Objective To probe into the total number and development status of respiratory physicians in China, and to analyze existing problems, for reference in making strategy and policy decisions in professional development of respiratory physicians in the country. Methods The basic information of China′s healthcare institutions and healthcare manpower in 2015 were provided by the Statistics Center of the National Health Commission. The number of respiratory specialists and their profile, drawn from such basic information, were subject to descriptive statistics. Results In 2015, there were 2.729 million medical practitioners ( including assistant doctors ) nationwide, 30.3 thousand among whom being respiratory physicians, accounting for 1.11% of the total. Among these respiratory physicians, practicing respiratory physicians account for 87.1% , respiratory physicians with a master degree and above account for only 19.7% , respiratory physicians with over 10 years of working experience account for 65.8% , respiratory physicians with senior titles account for only 21.4% , and 89.4% of the respiratory physicians work in general hospitals. Conclusions By the criteria of specializing in respiratory specialty over five years and being attending or above, the number approximates 14. 6 thousand nationwide. If practicing ( assistant) physicians specializing in respiratory specialty are also included, the number may hit 30. 3 thousand. Such a number, compared with other specialties, may be sufficient, yet their competence is far from satisfactory. Therefore it is imperative to build a standardized pulmonary medicine fellowship training system for their competence improvement.

Objective To investigate the cluster nursing in improving the weaning success rate of emergency neurally adjusted ventilatory assist (NAVA) in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD).Methods A total of 72 AECOPD patients treated with NAVA in the Second Affiliated Hospital of Wenzhou Medical University emergency intensive care unit (EICU) from January 2014 to January 2016 were collected and randomly divided into observation group and control group according to the random number table, 36 cases respectively. The observation group received the cluster nursing, and the control group used the traditional nursing. The incidence of ventilator associated pneumonia (VAP), length of stay, the service time and weaning success rate of ventilator were compared between two groups. Results The length of stay in EICU [(7.33±1.79) d]and the service time of ventilator [(3.17±0.63) d]in the observation group were shorter than those in the control group, the differences were statistically significant (t=8.912, 16.524;P<0.05). The incidence of VAP (13.89%) in the observation group was statistically significantly lower than those in the control group (χ2=9.425,P<0.05). The weaning success rate (86.11%) in the observation group was significantly higher than that in the control group (χ2=8.134,P<0.05).Conclusions The cluster nursing in AECOPD patients with emergency NAVA treatment can significantly reduce the ventilator-associated complications and improve the success rate of weaning. It is worthy of clinical promotion.

Objective To investigate the protective effect of protocatechuic acid(PCA)on the PC12 cell model of Alzheimer’s disease(AD)and to explore its mechanism . Methods Amyloid beta peptide 1-42(Aβ1-42)fiber polymers were identified by immunofluorescence. After PC12 cells were stimulated with the Aβ1-42 fiber polymers, the cellular morphology was observed at different time points of hour 0, 3, 6, 9, 12, 24 , and the cellular viability was tested by methyl thiazolyl tetrazolium(MTT)assay to monitor the modeling condition. The effect of PCA on PC12 cells was detected after PC12 cells were pretreated with the different contentions of PCA. Autophagy-related marker Beclin1 protein level was detected by Western blotting method to investigate the protective mechanism of PCA. Results Aggregated white Aβ1-42 mass was stable at hour 12 and 24, and showed no significant difference between the two time points, the cell damage rate being 40%. Therefore, we defined culturing time being 12 and 24 hours as the modeling condition of AD model. The cell viability was increased with 200-800 μmol/L of PCA after culturing for 24 hours(P<0.01) , and the Western blotting results showed that the Beclin1 protein expression was up-regulated by PCA. Conclusion PCA prevents PC12 cells from Aβ1-42-induced toxicity, the mechanism being related with the increase of cellular autophagy.
