1.Inhibitory Effects of the Slit Guidance Ligand 1-3’ Untranslated Region on the Fibrotic Phenotype of Cardiac Fibroblasts

		                        			ObjectiveTo study the regulatory effect of the partial sequence within the 3’ untranslated region (3’UTR) of slit guidance ligand 1 (Slit1) (Slit1-3’UTR) on the fibrotic phenotypes of cardiac fibroblasts (CFs) and its potential mechanism. MethodsThe adenovirus vector was used to overexpress the 1526nt sequence of Slit1-3’UTR in ICR neonatal mouse CFs (mCFs). The expression of fibrosis-related genes in mCFs, such as collagen type 1 alpha1(COL1A1), collagen type 3 alpha3 (COL3A1) and alpha smooth muscle actin (α-SMA) were detected by Western blot assay. The effect of Slit1-3’UTR 1526nt on the proliferation and migration of mCFs was assessed by EdU staining and Trans-well assays. Angiotensin Ⅱ (Ang Ⅱ) was used to treat mCFs, and the impact of Slit1-3’UTR 1526nt on the fibrotic phenotypes of Ang Ⅱ-induced mCFs was evaluated. After overexpression of Slit1-3’UTR 1526nt, miR-34a-5p mimic was transfected into mCFs, followed by actinomycin D treatment to detect the mRNA stability of Slit1-3’UTR 1526nt, and the levels of miR-34a-5p and its target gene SIRT1(si-SIRT1) in mCFs were determined. The effects of miR-34a-5p and small interfering RNA targeting SIRT1 on the Slit1-3’UTR 1526nt-mediated regulation of fibrotic phenotypes were also determined. ResultsAdenovirus-mediated overexpression of Slit 1-3’UTR 1526nt was achieved in mCFs. Overexpression of Slit 1-3’UTR 1526nt markedly inhibited the expression of the fibrosis-related genes, proliferation and migration of mCFs and fibrotic phenotypes of Ang Ⅱ. The results of actinomycin D assay showed that miR-34a-5p inhibited the stability of Slit1-3’UTR 1526nt in mCFs, while the level of miR-34a-5p was reduced in mCFs with overexpression of Slit1-3’UTR 1526nt. Transfection of miR-34a-5p promoted the fibrotic phenotypes, and reversed the inhibitory effect of Slit1-3’UTR 1526nt on the fibrotic phenotypes of mCFs. Overexpression of Slit1-3’UTR 1526nt significantly increased the level of miR-34a-5p target gene SIRT1 in mCFs. Transfection of miR-34a-5p and si-SIRT1 consistently reversed the inhibitory effects of Slit1-3’UTR 1526nt on the fibrotic phenotypes of mCFs. ConclusionSlit1-3’UTR1526nt inhibits the fibrotic phenotypes of mCFs by binding to miR-34a-5p and increasing the expression of its target gene of SIRT1. 
		                        		
		                        		
		                        		
		                        	
2.CircSLC8A1_005 Inhibits the Fibrotic Phenotype of Cardiac Fibroblasts by Encoding Protein

		                        			ObjectiveTo investigate the effect of circSLC8A1_005 on the fibrotic phenotype of cardiac fibroblasts and the potential mechanism involved. MethodsThe effect of adenovirus-mediated overexpression of circSLC8A1_005 on the expression of fibrosis-related genes, collagen type I alpha 1 chain (Col1a1), collagen type Ⅲ alpha 1 chain (Col3a1) and smooth muscle actin alpha 2 (Acta2), in mouse cardiac fibroblasts (mCFs) were detected. The proliferation and migration activities of mCFs were detected by EdU and wound-healing assay, respectively. Dual luciferase reporter gene assay was performed to detect the activity of potential internal ribozyme entry site (IRES) in circSLC8A1_005. CircSLC8A1_005-translated protein, SLC8A1-605aa, and its intracellular distribution was identified by Western blot assay. The effect of SLC8A1-605aa protein on transcription activity of Sod2 gene was detected by the dual luciferase reporter gene assay. RNA binding protein immunoprecipitation (RIP) was utilized to verify the interaction between SLC8A1-605aa and superoxide dismutase 2 (Sod2) mRNA. Actinomycin D treatment was used to detect the effect of SLC8A1-605aa on Sod2 mRNA stability in mCFs. ResultsAn efficient adenovirus-mediated overexpression of circSLC8A1_005 was achieved in mCFs. The enforced expression of circSLC8A1_005 suppressed proliferation and migration of mCFs, and inhibited the expression of fibrosis-related genes in mCFs. The dual luciferase reporter gene assay revealed the activities of 2 IRES in circSLC8A1_005. Results of Western blot assay showed that circSLC8A1_005 could translate protein SLC8A1-605aa with the prospected molecular weight of 70 ku, which is predominantly distributed in the nucleus. Overexpression of the circSLC8A1_005 and SLC8A1-605aa could consistently inhibit the fibrotic phenotype of mCFs. SLC8A1-605aa could up-regulate superoxide dismutase 2 (Sod2) expression, but not at the transcriptional level. RIP assay indicated that SLC8A1-605aa could specifically interact with Sod2 mRNA, and the results of actinomycin D assay showed that SLC8A1-605aa could enhance the stability of Sod2 mRNA in mCFs. ConclusionCircSLC8A1_005 inhibits the fibrotic phenotype of cardiac fibroblasts via translating SLC8A1-605aa protein, and SLC8A1-605aa may be a potential target for the treatment of myocardial fibrosis. 
		                        		
		                        		
		                        		
		                        	
3.One case of severe cardiac arrhythmias caused by Huoxiang Zhengqi liquid

		                        			A 22-year-old male self-administered Huoxiang Zhengqi liquid(10 mL,bid)orally due to abdominal pain and diarrhea.The symptoms improved significantly on the same day.Palpitations occurred about 30 minutes after 10 mL Huoxiang Zhengqi Liuquid administration in the morning and afternoon of the next day and in the morning of the third day,respectively,which was spontaneously relieved.Electrocardiogram showed that multiple arrhythmias alternated,including disordered atrial rhythm,paroxysmal atrial fibrillation,paroxysmal supraventricular tachycardia,frequent premature ventricular contractions,junctional premature contractions,atrioventricular block,and sinus bradycardia.After admission,the patient was given electrocardiography monitoring,polarizing fluid,verapamil injection,and other treatment methods to relieve symptoms,after 12 days of treatment,the patient's condition improved,and the electrocardiogram showed sinus rhythm.This case suggests that as an over-the-counter drug,the safety of Huoxiang Zhengqi liquid should be paid more attention by clinic and public.If patients manifest palpitation symptoms while using Huoxiang Zhengqi liquid,it is imperative to consider the potential for drug-induced serious arrhythmia and get medical treatment promptly.
		                        		
		                        		
		                        		
		                        	
4.Summary of the best evidence on health education for patients with temporary enterostomies

		                        			Objective To systematically retrieve and summarize high-quality evidence on health education for patients with temporary enterostomies to provide an evidence-based basis for clinical practice.Methods We searched UpToDate,BMJ Best Practice,AHRQ,NICE,RNAO,MEDLIVE,WCET,UOAA,NSWOCC,NHMRC,CAN,CNKI,Wanfang,VIP,CBM,PubMed,Web of Science,CINAHL,JBI,and Cochrane Collaboration databases of evidence related to health education for patients with temporary enterostomies,including clinical decisions,guidelines,expert consensuses,evidence summaries,and systematic reviews.The search time limit was from January 1,2013 to August 1,2023.2 researchers who were trained in systematic evidence-based knowledge independently conducted literature screening,quality assessment,extraction and summarization of evidence based on inclusion and exclusion criteria.Results A total of 16 articles were included,including 5 guidelines,2 clinical decisions,4 evidence summaries,2 systematic reviews,2 expert consensuses,and 1 standard.The 24 pieces of the best evidences were summarized in 7 aspects:qualification of health educators,subjects of health education,opportunity of health education,mode and evaluation index of health education,preoperative health education,postoperative health education and home follow-up management.Conclusion The best evidence of health education for patients with temporary enterostomies summarized in this study is scientific and practical,providing evidence-based basis for healthcare professionals to implement health education for patients with temporary enterostomies.
		                        		
		                        		
		                        		
		                        	
5.Expert consensus on subcutaneous injection nursing for allergic asthma in children

		                        			Objective To develop an expert consensus on subcutaneous injection nursing for allergic asthma in children,standardize nursing practice to reduce the occurrence of related adverse reactions.Methods The clinical guideline,expert consensus,systematic review,evidence summary and original research on subcutaneous injection of monoclonal antibody drug for children with allergic asthma were comprehensively searched in domestic and foreign databases.The time limit for retrieval was from the establishment of databases until August 2023.Combined with clinical practice experience,the first draft of the consensus was formed.From December 2023 to February 2024,27 experts were invited to conduct 2 rounds of expert letter consultation,revise and improve the contents of the first draft,and expert demonstration was conducted,and finally a consensus final draft was formed.Results The effective recovery rate of the 2 rounds of letter consultation questionnaires was 100%;the authority coefficient of experts was 0.88;the judging basis coefficient was 0.93;the familiarity coefficient was 0.83.In the 2 rounds of correspondence,the Kendall concordant coefficients of expert opinions were 0.241 and 0.252,respectively(P<0.001 for both).The consensus includes 6 parts,including personnel management,environmental layout,indications and contraindications,subcutaneous injection operation norms,identification and treatment of adverse reactions,and health education.Conclusion The consensus is strongly scientific and practical,and can provide guidance for nursing practice of subcutaneous injection of monoclonal antibodies in children with allergic asthma.
		                        		
		                        		
		                        		
		                        	
6.Effect of Modified Simiaosan on miR-223-3p and NLRP3/IL-1β Signaling Pathway in Rats with Acute Gouty Arthritis

		                        			ObjectiveTo investigate the effect of modified Simiaosan on miR-223-3p and NOD-like receptor thermal protein domain associated protein 3 (NLRP3)/interleukin-1β (IL-1β) signaling pathway in rat model with acute gouty arthritis (AGA) and explore the anti-inflammatory mechanism of modified Simiaosan on AGA. MethodA total of 72 8-week-old male SD rats were selected. They were divided into blank group, model group, colchicine group (0.3 mg·kg-1), high-dose modified Simiaosan group (31.75 g·kg-1), medium-dose modified Simiaosan group (15.75 g·kg-1), and low-dose modified Simiaosan group (7.875 g·kg-1) according to random number table method, with 12 rats in each group. Except for the blank group, monosodium urate (MSU) crystal suspension was injected into the right ankle joint of rats by the Coderre method in other groups to replicate the rat model with AGA. The drug administration groups were given the corresponding drug solution by gavage, and the model group and the blank group were given an equal volume of sterile sodium chloride solution by gavage for one week. The circumference of the rats' ankle joint was measured, and the swelling degree of the ankle joint was calculated. Hematoxylin-eosin (HE) staining was used to detect the pathological morphological changes in the synovial tissue of the ankle joint. The levels of IL-1β, tumor necrosis factor-α (TNF-α), and interleukin-6 (IL-6) in the serum of rats in each group were detected by enzyme-linked immunosorbent assay (ELISA). Western blot was used to detect the protein expressions of NLRP3, Caspase-1, and apoptosis-related spot-like protein (ASC) in synovial tissue of rats in each group, and real-time fluorescence quantitative polymerase chain reaction (Real-time PCR) was used to detect the mRNA expressions of NLRP3, Caspase-1, and ASC and the expression of miR-223-3p in synovial tissue of rats. ResultCompared with that in the normal group, the swelling degree of the ankle joint in the model group was higher (P<0.01), and the synovial tissue structure was disordered. Synovial cells proliferated obviously, and a large number of inflammatory cells were infiltrated. The levels of IL-1β, IL-6, and TNF-α in the serum of the model group increased significantly (P<0.01), and the protein and mRNA expressions of NLRP3, Caspase-1, and ASC increased, while expression of miR-223-3 decreased. Compared with the model group, the swelling degree of ankle joint in the colchicine group and high-dose and medium-dose modified Simiaosan groups was lower (P<0.05). Synovial cell proliferation and inflammatory cell infiltration of the colchicine group and high-dose, medium-dose, and low-dose modified Simiaosan groups were reduced to varying degrees, among which the colchicine group and high-dose modified Simiaosan group improved most obviously. The levels of IL-1β, IL-6, and TNF-α in the serum of rats in different dose groups of modified Simiaosan and colchicine group decreased significantly (P<0.01), while the protein and mRNA expressions of NLRP3, Caspase-1, and ASC increased (P<0.01). The expression of miR-223-3p in synovial tissue of the medium-dose and high-dose modified Simiaosan groups and colchicine group increased significantly (P<0.01). Compared with those in the colchicine group, the levels of IL-1β, IL-6, and TNF-α in the low-dose modified Simiaosan group increased greatly (P<0.01). In the medium-dose modified Simiaosan group, the protein expressions of NLRP3, Caspase-1, and ASC increased, and the expression of miR-223-3p decreased (P<0.05). In the low-dose modified Simiaosan group, the levels of IL-1β, IL-6, and TNF-α increased greatly (P<0.01), as well as the protein and mRNA expressions of NLRP3, Caspase-1, and ASC, while the expression of miR-223-3p was decreased (P<0.01). 7.Characteristics and changes of cardiac injury with age in children of Duchenne muscular dystrophy: a prospective cohort study

		                        			Objective:To explore the characteristics and changes of cardiac injury with age in Duchenne muscular dystrophy (DMD) and its clinical significance.Methods:A prospective cohort study was conducted. The 215 patients diagnosed with DMD in West China Second Hospital from January 2019 to November 2022 and aged from 6 to 18 years were enrolled. Their clinical data, myocardial injury markers, routine electrocardiogram, cardiac magnetic resonance (CMR) and echocardiography were collected. The patients were divided into five age groups: 6-<8, 8-<10, 10-<12, 12-<14 and 14-18 years of age, and matched with healthy boys respectively. Independent sample t test or Mann-Whitney U test was used to compare the clinical data and CMR indexes between DMD patients and controls in all age subgroups, and to compare the value of left ventricular ejection fraction (LVEF) measured by echocardiography and CMR in each subgroup of DMD patitents. Pearson correlation analysis or Spearman correlation analysis was used to explore the relation between the CMR indexes and age in DMD patients. Results:A total of 215 patients with DMD (all male) and 122 healthy boys were included in the study. There were 75 DMD patients and 23 controls in 6-<8 years of age group, 77 DMD and 28 controls in 8-<10 years of age group, 39 DMD and 23 controls in 10-<12 years of age group, 10 DMD and 31 controls in the 12-<14 years of age group, and 14 DMD and 17 controls in 14-18 years of age group. In the DMD patients, the older the age, the lower the levels of creatine kinase (CK) and creatine kinase isoenzyme (CK-MB). In the 6-<8 years of age group, the CK level was 10 760 (7 800, 15 757) U/L, while in the group of 14-18 years of age, it was 2 369 (1 480, 6 944) U/L. As for CK-MB, it was (189±17) μg/L in the 6-<8 years of age group and (62±16) μg/L in the 14-18 years of age group. Cardiac troponin I remained unchanged in <12 years of age groups, but significantly increased in 12-<14 years of age group, reaching the highest value of 0.112 (0.006, 0.085) μg/L. In the DMD patients, the older the age, the higher the proportion of abnormal electrocardiogram (ECG). In the 6-<8 years of age group, the proportion is 29.3% (22/75), while in the 14-18 years of age group, it was 10/14. Correlation analysis showed that the left ventricular end-diastolic volume index was positively related with age ( r=0.18, P=0.015), and the left ventricular stroke volume index and cardiac output index were negatively related with age ( r=-0.34 and -0.31, respectively, both P<0.001). In the DMD patients, the older the age, the lower LVEF, with the LVEF decreasing to (49.3±3.1)% in the 14-18 years of age group. The LVEF of DMD cases was significantly lower than that of controls in the age subgroups of 8-<10, 10-<12, 12-<14 and 14-18 years of age groups ((57.9±5.2) % vs. (63.6±0.8)%, 60.7% (55.9%, 61.9%) vs. 63.7% (60.2%, 66.0%), 57.1% (51.8%, 63.4%) vs. 62.1 % (59.5%, 64.5)%, (49.3±3.1) % vs. (61.6±1.3)%, respectively; all P<0.01). In the DMD patients, the older the age, the higher the proportion of positive late gadolinium enhancement (LGE). In the 6-<8 years of age group, it was 22% (11/51), in the 12-<14 years of age group, it was 13/14, and in the 14-18 years of age group, all DMD showed positive LGE. The value of LVEF of DMD cases measured by echocardiography was significantly higher than that measured by CMR in 6-<8 years of age group and 8-<10 years of age group (63.2% (60.1%, 66.4%) vs. 59.1 % (55.4%, 62.9%), and (62.8±5.2) % vs. (57.9±5.2)%, all P<0.001). Conclusion:DMD patients develop cardiac injury in the early stage of the disease, and the incidence of cardiac damage gradually increases with both age and the progression of disease.
		                        		
		                        		
		                        		
		                        	
8.Desmoid fibromatosis of the breast:clinicopathological analysis of twenty-six cases and review of literatures

		                        			Purpose To investigate the clinicopathological features,diagnosis,differential diagnosis and molecular charac-teristics desmoid fibromatosis the breast(DFB).Methods The clinicopathological data and prognostic information of 26 pa-tients with desmoid fibromatosis of the breast were collected.Their clinical characteristics,histological morphologies,immu-nophenotypes and molecular characteristics were analyzed.Re-sults All the 26 patients were female,with a median age of 34.5 years and an average age of 36.8 years(range from 13 to 69 years).There were 10 cases in left breast,14 cases in right breast and 2 cases in bilateral breast.Isolated and painless breast masses were found in all patients,and 3 cases were ac-companied by nipple depression.Grossly,most of them showed a poorly circumscribed tan-white to gray mass with a trabeculated appearance.Microscopically,all the tumors were composed of mild spindle cells with varying intervals of collagen fiber bun-dles.The boundary between the tumor and the surrounding breast tissue was not clear,and the tumor cells infiltrated adja-cent tissues,such as breast ducts,lobules,adipose tissue,and skeletal muscle.Uniform spindle or oval cells formed in fascicles and interwoven,without atypical or polymorphism;the nuclear chromatin was sparse or vacuolar,with small nucleoli,but mito-ses were rare or absent.Nuclear accumulation of β-catenin was present in 20 of 26 tumors,SMA was variously positive in 20 of 26,desmin was focally positive in 6 of 26;CKpan,CK5/6,p63,CD34,CD10 and S-100 were all negative.The Ki67 index was 5%-10%.The gene mutations of CTNNB1 exon 3 were found in 18 of 26 by Sanger sequencing,including T41A(83.3%),S45P(11.1%)and S45F(5.6%).Two patients also had familial adenomatous polyposis.Local resection was performed in 23 cases,mastectomy was performed in 2 cases,and one case did not require any additional treatment after core needle biopsy.20 cases were followed up for 1-108 months,and no recurrence occurred.Conclusion Desmoid fibromatosis of the breast is rare,and can mimic breast cancer clinically,ra-diologically and histologically.It should be always considered in differential diagnosis for the spindle cell proliferations of the breast.A diagnosis of DFB can be achieved basing on the typi-cal histopathology,immunohistochemistry,history and gene se-quencing.
		                        		
		                        		
		                        		
		                        	
9.Mechanism of Qingre Lishi Huazhuo Method(Modified Simiaosan) on Chronic Gouty Arthritis of Dampness-heat Obstruction Syndrome Based on NLRP3/IL-1β Signaling Pathway

		                        			ObjectiveTo study the clinical efficacy of the Qingre Lishi Huazhuo method on patients with chronic gouty arthritis of dampness-heat obstruction syndrome and the effect on nucleotide-binding oligomerization domain-like receptor 3(NLRP3)/interleukin-1β (IL-1β) signaling pathway to preliminarily explore its mechanism. MethodSixty patients with chronic gouty arthritis of dampness-heat obstruction syndrome were enrolled and divided into a treatment group (30 cases) and a control group (30 cases) according to the random number table method. Thirty people were assigned to the healthy group. Patients in the control group were treated with oral Febuxostat, while those in the treatment group were treated with modified Simiaosan combined with Febuxostat. Treatment lasted four weeks. The general clinical data, traditional Chinese medicine (TCM) syndrome scores, serum uric acid (UA), serum creatinine (SCr), blood urea nitrogen (BUN), fasting blood glucose (FPG), low-density lipoprotein (LDL), triglyceride (TG), total cholesterol (TC), erythrocyte sedimentation rate (ESR), and C-reactive protein (CRP) of patients were recorded. Enzyme-linked immunosorbent assay (ELISA) was employed to measure the levels of IL-1β,TNF-α, and IL-6,and the levels of NLRP3,cysteinyl aspartate-specific protease-1 (Caspase-1), and apoptosis-associated speck-like protein containing a CARD (ASC) were detected by Western blot. ResultBefore treatment, the levels of body mass index (BMI), systolic blood pressure (SBP),diastolic blood pressure (DBP),UA,SCr,BUN,FPG,LDL,TG,and TC in both groups significantly increased (P<0.05,P<0.01),and the levels of HDL significantly decreased as compared with those in the healthy group(P<0.05). Additionally, the levels of IL-1β, TNF-α, and IL-6 in both groups significantly increased before treatment (P<0.01). Compared with the results before treatment, patients in the two groups had significant reductions in tube pain, joint tenderness, joint swelling,joint fever, activity disorders, body fatigue, sliminess, bitter mouth, yellow and red urine, and tongue manifestation scores (P<0.05,P<0.01). Compared with patients in the control group after treatment, those in the treatment group had a significant decrease in joint fever, body fatigue, sliminess, bitter mouth,sticky stool,yellow and red urine, tongue manifestation score, and pulse score (P<0.05). The total effective rate in the treatment group was 80.0% (24/30), higher than 56.7% (17/30)in the control group(χ2=11.916,P<0.05). Compared with the results before treatment, BMI, SBP, DBP, UA, SCr, BUN, FPG, LDL, TG, TC, ESR,CRP, IL-1β, TNF-α, IL-6 levels, and VAS score in both groups significantly decreased (P<0.05,P<0.01). Compared with patients in the control group after treatment, those in the treatment group had decreased DBP,ESR, IL-1β levels, and VAS score (P<0.05). Western blot results showed that before treatment, the protein expression of NLRP3, Caspase-1, and ASC in peripheral blood mononuclear cells (PBMCs) of patients in both groups were higher than those in the healthy group (P<0.01). Compared with the results before treatment, the protein expression of NLRP3, Caspase-1, and ASC in PBMCs in patients of both groups after treatment decreased (P<0.05,P<0.01). Compared with the control group after treatment, the treatment group showed decreased expression levels of NLRP3 and Caspase-1(P<0.05). ConclusionThe Qingre Lishi Huazhuo method can effectively improve the clinical symptoms and reduce inflammation of chronic gouty arthritis of dampness-heat obstruction syndrome with good safety.
