Objective:To study the effect of bridge orbicularis oculi muscle flap blepharoptosis in the treatment of post-burn scar contracture ectropion. Methods:The clinical data of patients wih post-burn cicatricial ectropion treated with bridge orbicularis oculi muscle flap from April 2006 to September 2020 of Department of Plastic Surgery, Affiliated Hangzhou First People’s Hospital, School of Medicine, Westlake University were analyzed retrospectively. For patients with severe eyelid scar contracture caused by head and face burns, after releasing the scar and resetting the eyelid margin, orbicularis oculi muscle flaps were made at the base of the upper and lower eyelid release incision, crossed the muscle flap over the eyelid adhesion point of the eye fissure, by performed bridge cross transfer, and fixed to the orbicularis oculi muscle at the base of the release incision to form eyelid adhesion. H-shaped full-thickness skin graft or medium-thickness skin graft was performed on the defect wound during the operation. Tie-over dressing fixation in the skin graft area. One year after the operation, it was confirmed that the skin graft was stable and there was no recurrence of ectropion, and then cut the adhesion. Follow-up for more than one year after the operation, foreign body sensation, conjunctival sac pusc, skin graft survival, success rate of eyelid adhesion, anti-stretching effect of muscle flap, skin retraction rate and complications were observed. The distances between eyelid margins preoperative and after incision of eyelid adhesion were measured. SPSS 25.0 software was used for data statistics, measurement data was represented by M ( Q1, Q3), and counting data was represented by cases. Results:Fourteen patients (23 eyes) were included in this study, including 11 males and 3 females, aged 41.5 (32.5, 47.0) years, and the time from burn to this treatment was 13.5 (10, 24) months. 5 cases (7 eyes) were not treated with skin grafting or blepharoptosis after burn, and 9 cases (16 eyes) were treated with blepharoptosis and/or skin transplantation. The area of skin graft during operation was 37 mm ×18 mm to 52 mm ×39 mm. Before operation, the distance between the upper and lower eyelid margins was 14 (12, 14) mm when eyes were open, and 6 (5, 9) mm when eyes were closed. After eyelid adhesion surgery, the patient had no foreign body sensation and infection, and all the skin grafts survived. The success rate of blepharoptosis was 100%, and there was no accidental rupture or elongation. The eyelid adhesions contracted synchronously when the patients closed their eyes, and the muscle flap had good anti-stretching effect. The duration of blepharoptosis was 13(12, 24) months, during which no complications or adverse reactions occurred. The average skin graft retraction rate was 9% (5%, 10%) in half a year. After incision of the orbicularis oculi muscle flap, the distance between the upper and lower eyelid margins was 9 (8, 9) mm when the eyes were open, and 0 (0, 0) mm when the eyes were closed. Conclusion:Bridge orbicularis oculi muscle flap tarsorrhaphy can provide long-term tarsorrhaphy, effectively reduce skin contracture after eyelid skin grafting, and has a stable therapeutic effect on postburn ectropion.

Objective:To investigate the epidemic situation of drinking water-borne endemic fluorosis in Henan Province, evaluate the effectiveness of prevention and control measures, and provide a basis for formulating further prevention and control policies. Methods:From April to November 2021, a census of all diseased villages registered in 112 drinking water-borne endemic fluorosis counties (cities, districts, hereinafter referred to as counties) of 17 provincial-level cities and 9 provincial-level counties (cities) in Henan Province was conducted to collect the basic information of diseased counties and villages. The content of fluoride in drinking water and the prevalence of dental fluorosis in children aged 8 - 12 years old were measured on a village by village basis. Results:By the end of 2021, drinking water-borne endemic fluorosis was prevalent in 112 diseased counties in Henan Province, involving 17 450 diseased villages. Among them, 17 433 villages had implemented measures to reduce fluoride and improve water quality, with a water improvement rate of 99.90% (17 433/17 450). There were 14 920 villages with qualified water, and the qualified rate of water fluoride was 85.50% (14 920/17 450). There were a total of 4 232 water improvement projects in all diseased villages, with a normal operation rate of 97.09% (4 109/4 232) and a qualified rate of 93.03% (3 937/4 232) for water fluoride in the projects. The total detection rate of dental fluorosis in children in the province was 9.64% (39 391/408 584), with moderate to severe dental fluorosis accounted for 8.08% (3 182/39 391), the dental fluorosis index was 0.18. The prevalence of fluorosis was negative. Conclusions:In 2021, the prevalence of drinking water-borne endemic fluorosis is widespread in Henan Province, and measures to reduce fluoride and improve water quality in diseased villages have been basically implemented. However, the fluoride content of water in some villages still does not meet the standard, and some water improvement projects cannot operate normally throughout the year. In the future, it is necessary to address the above issues one by one and consolidate the achievements in prevention and control of drinking water-borne endemic fluorosis.

Objective:To understand the status of drinking water-borne endemic fluorosis in Henan Province, so as to provide scientific basis for improving prevention and control strategies. Methods:Ten counties (districts, referred to as counties) were selected from drinking water-borne endemic fluorosis areas of Henan Province in 2018. Three villages in each project county were selected according to the disease status of mild, moderate and severe conditions, and the operation status of water improvement projects, water fluoride content and dental fluorosis among children aged 8 to 12 years in each diseased village were monitored. Results:Totally 30 villages were surveyed, all of which were in the water improvement projects; a total of 25 water improvement projects were monitored, all of them were in normal operation, and the water fluoride exceeding standard rate was 28.00% (7/25), the qualified rate of water fluoride in the water improvement projects was 72.00% (18/25). Among them, 21 villages were in normal operation and the water fluoride in the water improvement projects met national standard. In which the detection rate of dental fluorosis among children aged 8 to 12 years was 36.71% (606/1 651), the index of dental fluorosis was 0.74, and the epidemic intensity was extremely mild. Water fluoride of water improvement projects in 9 villages exceeded the national standard, where the detection rate of dental fluorosis among children aged 8 to 12 years was 43.57% (261/599), the index of dental fluorosis was 0.78, and the epidemic intensity was extremely mild. The detection rate of dental fluorosis among children in the villages where the water fluoride of water improvement projects exceeded the national standard was higher than that in the villages where the water fluoride of water improvement projects met the national standard (χ 2=8.752, P < 0.01). Conclusions:The excessive fluorine content in the water of water improvement projects is still serious, and the epidemic is still severe. It is necessary to strengthen the scientific demonstration of the water improvement project construction and the project acceptance upon completion, so as to prevent unqualified projects from being put into use. We will carry out regular water quality monitoring and promptly rectify projects with water quality exceeding the standards.

Objective To study the changes of transcriptome levels in a Bama minipig model of hypertrophic scar during wound healing and pressure therapy by using RNA sequencing (RNA-seq) technique. Methods The Bama minipig model was established by skin wounds from the back and pressure (3.4 kPa) was initiated at 60 days after skin injury. Total RNA was extracted from scar tissues at 0, 14, 30, 60, and 90 days after skin injury and then sequenced. The resulting sequences were mapped to porcine reference genomes and transcriptomes were reconstructed to search for differentially expressed genes (DEGs). The DEGs were further subjected to GO and KEGG analysis using bioinformatics method, while part of the genes were selected for verification using qRT-PCR. Results After preprocessing, more than 78% reads in each group were accurately aligned to the reference sequence. The DEGs identification result showed that 568 genes were differentially expressed after pressure treatment, with 289 up-regulated and 279 down-regulated. GO enrichment analysis revealed that DEGs in each group were mainly associated with extracellular matrix, tissue development and skin development. KEGG analysis showed that the DEGs in each group during wound healing were mainly enriched in extracellular matrix-receptor interactions, focal adhesion and apoptosis pathways; while the DEGs after pressure treatment were mainly enriched in PI3K-Akt and MAPK signaling pathway except the pathways mentioned. qRT-PCR showed that the expression patterns of 6 DEGs were consistent with RNA-seq analysis, confirming the reliability of RNA-seq result. Conclusions RNA-Seq analysis identified differentially expressed genes in animal model of scars during wound healing and pressure therapy, which provided experimental evidence for clinical scar treatment.
