1.A study of suctioning flexible ureteroscopy with intelligent pressure-control in treating patients with urogenic sepsis after drainage at different times

【Objective】 To investigate the safety and effectiveness of suctioning flexible ureteroscopy with intelligent pressure-control at different times after drainage for patients with urogenic sepsis complicated with upper urinary tract stones. 【Methods】 Clinical data of 59 patients treated in the Department of Urology, Affiliated Hospital of Nantong University during May 2022 and May 2023 were collected.The patients were divided into early lithotripsy (≤1 week) group (n=27) and late lithotripsy (>1 week) group (n=32).Baseline data, imaging data and postoperative data of the two groups were compared. 【Results】 There were no significant differences between the two groups in the stone-free rate, total incidence of complications, incidence of high-grade complications, length of stay after lithotripsy, hospitalization costs after lithotripsy and total hospitalization costs (P>0.05). 【Conclusion】 Both early lithotripsy (<1 week) and late lithotripsy (>1 week) are safe and effective in the treatment of urogenic sepsis after drainage.

Anti-seizure medications (ASMs) are the main therapy for epilepsy.There are many kinds of ASMs with complex mechanism of action, so it is difficult for pharmacists to examine prescriptions.This paper put forward some suggestions on the indications, dosage forms/routes of administration, appropriateness of usage and dosage, combined medication and drug interaction, long-term prescription review, individual differences in pathophysiology of children, and drug selection when complicated with common epilepsy, for the reference of doctors and pharmacists.

Antipyretic-analgesics are currently one of the most prescribed drugs in children.The clinical application of antipyretic-analgesics for children in our country still have irrational phenomenon, which affects the therapeutic effect and even poses hidden dangers to the safety of children.In this paper, suggestions were put forward from the indications, dosage form/route, dosage suitability, pathophysiological characteristics of children with individual differences and drug interactions in the symptomatic treatment of febrile children, so as to provide reference for the general pharmacists when conducting prescription review.
5.Application of self-made multifunctional accessory stent in endoscopic mucosal dissection

Objective:To investigate the safety and efficacy of self-made multifunctional endoscopic instrument accessory stent in endoscopic mucosal dissection (ESD).Methods:A total of 80 patients who received ESD in 924th Hospital of Joint Logistic Support Force of Chinese People′s Liberation Army from May 2019 to February 2021 were selected as research object. Random number table method was used to divide patients into control group and experimental group, 40 cases in each group. The experimental group used self-made endoscopic instrument accessory stent, and the control group did not use self-made endoscopic instrument accessory stent. The length of operation, the number of instruments taken by mistake, the number of instruments polluted during operation and the infection of postoperative 3-7 days under the same operation position, operating doctors and nurses were analyzed and compared between the two groups.Results:The incidence of instruments taken by mistake, the incidence of instruments polluted during operation and the infection rate of postoperative 3-7 days were 0.9% (8/856), 1.4% (12/856) and 2.5% (1/40) in the experimental group, which in the control group were 10.8% (96/887), 11.8% (105/887) and 15.0% (6/40) respectively, there were significant differences between the two groups ( χ2=75.92, 75.76, 3.91, all P<0.05). The length of operation in the experimental group was (51.56 ± 2.32) min, and that in the control group was (79.02 ± 2.83) min, the difference was statistically significant ( t=-8.72, P<0.05). Conclusions:When the patients underwent ESD surgery, the self-made multifunctional accessory stent was used to place the required instrument accessory. The length of operation was shorter, the incidence of instruments taken by mistake, the incidence of instruments polluted during operation and the infection rate of postoperative 3-7 days were reduced, the quality of minimally invasive endoscopic surgery was improved.
6.New avenues for systematically inferring cell-cell communication: through single-cell transcriptomics data.

For multicellular organisms, cell-cell communication is essential to numerous biological processes. Drawing upon the latest development of single-cell RNA-sequencing (scRNA-seq), high-resolution transcriptomic data have deepened our understanding of cellular phenotype heterogeneity and composition of complex tissues, which enables systematic cell-cell communication studies at a single-cell level. We first summarize a common workflow of cell-cell communication study using scRNA-seq data, which often includes data preparation, construction of communication networks, and result validation. Two common strategies taken to uncover cell-cell communications are reviewed, e.g., physically vicinal structure-based and ligand-receptor interaction-based one. To conclude, challenges and current applications of cell-cell communication studies at a single-cell resolution are discussed in details and future perspectives are proposed.

7.A multicenter survey of the accessibility of essential medicines for children in China

Objective:To investigate the availability, prices and affordability of essential medicines in pediatric population across China, in the hope of improving rational use of medicines.Methods:A multicenter cross-sectional survey of medicine prices, availability and affordability was conducted in 17 provinces, municipalities and autonomous region across east, south-central part, west and north of China. Data on 42 medicines used in pediatric population, both original and generic, were collected in 55 public hospitals from May 26 to June 2, 2017. Availability was expressed as the percentage of hospitals with stock of the target medicine on the day of data collection,and median price ratio (MPR) was the ratio of price upon investigation to international reference. Based on national minimum daily wage, affordability represents the number of working days needed to earn the expense which covers a standard course using the target medicine. Statistical software SPSS 13.0 was applied for descriptive analysis of availability, MPR and affordability.Results:Mean Availability of original and generic medicine was 33% and 32%, with median MPR being 5.43 and 1.55. Among the 19 medicines with price information for both original and generic product, the median MPR was 7.73 and 2.04 respectively. Regarding the five medicines used to treat four common pediatric diseases (pneumonia,peptic ulcer, congenital hypothyroidism, refractory nephrotic syndrome), the affordability was 0.63 (0.16-6.17) d for generic medicine, and 1.03 (0.16-11.53) d for its original counterpart.Conclusions:The availability to both original and generic products of the 42 medicines used in pediatric population was low in China. The prices of generic medicines seem to be lower and affordability higher than those of original medicines. There is an urgent need to improve the availability and affordability of pediatric medicines.
8.Comparative study on selective hepatic vascular exclsion and total hepatic vascular exclusion for hepatic trauma with major hepatic vein injury

Objective To compare the effect of selective hepatic vascular exclsion ( SHVE) and total hepatic vascular exclusion ( THVE ) in the treatment of hepatic trauma with major hepatic vein injury. Methods A retrospective case control study was conducted to analyze the clinical data of 42 patients with hepatic trauma accompanied by hepatic vein injury admitted to multiple centers from April 2000 to December 2017. There were 30 males and 12 females, aged 14-65 years [(40. 2 ± 18. 8)years]. Blood flow exclusion was operated through HVE in 22 patients ( SHVE group ) and through THVE in 20 patients (THVE group). SHVE group included 22 patients (16 males and six females), aged (40. 1 ±19. 4)years. There were 10 patients with grade IV and 12 with grade V according to American Association of Traumatic Surgery ( AAST) classification of liver injury. In terms of the hepatic vein injury, there were 13 patients with type I, eight with type III, and one with type IV. THVE group included 20 patients (14 males and six females), aged (39.9 ±18.2)years. There were nine patients with grade IV and 11 with grade V according to AAST classification of liver injury. In terms of the hepatic vein injury, there were 11 patients with type I, seven with type III, and two with type IV. The operation approach, operation time, hepatic warm ischemia time, blocking time of hepatic vein blood flow, amount of abdominal hemorrhage, intraoperative blood loss, postoperative blood loss, intraoperative infusion, total blood transfusion, length of ICU stay after operation, length of hospital stay after operation, function of liver and kidney after operation, incidence of complications and mortality were compared between the two groups. Results There were no significant differences in the amount of abdominal hemorrhage, intraoperative blood loss, postoperative blood loss, perioperative blood transfusion, surgical procedure, and postoperative liver and kidney function between the two groups (P>0. 05). The THVE group had significantly longer operation time, hepatic warm ischemia time, hepatic venous blood flow blocking time, postoperative ICU time and postoperative hospital stay than the SHVE group (P<0. 05). The amount of infusion in the SHVE group was less than that in the THVE group (P <0. 05). The incidence of complications in SHVE group was 27% (6/22), lower than that in THVE group [60% (12/20)] (P<0. 05). The mortality of SHVE group was 14% (3/22), lower than that of THVE group [45% (9/20)] (P<0. 05). Conclusions SHVE and THVE can effectively control bleeding in the treatment of hepatic trauma with main hepatic vein injury. SHVE has more advantages over THVE in shortening operation time, warm ischemia time of liver, blocking time of hepatic vein blood flow, ICU stay after operation, hospital stay after operation and reducing intraoperative infusion volume, and can reduce the incidence of complications and mortality.
9.Glutamine enriched enteral nutrition for severe traumatic brain injury: a meta-analysis

Objective To evaluate the efficacy of Glutamine enriched enteral nutrition in the treatment of severe traumatic brain injury (sTBI).Methods PubMed,Cochrane Library,Chinese National Knowledge Infrastructure (CNKI),VIP Database for Chinese Technical Periodicals,and Wanfang databases were searched to identify randomized controlled trials (RCT) on the application of Glutamine enriched enteral nutrition for severe TBI patients from database establishment time to May 2017.Two investigators screened the literature strictly according to the inclusion and exclusion criteria,extracted the data,evaluated the literature quality,and performed meta-analysis using RevMan 5.3 software.The effects of glutamine enhanced enteral nutrition on albumin content,immunoglobulin G (IgG) level,the incidence of diarrhea,incidence of pulmonary infection,blood glucose,Glasgow Coma Scale (GCS),length of hospital stay,and mortality were evaluated.Results A total of 17 articles involving 939 sTBI patients were included,with 512 patients in Glutamine group and 427 controls in control group.There were no significant differences in the length of hospital stay and mortality between the two groups (P > 0.05).Significant differences were found in albumin content (95 % CI 0.19-2.54,Z =2.27,P<0.05),level of IgG (95% CI 0.67-1.80,Z =4.25,P <0.01),incidence of diarrhea (95% CI 0.23-0.57,Z =4.41,P < 0.01),incidence of lung infections (95% CI 0.14-0.56,Z =3.62,P<0.01),blood sugar (95% CI-2.53--0.52,Z=2.98,P<0.01),and the GCS score (95%CI0.50-2.68,Z=1.49,P<0.01) between the two groups.Conclusion Compared with routine enteral nutrition,Glutamine enriched enteral nutrition can increase albumin content and IgG level,reduce the incidence of diarrhea and lung infections,reduce blood sugar,and improve the GCS score,but it cannot shorten hospital stay or reduce mortality.
10.The effect of microRNA-224 on the anti-apoptosis and proliferation promoting of cholangiocarcinoma cells in vitro

Objective To explore the regulatory mechanism of microRNA-224 (miR-224) in the apoptosis and proliferation of human cholangiocarcinoma (CC) cells and examine whether the pathway of miR-224-HOXD10-PAK4 exist in cc cells or not.Methods The expression of miR-224 in CC and adjacent normal tissue were measured by RT-PCR.QBC939 cells were chosen and divided into two groups:cell groups transducted with negative control viruses (NC group) and cell groups transducted with target gene miRNA-down virus (DOMN group).The rate of apoptosis and the number of colonies formed in QBC939 cell lines of DOWN group and NC group were counted.The expression of miR-224,API5,PAK4 and HOXD10 in QBC939 cell lines of DOWN group and NC group were measured by RT-PCR and western blotting.Results (1) MiR-224 expression was higher in CC tissues than that in normal bile duct tissues (P < 0.05).(2) In QBC939 cells lines,miR-224 expression was lower in DOMN group than that in NC group.The clone formation of QBC939 cells in DOWN group was significantly lower than that of NC group.The apoptosis rate of QBC939 cells of DOWN group was significantly higher than that of NC group.(3) The expression of API5 and HOXD10 in QBC939 cells of DOWN group was significantly higher than that of NC group in the RT-PCR and western blotting experiments.(4) The expression of PAK4 in QBC939 cells of DOWN group was significantly lower than that of NC group in the RT-PCR and western blotting.Conclusions MiR-224 has anti-apoptotic effects and can promote the proliferation of QBC939 cells and may serve as an important regulator of QBC939 cell proliferation in vitro.There are miR-224-HOXD10-PAK4 signaling pathways in cholangiocarcinoma.
