Objective To compare the results of accumulation and continuation methods in indoor radon measurement. Methods The radon concentrations in 30 households in 7 provincial capital cities of China were simultaneously measured using both accumulation and continuation methods. Results The radon concentration measured by accumulation method in 30 households ranged from 21 to 323 Bq/m3, with a median M(P25, P75) of 70.5 (43.8, 111). The radon concentration measured by the continuation method ranged from 16.1 to 258 Bq/m³, with a median M(P25, P75) of 100 (51.3, 141). The average relative percent deviation between the two measurement methods was 41.8%. There were significant differences among the measurement results at 8, 16, and 24 h by the continuation method. Pairwise comparisons showed there were statistical diffferences between 8 h and 16 h, as well as between 8 h and 24h; however, no statistical significance was found (P < 0.05), between 16 h and 24 h. Conclusion The overall continuous measurement results of the 30 households were higher than those of cumulation method. The comparison between accumulation and continuation measurement results did not show a high level of correlation (r=0.49). The continuation method is significantly affected by environmental factors, and the length of the measurement period can affect the measurement results. The selection of indoor radon measurement methods should consider the purpose, sample size, and environmental conditions.

Objective:To assess the capability of seven reference medical laboratories to detect BCR::ABL1 p210 transcription levels and to compare the results among those laboratories. Methods:The interlaboratory comparison was carried out in two stages. The samples were prepared by the reference laboratory. The quantitative values of BCR::ABL1 p210 of the comparison samples covered 0.001%-0.01%, 0.01%-0.1%, 0.1%-1%, 1%-10% and>10% in each stage. Real-time quantitative PCR (RT-PCR) and dPCR (digital PCR) were used to examine the samples. The conversion factor (CF) was calculated and validated for each laboratory. Results:In the RT-PCR comparison, one laboratory was failed to detect BCR::ABL1 p210 in fourteen samples at the first stage. The results of the other six laboratories were qualified with the bias <±1.2 folds (-0.133-0.338) and 95% limits of agreement within ±5 folds (upper limit 0.147-0.785, lower limit -0.770--0.109), and the corresponding CF values were calculated and validated. In the dPCR comparison, one laboratory did not report results at the second stage. The results of the other six laboratories were qualified with the bias <±1.2 folds (-0.026-0.267) and 95% limits of agreement within±5 folds (upper limit 0.084-0.991, lower limit -0.669--0.135), and the corresponding CF values were calculated and validated. The samples with BCR::ABL1 p210 quantitative values of 0.01%-0.1%, 0.1%-1%, 1%-10% and >10% could be detected by both RT-PCR and qPCR. When the quantitative value of BCR::ABL1 p210 was 0.001%-0.01%, the detection rate of dPCR was higher than that of RT-PCR (85.56% vs. 68.00%).Conclusions:A good consistency is present among various laboratories. The quantitative value of BCR::ABL1 p210 is comparable among laboratories as shown by the CF value conversion. For quantitative detection of BCR::ABL1 p210 deep molecular reaction, dPCR has a higher positive detection rate and more advantages than RT-PCR. To ensure the accuracy and reproducibility of the BCR::ABL1 p210 test, it is imperative for every laboratory to enhance their daily quality control practices.

Objective:To observe any effect of combining electro-acupuncture with aerobic exercise on the expression of SR-BI receptor and caveolin-1 and ABCA1 proteins in the liver and on reverse cholesterol transport. Any relationship with inflammatory factors NF-κBp65, COX-2 and TNF-α in the aortic arch was also investigated. Methods:Thirty male SPF Sprague-Dawley rats were randomly divided into a normal group (NG), a model group (MG), and a group which received electro-acupuncture combined with aerobic exercise (the EAG group), each of 10. A hyperlipidemia model was induced in the rats of the MG and EAG groups by feeding a high-fat diet. For 4 weeks the EAG group received 30 minutes of electroacupuncture daily (frequencies of sparse/dense waves 2/100Hz, 1mA) at the ST40 and SP9 acupoints, followed by 60 minutes of no weight bearing swimming. No intervention was applied to the other groups. Before and after the intervention, serum was collected to observe lipid levels. Liver and aortic arch issue were resected to measure lipid deposition and inflammation using hematoxylin eosin staining. Caveolin-1, SR-BI and ABCA1 in the liver and NF-κBp65, COX-2 and TNF-α in the aortic arch were quantified using real-time PCRs and western blotting. Results:After successful modeling the levels of total cholesterol (TC) and low density lipoprotein cholesterol (LDL-C), and the protein and mRNA expressions of NF-κBp65, TNF-α, COX-2 and COX-2 in the aortic arches of the MG had increased significantly. High density lipoprotein cholesterol (HDL-C) and the protein and mRNA expressions of caveolin-1, SR-BI and ABCA1 in the liver had decreased significantly. Compared with the MG, the levels of TC and LDL-C, as well as the protein and mRNA expressions of NF-κBp65, TNF-α, COX-2 and COX-2 in the aortic arches of the EAG had decreased significantly, while the average HDL-C l level, as well as the protein and mRNA of caveolin-1, SR-BI and ABCA1 in the liver were up-regulated significantly. Conclusions:Electroacupuncture combined with aerobic exercise can relieve hyperlipidemia through promoting cholesterol reverse transport and inhibiting inflammation in endothelial cells. Those effects may be related to up-regulating the expression of caveolin-1, SR-BI and ABCA1 in the liver and down-regulating inflammatory factors in the aortic arch.

Objective To investigate the indoor radon concentration and its changing trend in northeastern China. Methods We measured indoor radon levels cumulatively for over three months by solid state nuclear track detection in a total of 261 houses in multi-story or high-rise buildings in Shenyang, Changchun, Harbin, Heihe, and Yichun in northeastern China. The measurement lasted one year in Changchun for seasonal changes. Results The average indoor radon concentration in the five cities was 88 Bq/m3, ranging from 12 to 558 Bq/m3. The indoor radon concentrations were ≤ 100 Bq/m3 in 75.1% of the houses, and ≤ 300 Bq/m3 in 97.7% of the houses. The indoor radon concentration increased with the age of buildings. The indoor radon concentration was highest in winter, and it was higher in summer than in autumn and spring. Conclusion The indoor radon concentration in northeastern China increased compared with the data of 1980s and 1990s. It is highest in the winter heating season, and higher in summer than in spring and autumn. Indoor radon exposure deserves attention.

Objective:To analyze the clinical and laboratory characteristics of acute myeloid leukemia (AML) patients with NPM1 mutation, and to explore the prognostic factors. Methods:A total of 77 AML patients with NPM1 gene mutation admitted to Hebei Yanda Ludaopei Hospital from May 1st 2012 to December 31st 2021 were enrolled in the study, including 34 male and 43 female patients. The median age was 40 (3, 68) years old. Patients were selected and divided into 4 groups according to the morphological FAB classification. There were 29 cases (37.7%) of M1 type, 13 cases (16.9%) of M2 type, 23 cases (29.9%) of M4 type, and 12 cases (15.5%) of M5 type. The clinical characteristics, bone marrow/peripheral blood cell morphology, immunophenotype, cytogenetics, molecular biology and overall survival of different groups were retrospectively analyzed, and the risk factors affecting the prognosis of AML were also explored. Cox multivariate regression was used to analyze the clinical influencing factors of survival and prognosis. Results:The white blood cell counts were highest in M4 and M5 patients and lowest in M2 patients, while no significant difference in the red blood cell, hemoglobin, and platelet counts( P>0.05). Morphologically, there were significant differences in the percentage of blasts and blasts with cup-like nuclei on bone marrow (BM) and peripheral blood (PB). The proportion of blasts in BM and PB was the highest in M1 and the lowest in M2 ( P<0.001). The positive rate of blasts with cup-like nuclei was the highest in M1 and the lowest in M5 of BM ( P<0.001), while the highest in M2 and the lowest in M5 of PB ( P=0.006). The scores of myeloperoxidase and chloroacetate esterase were all the highest in M1 and the lowest in M5 ( P<0.001, 0.001, respectively). In terms of molecular biology, the occurence rate of blasts combined with DNMT3A mutation was the highest in M4 and the lowest in M2 ( P=0.044), while those combined with FLT3-ITD mutation was the highest in M4 and the lowest in M5 ( P=0.002). In immunophenotype, there were significant differences in the expression positivities of seven antigens including HLA-DR, CD56, CD11c, CD15, CD14, CD96 and cMPO ( P<0.05). Multivariate COX regression analysis showed that no recurrence after treatment ( P<0.001), complete remission after treatment ( P=0.015) and transplantation ( P<0.001) were correlated with overall survival (OS). No recurrence after treatment ( P=0.033), transplantation ( P=0.027), no mutation of FLT3-ITD ( P=0.040), and hemoglobin concentration ( P=0.023) were associated with relapse-free survival (RFS). Survival analysis by Kaplan-Meier curve showed that there was no significant difference in survival time between the M1, M2, M4 and M5 groups in OS and RFS. Conclusion:There were significant differences in the white blood count, the percentage of blasts and blasts with cup-like nuclear morphology, cytochemical staining (MPO integration, CE integration and percentage of NAS-DCE), gene mutation (DNMT3A and FLT3-ITD) and immunophenotypes (HLA-DR, CD56, CD11c, CD15, CD14, CD96 and cMPO) between the four groups. The multivariate analysis revealed that no recurrence after treatment and transplantation were independent prognostic factors in NPM1 mut AML patients. On the other hand, FLT3-ITD mutation and hemoglobin concentration were associated with RFS and complete remission after treatment was associated with OS in the entire NPM1 mut cohort.

10.MicroRNA in acute kidney injury due to sepsis
Clinical Medicine of China 2023;39(5):385-388
Sepsis is a life-threatening clinical syndrome characterized by organ dysfunction caused by dysregulation of the body's immune response to infection, and is the primary cause of acute kidney injury in critically ill patients. Sepsis induced AKI (S-AKI) is a common clinical critical illness with a high mortality rate and is closely associated with the clinical outcome of critically ill patients. At present, the clinical diagnosis of AKI is still based on the traditional indicators such as serum creatinine and urine output, but it is easily interfered by age, gender, drugs and other factors, and cannot accurately and sensitively reflect the renal damage of patients, so that some AKI patients lose the best time for treatment. Therefore, some biomarkers with strong sensitivity and high specificity are urgently needed in the clinic to help the diagnosis of AKI, so as to take active therapeutic measures to alleviate the prognosis of AKI.Numerous studies have shown that microRNAs (miRNAs) are involved in the development of S-AKI. Therefore, miRNAs can not only be early diagnostic markers of S-AKI, but also have important potential value for the treatment and prognosis of S-AKI. In this paper, we review the potential value of miRNA for the diagnosis, prognosis and treatment of S-AKI.
