1.Network meta-analysis for efficacy and safety of aromatase inhibitors for postmenopausal hormone receptor-positive early breast cancer

		                        			OBJECTIVE To evaluate the efficacy and safety of three aromatase inhibitors (exemestane, anastrozole, letrozole) for postmenopausal hormone receptor (HR)-positive early breast cancer. METHODS PubMed, the Cochrane Library, Embase, CNKI, Wanfang Data, VIP and SinoMed were searched to collect randomized controlled trials (RCTs) of the above three drugs in the treatment of postmenopausal HR-positive early breast cancer patients. The retrieval time limit was from the establishment of the database to October 25, 2024. After literature screening, data extraction and literature quality evaluation, network meta-analysis was performed by using RevMan 5.3 and Stata 18.0 software. RESULTS A total of 15 RCTs involving 44 055 patients were included. The results of network meta-analysis showed that the objective response rate of letrozole group was significantly higher than anastrozole group (P<0.05), and the order of surface under the cumulative ranking curve (SUCRA) from high to low was letrozole (85.6%)>anastrozole (61.5%)>exemestane (2.8%). The disease-free survival rate of anastrozole group was significantly higher than exemestane and placebo groups (P<0.05), and the order of SUCRA from high to low was letrozole (85.8%)> anastrozole (67.3%)>exemestane (41.4%)>placebo (5.5%). The total incidence of adverse reactions in anastrozole group was significantly higher than letrozole and placebo groups (P<0.05), and the order of SUCRA from high to low was exemestane (87.4%)>letrozole (63.9%)>anastrozole (47.0%)>placebo (1.7%). The results of subgroup analysis according to the course of treatment≥104 weeks were consistent with them. CONCLUSIONS Compared with anastrozole, letrozole has better efficacy and safety in the treatment of postmenopausal HR-positive early breast cancer, and the efficacy of exemestane is limited.
		                        		
		                        		
		                        		
		                        	
2.Analysis of in-service occupational medical examination results during employment for high-temperature workers in Longhua District, Shenzhen City

		                        			Objective To investigate the health status of high-temperature workers in Longhua District, Shenzhen City. Methods A total of 10 323 high-temperature workers in Longhua District from 2019 to 2022 were selected as the research subjects using the judgment sampling method. Their in-service occupational medical examination results during employment were collected and analyzed by grouping. Results The abnormal rates of urinalysis, blood pressure, serum alanine aminotransferase (ALT), electrocardiogram (ECG), and blood glucose among the research subjects were 34.9%, 12.5%, 8.5%, 7.6%, and 7.2%, respectively. Women had a higher rate of abnormal urinalysis than men (63.0% vs 25.9%, P<0.01), while men had higher rates of abnormal blood pressure and serum ALT than women (13.5% vs 9.4%, 10.2% vs 3.1%, both P<0.01). The abnormal rates of urinalysis, blood pressure, ECG, and blood glucose among the research subjects increased with age (all P<0.05), while the abnormal rate of serum ALT decreased with age (P<0.01). The abnormal rates of blood pressure and blood glucose increased with the length of high-temperature work time (all P<0.01). The most common occupation exposed to high temperatures was injection molding (accounting for 22.2%). The abnormal rates of urinalysis, blood pressure, serum ALT, ECG, and blood glucose among different occupations had significant differences (all P<0.01). Among them, injection molding workers had the highest rate of abnormal urinalysis (43.5%), while inflation film workers had the highest rate of abnormal blood pressure (24.2%). Conclusion The health status of high-temperature workers in Longhua District is concerning. Priority should be given to addressing abnormalities in urinalysis, ALT, ECG, blood pressure, and blood glucose. 3.Exploring the Characteristics of Medication Used in SUN Simiao's Wings of the Thousand Gold Pieces Formulary (《千金翼方》) for Regulating the Spleen:from the Principle of "Supplementing or Draining by Preference and Aversions"

		                        			The principle of "supplementing or draining by the preference and aversions of the the five zang organs" emphasizes that prescriptions should align the five flavors of medicinals with the needs of the five organs, achieving supplementation through harmonizing flavors and drainage through opposing flavors. This paper analyzed the formulas for regulating the spleen found in Wings of the Thousand Gold Pieces Formulary (《千金翼方》) from the perspective of "supplementing or draining by the preference and aversions", to summarize SUN Simiao's prescription characteristics for treating spleen-related disorders. In terms of the combination rule of medicinal properties and flavors, formulas like Bupi Decoction (补脾汤) for supplementing the spleen use sweet and warm medicinals paired with bitter and drying medicinals; those for draining the spleen use acrid and warm medicinals paired with bitter and cold medicinals; those for warming the spleen employs acrid and hot medicinals with sweet and warm medicinals; those like Jianpi Decoction (建脾汤) for strengthening spleen combines sweet and warm with sour and bitter medicinals; Zhuanpi Pill (转脾丸) pairs acrid and warm medicinals with sweet and warm medicinals; and Roupi Decoction (柔脾汤) uses sweet and warm medicinals with sweet and bitter medicinals. Regarding the medicinal flavors, warm dominates the four qi (the four properties), while sweet, bitter, and acrid are the main among the five flavors. Clinical practice involves applying comprehensive therapy methods such as the combination of sweet and bitter to harmonize, acrid and sweet to transform yang, pungent to disperse and bitter to descend, balancing cold and warm, and combining supplementation and drainage. The foundational principle is to tonify spleen qi and protect the middle jiao, and treatment is tailored based on the disease condition, location, nature, and the characteristics of the medicinals, with careful selection of suitable dosage forms and administration methods. 
		                        		
		                        		
		                        		
		                        	
4.Advances in influential factor and drug treatment studies for taxane-induced peripheral neuropathy

		                        			There are millions of patients with taxane-induced peripheral neuropathy (TIPN), and there is no effective treatment or prevention measure in clinical practice. The occurrence of TIPN may be related to the dosage form of paclitaxel drugs, genetic and molecular markers, drug dosage and chemotherapy cycle, patient factors, etc. At present, drugs for treating TIPN mainly include those that inhibit axonal degeneration (such as dosazosin, tamsulosin), prevent mitochondrial dysfunction (such as glutathione trisulfides, antioxidants α -lipoic acid), improve calcium imbalance in the internal environment (Shaoyao gancao decoction, N-type voltage-gated calcium channel inhibitor IPPQ), and inhibit neuroinflammation (such as chemokine inhibitors and selective interleukin-8 receptor inhibitors DF2726A). 5.Study on the Mechanism of Hepatotoxicity Induced by Rhubarb Based on Network Pharmacology and Experimental Verification

		                        			Objective The potential mechanism of hepatotoxicity induced by rhubarb was preliminarily explored by network pharmacology and verified by cell experiments.Methods Based on network pharmacology,component collection and target prediction are carried out through multiple databases.PPI network construction,GO enrichment analysis and KEGG pathway analysis were combined with software to systematically predict the mechanism of hepatotoxicity induced by rhubarb.The pathway information predicted by network pharmacology was verified by primary hepatocyte experiments and Western blot experiments.Results The results of network pharmacology showed that RH was the main component of hepatotoxicity induced by rhubarb.Seventeen core targets of hepatotoxicity induced by rhubarb were obtained.KEGG results suggested that DNA damage and apoptosis were one of the key mechanisms of hepatotoxicity induced by rhubarb.The results of primary hepatocytes and Western blot showed that RH could inhibit the viability of primary hepatocytes in a time-dose dependent manner.ABT and SFP can significantly reduce the toxicity of RH on primary liver cells in mice,and RFP can increase the toxicity of RH to mouse primary liver cells.Upregulation of γ-H2AX and PARP-1 protein in primary liver cells of mice after treatment with different concentrations of RH.Conclusion RH in rhubarb can significantly inhibit the viability of mouse primary hepatocytes,and its toxicity to mouse primary hepatocytes is mainly caused by the metabolic activation of RH by CYP 2C9.RH can activate PARP-1 protein,phosphorylate H2AX,induce DNA damage and apoptosis in mouse primary hepatocytes.
		                        		
		                        		
		                        		
		                        	
6.Association of high triglyceride glucose index with increased mortality in peritoneal dialysis:A cohort study

		                        			Objective The objective of this study is to investigate whether there is a correlation between a high TyG index(serum triglyceride glucose index)and higher mortality rates among patients undergoing peritoneal dialysis(PD).Methods This study utilized a single-center retrospective cohort as the basis for its methods..From January 1,2007 to December 31,2015,a total of 519 PD patients kept under observation until December 31,2018.There searchers employed the Kaplan-Meier method and Cox proportional hazards modelsto examine the cor-relation between TyG index levels and mortality.Results Over a period of 40.5 months,104(20.0%)individuals with Parkinson's disease passed away,with 55(52.9%)of these deaths attributed to cardiovascular disease(CVD).The serum median TyG index at baseline was 8.44(6.48,11.94).Through Cox regression analysis subject to the adjustments of such parameters as gender,age,body mass index(BMI),presence of cardiovascular disease,hypertension,diabetes mellitus,hemoglobin,serum albumin,serum Ferritin,total cholesterol,renal residual function(RRF),An increased risk of all-cause mortality(HR = 2.22,95%CI:1.43~3.44,P<0.001)and CVD mortality(HR = 2.50,95%CI:1.34~4.65,P = 0.004)was observed with a higher baseline TyG index(8.44).A comparable impact was observed in the correlation between the average TyG index over time(TA-TyG index)and both all-cause mortality and CVD mortality.(HR = 1.90,95%CI:1.25~2.90,P = 0.003;HR = 2.05,95%CI:1.14~3.70,P = 0.017,respectively).Conclusion PD patients with a higher serum TyG index have a greater risk of all-cause mortality and mortality related to cardiovascular disease.
		                        		
		                        		
		                        		
		                        	
7.Screening and experimental validation of hub genes for myocardial isch-emia-reperfusion injury based on bioinformatics

		                        			AIM:Using bioinformatics analysis methods to identify the hub genes involved in myocardial isch-emia-reperfusion injury(MIRI).METHODS:Firstly,the rat MIRI related dataset GSE122020,E-MEXP-2098,and E-GEOD-4105 were downloaded from the database.Secondly,differentially expressed genes(DEGs)were screened from each dataset using the linear models for microarray data(limma)package,and robust DEGs were filtered using the robust rank aggregation(RRA)method.In addition,the surrogate variable analysis(SVA)package was used to merge all datas-ets into one,and merged DEGs were screened using the limma package.The common DEGs were obtained by taking the intersection of the two channels of DEGs.Next,the protein-protein interaction(PPI)network of common DEGs was con-structed,and the hub genes were identified using the density-maximizing neighborhood component(DMNC)algorithm.The receiver operating characteristic curve(ROC)was plotted to evaluate the diagnostic performance of the hub gene.Then,the mRNA and protein expression levels of hub genes were detected in the rat MIRI model,and the literature re-view analysis was carried out on the involvement of hub genes in MIRI.Finally,the gene set enrichment analysis(GSEA)was performed on hub gene to further reveal the possible mechanism in mediating MIRI.RESULTS:A total of 143 robust DEGs and 48 merged DEGs were identified.After taking the intersection of the two,48 common DEGs were obtained.In the PPI network of common DEGs,5 hub genes were screened out,namely MYC proto-oncogene bHLH transcription fac-tor(MYC),prostaglandin-endoperoxide synthase 2(PTGS2),heme oxygenase 1(HMOX1),caspase-3(CASP3),and plasminogen activator urokinase receptor(PLAUR).The ROC results showed that the area under the curve values for all hub genes were greater than 0.8.MYC,PTGS2,CASP3,and PLAUR showed high mRNA and protein expression in rat MIRI,while there was no difference in mRNA and protein expression for HMOX1.The literature review revealed that among the 5 hub genes,only PLAUR has not been reported to be involved in MIRI.The GSEA results for PLAUR indicat-ed that its functional enrichment mainly focused on pathways such as NOD-like receptor signaling pathway,P53 signaling pathway,Toll-like receptor signaling pathway,apoptosis,and fatty acid metabolism.CONCLUSION:MYC,PTGS2,CASP3,HMOX1,and PLAUR are involved in the pathological process of MIRI.PLAUR is a potential hub gene that can mediate MIRI by regulating pathways such as NOD like receptor signaling,P53 signaling,Toll like receptor signaling,cell apoptosis,and fatty acid metabolism.The results can provide reference for further investigation into the molecular mechanisms and therapeutic targets of MIRI.
		                        		
		                        		
		                        		
		                        	
8.Treatment strategies for human brucellosis

		                        			Brucellosis is a zoonotic infectious disease caused by Brucella infection. So far, animal to animal Brucellosis has not been eradicated, and there is a lack of safe and effective human vaccine. Therefore, "early, combined, sufficient, and full course" drug treatment remains an important strategy in the management of human Brucellosis. The goal of treating brucellosis is to alleviate and shorten the symptom period, reduce complications, relapses, and chronicity. At present, although antibiotic treatment is effective for most patients, there are still some patients who experience treatment failure or later recurrence, so the treatment strategy for brucellosis urgently needs to be optimized. 9.Use of mobile fitness APP in college students and its relationship with healthy lifestyle

		                        			To investigate the use of mobile fitness application (APP) and healthy lifestyles of college students, so as to provide a theoretical basis for improving the health quality of college students and promoting the national scientific fitness.
		                        		
		                        			Methods:
		                        			From August to October 2023, 591 college students from three universities in Jinan and Weifang Cities, Shandong Province were selected by stratified cluster random sampling method, and the online questionnaire was conducted on the mobile fitness APP use behavior and healthy lifestyle. The ttest and analysis of variance were used to assess the differences in healthy lifestyle of college students, and stratified linear regression was used to analyze the effect of mobile fitness APP use on healthy lifestyle.
		                        		
		                        			Results:
		                        			The utilization rate of mobile fitness APP among college students was 82.57% (n=488), and the average score of healthy lifestyles was (3.49±0.66). The differences in students healthy lifestyle scores between different residences, grades, majors, monthly living expenses, annual personal visits, and family medical expenses were statistically significant (t/F=23.82, 4.87, 6.07, 10.17, 3.79, 16.92, P<0.05). The results of stratified regression analysis showed that the overall evaluation of mobile fitness APP, and the recommendation intention of mobile fitness phone APP were the main related factors of healthy lifestyles among college students (β=0.47, 0.06, P<0.05).
		                        		
		                        			Conclusions
		                        			College students have a high degree of use of the mobile fitness APP, and the mobile fitness APP is positively associated with a healthy lifestyle.It should increase the publicity and promotion of mobile fitness APP to promote college students to develop a healthy lifestyle.
		                        		
		                        		
		                        		
		                        	
10.Cases Analysis and Countermeasures Discussion on Ethical Review of Clinical Trial Protocol Violation

		                        			To ensure the rights and safety of the subjects and improve the quality of clinical trials, the author analyzed and discussed the deviation type and typical cases from 184 cases of protocol violation reviewed by the ethics committee in 56 clinical trials in a tertiary hospital in 2020. Among the 184 cases of violating the protocol, there were 29 major protocol violation cases and its proportion is 16%; 99 cases (54%) violated the GCP principle; 56 cases of other violations of the protocol that require to be reported, accounting for 30%. Through the case analysis of the researcher gave the wrong doses to subjects without following the protocol and drug administration did not conform to the rules, analyzed and discussed from the five perspectives of the research protocol design, the researcher, the clinical trial institution, the sponsor and the ethics committee, and put forward solutions and suggestions, so as to provide reference to improve the compliance of clinical trial protocol, reduce the risk of subjects and protect their rights and safety and ensure the successful progress of clinical trials.
