1.A postoperative in-hospital mortality risk model for elderly patients undergoing cardiac valvular surgery based on LASSO-logistic regression

Objective To evaluate the risk factors for postoperative in-hospital mortality in elderly patients receiving cardiac valvular surgery, and develop a new prediction models using the least absolute shrinkage and selection operator (LASSO)-logistic regression. Methods The patients≥65 years who underwent cardiac valvular surgery from 2016 to 2018 were collected from the Chinese Cardiac Surgery Registry (CCSR). The patients who received the surgery from January 2016 to June 2018 were allocated to a training set, and the patients who received the surgery from July to December 2018 were allocated to a testing set. The risk factors for postoperative mortality were analyzed and a LASSO-logistic regression prediction model was developed and compared with the EuroSCOREⅡ. Results A total of 7 163 patients were collected in this study, including 3 939 males and 3 224 females, with a mean age of 69.8±4.5 years. There were 5 774 patients in the training set and 1 389 patients in the testing set. Overall, the in-hospital mortality was 4.0% (290/7 163). The final LASSO-logistic regression model included 7 risk factors: age, preoperative left ventricular ejection fraction, combined coronary artery bypass grafting, creatinine clearance rate, cardiopulmonary bypass time, New York Heart Association cardiac classification. LASSO-logistic regression had a satisfying discrimination and calibration in both training [area under the curve (AUC)=0.785, 0.627] and testing cohorts (AUC=0.739, 0.642), which was superior to EuroSCOREⅡ. Conclusion The mortality rate for elderly patients undergoing cardiac valvular surgery is relatively high. LASSO-logistic regression model can predict the risk of in-hospital mortality in elderly patients receiving cardiac valvular surgery.
2.Blood concentration monitoring of tacrolimus in patients with nephrotic syndrome and establishment of MLP prediction model

OBJECTIVE To investigate the monitoring of tacrolimus blood concentration in patients with nephrotic syndrome (NS),and to establish a prediction model for tacrolimus blood concentration. METHODS Data from 509 concentration monitoring sessions of 166 NS patients using tacrolimus were collected from January 1, 2020 to August 31, 2023 in Zhongshan Hospital Affiliated to Xiamen University. The relationship of efficacy and adverse drug reaction(ADR) with blood concentration was analyzed. A multilayer perceptron (MLP) prediction model was established by using the blood concentration monitoring data of 302 times from 109 NS patients with genetic information, and then verified. RESULTS In terms of efficacy, the median blood concentration of tacrolimus in the non-remission group was 2.20 ng/mL, which was significantly lower than that in the partial remission group (4.00 ng/mL, P<0.001) and the complete remission group (3.60 ng/mL, P=0.002). In terms of ADR, the median blood concentration of tacrolimus in the ADR group was 5.01 ng/mL, which was significantly higher than that in the non-ADR group (3.37 ng/mL) (P=0.001). According to the subgroup analysis of the receiver operating characteristic curve, when the blood concentration of tacrolimus was ≥6.65 ng/mL, patients were more likely to develop elevated blood creatinine [area under the curve (AUC) was 0.764, P<0.001); when the blood concentration of tacrolimus was ≥6.55 ng/mL, patients were more likely to develop blood glucose (AUC=0.615, P= 0.005). The established MLP prediction model has a loss function of 0.9, with an average absolute error of 0.279 5 ng/mL between the predicted and measured values. The determination coefficient of the validation scatter plot was 0.984, indicating an excellent predictive performance of the model. CONCLUSION Tacrolimus blood concentration has an impact on both efficacy and ADR in NS patients. The use of the MLP model for predicting blood concentration exhibits high accuracy with minimal error between predicted and measured values. The model can be used as an important tool in clinical individualized medication regimens.
3.Establishment of an In-hospital Mortality Risk Model for Elderly Patients Undergoing Cardiac Valvular Surgery Based on Machine Learning

Objectives:To evaluate and predict the risk for in-hospital mortality in elderly patients receiving cardiac valvular surgery preferably,we developed a new prediction model using machine learning. Methods:Clinical data including baseline characteristics,peri-operative data and primary endpoint of 7 163 elderly patients aged 65 years or older undergoing cardiac valvular surgery from January 2016 to December 2018 from 87 hospitals were collected from the Chinese Cardiac Surgery Registry(CCSR).Patients from January 2016 to June 2018 were assigened to the training cohort(n=5 774)and patients from July to December 2018 were assigened to the validation cohort(n=1 389).The primary endpoint was in-hospital mortality.Machine learning algorithms were used to analyze risk factors and develop prediction model. Results:Overall in-hospital mortality was 4.1%.Linear discriminant analysis(LDA),support vector classification(SVC)and logistic regression(LR)models in the training cohort all have high AUCs and low Brier scores,with good discrimination and calibration.In validation cohort,the AUC of LDA,SVC and LR were 0.744,0.744 and 0.746 respectively,which were significantly better than that of 0.642 using the European System for Cardiac Operative Risk Evaluation II(EuroSCORE II)model(P<0.05). Conclusions:The mortality rate for elderly patients undergoing cardiac valvular surgery is relatively high.LDA,SVC and LR can predict the risk for in-hospital mortality in elderly patients receiving cardiac valvular surgery with high accuracy.

As the foremost among the four examinations in traditional Chinese medicine (TCM), inspection and related equipment research face challenges in landing and transformation due to variations in evidence quality, lack of standardization, insufficient algorithm transparency, and poor reliability and stability of decision-making. Against the backdrop of rapid development of emerging technologies such as big data, the internet of things, and artificial intelligence, coupled with macro policy support from the government, digital and intelligent generalized inspection in TCM has emerged, with the aim of utilizing digital technologies to overcome the limitations of naked-eye inspection and comprehensively perceive and analyze facial and bodily expressions. The research in this field intelligently correlates Zang-fu organ functions with health conditions and disease progression and establishes a technical system for digital and intelligent inspection, multi-dimensional and multimodal perception, fusion analysis, and decision-making. This system aims to enhance the accuracy of disease risk warning and diagnosis, bridging the gap between inspection equipment and assistance in clinical decision-making. From an evidence-based perspective, this paper systematically examines the research ideas of digital and intelligent inspection and the development of related equipment, deeply explores how to propose clinical practice-oriented key scientific issues, comprehensively acquire and co-apply multi-dimensional data, establish precise inspection models driven by digital intelligence, optimize standards to enhance equipment interoperability and reliability, construct post-effect evaluation mechanisms to promote improvement, and actively address potential risks such as the black box nature and information security in the application of intelligent technology. This paper not only demonstrates the tremendous potential of digital technologies in improving the accuracy and clinical application efficiency of inspection but also provides new perspectives and ideas for the modernization of inspection in TCM, paving the way for the application of inspection in the global medical and health field.

Research in cellular mechanics has rapidly advanced in recent years,uncovering how cells regulate their functions and behaviors by sensing and responding to mechanical cues from their environment.In 2023,significant breakthroughs were made in understanding the mechanical properties of cells,their mechanosensing of solid and fluid environments,and their adaptability under dynamic mechanical conditions.Meanwhile,advanced measurement techniques and mechanical models have provided new tools for investigation.These achievements have deepened the understanding of physiological and pathological processes,offering new approaches for disease diagnosis,prevention,and intervention.This review summarizes the research progress by Chinese scholars in cellular mechanics and explores the mechanical mechanisms exhibited in various biological processes in the year 2023.
6.Preparation of no-carrier-added 161Tb by lanthanide resin

Objective:To produce 161Tb from enriched 160Gd 2O 3 isotope-enriched target material and realize domestic production of the novel medical isotope 161Tb. Methods:The 160Gd 2O 3 isotope-enriched target material was irradiated with neutrons by the China Mianyang Research Reactor (CMRR). The no-carrier-added 161Tb product was obtained after the processes of target broken, sample dissolution, separation and purification with lanthanide (LN) resin and solution replacement with diglycolamide (DGA) column. Various key indicators such as γ spectral purity, metal impurity content, specific activity, radiochemical purity, and radioactive concentration were used to conduct the quality inspection and the control of 161Tb products. Results:161TbCl 3 of 33.4 GBq was obtained in a single time with the radioactive concentration of 16.8 GBq/ml, nuclear purity more than 99.9%, and radiochemical purity of 99.2%. Metal impurity content was met the established standards, with the specific activity of 6.02×10 17 Bq/mol. The radiochemical purities of 161Tb labeling with 1, 4, 7, 10-tetraazacyclododecane-1, 4, 7, 10-tetraacetic acid- D-Phe1-Tyr3-Thr8-octreotide (DOTATATE) after 0 and 72 h were 100% and 95.8% respectively. Conclusion:The preparation of no-carrier-added 161Tb by using LN resin has the advantages of high separation performance and high sample loading, which has great significance in the field of medical isotope preparation and lays a good nuclide guarantee for the research and development of domestic 161Tb-labeled drugs.
7.Effects of 4-hydroxy-2(3H)-benzoxazolone on Inflammatory and Apoptosis Signaling Pathways in Non-alcoholic Fatty Liver Disease Model Rats

OBJECTIVE:To inv estigate the effects of 4-hydroxy-2(3H)-benzoxazolone on inflammatory and apoptosis signaling pathways in non-alcoholic fatty liver disease (NAFLD)model rats. METHODS :SD rats were divided into normal control group(10 rats)and modeling group (50 rats). Normal control group was given basic diet ,and modeling group were given high-fat diet to induce NAFLD model. After modeling ,the rats were divided into normal control group ,model group ,silibinin group (26.25 mg/kg),and 4-hydroxy-2(3H)-benzoxazolone high-dose ,medium-dose and low-dose groups (100,50,25 mg/kg),with 8 rats in each group. Normal control group and modeling group were given 0.6% CMC-Na intragastrically ,and other groups were given relevant medicine 10 mL/kg intragastrically ,once a day ,for consecutive 4 weeks. After last medication ,the serum levels of albumin(ALB),total protein (TP),globulin(GLB),ALB/GLB and free fatty acid (FFA)were detected ;TUNEL staining was used to observe the apoptosis of rat hepatocytes. Western blot assay was used to detect the protein expression and phosphorylation level of inflammatory signaling pathway related proteins [Toll-like receptor 4(TLR4),myeloid differentiation factor 88(MyD88), nuclear factor-κB p65(NF-κB p65),NF-κB inhibitor protein(IκBα)] in liver tissue as well as the expression of apoptosis signaling pathway related proteins [B cell lymphoma 2(Bcl-2),Bax,caspase-3]. RESULTS :Compared with model group ,serum levels of TP (except for low-dose group ),GLB and FFA ,the protein expression of TLR 4(except for low-dose group ),MyD88 (except for medium-dose group )and caspase- 3,the phosphorylation levels of NF-κB p65 and IκBα protein were decreased significantly(P<0.05 or P<0.01). The ratio of A LB/GLB in serum and the ratio of Bcl- 2/Bax in liver tissue were significantly increased(P<0.05 or P<0.01),and the phenomenon of hepatocyte apoptosis was improved. CONCLUSIONS :4-hydroxy-2 (3H)-benzoxazolone can ameliorate NAFLD in rats ,and the mechanism may be associated with inhibiting the expression TLR 4/ MyD88/NF-κB signaling pathway-related proteins and apoptosis-related proteins in liver tissues.
8.Preparation and preliminary biological evaluation of domestic 177Lu-PSMA-617

Objective:To synthesize 177Lu-prostate-specific membrane antigen (PSMA)-617 with domestic 177Lu (made in China), and explore its optimal labeling condition, biodistribution, stability, and safety. Methods:177Lu-PSMA-617 was prepared with domestic 177Lu by a manual method. The optimal labeling condition, radiochemical purity, stability ( in vivo and in vitro), lipid-water partition coefficient, and plasma protein binding rate were determined. The uptake rate of 177Lu-PSMA-617 was evaluated by using 22RV1 cells. Biodistribution and SPECT/CT imaging were performed on normal mice with imported 177Lu-PSMA-617 as control group. The blood routine test was performed to evaluate the safety. Results:The best labeling result of domestic 177Lu-PSMA-617 can be obtained under the following conditions: pH=4.5, 100 ℃ for 30 min. And the radiochemical purity was ≥99%. The product was stable in vivo and in vitro, with the radiochemical purity >95% in 72 h. The plasma protein binding rate was (35.3±5.3)%, the lipid-water partition coefficient was -2.27±0.06, and the specific uptake rate of domestic 177Lu-PSMA-617 by 22RV1 cells reached the highest in 1 h ((7.58±0.84)%), which was slightly lower than the imported 177Lu-PSMA-617 ((7.86±0.96)%), but there was no significant difference between them ( t=-0.439, P>0.05). The distribution and SPECT/CT imaging of normal mice showed that domestic and imported 177Lu-PSMA-617 in blood were cleared quite fast, and both of them were excreted mainly through the kidneys. No obvious adverse reactions were found in the toxicity test of domestic and imported 177Lu-PSMA-617. There was no obvious abnormality in blood routine and liver and kidney metabolism. Conclusion:The domestic 177Lu-PSMA-617 has many advantages, such as qualified quality control, good biological properties and safety, which support its potential application value in diagnosis of prostatic neoplasms.
9.Study on the Improvement Effect and Mechanism of Ilicifoliosids Alkaloid A on Non-alcoholic Fatty Liver Di- sease in Rats

OBJECTIVE:To s tudy improvement effect and mech anism of ilicifoliosids alkaloid A (HBOA)on non-alcoholic fatty liver disease in rats. METHODS :SD rats were randomly divided into blank control group ,model group ,Silybin capsule group(positive control ,26.25 mg/kg),HBOA high-dose ,medium-dose and low-dose groups (100,50,25 mg/kg),with 10 rats in each group. Except that blank control group fed normal feed ,the other groups were continuously fed with high-fat diet for 8 weeks to induce non-alcoholic fatty liver disease model. Form the 9th week ,blank control group and model group were given constant volume of 0.6% CMC-Na solution ,and administration groups were given corresponding drugs by intragastric admini- stration,once a day ,for consecutive 4 weeks. The general information of rats were observed and the body weight increase ,organ (liver,kidney and spleen )indexes were calculated ;the contents of AST ,ALT,TC,TG and NEFA in liver tissue were detected , and SOD,GSH-Px activities and MDA content in the serum were also determined. The protein expression of PPARα in liver tissue was detected by immunohistochemistry. RESULTS :Compared with blank control group ,the body mass increase and liver index of rats in model group were increased significantly (P<0.01);fat deposition could be observed in the liver ;the activities of SOD and GSH-Px in serum were reduced significantly ,and the contents of MDA ,the contents of AST ,ALT,TC,TG and NEFA in liver tissue were significantly increased ,and the protein expression of PPARα was decreased significantly(P<0.01). Compared with model group ,the body mass increase and liver index of the rats were decreased significantly in administration groups (P<0.05 or P<0.01),liver fat deposition was improved ,the activity of SOD and GSH-Px in serum (except for HBOA low-dose group )were increased significantly while MDA content ,the contents of AST ,ALT,TC(except for HBOA low-dose group ),TG(except for HBOA low-dose group ) and NEFA in liver tissue were decreased significantl y,while protein expression of PPAR α 15177460685@163.com was increased significantly (P<0.05 or P<0.01). 10.Early survival analysis of acute type A aortic dissection and intramural hematoma

Objective To explore the early clinical outcomes of patients with acute type A aortic dissection and intramural hematoma. Methods The clinical data of 61 patients with acute type A aortic dissection or intramural hematoma in our hospital from January 23, 2020 to March 10, 2020 were retrospectively analyzed, including 43 males and 18 females, aged 22-81 (52.1±13.0) years. The patient's time of visit, clinical characteristics and early survival were analyzed. Kaplan-Mier survival curve and log-rank test were used for the survival analysis. Results There were 48 (78.7%) patients diagnosed with acute type A aortic dissection and 13 (21.3%) patients with intramural hematoma; 34 patients received operation and 11 were emergent. The 30-day mortality was 2.9% among the patients receiving operation. There were 48 patients alive and 13 patients dead during the study period. The cumulative survival rates for all the patients on postoperative 1 day, 3 days and 7 days were 93.4%, 86.4% and 77.5%, respectively. The cumulative survival rates for the patients with dissection on postoperative 1 day, 3 days and 7 days were 95.7%, 88.7% and 79.4%, respectively. The cumulative survival rates for the patients with hematoma on postoperative 1 day, 3 days and 7 days were 92.3%, 84.6% and 84.6%, respectively. The difference of survival rates between the two groups was not statistically significant (P>0.05). The cumulative survival rate of all the patients on postoperative 14 days was 74.5%. No statistically significant difference in survival rate on postoperative 14 days was found between patients with intramural hematoma and patients with aortic dissection (P>0.05). The proportions of the patients with unstable hemodynamics were found statistically significant between the survival patients and the dead patients (P<0.05). Conclusion Patients with acute aortic dissection and intramural hematoma who survive to the hospital still have the risk of death under active drug therapy, and rupture of the dissection is the leading cause of death in these patients, especially for those with hemodynamic unstability.
