Objective To observe the guiding value of cytochrome P450(CYP450)enzyme related gene polymorphism for tamoxifen(TAM)in the precise treatment of relapsed metastatic breast cancer. Methods Patients with recurrent metastatic breast cancer were selected, all of whom were treated with oral TAM,and complete remission and partial remission were included in the sensitive group, while stable and progressive were included in the drug-resistant group.Patients were followed up by outpatient and telephone visits until December 2022,and median survival time was recorded.CYP2D6*10 genotype distribution, general situation, clinicopathological characteristics of sensitive group and drug-resistant group were compared.COX proportional risk regression model was used and Kaplan-Meier curve was drawn to analyze the relationship between CYP2D6*10 genotype distribution and clinical efficacy of TAM in the treatment of recurrent and metastatic breast cancer. Results As of the date of follow-up,the median survival time of patients with CYP2D6*10 genotype C/C+C/T(58 patients)was 18 months(95%CI:14.48-21.52)and that of patients with CYP2D6*10 genotype TT(34 patients)was 13 months(95%CI:10.69-15.31).The survival rate of CYP2D6*10 genotype C/C+C/T patients was higher than that of CYP2D6*10 genotype TT patients, the Log-rank test showed that the difference in Kaplan-Meier curves was statistically significant(x2=6.901,P<0.05).Compared with the sensitive group, the CYP2D6*10 wild-type C/C genotype component ratio decreased, while the mutant homozygous T/T genotype component ratio increased in the drug-resistant group(P<0.05).There was no significant difference in CYP2D6*10 heterozygote C/T genotype composition between the two groups(P>0.05).Compared with sensitive group, the size of primary tumor>5 cm,number of lymph node metastasis ≥4,estrogen receptor positive, progesterone receptor positive,CYP2D6*10 genotype distribution T/T genotype composition ratio were increased in drug-resistant group(P<0.05).COX proportional risk regression model showed that primary tumor size, number of lymph node metastases, estrogen receptor status, progesterone receptor status and CYP2D6*10 genotype distribution were all independent predictors of clinical efficacy of TAM in the treatment of recurrent metastatic breast cancer(P<0.05).Conclusions CYP2D6*10 gene polymorphism is closely associated with the clinical efficacy and median survival of TAM in the treatment of recurrent metastatic breast cancer.

The breakage pattern of unit particles during the production of oral solid dosage forms (OSD) is closely related to the quality of intermediate or final products. To accurately characterize the particles and study the evolution law of particle breakage, the Bonding model of the discrete element method (DEM) was used to investigate the breakage patterns of model parameters, particle shape and process conditions (loading mode and loading rate) on the dynamic breakage, force-time curve, breakage rate, maximum breakage size ratio and fracture strength of particles. The results showed that the particle breakage force was positively correlated with normal strength and bonded disk scale, negatively correlated with normal stiffness per unit area and tangential stiffness per unit area, and weakly correlated with tangential strength. The particle breakage rate was negatively correlated with the aspect ratio of the particles, and the maximum breakage size ratio was positively correlated with the aspect ratio of the particles; among the three loading modes, the breakage rate of compression breakage model was the largest, the breakage rate of shear breakage model was the second largest, and the breakage rate of wear breakage model was the smallest; the maximum breakage size ratio was positively correlated with the loading rate, the loading mode and the loading rate had no mutual influence on particle breakage rate, but had mutual influence on the maximum breakage size ratio. The research results will provide a theoretical basis for the shift of OSD from batch manufacturing to advanced manufacturing.

This paper summarized professor WANG Shouchuan's experience in differentiating and treating children epistaxis from the perspective of "four excess and three deficiency". It is believed that the pathogenesis of children epistaxis is concluded as "four excess and three deficiency", of which the four excess syndromes are exuberant heat in the lung channel, intense stomach fire, heart fire hyperactivity, and liver fire flaming upward, while the three deficiency syndromes include qi, yin and yang deficiency. Seven methods for treating children epistaxis are summarized. For exuberant heat in the lung channel syndrome, it is recommended to clear lung and direct qi downward, using self-made Xiebai Zhiniu Decoction (泻白止衄汤). For intense stomach fire syndrome, the method of clearing stomach and draining fire can be used with self-made Qingwei Zhiniu Decoction (清胃止衄汤). In terms of heart fire hyperactivity syndrome, it is better to clear heart and drain fire, using self-made Daochi Zhiniu Decoction (导赤止衄汤). For liver fire flaming upward syndrome, it is advised to clear liver and drain fire, using self-made Yimu Zhiniu Decoction (抑木止衄汤). In terms of qi deficiency syndrome, the method of fortifying spleen and boosting qi and containing blood should be used with self-made Futu Zhiniu Decoction (扶土止衄汤). If there is yin deficiency syndrome, it is advised to supplement kidney, enrich yin and clear heat, using self-made Zishui Zhiniu Decoction (滋水止衄汤). If there is yang deficiency syndrome, the method of boosting qi, warming yang and nourishing blood can be used, using self-made Wenpi Zhiniu Decoction (温脾止衄汤).

Hip fracture in the elderly is characterized by high incidence, high disability rate, and high mortality and has been recognized as a public health issue threatening their health. Surgery is the preferred choice for the treatment of elderly patients with hip fracture. However, lower extremity deep venous thrombosis (DVT) has an extremely high incidence rate during the perioperative period, and may significantly increase the risk of patients′ death once it progresses to pulmonary embolism. In response to this issue, the clinical guidelines and expert consensuses all emphasize active application of comprehensive preventive measures, including basic prevention, physical prevention, and pharmacological prevention. In this prevention system, basic prevention is the basis of physical and pharmacological prevention. However, there is a lack of unified and definite recommendations for basic preventive measures in clinical practice. To this end, the Orthopedic Nursing Professional Committee of the Chinese Nursing Association and Nursing Department of the Orthopedic Branch of the China International Exchange and Promotive Association for Medical and Health Care organized relevant nursing experts to formulate Expert consensus on perioperative basic prevention for lower extremity deep venous thrombosis in elderly patients with hip fracture ( version 2024) . A total of 10 recommendations were proposed, aiming to standardize the basic preventive measures for lower extremity DVT in elderly patients with hip fractures during the perioperative period and promote their subsequent rehabilitation.

Patients with severe trauma require an extremely timely treatment and transfusion plays an irreplaceable role in the emergency treatment of such patients. An increasing number of evidence-based medicinal evidences and clinical practices suggest that patients with severe traumatic bleeding benefit from early transfusion of low-titer group O whole blood or hemostatic resuscitation with red blood cells, plasma and platelet of a balanced ratio. However, the current domestic mode of blood supply cannot fully meet the requirements of timely and effective blood transfusion for emergency treatment of patients with severe trauma in clinical practice. In order to solve the key problems in blood supply and blood transfusion strategies for emergency treatment of severe trauma, Branch of Clinical Transfusion Medicine of Chinese Medical Association, Group for Trauma Emergency Care and Multiple Injuries of Trauma Branch of Chinese Medical Association, Young Scholar Group of Disaster Medicine Branch of Chinese Medical Association organized domestic experts of blood transfusion medicine and trauma treatment to jointly formulate Chinese expert consensus on blood support mode and blood transfusion strategies for emergency treatment of severe trauma patients ( version 2024). Based on the evidence-based medical evidence and Delphi method of expert consultation and voting, 10 recommendations were put forward from two aspects of blood support mode and transfusion strategies, aiming to provide a reference for transfusion resuscitation in the emergency treatment of severe trauma and further improve the success rate of treatment of patients with severe trauma.
