1.Inhibitory Effects of the Slit Guidance Ligand 1-3’ Untranslated Region on the Fibrotic Phenotype of Cardiac Fibroblasts

ObjectiveTo study the regulatory effect of the partial sequence within the 3’ untranslated region (3’UTR) of slit guidance ligand 1 (Slit1) (Slit1-3’UTR) on the fibrotic phenotypes of cardiac fibroblasts (CFs) and its potential mechanism. MethodsThe adenovirus vector was used to overexpress the 1526nt sequence of Slit1-3’UTR in ICR neonatal mouse CFs (mCFs). The expression of fibrosis-related genes in mCFs, such as collagen type 1 alpha1(COL1A1), collagen type 3 alpha3 (COL3A1) and alpha smooth muscle actin (α-SMA) were detected by Western blot assay. The effect of Slit1-3’UTR 1526nt on the proliferation and migration of mCFs was assessed by EdU staining and Trans-well assays. Angiotensin Ⅱ (Ang Ⅱ) was used to treat mCFs, and the impact of Slit1-3’UTR 1526nt on the fibrotic phenotypes of Ang Ⅱ-induced mCFs was evaluated. After overexpression of Slit1-3’UTR 1526nt, miR-34a-5p mimic was transfected into mCFs, followed by actinomycin D treatment to detect the mRNA stability of Slit1-3’UTR 1526nt, and the levels of miR-34a-5p and its target gene SIRT1(si-SIRT1) in mCFs were determined. The effects of miR-34a-5p and small interfering RNA targeting SIRT1 on the Slit1-3’UTR 1526nt-mediated regulation of fibrotic phenotypes were also determined. ResultsAdenovirus-mediated overexpression of Slit 1-3’UTR 1526nt was achieved in mCFs. Overexpression of Slit 1-3’UTR 1526nt markedly inhibited the expression of the fibrosis-related genes, proliferation and migration of mCFs and fibrotic phenotypes of Ang Ⅱ. The results of actinomycin D assay showed that miR-34a-5p inhibited the stability of Slit1-3’UTR 1526nt in mCFs, while the level of miR-34a-5p was reduced in mCFs with overexpression of Slit1-3’UTR 1526nt. Transfection of miR-34a-5p promoted the fibrotic phenotypes, and reversed the inhibitory effect of Slit1-3’UTR 1526nt on the fibrotic phenotypes of mCFs. Overexpression of Slit1-3’UTR 1526nt significantly increased the level of miR-34a-5p target gene SIRT1 in mCFs. Transfection of miR-34a-5p and si-SIRT1 consistently reversed the inhibitory effects of Slit1-3’UTR 1526nt on the fibrotic phenotypes of mCFs. ConclusionSlit1-3’UTR1526nt inhibits the fibrotic phenotypes of mCFs by binding to miR-34a-5p and increasing the expression of its target gene of SIRT1.
2.IDH3A Inhibits Cardiomyocyte Hypertrophy via Elevating α-Ketoglutarate Level

ObjectiveTo investigate the regulatory effect and potential mechanisms of isocitrate dehydrogenase 3A (IDH3A) on cardiomyocyte hypertrophy. MethodsThe expression of IDH3A in the myocardium of healthy volunteers (n=10) and patients with heart failure (HF) (n=10), and in the myocardium of mice subjected to transverse aortic constriction (TAC) surgery and sham operation, as well as in phenylephrine (PE)-induced neonatal rat ventricular cardiomyocytes (NRVCs), was assessed by real-time quantitative polymerase chain reaction (RT-qPCR) and Western blot assay. The effect of adenovirus-mediated overexpression of IDH3A on the expression of hypertrophy-related genes in PE-induced NRVCs was also evaluated. The effect of IDH3A on NRVCs area was examined by phalloidin staining assay. A mutant of IDH3A with abolished enzymatic activity, IDH3A_D208A, was generated through site-directed mutagenesis. The impact of this IDH3A mutant on the hypertrophic phenotype, ATP and ROS levels in NRVCs was evaluated to investigate whether the regulatory role of IDH3A in cardiomyocyte hypertrophy was dependent on its enzymatic activity. The effect of exogenous α-ketoglutaric acid (AKG) on cardiomyocyte hypertrophy was also detected by Western blot and phalloidin staining assay, respectively. ResultsIDH3A was significantly decreased in the myocardium of HF patients, in the myocardium of TAC-operated mice, and in PE-induced NRVCs (P = 0.005 2,P = 0.026 6,P = 0.041 3 and P = 0.006 6, respectively). Overexpression of IDH3A markedly suppressed the expression of hypertrophy-related genes and the increase of cell size of PE-induced NRVCs (P < 0.000 1, P = 0.000 1 and P = 0.000 2, respectively). The ATP and ROS analysis indicated that IDH3A inhibited the increases of ATP and ROS levels in PE-induced NRVCs (P = 0.001 2 and P<0.000 1, respectively), whereas the enzymatically inactive IDH3A mutant lacked this effect. Exogenous AKG provision could, but overexpression of IDH3A mutant failed to suppress PE-induced NRVCs hypertrophy. ConclusionIDH3A inhibits cardiomyocyte hypertrophy via elevating AKG level, providing scientific evidence for study on IDH3A-based treatment of cardiac hypertrophy.
3.Effect of Modified Simiaosan on miR-223-3p and NLRP3/IL-1β Signaling Pathway in Rats with Acute Gouty Arthritis

ObjectiveTo investigate the effect of modified Simiaosan on miR-223-3p and NOD-like receptor thermal protein domain associated protein 3 (NLRP3)/interleukin-1β (IL-1β) signaling pathway in rat model with acute gouty arthritis (AGA) and explore the anti-inflammatory mechanism of modified Simiaosan on AGA. MethodA total of 72 8-week-old male SD rats were selected. They were divided into blank group, model group, colchicine group (0.3 mg·kg-1), high-dose modified Simiaosan group (31.75 g·kg-1), medium-dose modified Simiaosan group (15.75 g·kg-1), and low-dose modified Simiaosan group (7.875 g·kg-1) according to random number table method, with 12 rats in each group. Except for the blank group, monosodium urate (MSU) crystal suspension was injected into the right ankle joint of rats by the Coderre method in other groups to replicate the rat model with AGA. The drug administration groups were given the corresponding drug solution by gavage, and the model group and the blank group were given an equal volume of sterile sodium chloride solution by gavage for one week. The circumference of the rats' ankle joint was measured, and the swelling degree of the ankle joint was calculated. Hematoxylin-eosin (HE) staining was used to detect the pathological morphological changes in the synovial tissue of the ankle joint. The levels of IL-1β, tumor necrosis factor-α (TNF-α), and interleukin-6 (IL-6) in the serum of rats in each group were detected by enzyme-linked immunosorbent assay (ELISA). Western blot was used to detect the protein expressions of NLRP3, Caspase-1, and apoptosis-related spot-like protein (ASC) in synovial tissue of rats in each group, and real-time fluorescence quantitative polymerase chain reaction (Real-time PCR) was used to detect the mRNA expressions of NLRP3, Caspase-1, and ASC and the expression of miR-223-3p in synovial tissue of rats. ResultCompared with that in the normal group, the swelling degree of the ankle joint in the model group was higher (P<0.01), and the synovial tissue structure was disordered. Synovial cells proliferated obviously, and a large number of inflammatory cells were infiltrated. The levels of IL-1β, IL-6, and TNF-α in the serum of the model group increased significantly (P<0.01), and the protein and mRNA expressions of NLRP3, Caspase-1, and ASC increased, while expression of miR-223-3 decreased. Compared with the model group, the swelling degree of ankle joint in the colchicine group and high-dose and medium-dose modified Simiaosan groups was lower (P<0.05). Synovial cell proliferation and inflammatory cell infiltration of the colchicine group and high-dose, medium-dose, and low-dose modified Simiaosan groups were reduced to varying degrees, among which the colchicine group and high-dose modified Simiaosan group improved most obviously. The levels of IL-1β, IL-6, and TNF-α in the serum of rats in different dose groups of modified Simiaosan and colchicine group decreased significantly (P<0.01), while the protein and mRNA expressions of NLRP3, Caspase-1, and ASC increased (P<0.01). The expression of miR-223-3p in synovial tissue of the medium-dose and high-dose modified Simiaosan groups and colchicine group increased significantly (P<0.01). Compared with those in the colchicine group, the levels of IL-1β, IL-6, and TNF-α in the low-dose modified Simiaosan group increased greatly (P<0.01). In the medium-dose modified Simiaosan group, the protein expressions of NLRP3, Caspase-1, and ASC increased, and the expression of miR-223-3p decreased (P<0.05). In the low-dose modified Simiaosan group, the levels of IL-1β, IL-6, and TNF-α increased greatly (P<0.01), as well as the protein and mRNA expressions of NLRP3, Caspase-1, and ASC, while the expression of miR-223-3p was decreased (P<0.01). ConclusionModified Simiaosan may play an anti-inflammatory role by intervening in the NLRP3/IL-1β signaling pathway via regulating miR-223-3p.
4.Summary of the best evidence on health education for patients with temporary enterostomies

Objective To systematically retrieve and summarize high-quality evidence on health education for patients with temporary enterostomies to provide an evidence-based basis for clinical practice.Methods We searched UpToDate,BMJ Best Practice,AHRQ,NICE,RNAO,MEDLIVE,WCET,UOAA,NSWOCC,NHMRC,CAN,CNKI,Wanfang,VIP,CBM,PubMed,Web of Science,CINAHL,JBI,and Cochrane Collaboration databases of evidence related to health education for patients with temporary enterostomies,including clinical decisions,guidelines,expert consensuses,evidence summaries,and systematic reviews.The search time limit was from January 1,2013 to August 1,2023.2 researchers who were trained in systematic evidence-based knowledge independently conducted literature screening,quality assessment,extraction and summarization of evidence based on inclusion and exclusion criteria.Results A total of 16 articles were included,including 5 guidelines,2 clinical decisions,4 evidence summaries,2 systematic reviews,2 expert consensuses,and 1 standard.The 24 pieces of the best evidences were summarized in 7 aspects:qualification of health educators,subjects of health education,opportunity of health education,mode and evaluation index of health education,preoperative health education,postoperative health education and home follow-up management.Conclusion The best evidence of health education for patients with temporary enterostomies summarized in this study is scientific and practical,providing evidence-based basis for healthcare professionals to implement health education for patients with temporary enterostomies.
5.Expert consensus on subcutaneous injection nursing for allergic asthma in children

Objective To develop an expert consensus on subcutaneous injection nursing for allergic asthma in children,standardize nursing practice to reduce the occurrence of related adverse reactions.Methods The clinical guideline,expert consensus,systematic review,evidence summary and original research on subcutaneous injection of monoclonal antibody drug for children with allergic asthma were comprehensively searched in domestic and foreign databases.The time limit for retrieval was from the establishment of databases until August 2023.Combined with clinical practice experience,the first draft of the consensus was formed.From December 2023 to February 2024,27 experts were invited to conduct 2 rounds of expert letter consultation,revise and improve the contents of the first draft,and expert demonstration was conducted,and finally a consensus final draft was formed.Results The effective recovery rate of the 2 rounds of letter consultation questionnaires was 100%;the authority coefficient of experts was 0.88;the judging basis coefficient was 0.93;the familiarity coefficient was 0.83.In the 2 rounds of correspondence,the Kendall concordant coefficients of expert opinions were 0.241 and 0.252,respectively(P<0.001 for both).The consensus includes 6 parts,including personnel management,environmental layout,indications and contraindications,subcutaneous injection operation norms,identification and treatment of adverse reactions,and health education.Conclusion The consensus is strongly scientific and practical,and can provide guidance for nursing practice of subcutaneous injection of monoclonal antibodies in children with allergic asthma.

The Chinese Neonatal Network(CHNN) was established in 2018 with the mission of establishing a national collaboration platform, conducting high-quality and collaborative research, and ultimately improving the quality of neonatal-perinatal care and health in China.At present, 112 hospitals across the country have joined CHNN.CHNN has established a national standardized cohort of very premature infants/very low birth weight infants with >10 000 enrollments each year, has been leading data-driven collaborative quality improvement initiatives, conducting multicenter clinical studies, and performing multi-level training programs.Guided by the principles of collaboration and sharing, data-driven, continuous improvement, and international integration, CHNN has become an important platform for clinical and research collaboration in neonatal medicine in China.
7.Mediating effects of disease perception on disease uncertainty and health behaviors among thrombolysis therapy in patients with acute ischemic stroke

Objective:To explore the mediating effects of disease perception on disease uncertainty and health behaviors among thrombolysis therapy in patients with acute ischemic stroke, to provide a theoretical basis for improving patient health behavior.Methods:By cross-sectional survey methods, from June 2021 to June 2022, a total of 123 thrombolysis therapy in patients with acute ischemic stroke were investigated by general information, Mishel Uncertainty in Illness Scale, Health Behavior Scale for Stroke Patients and Revised Illness Perception Questionnaire by convenience sampling method. The mediating effect of disease perception between disease uncertainty and health behavior was verified by structural equation model.Results:The scores of disease uncertainty, health behaviors and disease perception were (52.21 ± 7.67), (63.77 ± 9.40), (127.70 ± 26.19) points in patients with acute ischemic stroke. Health behaviors and each dimension including exercise, medication, instruction, nutrition, responsibility, tobacco and alcohol were negatively correlation between disease uncertainty ( r values were -0.762 to -0.411, all P<0.01), and positively correlation between disease perception ( r values were 0.689 to 0.912, all P<0.01). Disease uncertainty was a significantly negative predictor for health behaviors ( β= -0.71, P<0.01); disease perception was also a significantly negative predictor for disease perception ( β = -0.85, P<0.01). Disease uncertainty had a significantly indirect effect on the health behaviors for thrombolysis therapy in patients with acute ischemic stroke through disease perception, which showed the mediating effect of health behaviors accounts for 15.2% of total effect. Conclusions:Disease perception play a mediating role in the relationship between disease uncertainty and health behaviors among thrombolysis therapy in patients with acute ischemic stroke.
8.Clinical study of radiation exposure from atrial fibrillation catheter ablation guided by magnetic navigation system and cryoballoon

Objective:To identify the method to reduce X-ray exposure during ablation of atrial fibrillation (AF) by comparing the cryoballoon (CRYO) ablation and remote magnetic navigation (RMN) ablation.Methods:A retrospective analysis was conducted on 144 patients undergoing CRYO ablation (CRYO group) and 121 patients undergoing RMN ablation (RMN group) in our hospital. Entrance surface doses at reference points online, exposure time during procedure and outcomes were analyzed for different types of patients.Results:Compared with the RMN group, the procedure time for the CRYO group significantly decreased [(165.0±23.6), (97.8±18.4) min, t=26.05, P<0.001]. However, the entrance surface dose value [(232.3±130.7), (669.0±387.5) mGy, Z=-12.29, P<0.001] and X-ray exposure time [(8.1±3.1), (23.4±6.2) min, t=-24.57, P<0.001] increased significantly for the CRYO group. No significant difference was found between the two groups in the proportion of maintaining sinus rhythm during follow-up of patients (71.9%, 75.7%, P=0.618). Multiple regression analysis showed that obese patients, patients with non-paroxysmal AF and patients with variant pulmonary veins were associated with an increase in entrance surface dose values in the CRYO group ( t=5.47, 2.23, 3.39, P<0.05). The X-ray exposure time for the three types patients above in the CRYO group also increased ( t=2.87, 3.86, 3.25, P<0.05) in the CRYO group. However, only obese patients in the RMN group had an increase in entrance surface dose value ( Z=-4.15, P<0.001) and no increase in exposure time. For the three types of patients above, there was no significant difference in proportion of maintaining sinus rhythm between the CRYO group and the RMN group during follow-up ( P>0.05). Conclusions:Compared with RMN ablation, the radiation exposure of CRYO AF ablation significantly increased, especially in obese patients, patients with non-paroxysmal AF and patients with pulmonary veins variation. The use of RMN for these types of patients may reduce the radiation exposure without affecting the procedure outcomes.
9.Mechanism of Qingre Lishi Huazhuo Method(Modified Simiaosan) on Chronic Gouty Arthritis of Dampness-heat Obstruction Syndrome Based on NLRP3/IL-1β Signaling Pathway

ObjectiveTo study the clinical efficacy of the Qingre Lishi Huazhuo method on patients with chronic gouty arthritis of dampness-heat obstruction syndrome and the effect on nucleotide-binding oligomerization domain-like receptor 3(NLRP3)/interleukin-1β (IL-1β) signaling pathway to preliminarily explore its mechanism. MethodSixty patients with chronic gouty arthritis of dampness-heat obstruction syndrome were enrolled and divided into a treatment group (30 cases) and a control group (30 cases) according to the random number table method. Thirty people were assigned to the healthy group. Patients in the control group were treated with oral Febuxostat, while those in the treatment group were treated with modified Simiaosan combined with Febuxostat. Treatment lasted four weeks. The general clinical data, traditional Chinese medicine (TCM) syndrome scores, serum uric acid (UA), serum creatinine (SCr), blood urea nitrogen (BUN), fasting blood glucose (FPG), low-density lipoprotein (LDL), triglyceride (TG), total cholesterol (TC), erythrocyte sedimentation rate (ESR), and C-reactive protein (CRP) of patients were recorded. Enzyme-linked immunosorbent assay (ELISA) was employed to measure the levels of IL-1β,TNF-α, and IL-6,and the levels of NLRP3,cysteinyl aspartate-specific protease-1 (Caspase-1), and apoptosis-associated speck-like protein containing a CARD (ASC) were detected by Western blot. ResultBefore treatment, the levels of body mass index (BMI), systolic blood pressure (SBP),diastolic blood pressure (DBP),UA,SCr,BUN,FPG,LDL,TG,and TC in both groups significantly increased (P<0.05,P<0.01),and the levels of HDL significantly decreased as compared with those in the healthy group(P<0.05). Additionally, the levels of IL-1β, TNF-α, and IL-6 in both groups significantly increased before treatment (P<0.01). Compared with the results before treatment, patients in the two groups had significant reductions in tube pain, joint tenderness, joint swelling,joint fever, activity disorders, body fatigue, sliminess, bitter mouth, yellow and red urine, and tongue manifestation scores (P<0.05,P<0.01). Compared with patients in the control group after treatment, those in the treatment group had a significant decrease in joint fever, body fatigue, sliminess, bitter mouth,sticky stool,yellow and red urine, tongue manifestation score, and pulse score (P<0.05). The total effective rate in the treatment group was 80.0% (24/30), higher than 56.7% (17/30)in the control group(χ2=11.916,P<0.05). Compared with the results before treatment, BMI, SBP, DBP, UA, SCr, BUN, FPG, LDL, TG, TC, ESR,CRP, IL-1β, TNF-α, IL-6 levels, and VAS score in both groups significantly decreased (P<0.05,P<0.01). Compared with patients in the control group after treatment, those in the treatment group had decreased DBP,ESR, IL-1β levels, and VAS score (P<0.05). Western blot results showed that before treatment, the protein expression of NLRP3, Caspase-1, and ASC in peripheral blood mononuclear cells (PBMCs) of patients in both groups were higher than those in the healthy group (P<0.01). Compared with the results before treatment, the protein expression of NLRP3, Caspase-1, and ASC in PBMCs in patients of both groups after treatment decreased (P<0.05,P<0.01). Compared with the control group after treatment, the treatment group showed decreased expression levels of NLRP3 and Caspase-1(P<0.05). ConclusionThe Qingre Lishi Huazhuo method can effectively improve the clinical symptoms and reduce inflammation of chronic gouty arthritis of dampness-heat obstruction syndrome with good safety. The mechanism may be related to the inhibition of the NLRP3/IL-1β signaling pathway.
10.A Chromosome-level Genome Assembly of Wild Castor Provides New Insights into Its Adaptive Evolution in Tropical Desert

Wild castor grows in the high-altitude tropical desert of the African Plateau,a region known for high ultraviolet radiation,strong light,and extremely dry condition.To investigate the potential genetic basis of adaptation to both highland and tropical deserts,we generated a chromosome-level genome sequence assembly of the wild castor accession WT05,with a genome size of 316 Mb,a scaffold N50 of 31.93 Mb,and a contig N50 of 8.96 Mb,respectively.Compared with cultivated castor and other Euphorbiaceae species,the wild castor exhibits positive selection and gene family expansion for genes involved in DNA repair,photosynthesis,and abiotic stress responses.Genetic variations associated with positive selection were identified in several key genes,such as LIG1,DDB2,and RECGI,involved in nucleotide excision repair.Moreover,a study of genomic diversity among wild and cultivated accessions revealed genomic regions containing selection signatures associated with the adaptation to extreme environments.The identification of the genes and alleles with selection signatures provides insights into the genetic mechanisms under-lying the adaptation of wild castor to the high-altitude tropical desert and would facilitate direct improvement of modern castor varieties.
