1.Comparative analysis of tumor staging versus clinical staging in reimbursement scope restrictions under medical insurance for antineoplastic agents

OBJECTIVE To comparatively analyze tumor staging versus clinical staging in reimbursement scope restrictions under medical insurance for antineoplastic agents in order to better implement the medicare drug payment policy. METHODS Antineoplastic agents included in the National Basic Medical Insurance, Workers’ Compensation Insurance and Maternity Insurance Drug Catalogue (2024) (hereinafter referred to as the “Medical Insurance Catalog”) were used as research subject to compile and analyze reimbursement scope restrictions regarding tumor staging. By consulting clinical diagnosis and treatment guidelines and relevant literature, the tumor staging in reimbursement scope restrictions of the Medical Insurance Catalog was mapped and compared with clinical staging. RESULTS & CONCLUSIONS A total of 89 antineoplastic agents’ medical insurance payments had tumor staging. Among these, there were 86 western drugs (including 17 ordinary western drugs, 68 negotiated drugs, and 1 competitive drug) and 3 Chinese patent medicines (including 1 ordinary Chinese patent medicine and 2 negotiated drugs). Non-small cell lung cancer involved the most restricted payment drugs, with 36 drugs. The tumor staging in reimbursement scope restrictions was mostly “metastatic” and “locally advanced”, involving 67 and 48 drugs respectively. Tumor staging in most reimbursement scope restrictions could correspond to the clinical staging of the tumor. However, mid-advanced esophageal cancer, unresectable gastrointestinal stromal tumors, unresectable locally advanced neuroendocrine tumors, locally advanced basal cell carcinoma, and unresectable neurofibromatosis type Ⅰ did not have a corresponding clinical staging mentioned in authoritative guidelines or high-quality clinical studies and need to be determined by the clinic according to the actual situation of the patient. Therefore, it is recommended that the interpretation of tumor staging in reimbursement scope restrictions should be accurately defined and standardized, so as to improve the accuracy of the drug payment policy in the actual implementation process.

Glioma is the most common primary tumor of the brain,accounting for 81%of central nervous system(CNS)malignant tumors.The degree of malignancy is high,and the current treatment methods are limited.In recent years,with the in-depth study of intestinal flora and brain-gut axis,it has been found that the diversity of gut microbiota plays an important role in the regulation of glioma.The mechanism is that the intestinal flora affects the development of glioma through the role of immune regulation and metabolites.In addition,it has been con-firmed that there is a certain correlation between some probiotics and glioma,which provides a new application prospect for the treatment of glioma.This paper discusses the main intestinal bacteria that regulate gliomas as well as the role and regulatory mechanisms of intestinal flora in the development of gliomas,and provides ideas for the discovery of new targets for glioma treatment and further improvement of treatment options.
3.A New Calculation Model for Calcium Requirements After Parathyroidectomy in Patients With Secondary Hyperparathyroidism

Objectives:
. We aimed to develop a new calculation model for calcium requirements in dialysis patients following parathyroidectomy.
Methods:
. A total of 98 patients with secondary hyperparathyroidism receiving parathyroidectomy from January 2014 to January 2022 were enrolled in this study. Among these patients, 78 were randomly selected for construction of the calcium requirement calculation model, and the remaining 20 patients were selected for model validation. The calcium requirement model estimated the total calcium supplementation for 1 week after surgery using variables with significant relationships in the derivation group by stepwise multiple linear regression analysis. Bias, precision, and accuracy were measured in the validation group to determine the performance of the model.
Results:
. The model was as follows: calcium requirement for 1 week after surgery=33.798–8.929×immediate postoperative calcium+0.190×C-reactive protein–0.125×age+0.002×preoperative intact parathyroid hormone+0.003×preoperative alkaline phosphatase (R2=0.8). The model was successfully validated.
Conclusion
. We generated a novel model to guide calcium supplementation. This model can assist in stabilizing the serum calcium levels of patients during the early postoperative period. Furthermore, it contributes to the individualized and precise treatment of hypocalcemia in patients following parathyroidectomy.
4.Analysis on the difference between the payment limitations of anti-cancer drugs and the application scope of drug indications

OBJECTIVE To analyze the difference between the payment limitations of anti-cancer drugs and application scope of drug instructions, so as to better implement the payment policy of medical insurance drugs. METHODS The differences between the payment limitations of anti-cancer drugs and application scope of drug instructions in the National Catalogue of Drugs for Basic Medical Insurance, Industrial Injury Insurance and Maternity Insurance (2022) were compared and analyzed; the evidence-based basis of the difference was discussed, and the scope of limited payment was interpreted. RESULTS Totally 118 drugs had payment limitations; limitations scope mainly included limited evidence of gene detection results, limited indications, limited second-line and above treatment, limited payment duration, limited specialist prescription, limited medical institution grade, etc. Among them, 43 drugs had differences between the payment limitations and drug instructions, and the indications of 31 drugs were greater than payment limitations; for seven drugs, the drug indications beyond the payment limitations were recommended by the guidelines. The payment limitations of 75 drugs were consistent with drug instructions. The second-line and multi-line treatment was ineffective or intolerable with first-line drugs. There was a certain relationship between locally advanced, advanced or metastatic tumor and tumor stage, but different tumors had different criteria. Systemic treatment mainly referred to systemic treatment with drug. The results of limited genetic test required that the result was positive or negative. In addition, six kinds of TCM injections were limited to the level of medical institutions; the payment of two drugs did not exceed 12 months; when lenalidomide was combined with isazomide citrate, the medical insurance only paid for one of the drugs. CONCLUSIONS The payment limitations of some anti- cancer drugs are inconsistent with the drug indications. The drug payment limitations should be expanded according to the actual situation of clinical medication and the recommendations of guidelines. At the same time, the payment limitations should be formulated accurately and in detail, thus clinical and medical insurance staff can understand it and fully protect the interests of patients.
5.Risk factors for hypocalcemia after parathyroidectomy in maintenance hemodialysis patients with secondary hyperparathyroidism

Objective:To explore the risk factors of hypocalcemia and the correlation between calcium supplementation and clinical parameters after parathyroidectomy (PTX) in maintenance hemodialysis patients with secondary hyperparathyroidism (SHPT), and to analyze the effect of calcium supplementation after PTX on the long-term prognosis of patients.Methods:This study was a single-center retrospective study. The patients who underwent PTX in maintenance hemodialysis patients with SHPT in the Huashan Hospital affiliated to Fudan University from October 2014 to March 2021 were retrospectively enrolled. Total PTX with auto transplantation or total PTX alone were the surgical procedures. According to the postoperative requirement of calcium in the first week, the patients were divided into two groups: high calcium supplement (>16.05 g/week) group and low calcium supplement group (≤16.05 g/week). According to the average serum calcium level in the first week after operation, the patients were divided into hypocalcemia group (≤2.1 mmol/L) and non-hypocalcemia group (>2.1 mmol/L) and the differences of clinical parameters between the two groups were compared. The correlation between clinical parameters and the postoperative calcium requirement was examined through Pearson or Spearman correlation analysis. The influencing factors for hypocalcemia after PTX were examined through logistic regression analysis. The survival curve was made by Kaplan-Meier method, and the difference of cumulative survival rate between the two groups was compared by log-rank test.Results:A total of 98 maintenance hemodialysis patients with SHPT were enrolled. The levels of serum calcium, phosphorus, and intact parathyroid hormone (iPTH) after the operation decreased significantly than those of preoperation (all P<0.05). Multiple linear regression analysis showed age ( β=-0.160, P=0.030), iPTH ( β=0.004, P=0.025) and C-reactive protein ( β=0.186, P=0.011) were correlated with postoperative calcium requirement. Preoperative alkaline phosphatase ( OR=1.002, 95% CI 1.000-1.004, P=0.018) and hemoglobin ( OR=0.977, 95% CI 0.954-1.000, P=0.048) independently predicted the occurrence risk of postoperative hypocalcemia through multivariate logistic regression analysis. The recurrence rate of high calcium supplement group was higher than that of low calcium supplement group (10.26% vs 0, P=0.023) and there was no significant difference in all-cause mortality between the two groups (17.95% vs 5.08%, P=0.086). The recurrence rate between the hypocalcemia group and non-hypocalcemia group was no significantly different (8.3% vs 1.8%, P=0.451) and there was no significant difference in all cause mortality between the two groups (12.5% vs 12.7%, P=1.000). Kaplan-Meier survival curve showed that the cumulative survival rate between the two groups was no significantly different (log-rank test χ2=0.147, P=0.702). Conclusions:PTX is a safe and effective therapeutic method to reduce the level of iPTH and improve the metabolism of calcium and phosphorus in SHPT patients. Age, iPTH and C-reactive protein are correlated with the postoperative requirement of calcium in the first week. Preoperative alkaline phosphatase and hemoglobin are independent risk factors for postoperative hypocalcemia. Correcting preoperative electrolyte disorder, improving infection and anemia can reduce the incidence of hypocalcemia after PTX.
6.Changes of brain oxygenated hemoglobin concentration in adolescent patients with depression under speech fluency task

Objective:To explore the characteristics and differences of oxygenated hemoglobin responses in different brain regions in adolescent depressive disorder patients and healthy adolescents during speech fluency task based on functional near-infrared spectroscopy imaging.Methods:From March 2021 to June 2021, twenty-nine adolescents with depression(depressed group) and 26 healthy adolescents(normal group) were enrolled. The severity of depression was assessed by Hamilton depression scale, and the speech fluency task was performed by functional near-infrared spectroscopy brain imaging, and the relative concentrations of oxyhemoglobin in the prefrontal and bilateral temporal lobes were measured.SPSS 26.0 statistical software was used for data analysis, chi-square test was used for gender count data. The t-test was used for comparison of two groups. The measurement data that did not conform to normal distribution were compared between the two groups by Mann-Whitney U test. Results:In the speech fluency task, there was no significant difference in the concentration of oxygenated hemoglobin in prefrontal and left temporal between the depressed group and the normal group (both P>0.05); the oxygenated hemoglobin concentration in the right temporal lobe(-9.179(-22.231, 4.789)) of depressed group was significantly lower than that of the normal group (12.754(-9.438, 35.008)), and the difference was statistically significant ( U=538.00, P<0.05). The oxygenated hemoglobin concentration in right temporal lobe of depressed group had no correlation with the score of Hamilton depression rating scale ( r=0.092, P>0.05). Conclusion:The right temporal lobe function of adolescent depression patients is lower than that of healthy controls during speech fluency tasks, which provides a basis for study of the relationship between depression and some cognitive sensitivity impairment and the function of right temporal lobe.
7.Effect of multidisciplinary cooperation model on perioperative nursing of chronic ulcer of diabetes mellitus patients following lower extremity trauma

Objective:To explore the effect of multidisciplinary collaboration(MDT)mode on perioperative nursing of chronic ulcer of diabetes mellitus patients following lower extremity trauma.Methods:A retrospective case-control study was conducted to analyze the clinical data of 122 diabetes mellitus patients combined with chronic ulcer following lower extremity trauma admitted to Sir Run Run Shaw Hospital affiliated to Zhejiang University School of Medicine from January 2015 to December 2019. There were 58 males and 64 females at age of 40-76 years[(56.0 ± 4.7)years]. The wounds were located at the heel in 10 patients,at the lateral ankle in 12,at the toe in 22,at the calf in 59 and at the thigh in 19. Sixty patients received MDT care(collaborative care group),and 62 patients received traditional care(traditional care group). Visual analogue scale(VAS)and level of fasting plasma glucose were measured at days 1 and 3 postoperatively and on the day of discharge. Mental status of the patients was evaluated using self-evaluation of anxiety scale(SAS)and self-rating depression scale(SDS)after nursing. Area and depth of wounds was detected at postoperative 2 weeks and 1 month,and level of fasting glucose was measured again within 1 month after operation. The rate of amputation,incidence of debridement and direct suture rate were documented while hospitalized again at postoperative 1 month.Results:All patients were followed up for 0.5-3 months[(1.2 ± 0.7)months]. VAS was 1.0(1.0,2.0)points,1.0(0.0,1.0)points and 1.0(0.0,1.0)points in collaborative care group at days 1 and 3 postoperatively and on the day of discharge,compared to 2.0(2.0,2.3)points,2.0(2.0,2.0)points and 1.0(1.0,2.0)points in traditional care group( P < 0.05). Level of fasting blood glucose was(7.2 ± 0.8)mmol/L,(6.9 ± 0.8)mmol/L and(6.9 ± 0.7)mmol/L in collaborative care group on days 1 and 3 postoperatively and on the day of discharge,compared to(7.8 ± 0.8)mmol/L,(7.8 ± 0.8)mmol/L and(7.7 ± 0.9)mmol/L in traditional care group( P < 0.05). Scores of SAS and SDS were(8.4 ± 0.8)points and(11.2 ± 1.0)points in collaborative care group after nursing,compared to(8.7 ± 0.7)points and(12.3 ± 1.0)points in traditional care group( P < 0.05). Area and depth of wounds were(29.4 ± 3.9)cm 2 and(1.4 ± 0.4)cm in collaborative care group at postoprative 2 weeks,compared to(33.3 ± 3.6)cm 2 and(1.5 ± 0.5)cm in traditional care group( P < 0.05). Area and depth of wounds were(24.5 ± 3.8)cm 2 and(0.9 ± 0.4)cm in collaborative care group at postoprative 1 month,compared to(30.6 ± 4.8)cm 2 and(1.2 ± 0.5)cm in traditional care group( P < 0.05). Level of fasting blood glucose in collaborative care group was significantly lower than that in traditional care group at postoprative 1 month( P < 0.05). During hospital re-admission 1 month after operation,rate of amputation and incidence of re-debridement were 5%(3/60)and 7%(4/60)in collaborative care group,significantly lower than those in traditional care group[18%(11/62),22%(13/62)]( P < 0.05),and direct repair suture rate was 88%(53/60)in collaborative care group,significantly higher than that in traditional care group[61%(38/62)]( P < 0.05). Conclusion:For chronic ulcer of diabetes mellitus patients following lower extremity trauma,MDT model is superior over traditional nursing for alleviated pain,controlled blood glucose,improved psychological state,promoted wound healing and reduced rate of amputation and incidence of re-debridement.
8. Prenatal ultrasonographic diagnosis and prognosis of fetal meconium peritonitis

To investigate the maternal and neonatal outcomes of fetal meconium peritonitis (FMP) cases with different ultrasonic manifestations.
Methods:
The clinical data of 31 pregnant women with FMP diagnosed by prenatal ultrasound and confirmed by postnatal imaging examination in Guangzhou Women and Children's Medical Center from January 2011 to December 2018 were analyzed retrospectively. According to the last prenatal ultrasonographic findings, the 31 cases were classified into the following grades: grade 0 (three cases), grade 1 (20 cases, grade 1A: nine cases, grade 1B: three cases, grade 1C: eight cases), grade 2 (seven cases) and grade 3 (one case). 9.Expression of DDX46 protein in colorectal cancer and its correlation with expressions of C-erbB-2, Ki-67, p53 and nm23 proteins

Objective To investigate the expression of DDX46 protein in colorectal cancer and its correlation with expressions of C-erbB-2, Ki-67, p53 and nm23 proteins. Methods A total of 149 cases of colorectal cancer tissues and 45 adjacent mucosa tissues after operation in the Wuxi No.2 Hospital Affiliated to Nanjing Medical University from January 2010 to December 2012 were collected. The expressions of DDX46 protein and C-erbB-2, Ki-67, p53 and nm23 proteins in tissues of colorectal cancer were detected by using immunohistochemistry. The relations between the expression level of DDX46 protein and the clinicopathological features including the age, gender, tumor differentiation, tumor infiltration, lymph node metastasis, TNM stage were analyzed, and the relations between DDX46 protein expression and expressions of C-erbB-2, Ki-67, p53 and nm23 proteins were also analyzed. Results Of 149 colorectal cancer tissues, 50 (33.6%) showed low expressions of DDX46 protein and 99 (66.4%) showed high expressions. Of 45 adjacent mucosa tissues, 37 (82.2%) showed low expressions of DDX46 protein, and 8 (17.8%) showed high expressions. There were significant differences between the high expression rate of DDX46 protein in cancer tissues and adjacent mucosa tissues (χ2=33.09, P<0.01). There were statistical differences in the expression level of DDX46 protein and tumor differentiation, tumor infiltration, lymph node metastasis, TNM staging (all P<0.05). The ratio of high-expressed DDX46 protein was increased with the low differentiation, high invasion degree, lymph node metastasis and late TNM staging. DDX46 protein expression was correlated with expressions of Ki-67, p53 and nm23 proteins (all P< 0.05), and the expression of DDX46 protein had a positive correlation with expressions of Ki-67 and p53 proteins (r values were 0.161 and 0.347), and a negative correlation with expression of nm23 protein (r= -0.561). Conclusions The high expression of DDX46 protein in colorectal cancer tissues is related with malignant biological behavior of the tumors and the key oncogenes. DDX46 protein could be regarded as the potential marker for the diagnosis and molecular targeting therapy of colorectal cancer.
10.Effects of DDX46 gene on the radiosensitivity of colorectal cancer cells

Objective To investigate the effect of DDX46 gene on the radiosensitivity of colorectal cancer cells and underlying mechanism.Methods SW480 cells transfected with DDX46 RNAi or its empty plasmid by lentivirus were set as experimental group and control group,respectively.After 72 h of transfection,the cells were irradiated with 4 Gy X rays.The cell viabilities of these two groups were detected by CCK-8 assay.The number of γ-H2AX foci and the expressions of some key proteins related to DNA damage repair were detected by immunofluorescence technique and Western blot at 24 h after irradiation.Results At 24 h after 4 Gy irradiation,the cell vitality of experimental group was decreased to (15.02±3.92)%(t=-4.696,P < 0.05) of control and(17.43 ±1.83)%(t=4.844,P<0.01) of nonirradiated cells,but there was no significant difference between 4 Gy irradiated control cells and nonirradiated cells.Meanwhile,compared with the control group,the ATM protein expression level (t =7.530,P <0.01) and the number of γ-H2AX foci (t =-3.108,P <0.05) were significantly increased in the experimental SW480 cells,while the expression levels of p-ATM and Rad50 were significantly reduced (t =4.260,4.260,P < 0.05),and the protein levels of DNA-PK in these two groups had tiny difference.Conclusions DDX46 RNA silence increases the radiation sensitivity of SW480 cells by inhibiting ATM activation and DNA repair.
