1.Mechanism of Huangqi Gegen Decoction in Treatment of Type 2 Diabetes Mellitus via Intestinal Mucosal Barrier

		                        			ObjectiveTo investigate the mechanism of Huangqi Gegentang (HGT) in the treatment of type 2 diabetes mellitus (T2DM) through the application of proteomic techniques. MethodsThe rat model of T2DM was established by streptozotocin combined with a high-fat, high-sugar diet. Thirty-two male SD rats were randomized into four groups: blank, model, HGT (8.10 g·kg-1·d-1), and positive control (metformin hydrochloride, 76.5 mg·kg-1·d-1). After 6 weeks of drug intervention, the fasting blood glucose level was measured, and an oral glucose tolerance test (OGTT) was performed. The area under the curve (AUC) was calculated. Enzyme-linked immunosorbent assay was performed to assess the level of glycated hemoglobin (GHbA1c) in the serum. The limulus amebocyte lysate assay was employed to measure the serum level of lipopolysaccharide (LPS). Pathological changes in the colon were observed by hematoxylin-eosin staining. The mRNA levels of pro-inflammatory cytokines including tumor necrosis factor (TNF)-α, interleukin (IL)-6, and IL-1β in the colon tissue were quantified via Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR). Additionally, the protein and mRNA levels of zonula occludens-1 (ZO-1), Occludin, and Claudin-1 in the colon tissue were assessed by Western blot and Real-time PCR, respectively. Label-free quantitative proteomics was employed to identify the differentially expressed proteins between the colon tissue samples from the blank, model, and HGT groups. Key proteins identified were subsequently validated by Western blot and Real-time PCR. Finally, bioinformatics analysis was conducted on the differentially expressed proteins. ResultsCompared with the blank group, the model group exhibited increased fasting blood glucose, AUC, and GHbA1c levels (P<0.01), damaged colonic mucosal epithelial structure and inflammatory cell infiltration, up-regulated mRNA levels of TNF-α, IL-6, and IL-1β in the colon and an increase in serum LPS content (P<0.05, P<0.01), and down-regulated protein and mRNA levels of ZO-1, Occludin, and Claudin-1 in the colon (P<0.01). Compared with the model group, the HGT group showed reductions in fasting blood glucose, AUC, and GHbA1c (P<0.01), alleviated damage to the colonic mucosal epithelium, down-regulated mRNA levels of TNF-α, IL-6, and IL-1β in the colon, a reduction in serum LPS content (P<0.05, P<0.01), and up-regulated protein and mRNA levels of ZO-1, Occludin, and Claudin-1 in the colon (P<0.05, P<0.01). Proteomics analysis identified 70 differentially expressed proteins that exhibited a downward trend in the model group relative to the blank group and an upward trend in the HGT group relative to the model group. These findings were corroborated by Western blot and Real-time PCR, which confirmed that the protein and mRNA levels of mucin 2 (Muc2) and transforming growth factor (TGF)-beta receptor 1 (Tgfbr1) in the colon tissue were consistent with the proteomic data. Bioinformatics analysis showed that these 70 differentially expressed proteins identified were significantly enriched in multiple signaling pathways, among which the TGF-β and advanced glycation endproduct (AGE)/receptor for advanced glycation endproduct (RAGE) signaling pathways were closely associated with damage to the intestinal mucosal barrier. This suggests that HGT may ameliorate intestinal mucosal barrier damage by regulating these pathways. ConclusionHGT potentially exerts anti-T2DM effects by influencing AGE/RAGE and TGF-β signaling pathways, thereby contributing to the restoration of the intestinal mucosal barrier. 
		                        		
		                        		
		                        		
		                        	
2.Diagnostic value of 3D reconstruction of CBCT and nine zone method of alveolar bone for ambush teeth of anterior maxilla

		                        			Objective:To explore the diagnostic value of three-dimensional(3D)reconstruction of cone beam computed tomography(CBCT)and nine zone method of alveolar bone for ambush teeth of anterior maxilla.Methods:From January 2022 to January 2023,a total of 80 patients with ambush teeth of anterior maxilla were selected from the Stomatology Department of the Third Medical Center of Chinese PLA General Hospital.All patients sought medical attention due to uneven or missing dental alignment.A total of 88 ambush teeth of anterior maxilla were treated.After alveolar bone was conducted by nine zone method,the ambush teeth were found by nine zone method combined with temporomandibular bone CBCT.The number of ambush teeth of anterior maxilla at the outer-upper,outer-middle,outer-lower,middle-upper,middle-middle,middle-lower,inner-upper,inner-middle and inner-lower at the second tooth root of dental crown,as well as the position of the ambush teeth,dental crown positioning and root curvature,were counted by using statistical method.The diagnostic accuracies of two examination methods for the position of ambush teeth,dental crown positioning and root curvature were compared.The comparison of the rates between the nine zone method of alveolar bone and the research results of CBCT was analyzed by using chi square test and other methods.Results:In the 88 ambush teeth,the dental crowns and roots of 50 teeth were at the same side of adjacent teeth,and these of 5 teeth were at between adjacent teeth,and these of 33 teeth were at the different side of adjacent teeth.The most of dental crowns and roots of ambush teeth located in two regions,which were respectively inner-middle and outer-upper regions(23.86%and 19.32%)of maxilla.The diagnostic results of 88 ambush teeth were consistent with the results of intraoperative findings,with a diagnostic accuracy of 100%.The accuracies of the nine zone classification method of alveolar bone in the positions of ambush teeth and dental crown were significantly higher than those of CBCT,and the differences were statistically significant(x2=4.020,1.340,4.460,4.570,0.900,0.930,0.030,0.040,0.200,P<0.05),respectively.Conclusion:CBCT has the advantages of clearness,intuitiveness,accuracy and convenience in the localization of ambush teeth of the anterior maxilla.The nine zone method of alveolar bone has a significance of precise localization for the treatment of complex ambush teeth in the anterior maxilla,and also has a certain guidance value of diagnosis and treatment.
		                        		
		                        		
		                        		
		                        	
3.Detection of PIK3CA gene mutation and its related prognosis in colorectal cancer based on next-generation sequencing

		                        			Objectives:To investigate the mutation of PIK3CA in colorectal cancer and to analyze their clinicopathological features, and evaluate their role in clinical treatment and prognostication.Methods:A total of 128 paraffin-embbeded tissue samples of colorectal cancer from Shanxi Cancer Hospital from 2018 to 2021 were collected. DNA was extracted from the samples, and next-generation sequencing (NGS) was used to detect PIK3CA mutation. The relationship between PIK3CA mutation, their clinicopathological features, and prognosis were analyzed.Results:Among the 128 colorectal cancer samples, there were 75 males and 53 females; with aged range 32-86 years, median 61.5 years, 27 (21.09%) had PIK3CA mutations. Colorectal cancer with PIK3CA mutation was more likely to occur in male patients ( P=0.007), which was related to tumor site ( P=0.032), tumor size ( P=0.029) and TP53 wild-type ( P=0.001). The common site mutations of PIK3CA mostly occurred in tumors with tumor mutation burden≥10 Muts/Mb ( P=0.031).PIK3CA mutation had no significant effect on the survival prognosis of patients, but the efficacy of anti-angiogenic therapy was poor in these patients. Conclusions:PIK3CA mutation is a common mutation in colorectal cancer and plays an important role in the occurrence and development of colorectal cancer. PIK3CA mutation may lead to resistance to anti-angiogenic drugs in colorectal cancer, but its impact on survival and prognosis to patients needs further study.
		                        		
		                        		
		                        		
		                        	
4.Molecular epidemiological characteristics of carbapenem-resistant Klebsiella pneumoniae in intensive care unit of a hospital in Hunan Province from 2020 to 2021

		                        			Objective To investigate the clinical distribution,antimicrobial resistance,carbapenemase resistance genes,virulence genes,capsular serotypes and ST subtypes of carbapenem-resistant Klebsiella pneumoniae(CRKP)strains in intensive care unit of a tertiary hospital in Hunan Province for better management of CRKP infections.Methods CRKP strains were isolated from 8 intensive care units of the First People's Hospital of Chenzhou City from January 2020 to December 2021.The isolates were identified by matrix-assisted laser desorption/ionization time-of-flight mass spectrometry and tested by VITEK Compact 2 for antimicrobial susceptibility.Carbapenemase phenotype was detected by modified carbapenem inactivation method(mCIM).The capsular serotypes were determined by wzi sequencing.Carbapenem resistance genes and virulence genes were identified by PCR and Sanger sequencing.The strains were also analyzed by multilocus sequence typing(MLST)in terms of ST subtypes.Results The 75 CRKP strains were mainly isolated from geriatric ICU(28.0%)and neurosurgery ICU(20.0%).Overall,6.7%(5/75)and 16.0%(12/75)of the CRKP strains were resistant to tigecycline and ceftazidime-avibactam,respectively.The CRKP strains(>96.0%resistant)were highly resistant to carbapenems,cephalosporins,β-lactam/β-lactamase inhibitor combinations,and levofloxacin.PCR and sequencing analysis found blaKPC-2 gene in 61 strains(81.3%),blaNDM-1 gene in 11 strains(14.7%),blaNDM-5 gene in 1 strain(1.3%),and blaOXA-48 gene in 2 strains(2.7%).MLST revealed that ST11(54.7%,41/75),ST1883(13.3%,10/75),and ST307(6.7%,5/75)were the top three ST subtypes.All ST11 and ST1883 CRKP strains harbored blaKPC-2.KL64(38.7%,29/75)and KL47(25.3%,19/75)were the most prevalent capsular serotypes among the 75 CRKP strains.The most common virulence genes among these CRKP strains were rmpA2(48.0%,36/75),iroN(38.7%,23/75)and iucA(37.3%,15/75).Conclusions The CRKP strains isolated from the intensive care units were mainly ST11-KL64 and ST11-KL47 types.Most of the strains harbor blaKPC-2 and virulence gene,and associated with high level antimicrobial resistance.It is urgent to strengthen the monitoring of molecular epidemiological characteristics of CRKP in order to inform individualized and precision treatment of CRKP infections.
		                        		
		                        		
		                        		
		                        	
5.Study on the mechanism of Inonotus obliquus extract in treatment of Crohn's disease based on proteomics

		                        			Objective To investigate the effect of Inonotus obliquus extract on Crohn's disease and its mechanism by proteomics technology.Methods Crohn's disease(CD)model was induced by 2,4,6-trinitrobenzene sulfonic acid(TNBS).A total of 48 SD male rats were randomized into control,model,Inonotus obliquus low-dose(200 mg/kg),medium-dose(400 mg/kg),high-dose(800 mg/kg)groups,and positive control group(mesalazine,225 mg/kg).The disease activity index(DAI)score and the colonic mucosal injury index(CMDI)score were assessed after one week of drug intervention.HE staining was used to observe the histopathological changes in the colon,and ELISA was used to detect the levels of IL-1β,IL-6,and TNF-α in the serum.Proteins were extracted from the colonic tissues of the control group,model group,and Inonotus obliquus high-dose group,and bioinformatics analysis was performed for the proteins identified by quantitative proteomics.Finally,Western blot and RT-qPCR were employed to verify the key proteins.Results Compared with the model group,the DAI,CMDI and HE staining scores were significantly decreased in the medium and high dose groups(P<0.05 or P<0.01),as well as the levels of inflammatory factors IL-1β,IL-6 and TNF-αin serum(P<0.05 or P<0.01).Proteomic tests showed that there were 199 differentially expressed proteins(DEPs)between the Inonotus obliquus high-dose group and the model group,of which 63 DEPs were related to CD.Bioinformatics analysis showed that these 63 DEPs were mainly involved in NOD-like receptor signaling pathway,calcium signaling pathway,necroptosis,and other pathways.Consistent with proteomic result,expressions of Vdac1 and Trpv2 were confirmed by Western blot and RT-qPCR in colon tissue.Conclusions Inonotus obliquus extract may regulate NOD-like receptor signaling pathway,calcium signaling pathway,and necroptosis by interfering with the expression of Vdac1 and Trpv2,so as to achieve the effect of treating CD.
		                        		
		                        		
		                        		
		                        	
6.Study on therapeutic effect of auricular acupuncture combined with drug in patients with frequent ventricular premature beat

		                        			Objective To compare the efficacies between auricular acupuncture combined with drugs and drug alone in the treatment of frequent ventricular premature beats(FVPB).Methods A total of 51 pa-tients with FVPB admitted to this hospital from January to December 2022 were selected as the study sub-jects,including 27 patients in the treatment group and 24 cases in the control group.The treatment group was given auricular acupuncture combined with oral metoprolol tartrate tablets,the heart and ear points and adre-nal ear points were selected,the two ears were treated alternately and the needles were replaced after 3 days.The control group was only given oral metoprolol tartrate tablets,and the adhesive tape of the same color and material were pasted at the same auricular acupoints.The number of premature beats,the TCM symptom score and clinical efficacy on 1 day before treatment and on 1,6 days after treatment were compared between the two groups.Results The number of premature beats on 1 day after treatment in the treatment group was 733(427,1 637)times,which in the control group was 2 244(1 023,2 808)times.On 6 days after treatment,the number of premature beats in the treatment group was 182(116,321)times,which in the control group was 1 025(678,1 127)times.On 6 days after treatment,the number of premature beats in the treatment group was averagely reduced by 765.49 times compared with the control group(95%CI:-976.41 to-554.56,P<0.001).On 1,6 days after treatment,the TCM symptom scores in the treatment group all were lower than those in the control group,and the difference was statistically significant(P<0.05).The effective rate of the treatment group was 92.59%,which was higher than 83.33%in the control group,and the differ-ence was statistically significant(P<0.05).Conclusion Compared with drug therapy alone,auricular acu-puncture combined with drug therapy for treating FVPB could achieve better clinical efficacy.

		                        			Purpose/Significance To investigate and analyze the current status and influencing factors of digital health literacy(DHL)of the elderly,and to provide references for relevant departments.Method/Process The eHEALS scale is used to investigate the DHL status of 419 elderly people aged 60 and above in Shanghai through convenience sampling,and the influencing factors are explored through binary logistic regression analysis.Result/Conclusion Low education,smoking,and health anxiety are risk factors,while regular physical exami-nation is protective factor.Relevant departments should focus on the elderly people with lower education,and strengthen research and edu-cation on health examinations,smoking hazards,and the alleviation of anxiety.
		                        		
		                        		
		                        		
		                        	
8.An investigation of hepatitis D virus infection among patients with chronic hepatitis B virus infection in some regions of China

		                        			 Objective To investigate the prevalence of hepatitis D virus (HDV) infection among patients with chronic hepatitis B virus (HBV) infection in some regions of China. Methods Serum samples were collected from 3 131 patients with chronic HBV infection in 10 provinces, cities, and autonomous regions of China from March 2021 to June 2022, and anti-HDV IgG ELISA was used for the detection of all serum samples. Nested reverse transcription-polymerase chain reaction (nRT-PCR) was used to detect HDV RNA in anti-HDV IgG-positive samples, and the nRT-PCR amplification products of HDV RNA-positive samples were sequenced and analyzed to determine HDV genotype. The clinical features of anti-HDV IgG-positive patients were analyzed. The Mann-Whitney U rank sum test was used for comparison of continuous data between two groups, and the chi-square test or the Fisher's exact test was used for comparison of categorical data between two groups. Results The positive rate of anti-HDV IgG in the 3 131 patients with chronic HBV infection was 0.70% (22/3 131), and that in the patients with chronic HBV infection in Inner Mongolia Autonomous Region, Xinjiang Uygur Autonomous Region, Beijing, and Hunan Province was 1.81% (16/886), 0.88% (2/226), 0.28% (2/708), and 1.00% (2/200), respectively; the patients with chronic HBV infection in Inner Mongolia Autonomous Region had a significantly higher positive rate of anti-HDV IgG than those in Beijing ( P =0.004), and there was no significant difference between the other regions ( P > 0.05). Clinical features of the patients with chronic HBV infection in Inner Mongolia Autonomous Region showed that compared with the anti-HDV IgG-negative group, the anti-HDV IgG-positive group had a significantly higher proportion of patients with Mongol nationality ( P =0.001), abnormal alanine aminotransferase ( P =0.007), or antiviral treatment ( P =0.029), as well as a significantly lower median HBV DNA level ( P =0.030). A total of 19 HDV RNA-positive samples were identified, all of which had HDV genotype 1. Conclusion The prevalence rate of HDV varies greatly across different regions of China, with a higher prevalence rate of HDV in patients with chronic HBV infection from Inner Mongolia Autonomous Region. 9.Prevalence and clinical characteristics of polymyositis in a Chinese cohort

		                        			Objective:To analyze the frequency and characteristics of polymyositis (PM) in idiopathic inflammatory myopathy (IIM), and to investigate whether PM is over-diagnosed.Methods:Patients diagnosed as IIM according to the Bohan & Peter criteria of IIM hospitalized in the Department of Rheumatology of China-Japan Friendship Hospital from 2008 to 2019 were involved in the study. Definite PM (dPM) was defined as typical clinical and pathological features including elevated creatine kinase (CK) level, muscle weakness and muscle biopsy findings with endomysial CD8 + T cell infiltration and expression of MHC-1 on sarcolemma. Meanwhile, dermatomyositis (DM), anti-synthase syndrome(ASS), immune-mediated necrotic myopathy(IMNM), sporadic inclusion body myositis(sIBM) and other myopathies were excluded according to the new classification criteria of IIM subtypes respectively. Statistical analysis was performed using SPSS software 24.0. The Kruskal-Wallis test and χ2 test were used to compare the clinical characteristics between the dPM group and other IIM subtypes. Results:A total of 1 259 patients with IIM including 1 015 (80.6%) DM and 244(19.4%) PM were enrolled in this study. According to the strict definition of PM criteria, only 0.5% of patients (6/1 259) in IIM could be diagnosed as dPM. Most PM patients were IMNM and ASS according to the new IIM subtypes criteria, of which 48.0% (117/244) were IMNM and 32.0% (78/244) were ASS. 66.7%(4/6) of dPM patients were women. One complicated with RA, and one was dPM overlaped with systemic sclerosis. All of them had muscle weakness, mild elevation of CK level [611(391,1 451) U/L], and were myositis-specific autoantibodies negative. Except one dPM patients who did not receive immunoregulatory therapy due to chronic obstructive pulmonary disease, the others were administrated with low or medium dose prednisone combined with or without immunosuppressive agents. After a median follow-up of (38±26) months, the muscle strength of dPM patients were improved.Conclusion:dPM is a very rare clinical subtype of IIM. PM is an over-diagnosed entity in clinical practice. 		                        			Oral microbial community, as an important part of human microbial community, is closely related to oral and general health. Oral microbiological research has become the forefront of international microbiological research. Standardized and unified nomenclature for oral microorganisms in Chinese is of great significance to support the development of oral medicine research. Standardized translation of microbial names is the basis for writing canonical and authoritative professional textbooks and reference books, which helps students to accurately acquire the characteristics and classifications of oral microbes. Unified translation of oral microorganisms is also conducive to academic communication and cooperation, and plays an important role in oral health education and science popularization, which enables oral microbiology knowledge to be accurately disseminated to the public. Therefore, in order to standardize the words in scientific research, funding application, publications, academic exchanges and science popularization within the field of oral medicine, we have fully discussed and revised the Chinese names of oral microorganisms in 2017 edition and ones of newly discovered oral microbes, finally reaching a consensus to form the 2023 edition of Chinese names of oral microorganisms.
