1.Gut microbiota and osteoporotic fractures

BACKGROUND:Osteoporotic fracture is the most serious complication of osteoporosis.Previous studies have demonstrated that gut microbiota has a regulatory effect on skeletal tissue and that gut microbiota has an important relationship with osteoporotic fracture,but the causal relationship between the two is unclear. OBJECTIVE:To explore the causal relationship between gut microbiota and osteoporotic fractures using Mendelian randomization method. METHODS:The genome-wide association study(GWAS)datasets of gut microbiota and osteoporotic fracture were obtained from the IEU Open GWAS database and the Finnish database R9,respectively.Using gut microbiota as the exposure factor and osteoporotic fracture as the outcome variable,Mendelian randomization analyses with random-effects inverse variance weighted,MR-Egger regression,weighted median,simple model,and weighted model methods were performed to assess whether there is a causal relationship between gut microbiota and osteoporotic fracture.Sensitivity analyses were performed to test the reliability and robustness of the results.Reverse Mendelian randomization analyses were performed to further validate the causal relationship identified in the forward Mendelian randomization analyses. RESULTS AND CONCLUSION:The results of this Mendelian randomization analysis indicated a causal relationship between gut microbiota and osteoporotic fracture.Elevated abundance of Actinomycetales[odds ratio(OR)=1.562,95%confidence interval(CI):1.027-2.375,P=0.037),Actinomycetaceae(OR=1.561,95%CI:1.027-2.374,P=0.037),Actinomyces(OR=1.544,95%CI:1.130-2.110,P=0.006),Butyricicoccus(OR=1.781,95%CI:1.194-2.657,P=0.005),Coprococcus 2(OR=1.550,95%CI:1.068-2.251,P=0.021),Family ⅩⅢ UCG-001(OR=1.473,95%CI:1.001-2.168,P=0.049),Methanobrevibacter(OR=1.274,95%CI:1.001-1.621,P=0.049),and Roseburia(OR=1.429,95%CI:1.015-2.013,P=0.041)would increase the risk of osteoporotic fractures in patients.Elevated abundance of Bacteroidia(OR=0.660,95%CI:0.455-0.959,P=0.029),Bacteroidales(OR=0.660,95%CI:0.455-0.959,P=0.029),Christensenellacea(OR=0.725,95%CI:0.529-0.995,P=0.047),Ruminococcaceae(OR=0.643,95%CI:0.443-0.933,P=0.020),Enterorhabdus(OR=0.558,95%CI:0.395-0.788,P=0.001),Eubacterium rectale group(OR=0.631,95%CI:0.435-0.916,P=0.016),Lachnospiraceae UCG008(OR=0.738,95%CI:0.546-0.998,P=0.048),and Ruminiclostridium 9(OR=0.492,95%CI:0.324-0.746,P=0.001)would reduce the risk of osteoporotic fractures in patients.We identified 16 gut microbiota associated with osteoporotic fracture by the Mendelian randomization method.That is,using gut microbiota as the exposure factor and osteoporotic fracture as the outcome variable,eight gut microbiota showed positive causal associations with osteoporotic fracture and another eight gut microbiota showed negative causal associations with osteoporotic fracture.The results of this study not only identify new biomarkers for the early prediction of osteoporotic fracture and potential therapeutic targets in clinical practice,but also provide an experimental basis and theoretical basis for the study of improving the occurrence and prognosis of osteoporotic fracture through gut microbiota in bone tissue engineering.
2.Effects of liraglutide on cardiovascular metabolism,left ventricular structure and function in NAFLD patients with T2DM

OBJECTIVE To observe the effects of liraglutide on cardiovascular metabolism, left ventricular structure and function of non-alcoholic fatty liver disease (NAFLD) patients with type 2 diabetes mellitus (T2DM). METHODS Totally 351 NAFLD patients with T2DM were enrolled retrospectively, who visited the Department of Endocrinology in our hospital from January 2019 to December 2022. They were divided into control group (196 cases) and observation group (155 cases) according to different treatment regimens. The control group received conventional standard treatment, and the observation group was additionally given Liraglutide injection 0.6 mg/d subcutaneously once a day based on the control group, adjusted to 1.2 mg/d after 7 days. Both groups received regular treatment for more than 12 months. The propensity matching method was used to match the two groups of patients at a ratio of 1∶1. The cardiovascular metabolism indexes and cardiac ultrasound parameters were compared, and the correlation between left ventricular structure, function parameters and cardiovascular metabolism indexes was analyzed. RESULTS After propensity score matching, there was no significant difference in baseline clinical data between the two groups (each 155 cases) before treatment (P>0.05). After 12 months of treatment, the waist circumference, weight, body mass index (BMI), systolic blood pressure (SBP), fasting blood glucose (FBG), glycosylated hemoglobin (HbA1c) and triglyceride (TG) of both groups, as well as the diastolic blood pressure (DBP), total cholesterol (TC), uric acid (UA) and left ventricular mass (LVM) of the observation group, exhibited a significant decrease compared to pre-treatment levels (P<0.05). The high-density lipoprotein cholesterol (HDL-C), estimated glomerular filtration rate (eGFR), and E/A ratio in both groups, as well as the aspartate aminotransferase (AST) in the control group and the left ventricular ejection fraction (LVEF) in the observation group, were all significantly increased compared with before treatment in the same group (P<0.05). Moreover, the improvement of the above indicators (except for TG and SBP) in the observation group was generally more significant than those in the control group (P<0.05). The left ventricular structure and functional parameters (LVM, LVEF, E/A ratio) of the two groups before and after treatment had varying degrees of correlation with the patients’ waist circumference, body weight, BMI, SBP, FBG and HbA1c. Moreover, BMI (observation group: β= 0.229, P=0.004) and SBP (control group: β=0.240, P=0.004; observation group: β=0.226, P=0.007) were independent influential factors for LVM of the patients. CONCLUSIONS Liraglutide combined with conventional standard treatment can effectively control blood glucose in NAFLD patients with T2DM, reduce waist circumference, body weight and blood pressure, improve blood lipid disorders, and protect their cardiac structure and function.
3.Application of tourniquet in amputation for lower limb gangrene

Objective To explore the efficacy and safety of using a tourniquet in amputation for lower limb gangrene. Methods All patients underwent amputation for lower limb gangrene from January,2009 to June,2023 in Beijing Bo'ai Hospital were reviewed,involving 41 patients with a total of 44 limbs,and they were divided into non-tourniquet group(n=28)and tourniquet group(n=16)according to whether a tourniquet was used during surgery.The am-putation field clearness,surgical bleeding,incision healing,reoperation rate within 30 days post-operation,intra-operative blood pressure and heart rate,and operation time were compared. Results The amputation field was clearer in the tourniquet group(χ2=42.385,P<0.001),with less bleeding(Z=-2.082,P<0.05).No tourniquet-related local damages,such as nerve damage and skin injuries,was observed in the limbs using tourniquets.The incidence of grade A of incision healing was not significantly different(χ2=0.028,P=0.624). Conclusion Application of tourniquet can improve the amputation field clearness and reduce bleeding during amputation for lower limb gangrene,without affecting incision healing.
4.Influence of clinical and rehabilitation characteristics on prognosis of diabetic foot amputees:a systematic review

Objective To analyze the impact of different clinical and rehabilitation characteristics on the prognosis of diabetic foot amputees. Methods Related literatures were searched in CNKI,Wanfang Data,PubMed,Cochrane Library and Google Scholar from establishment to August,2024.The literatures were screened and extracted by two researchers independent-ly,the Newcastle-Ottawa Scale(NOS)and the evaluation criteria recommended by Agency for Healthcare Re-search and Quality(US)were used for quality evaluation,and literatures with above medium quality were includ-ed,and a systematic review was conducted. Results A total of 17 articles involving 9 239 subjects were included,in which three were Chinese,and 14 were English.The study designs were case-control study,cohort study and cross-sectional study.They mainly came from the fields of rehabilitation medicine,orthopedics,sports medicine and disability studies,and were published between 1998 to 2023.Clinical and rehabilitation characteristics related to the prognosis of diabetic foot amputees includ-ed amputation level,socioeconomic determinants(educational attainment,economic status,social participation,etc.),psychological states(anxiety,depression,etc.)and physiological factors(age,gender,pain,prosthetic limb usage,and ambulatory capacity,etc.).These different characteristics could affect the quality of life of diabetic foot amputees,and even lead to re-amputation or death. Conclusion Factors of amputation level,socioeconomic status,psychological status and physiological status are impor-tant for poor prognosis in diabetic foot amputees.Controlling the above factors can effectively reduce the re-am-putation rate and mortality,and improve the quality of life in diabetic foot amputees,thus improving their progno-sis,and promoting functional rehabilitation.
5.Feasibility of Deep Learning Algorithm to Optimize the Noise and Texture of Children's Head CT

Purpose To evaluate the image quality improvement of deep learning iterative reconstruction(DLIR)on pediatric head CT images of head injury and to evaluate the performance of DLIR and conventional adaptive statistical iterative reconstruction-veo(ASIR-V)of noise and image texture of CT image in children's head trauma.Materials and Methods A total of 80 cases in Beijing Children's Hospital,Capital Medical University from December 7th to 11th 2020 of children's head low-dose CT were retrospectively selected.Scan voltage was 120 kV.Scan current was 150-220 mA.The raw data were reconstructed into 5 mm thick slice and 0.625 mm thin slice brain window and bone window images.50%ASIR-V and high weight DLIR images(DL-H)were reconstructed,respectively.A 4-point system was used to subjectively evaluate the display of sulcus,brain matter and bone.The number of lesions in each group was counted.The CT value and image noise values of gray matter and white matter were measured,and the contrast to noise ratio was calculated,then measured the blur metric index was measured in the same slice.The differences between the two image reconstruction methods were compared.Results Compared to 50%ASIR-V images,DL-H significantly improved the display ability of the sulcus and ventricles,as well as the display ability of the brain parenchyma(W=5.5-22.2,all P<0.05)in both slice thickness.There was no statistically significant difference in the display ability of the sulcus and ventricles between 5 mm 50%ASIR-V and 0.625 mm DL-H images(W=0.9,2.0,P=0.32,0.05,respectively).In terms of bone display ability,all images could achieve a maximum score of 4.0.A total of 35 lesions were found in 80 patients via 5 mm 50%ASIR-V and DL-H images,including 12 hemorrhagic lesions,1 intracranial gas,9 fractures,and 13 soft tissue swelling.In terms of objective evaluation,the noise level of DL-H images was significantly lower than that of 50%ASIR-V images(t=21.4-35.7,all P<0.05),and there was no statistically significant difference in noise and contrast noise ratio between 5 mm 50%ASIR-V and 0.625 mm DL-H images(t=1.7-2.2,all P≥0.05).The blur metric index showed that DL-H was superior to 50%ASIR-V images(t=6.1,10.0,both P<0.05),and there was no statistically significant difference in blur metric index between 0.625 mm DL-H and 5 mm 50%ASIR-V images(t=2.6,P=0.28).Conclusion DLIR can improve the CT image quality and image texture of children's head trauma,0.625 mm DL-H image quality is close to 5 mm 50%ASIR-V image,which can meet the diagnostic requirements,and possible to further reduce the radiation dose.
6.Promote the high-quality development of gastrointestinal surgery with technological concept innovation

In the past 30 years, gastrointestinal surgery in China has made significant progress, which is reflected in the gradual standardization of clinical diagnosis and treatment, significant improvement in surgical quality, improvement in short-term and long-term postoperative outcomes, and continuous development of high-quality clinical research. At present, the spectrum of disease in gastrointestinal surgery has changed from traditional benign diseases to malignant diseases represented by gastric cancer and colorectal cancer, metabolic diseases represented by obesity and diabetes, and immune diseases represented by inflammatory bowel disease. It is necessary to carry out full-cycle management for patients. In the new era full of opportunities and challenges, surgeons must be driven by innovation in surgical technology, guided by high-quality clinical research and guaranteed by standardized diagnosis and treatment of diseases, to continue to promote the high-quality development of gastrointestinal surgery in China.
7.An exploratory study of the domestic robotic surgical system for general surgery

Objective:To evaluate the safety and efficacy of the domestic robotic surgical system for general surgery.Methods:A prospective single-center, single-arm exploratory study was conducted at Gansu Provincial People's Hospital from Jun 2022 to Oct 2023, enrolling 54 patients undergoing general surgery using domestically produced Toumai? Endoscopic Surgical Robotic System. The primary study endpoint was surgical success rate, and the secondary study endpoints were intraoperative bleeding, operative time, complications, system performance, hospitalization days.Results:In this study, robotic surgery was successfully completed in 52 patients, and in 2 patients undergoing thyroid operation it was converted to open surgery due to bleeding, with a success rate of 96%, no organ injury or death during surgery, and no system failure. The types of surgery included cholecystectomy, radical gastric cancer resection, radical colorectal cancer resection, inguinal hernia repair, partial hepatectomy, total thyroidectomy and choledocho-jejunal anastomosis.Conclusion:The study provides preliminary evidence of the safety and efficacy of the Toumai? Endoscopic Surgical Robotic System for the treatment of general surgical diseases.

The ezrin,radixin,moesin(ERM)protein family plays a pivotal role in cell morphology,migration,and signal transduction.Ezrin,as a prominent member of this family,is highly involved in these processes.Ezrin phosphorylation is particularly crucial,by regulating the interaction between ezrin and the actin cytoskeleton.This interaction is a key mediator of cytotoxicity in host cells infected with Helicobacter pylori,significantly impacting cell morphology.In this review,we comprehensively summarize the multifaceted role of ezrin protein in H.pylori-induced nodular gastritis.We consider the relationships between ezrin's structure,function,signaling pathways,and phosphorylation in the context of nodular gastritis.Moreover,this review highlights the role of ezrin protein as a potential therapeutic target,offering novel insights for the prevention and treatment of nodular gastritis.
10.A Preliminary Study on the Efficacy of Percutaneous Coronary Intervention for Complex Left Main Stem Combined With Chronic Total Occlusion of the Right Coronary Artery

Objectives:to analyze the efficacy of percutaneous coronary intervention(PCI)for complex left main(LM)lesions combined with chronic total occlusion(CTO)of the right coronary artery. Methods:Ninety patients with complex left main lesions hospitalized in Hunan Provincial People's Hospital from January 2019 to December 2022 were consecutively included.According to the coronary angiographic vascular lesions,patients were divided into complex left main lesions combined with right coronary artery CTO(observation group,n=30)and complex left main lesions without right coronary artery CTO(control group,n=60).The baseline clinical data,intraoperative conditions,angiographic results,and postoperative follow-up results of the patients were analyzed and compared between the two groups. Results:Fifty-eight(64.4%)out of the 90 patients were male.There was no statistically significant difference between the two groups in terms of baseline clinical data(all P>0.05),left main lesion condition(P=1.000),left main calcification condition(P=0.249),and preoperative TIMI flow grading(P=1.000).In the comparison between observation group and the control group,intraoperative occurrence of no-reflow(3.3%vs.5.0%,P=1.000),hypotension(10.0%vs.8.3%,P=1.000),pericardial effusion(3.3%vs.0%,P=0.333),the percentage of intravascular ultrasound(IVUS)use(86.7%vs.90.0%,P=0.635),and the use of circulatory assist device(P=0.699),and the proportion of intraoperative coronary spinning(26.7%vs.21.7%,P=0.597)were all similar between the two groups.The median follow-up time was 14.50(11.83,15.85)months,and the differences in the incidence of major adverse cardiovascular events(MACE)such as recurrent angina,acute myocardial infarction,rebleeding,readmission for heart failure,and cardiac death(31.0%vs.32.1%,P=1.000)were not statistically significant between the observation group and the control group. Conclusions:PCI revascularization may be a viable approach for elderly patients with complex LM lesions with multiple underlying disease,and combined right coronary artery CTO,intolerance and reluctance to CABG.
