and EEA1 in the kidney of rats(P<0.001),attenuated fibro-sis in kidney.CONCLUSION The paper integrates UPLC-Q-TOF/MS,network pharmacology and spontaneously hypertensive rat model and preliminarily explores the effect mechanism of PGYSD in the treatment of hypertension and its target organ damage,provi-ding a scientific basis for further mechanism research and clinical application of PGYSD in the treatment of hypertension.
		                        		
		                        		
		                        		
		                        	
2.Prevalence and 5-year mortality of dementia and association with geriatric syndromes in elderly population in Beijing

		                        			Objective:To investigate the prevalence and mortality of dementia and assess the impact of geriatric syndromes (GS) on the risk for dementia and death in elderly population in Beijing.Methods:A cross-sectional survey was conducted in the elderly population aged ≥65 years and selected by a multi-stage sampling in Beijing during 2013-2015. Cognitive function was screened using the Chinese Revised Version of the Mini-Mental State Examination (MMSE). Then, neurological examination and psychiatric assessment were performed for those with the MMSE score lower than the cut-off value. The information about GS prevalence was also collected. The study also collected death records for all individuals from baseline until December 31, 2019. Based on the age and gender distribution from Beijing data of the 2010 Six th National Population Census, the dementia prevalence in the study population was directly standardized. Logistic regression analysis was used to evaluate the association of different forms of dementia with GS, and Cox proportional hazards regression model was used to estimate the hazard ratio ( HR) and 95% CI of death. Results:During 2013-2015, a total of 2 935 individuals completed dementia assessments, of which 167 were diagnosed with dementia. The standardized prevalence of dementia was 5.9% (95% CI: 5.0%-17.4%). The individuals with Alzheimer's disease (AD) and vascular dementia (VaD) accounted for 58.7% and 28.1% of total individuals with dementia, respectively. Aging, lower education level, urinary incontinence, and fall were risk factors for AD, while disability of activity of daily life dependence, hypertension, and stroke were found to be risk factors for VaD. After a median follow-up of 5.44 person-years, 399 deaths were recorded. The 5-year mortality risk was 2.87 (95% CI: 1.92-4.17) times and 4.93 (95% CI: 3.23-7.53) times higher for the elderly individuals with AD and VaD, respectively, compared to non-demented individuals. After adjusting for demographic, GS, and cardiovascular risk factors, the mortality risk in the elderly individuals with AD showed no significant difference compared with non-demented individuals ( HR=1.32, 95% CI: 0.89-1.97), while the mortality risk in those with VaD was 2.46 (95% CI: 1.49-4.05) times higher than that in non-demented individuals. Conclusions:The prevalence of dementia in Beijing increased significantly in the context of population aging, especially the prevalence of AD. The presence of GS increased the risks for AD and VaD, as well as the risk for death. 3.Intolerability of uncertainty and related factors in women receiving in vitro fertilization and embryo transfer

		                        			Objective:To explore the intolerable uncertainty level and its related factors in female patients re-ceiving in vitro fertilization and embryo transfer.Methods:A total of 503 female patients receiving in vitro fertiliza-tion and embryo transfer in a tertiary reproductive hospital in Shandong province were selected.Theywere assessed with a self-designed general information questionnaire,the Intolerance of Uncertainty scale-12(IUS-12),Fertility Problem Inventory(FPI,including social concern,sexual concern,relationship concern,need for parenthood,and re-jection of childfree lifestyle),and Hospital Anxiety and Depression Scale(HADS).Results:The score of IUS-12 was 28(13,60).Multiple stepwise regression analysis showed that having children,having anxiety symptoms,the scores of social concern,sexual concern and need for parenthood were positively associated with IUS-12 scores(β=0.11,0.19,0.21,0.13,0.25),and rejection of childfree lifestyle was negatively associated with IUS-12 scores(β=-0.18).Conclusion:It suggests that the unbearable uncertainty of women receiving in vitro fertilization and embryo transfer is related to whether they have children,social concerns,sexual concerns,need for parenthood,and rejection of childfree lifestyle.
		                        		
		                        		
		                        		
		                        	
4.Scoping review of continuous bladder irrigation practice following transurethral resection of the prostate

		                        			Objective:To comprehensively and systematically collect and review domestic and international studies on the practices of continuous bladder irrigation (CBI) following transurethral resection of the prostate (TURP), including the topics, research methods, and evaluation techniques used in these studies, and to provide clinical nursing staff with a reference.Methods:Nine Chinese and English databases were systematically searched, including CNKI, Wanfang, SinoMed, VIP, Embase, Cochrane Library, PubMed, CINAHL, and Web of Science. The search covered all literature up to April 10, 2023. The practices of CBI were extracted and systematically analyzed, and the results were reported in a standardized manner using a scoping review methodology.Results:A total of 23 eligible studies were included, comprising two English articles and 21 Chinese articles. The types of studies included 12 randomized controlled trials and 11 quasi-experimental studies. The interventions were categorized into single and comprehensive measures, including methods such as adjusting the temperature and flow rate of bladder irrigation, selection of indwelling catheters, traditional Chinese medicine-based nursing, perioperative psychological care, and rapid recovery protocols. The evaluation metrics used in the studies were classified into three levels: symptom indicators, process indicators, and management indicators.Conclusions:Research on CBI following TURP has achieved a certain scale, with a variety of intervention methods currently available. However, there is still a lack of high-quality evidence. Future studies should focus on enhancing research design, developing standardized quality assessment tools for bladder irrigation, and improving innovative irrigation devices to enhance clinical nursing quality.
		                        		
		                        		
		                        		
		                        	
5.Development of reproductive motivation scale for infertile women and its reliability and validity

		                        			Objective:To preliminarily develop a fertility motivation scale for infertile women (FMS-IW) and test its reliability and validity.Methods:The FMS-IW was developed according to the theory of self-determination and Maslow's hierarchy of needs, and the original item pool was established through clinical psychological counseling practice experience, literature review, consulting clinical psychotherapists, interviewing with infertile women and open-ended questionnaires. The original scale was constructed on item analysis and exploratory factor analysis in 257 infertile women. The formal version of FMS-IW was further adapted by Delphi method and tested in another 392 women to conduct confirmatory factor analysis and reliability test. Furthermore, 56 participants of 392 women were randomly retested with FMS-IW after two weeks.SPSS 25.0 and Amos 24.0 software were used for Spearman analysis, exploratory factor analysis and confirmatory factor analysis.Results:Exploratory factor analysis showed that the FMS-IW was composed of 16 items consisting of two factors: autonomous fertility motivation and controlled fertility motivation. The cumulative variance contribution rate was 64.18%. Confirmatory factor analysis showed that the model fitted well ( χ2/ df=3.292, RMSEA=0.077, SRMR=0.055, GFI=0.902, AGFI=0.871, IFI=0.938, CFI=0.938, TLI=0.928). The Cronbach's α coefficient of the FMS-IW was 0.908. The Cronbach's α coefficient for autonomous and controlled fertility motivation was 0.911 and 0.928, respectively. The parity split-half coefficient of the formal version of FMS-IW was 0.870. The test-retest reliability of the formal version of FMS-IW was 0.823. Conclusion:The FMS-IW has good reliability and validity, and can be used as an effective tool to measure fertility motivation in infertile women.
		                        		
		                        		
		                        		
		                        	
6.The safety and efficacy of a dedicated neonatal-infant 0.35 T brain MRI system

		                        			Objective:To evaluate the safety and efficacy of a dedicated neonatal-infant brain 0.35 T MRI system.Methods:A dual-center controlled clinical trial was conducted with single-arm objective performance criteria. From June to July 2020, sixty-six infants aged 0-12 (6.3±3.4) months were recruited from Children′s Hospital of Soochow University and the First People′s Hospital of Lianyungang prospectively. All infants underwent brain MRI with a dedicated neonatal-infant 0.35 T brain MRI system, using the dedicated two-channel transceiver head coil. MRI protocol included spin echo T 1WI, fast spin echo T 2WI, fluid attenuated inversion recovery, diffusion weighted imaging and 3D gradient echo sequence. MRI sequences were set with three orientations (axial, sagittal and coronal). Each case received at least two scanning planes and two scanning sequences. Five-point Likert scoring system was used to evaluate the image quality of acquired images, and the target value was set as at least 3 points per image. The temperature, heart rate and breathe of the infants were recorded before and after MRI; the acoustic noise of the MRI system was measured during the scanning process; and the adverse reactions were recorded if presented. Results:Five infants successfully completed their examination during non-sedated sleep in a single attempt, and 61 infants after sedation with chloral hydrate. Based on MRI-based five-point Likert scoring system, 41 cases achieved a score of 5, 21 cases with a score of 4 and 4 cases with a score of 3. Cases with score of 3 was due to movement of the infants during the scan, which resulted in motion related artifacts. The vital signs of all infants showed stable before and after imaging, with heart rate of (126.8±12.9) beats per minute, breathe of (38.2±6.8) times per minute. It was found that 47 cases showed no sign of temperature raise after brain MRI, 15 cases had less than 0.3 ℃ raise and 4 cases had 0.3 ℃ to 0.5 ℃ raise. The noise recorded during the scanning process was (57.5±1.8) dB(A). One case had mild diarrhea on the day of MR scan, and the symptoms disappeared on the second day without treatment; no adverse reactions were found for the rest subjects.Conclusion:Dedicated neonatal-infant 0.35 T brain MRI system allows data acquisition with high safety and excellent image quality, which has potentials in the clinical applications.
		                        		
		                        		
		                        		
		                        	
7.Construction of a chimeric antigen receptor CAR THP -1 cell line targeting HER2

		                        			To  obtain  chimeric antigen receptor macrophages  ( CAR-M) targeting HER2  stably transfected.
		                        		
		                        			Methods  :
		                        			CAR lentivirus vector targeting HER2 was constructed and infected with human monocytic leukemia cell line  (THP-1) .CAR  THP-1  cells with green fluorescent labeling were selected by sorting flow cytometry  and continued to be cultured in vitro.The CAR THP-1 cells targeting HER2 were co-cultured with the endometrial  cancer cell line Ishikawa with negative and positive HER2 expression,and their targeted phagocytosis of CAR-M to  HER2 positive  tumor  cells  was  detected  by  imaging  flow cytometry ,and the targeted phagocytosis efficiency of CAR-M to HER2 positive tumor cells was detected by flow cytometry.  
		                        		
		                        			Results :
		                        			CAR lentivirus infection with THP- 1 cells was less efficient ; After co-culture with cancer cells,flow cytometry and imaging flow cytometry showed that  CAR THP-1 cells had enhanced phagocytosis of HER2 positive Ishikawa cells compared with the empty body group  (P<0. 01) .
		                        		
		                        			Conclusion  
		                        			In this experiment,CAR THP-1  cell line targeting HER2 was established by constructing CAR lentivirus vector  and  transfecting  THP-1  cells ,and  it  was  proved  that  CAR  THP-1  could  phagocytize  HER2 positive Ishikawa cells through specific targeting.
		                        		
		                        		
		                        		
		                        	
8.Effect of case three-dimensional teaching method integrated with multimedia teaching on practice teaching of liver infectious diseases

		                        			Objective:To explore the effect of case three-dimensional teaching method (CTTM) combined with multimedia teaching on the practice teaching of liver infectious diseases.Methods:Twenty-five interns of liver infectious diseases received by The Second Hospital of Nanjing from May 2019 to January 2020 were taken as the control group, and 25 interns of liver infectious diseases received from February 2020 to October 2020 were taken as the observation group. The control group was given the conventional teaching mode including the conventional case teaching and the conventional multimedia teaching, while the observation group was given CTTM integrated with multimedia teaching method, and both groups were taught for 4 weeks. After teaching, the examination results of liver infectious disease related theory and practice, the changes of core competence before and after teaching, and the recognition of teaching were compared between the two groups. SPSS 25.0 was used for t test and chi-square test. Results:After teaching, the examination results of liver infectious disease related theory [(92.45±7.23) vs. (81.21±7.11)] and practice [(93.27±6.68) vs. (81.23±6.26)] in the observation group were higher than those in the control group. There was no significant difference in the scores of core competence between the two groups before teaching; after teaching, the scores of autonomous learning, problem analysis and solving, doctor-patient communication and teamwork ability of the two groups were higher than those before teaching, and the scores of observation group were significantly higher than those of the control group ( P<0.05). The recognition rates of interns in the observation group were higher than those in the control group in the improvement of learning initiative, the consolidation of theoretical knowledge, the enhancement of practical ability and the improvement of learning efficiency ( P<0.05). Conclusion:In the teaching of liver infectious disease interns, CTTM integrated with multimedia teaching method can not only improve the examination results, and enhance the core competence, but also improve the recognition of interns on the teaching mode.
		                        		
		                        		
		                        		
		                        	
9.Correlation between anxiety level of residents and needs of traditional Chinese medicine nursing during the COVID-19 epidemic

		                        			ObjectiveTo understand the current situation of anxiety level and traditional Chinese medicine (TCM) nursing needs among residents during the COVID-19 epidemic, and to discuss the relationship between them,so as to provide references for maintaining their mental health. MethodsA total of 560 residents were recruited using convenience sampling method, and the selected individuals completed three questionnaires based on online platform, including self-designed general information questionnaire, Generalized Anxiety Disorder Scale (GAD-7), and TCM nursing needs questionnaire. Then the correlation between anxiety level and TCM nursing needs was analyzed by Spearman correlation test. ResultsA total of 552 valid questionnaires were collected. The results of GAD-7 showed that the total score was (11.04±3.72), and average score of each item was (1.58±0.53). There were 328 cases (59.4%) without anxiety symptoms, 180 cases (32.6%) with mild anxiety, 33 cases (6.0%) with moderate anxiety, and 11 cases (2.0%) with severe anxiety. The analysis of TCM nursing needs questionnaire showed that the total score was (21.59±7.92), and dominated by the needs of medical knowledge (2.90±1.15) and TCM dietary care (2.82±1.13). The item score and total score of TCM nursing needs questionnaire of residents during the COVID-19 epidemic were positively correlated with GAD-7 total score (r=0.240~0.331, P<0.01). ConclusionResidents suffer from different levels of anxiety during the COVID-19 epidemic, and TCM nursing needs is on the rise, indicating that the anxiety level of residents is correlated with the needs of TCM nursing. 
		                        		
		                        		
		                        		
		                        	
10.Effects of intranasal administration of glial cell line-derived neurotrophic factor on postoperative cognitive dysfunction in aged rats

		                        			Objective:To evaluate the effects of intranasal administration of glial cell line-derived neurotrophic factor (GDNF) on postoperative cognitive dysfunction in aged rats.Methods:Forty healthy Sprague-Dawley rats of both sexes, aged 21-23 months, weighing 480-600 g, were divided into 4 groups ( n=10 each) using a random number table method: sham operation group (group S), operation group (group O), intranasal administration of low-dose GDNF group (group G1) and intranasal administration of high-dose GDNF group (group G2). Rats underwent exploratory laparotomy under anesthesia with chloral hydrate in O, G1 and G2 groups, while the rats in group S only received sham operation.The rats in group G1 and group G2 were intranasally treated with GDNF 25 and 50 μg (in 25 μl of PBS), respectively, and PBS 25 μl was nasally administered in group S and group O every day for 3 consecutive days after operation or sham operation.Morris water maze test was performed on days 3-7 after surgery, and then the rats were sacrificed, and hippocampal tissues were removed for determination of the expression of GDNF, high mobility group box 1 (HMGB1), interleukin-1β (IL-1β), tumor necrosis factor-α (TNF-α), activated caspase-3 and Bax (by Western blot). Results:Compared with group S, the escape latency was significantly prolonged on days 5-7 after operation, the number of crossing the platform was reduced, time spent in the target quadrant was shortened, expression of GDNF was down-regulated, and expression of IL-1β, TNF-α, HMGB1, activated caspase-3 and Bax in hippocampi was up-regulated in group O, and the number of crossing the platform was reduced, time spent in the target quadrant was shortened, and expression of IL-1β and TNF-α was up-regulated in G1 and G2 groups ( P<0.05). Compared with group O, the escape latency was significantly shortened on days 5-7 after operation, the number of crossing the platform was increased, time spent in the target quadrant was prolonged, expression of GDNF was up-regulated, expression of TNF-α, HMGB1, activated caspase-3 and Bax in hippocampi was down-regulated in G1 and G2 groups, and IL-1β in hippocampi was down-regulated in group G1 ( P<0.05). Compared with group G1, the expression of TNF-α in hippocampi was down-regulated ( P<0.05), and no significant change was found in the other parameters mentioned above in group G2 ( P>0.05). Conclusions:Intranasal administration of GDNF can improve postoperative cognitive dysfunction, and the mechanism may be related to inhibiting neuroinflammatory responses and neuroapoptosis in aged rats.
