1.Mechanism of action of macrophage efferocytosis in acute and chronic liver diseases and related targeted therapy

Efferocytosis refers to the process by which apoptotic cells are engulfed and cleared by phagocytes, including professional phagocytes, such as macrophages and dendritic cells, and non-professional phagocytes, such as epithelial cells. Liver macrophages are the main cells with the function of efferocytosis in the liver. In recent years, an increasing number of studies have shown that various acute and chronic liver diseases are associated with the efferocytosis function of liver macrophages, including acute liver injury, alcoholic liver disease, nonalcoholic fatty liver disease, autoimmune liver disease, liver fibrosis, and liver cancer. This article elaborates on the expression of molecules associated with the efferocytosis function of macrophages, the process of efferocytosis, and the role of efferocytosis function in different liver diseases, so as to provide new ideas for the treatment of liver diseases.
2.Analysis of the causes for abnormal dissolution of lansoprazole enteric-coated tablets by multiple techniques and different dimensions

Abstract: This study aims to find out the causes for the dissolution of unqualified samples found during evaluation sampling of lansoprazole enteric-coated preparations by the laser infrared imaging system and orbitrap high resolution mass spectrometry, with suggestions for improvement. Lansoprazole enteric-coated preparations were tested by current standard, the dissolution of a batch of samples was below the limit and other items were in line with the standard. Considering that this product is unstable to acid and alkali, the following exploratory experiments were designed from different dimensions, based on the conclusion of the unannounced inspection of the company, to explore the reasons for the unqualified batch, including the influence of high-temperature and high-humidity storage environment on the key quality properties of the sample, the influence of 2-hour acid resistance test on dissolution result, the imaging of the core and the measure of coating layer thickness, the optimization of chromatographic conditions of related substances, and the analysis of the source of impurities. It was found that improper storage in circulation and poor coating process caused the low dissolution of this batch: the high-temperature and high-humidity storage environment possible in the circulation process led to the decreasing efficacy of disintegrating agent in the samples and thus the difficulty to release the active pharmaceutical ingredient fully; the coating solution could not be uniformly sprayed on the core, resulting in thin isolation layer and different thickness, which then affected the protection of the main drug against acid degradation. The above two reasons together resulted in unqualified dissolution of this batch. The overall quality of lansoprazole enteric-soluble preparation is good, but the formulation and process in some companies need to be optimized; and the temperature and humidity in the circulation process need to be controlled in strict accordance with the regulations.
3.Efficacy of the ACS NSQIP surgical risk calculator in open radical hysterectomy

Objectives To evaluate the complications predicting efficacy of the American College of Surgeons(ACS)National Surgical Quality Improvement Program(NSQIP)surgical risk calculator for cervical cancer patients undergoing open radical hysterectomy in China.Methods This study enrolled the cervical cancer patients(139 cases)undertaken open radical hysterectomy at Women's Hospital of Nanjing Medical University from Janu-ary to December in 2021.Preoperative risk factors were abstracted from medical records and the surgical risk scores were calculated using ACS NSQIP surgical risk calculator.The association between risk scores and actual outcomes were assessed using logistic regression together with the c-statistic(area under ROC)and Brier score.Results The ACSNSQIP calculator did not predict accurately for serious complications,any complications,venous thrombo-embolism(VTE),readmission,return operation room and surgical site infection(SSI)compared with actual out-comes.There was significantly difference in the predicted and actual length of stay(3.93±0.42 days vs.13.11±4.71 days,P<0.001).Conclusions The ACS NSQIP surgical risk calculator failed to predict the postoperative complications and the length of hospital stay for cervical cancer patients undergoing open radical hysterectomy.
4.Research advances in primary biliary cholangitis with dyslipidemia

Primary biliary cholangitis (PBC) is an autoimmune liver disease characterized by progressive and non-purulent inflammation of small- and medium-sized bile ducts in the liver. Recent studies have shown that abnormal lipid metabolism is relatively common in patients with PBC, and 76% of PBC patients have dyslipidemia. The effects and harms of dyslipidemia have attracted much attention. Lipid metabolism disorders play an important role in the progression of PBC. This article mainly reviews the research advances in the manifestation, role, diagnosis, and treatment of lipid metabolism disorders in PBC, so as to provide new ideas for the treatment of PBC.
5.Role of T-cell immunoglobulin and mucin domain-containing molecule 3 in the development and progression of liver diseases

T-cell immunoglobulin and mucin domain-containing molecule-3(Tim-3)is a member of the Tim family and has been a research hotspot in recent years.As a negative regulatory factor,Tim-3 exerts different effects by binding to different ligands.Tim-3 is expressed in various types of immune cells,such as natural killer cells,dendritic cells,and monocytes,and Tim-3 has a regulatory effect on the functions of these immune cells.In recent years,a large number of studies have shown that Tim-3 is closely associated with the development and progression of liver diseases.This article reviews the studies on the role and mechanism of Tim-3 in different liver diseases and cells in recent years,in order to provide richer perspectives and ideas for the clinical diagnosis and treatment of liver diseases.
6.Combining robot-assisted gait training with task-oriented training can improve the walking ability of children with dyskinetic cerebral palsy

Objective:To observe the effect of combining robot-assisted gait training (RAGT) with task-oriented training (TOT) on the walking ability of children with dyskinetic cerebral palsy (DCP).Methods:Sixty DCP children were randomly divided into a conventional intervention group, an RAGT group, and a combined intervention group, each of 20. All of the children received conventional rehabilitation therapy, while the RAGT and combined intervention groups were additionally provided with RAGT, and RAGT combined with TOT, respectively. Before the experiment and after 12 and 24 weeks of treatment, the subjects′ walking ability was evaluated using the 10-metre walk test (10MWT), and the D energy zone (standing position) and the E energy zone (walking, running and jumping) of the Gross Motor Function Measurement-88 (GMFM-88) instrument.Results:After 12 and 24 weeks of treatment, the average 10MWT speed and D and E energy zone scores of all three groups had improved significantly. After 24 weeks the combined group′s averages on all three measures were significantly better than those of the other two groups.Conclusion:RAGT combined with TOT and conventional rehabilitation training significantly improves the walking ability of DCP children.
7.Application research of micronized acellular dermal matrix transplantation

With bio-materials being widely applied in clinical practice, acellular dermal matrix (ADM), one of the most competitive techniques in tissue engineering, has been developed rapidly in recent years, gaining popularity for its advantages in clinical application. What’s more, micronized acellular dermal matrix (mADM) has received an increasing amount of attention for its unique morphological characteristics, particularly in the field of cosmetic injection. Compared with other techniques, mADM has the advantages of great safety and sustainability, with less minimal invasion. However, further practical validation is still needed to determine the most suitable media for mADM injection as well as the most appropriate ratio of media to mADM transplantation. This paper reviews the relevant research findings.
8.Clinical efficacy of A-PRF on alveolar ridge preservation:A meta analysis

Objective:To evaluate the clinical efficacy of A-PRF in the preservation of alveolar ridge after tooth extraction.Methods:Electronic databases of VIP,CNKI,Wanfang,CQVIP,Medline,Pubmed and Cochrane library were searched.The meta-analysis was performed by using Revman 5.4.Results:5 RCTs including 217 patients were identified.A-PRF can significantly reduce the vertical bone absorption of the extraction socket,can increase the amount of new bone formation in the extraction socket(P<0.05).However,A-PRF can not effectively reduce the horizontal bone absorption and can not improve the bone density in the extraction socket(P>0.05).Conclusion:A-PRF is effective in the alveolar ridge preservation after tooth extraction.
9.OCT analysis of in-stent neointima over 5 years post-DES implantation

Objective:To investigate the characteristics of neointimal hyperplasia (NIH) in patients with in-stent restenosis (ISR) over 5 years post-drug-eluting stent (DES) implantation based on optical coherence tomography (OCT).Methods:In this cross-sectional study, patients with DES-ISR who underwent OCT examination at PLA General Hospital between March 2010 and March 2022 were retrospectively included. All patients were divided into≤5 years DES-ISR group and>5 years DES-ISR group according to the time interval after DES implantation. Quantitative and qualitative analyses were conducted on OCT images to compare the clinical data and lesion characteristics of two patient groups. Furthermore, the independent clinical predictive factors of in-stent neoatherosclerosis (ISNA) were analyzed by multivariable logistic regression.Results:A total of 230 DES-ISR patients with 249 lesions were included, with an age of (63.1±10.4) years and 188 males (81.7%). The median interval after DES implantation was 6 (2, 9) years. There were 117 patients (122 ISR lesions) in the≤5 years DES-ISR group, and 113 patients (127 ISR lesions) in the>5 years DES-ISR group. Compared with≤5 years DES-ISR,>5 years DES-ISR showed more heterogeneous patterns (65.4% (83/127) vs. 48.4% (59/122), P=0.007), diffuse patterns (46.5% (59/127) vs. 31.2% (38/122), P=0.013), macrophage accumulations (44.1% (56/127) vs. 31.2% (38/122), P=0.035) in NIH and higher prevalence of ISNA (83.5% (106/127) vs. 72.1% (88/122), P=0.031). According to multivariable logistic regression, the independent predictive factor for ISNA was female ( OR=0.44, 95% CI 0.21-0.90, P=0.026). Female ( OR=0.48, 95% CI 0.23-0.99, P=0.046) and low-density lipoprotein cholesterol level ( OR=1.62, 95% CI 1.01-2.59, P=0.046) were independent predictive factors, respectively, for lipid ISNA. Calcified ISNA was independently associated with time interval of post-DES implantation ( OR=1.18, 95% CI 1.07-1.29, P=0.001). Conclusion:DES-ISR patients with a time interval of>5 years after stent implantation have a higher prevalence of ISNA and more complex lesions. Gender, the level of low-density lipoprotein cholesterol, and the time interval post-DES implantation are independently correlated with ISNA, lipid ISNA, and calcified ISNA.
