1.Dosimetric analysis of radiation pneumonitis induced by 125I seed implantation for the treatment of malignant lung tumors

Objective:To investigate the incidence of radiation pneumonitis (RP) induced by 125I seed implantation for the treatment of malignant lung tumors and analyze related dosimetric parameters. Methods:A retrospective analysis was conducted on 31 cases of malignant lung tumors treated with 125I seed implantation from January 2017 to December 2022 at Hebei Provincial Tumor Radioactive Seeds Implantation Diagnosis and Treatment Center. These cases consisted of eight patients with squamous cell carcinoma, 10 patients with adenocarcinoma, and 13 patients with metastatic cancer in other sites. At 1-6 months after treatment, these patients received postoperative chest CT scans, with the efficacy evaluated based on the Response Evaluation Criteria in Solid Tumors version 1.1 (RECIST 1.1), including the objective response rate (ORR) and the disease control rate (DCR). The efficacy of RP was evaluated using the Radiation Therapy Oncology Group (RTOG) criteria. Postoperative dosimetric parameters, including D90 (minimum peripheral dose received by 90% of the target volume), V8 (percentage of lung volume receiving 8 Gy), V32 (percentage of lung volume receiving 32 Gy), and Dmean (mean radiation dose) of the affected lung, were statistically analyzed. The relationships of the RP occurrence with postoperative D90, V8, V32, and Dmean were analyzed by comparison with relevant external radiotherapy data, to identify the parameters that are correlated closely with RP occurrence. Results:All the patients underwent successful surgeries. The postoperative efficacy evaluation after six months showed complete response (CR) in 11 cases, partial response (PR) in 11 cases, stable disease (SD) in eight cases, and progressive disease (PD) in one case, with an overall response rate (ORR) of 71.0%, and a disease control rate (DCR) of 96.8%. Three patients suffered RP, with an incidence rate of 9.7%. Postoperative V8, V32, and Dmean could not serve as predictive indicators for RP. Follow-up observation revealed that three RP cases (3/5) exhibited postoperative D90 exceeding 170 Gy and no RP cases (0/26) showed postoperative D90 below 170 Gy. Conclusions:In the treatment of malignant lung tumors with 125I seed implantation, there is a certain correlation between RP and postoperative D90, while there is no correlation between it and V8, V32, and Dmean.
2.Translation and validation of Chinese version fear of falling avoidance behaviour questionnaire

Objective To introduce and translate the fear of falling avoidance behaviour questionnaire(FFABQ)into a Chinese version and test its reliability and validity.Methods The Brislin translation model was employed to translate and back-translate the original FFABQ.The back-translated FFABQ was modified based on expert consultations and a pre-survey,followed by cultural adaptation evaluation.To assess the reliability and validity of the Chinese version FFABQ(FFABQ-CHN),a convenient sampling method was used to select a total of 269 elderly patients from a general hospital in Xi'an to participate the survey.The item analysis,content validity,exploratory factor analysis,Cronbach α coefficient,split-half reliability,test-retest reliability and paired t-test were employed in the evaluation of the reliability and validity of FFABQ-CHN.Results The Chinese version FFABQ comprised 3 dimensions of walking,daily activities and work/leisure activities,with 14 items in total.A total of 254 patients had responded to the survey.Item analysis revealed that CR values of the 14 items ranged from 3.978 to 11.755(all P<0.05),with correlation coefficients between each item and the total score of the questionnaire from 0.473 to 0.930(all P<0.001).The content validity index at the item level ranged from 0.834 to 1.000,while the content validity index at the scale level was 0.968.Exploratory factor analysis extracted three common factors,with a cumulative variance contribution rate of 85.347%.The Cronbach α coefficient,half-half reliability and test-retest reliability of the questionnaire were 0.929,0.877,and 0.954,respectively.Paired t-test indicated no significant difference between the scores(all P>0.05).Conclusions The Chinese version FFABQ demonstrates strong a reliability and validity in the assessment of activity restriction related to fear of falling in the elderly.Thus,it serves as an effective tool for medical practitioners in evaluation of the restrictions in fear-of-falling-related activities of the elderly.

Wumei Pills can play a role in treating digestive system diseases through antioxidant stress, Inhibiting inflammatory response, regulating intestinal flora, anti-tumor, promoting intestinal mucosal repair, etc. Most of the related research on the mechanism of Wumei Pills in the treatment of digestive system diseases were studied in a single pathway. The relationship between other mechanisms and pathways is worthy of further exploration. In addition, the current research mostly focuses on the lower digestive system diseases with diarrhea as the main manifestation, and there is still room for research and reporting in the treatment of upper digestive system diseases.
4.Research Progress of Pyroptosis in Anti-tumor Nanodelivery System

Pyroptosis is a kind of inflammatory cell death mediated by the Gasdermin family,which has made great progress in tumor therapy in recent years.Basing on that cell pyroptosis can activate the anti-tumor immune response,and tumor immunotherapy is a new field of tumor therapy,the regulation of cell pyroptosis exhibits great potential for tumor therapy.Meanwhile,nanotechnology is the key means of tumor precision treatment with the advantages of precise targeting and continuous release.Based on these current situations,this paper summarizes the drugs that activate pyroptosis and the nanocarriers that use nanotechnology to promote pyroptosis to participate in tumor therapy,and summarizes the mechanism and application of their action on pyroptosis.This paper is aimed to provide certain references for anti-tumor therapy based on pyroptosis.
5.Synergistic antimicrobial effect of demethylzeylasteral and meropenem against NDM-1-positive Escherichia coli

This study aims to investigate the synergistic antimicrobial effect of demethylzeylasteral meropenem against NDM-1-positive Escherichia coli.Firstly,the nitrocefin hydrolysis assay and molecular docking assay were used to determine the inhibitory effect and mechanism of demethyl-zeylasteral on the hydrolytic activity of the NDM-1 protein.Secondly,the synergistic bacteriostatic effect of demethylzeylasteral with the carbapenem antibiotic meropenem was confirmed by the checkerboard minimum inhibitory concentration assay,growth curve assay,and inhibition zone as-say.The synergistic bactericidal effect of the combination was further determined by the time-kill-ing assay,live/dead bacterial staining assay,and membrane integrity assay.Finally,the G.mellonel-la infection model demonstrated the protective effect of demethylzeylasteral and meropenem.The results indicated that demethylzeylasteral significantly inhibited the hydrolytic activity of NDM-1(IC50=0.32 mg/L)and competitively affected the binding of NDM-1 to meropenem.In vitro tests showed that demethylzeylasteral had good a synergistic antibacterial effect with meropenem and could exert a synergistic bactericidal effect by enhancing the membrane damage of bacteria caused by meropenem.In vivo assays demonstrated that demethylzeylasteral increased the protective effect of meropenem from 67%to 80%.The results obtained in this study provide a therapeutic strategy and antibacterial synergist for treating NDM-1-positive bacterial infections.

Cardiovascular disease(CVD)is the leading cause of death in humans and has received widespread attention.The clinical indicators for predicting the risk of CVD occurrence and development are expensive and difficult to popularize in grassroots areas.Triglyceride glucose(TyG)index,as an alternative biomarker for insulin resistance,can predict the risk of CVD.The clinical TyG index is easy to obtain and is gradually receiving attention.This article reviews the research progress on the correlation between TyG index and CVD risk.
7.Application of optical surface monitoring system guided volumetric modulated arc therapy in total body irradiation

Objective To establish a novel clinical application process of the optical surface monitoring system(OSMS)guided volumetric modulated arc therapy(VMAT)for total body irradiation(TBI),and to assess the accuracy and effectiveness of OSMS in inter-fractional auxiliary positioning before radiotherapy and real-time monitoring of intra-fractional motion during radiotherapy.Methods A retrospective analysis was conducted on 15 leukemia patients who underwent OSMS-guided VMAT-TBI before hematopoietic stem cell transplantation.CT simulation positioning was performed,and the whole-body image data which were collected in head-first supine position(HFS)and feet-first supine position(FFS)were transmitted to the treatment planning system for image registration,multicenter VMAT planning and dose verification.The prescription dose was 800 cGy in 4 fractions twice daily.OSMS was used to assist positioning before delivery,and CBCT was used for position verification.During treatment,OSMS was used for monitoring.The intra-fractional error monitored by OSMS in real time was obtained by analyzing the offline log files.Results The mean dose and coverage of the target area in HFS plan were(905.4±19.0)cGy and 93.0%±2.8%.The mean doses to lung and kidney were(603.7±55.7)cGy and(600.4±49.6)cGy,respectively,and the maximum dose to the lens was(393.9±58.9)cGy.The mean dose and coverage of the target area in FFS plan were(888.5±58.9)cGy and 94.0%±3.2%;and the maximum dose at the junction was(1148.9±72.9)cGy.Fractional treatment delivery time was(75.1±15.1)min.OSMS-assisted positioning was carried out before delivery,and the total deviations of CBCT three-dimensional vector in translational and rotation directions were(2.71±1.96)mm and 0.91°±0.90°,respectively.The three-dimensional vector deviation of the intra-fractional motion amplitude in translational direction monitored by OSMS during the treatment was(1.95±1.88)mm,of which the deviation within 1 mm accounted for 57.5%,79.7%and 62.1%in longitudinal,lateral and vertical directions,respectively.The three-dimensional vector deviation in rotation direction was 0.76°±0.72°,of which the deviation within 1°accounted for 93.1%,85.7%and 94.3%in rotation,pitch and roll directions,respectively.Conclusion VMAT simplifies TBI process,while improving target coverage and organs-at-risk sparing.The use of OSMS can reduce positioning errors,especially rotation errors.In order to ensure the accurate implementation of TBI and the safety of patients,it is necessary to use OSMS for auxiliary positioning and intra-fractional position monitoring.
8.Study on the influence of reconstruction algorithms of image on the image quality and precision of automatic registration of imaging system with megavolt grade

Objective:To compare and study the improvement of different iterative reconstruction(IR)algorithms of the tomotherapy(TOMO)Radixact system on the image quality of megavoltage computed tomography(MVCT)imaging system,and the influence of that on the precision of automatic registration,and to explore the reconstruction algorithm that is suitable for clinical application.Methods:Using the MVCT imaging system to respectively scan the Tomo-Phantom HE phantom and the Catphan 604 phantom,and to analyze three groups of images were generated by three kinds of reconstruction algorithms,which included the Standard(STD)algorithm,IR General(IR-G)algorithm and IR Soft Tissue(IR-ST)algorithm,in MVCT image.The noise index(NI),uniformity index(UI)of image,modulation transfer function(MTF),low contrast visibility(LCV)index and contrast-to-noise ratio(CNR)of three groups of images were calculated respectively.The Lucy phantom was used to test the accuracy of automatic registration algorithm.The registration data of four dimensions,included left-right(X-axis),head-foot(Y-axis),vertical(Z-axis)and free rotation(Roll)around the Y-axis,were used to conduct verification analysis for the influences of them on the precision of automatic registration.Results:The NI values of IR-G and IR-ST reconstruction algorithms were respectively 39.58±0.10 and 14.62±0.26,which were better than 39.58±0.10 of STD algorithm,and the UI values of them were respectively 19.87±0.83 and 15.84±2.51,which were better than 24.51±1.81 of STD algorithm,and LCV values of them were respectively 2.50±0.03 and 1.74±0.11,which were better than 3.67±0.04 of STD algorithm.All of them appeared significant increase,but the resolution with high contrast of MTF were respectively 0.23 and 0.21,which were lower than 0.32 of STD.The overall image quality of the IR algorithms was superior to that of the STD algorithm.In the accuracy test of the automatic registration algorithm,the registration precisions of IR-G and IR-ST algorithms on Y-axis were respectively(0.360±0.142)mm and(0.245±0.050)mm,which were significantly higher than 0.145±0.136 of STD algorithm,and the differences of them were significant(Z=6.0,15.0,P<0.05).The differences of registration precisions of other directions were not significant(P>0.05).Conclusion:The IR algorithm has advantages in terms of noise,uniformity and resolution with low-contrast,however,it shows reduction on resolution with high-contrast.This reduction of resolution with high-contrast do not lead to the decrease of the precision of automatic registration.
9.Investigation and analysis of clinical characteristics and curative prognosis of patients with upper gastrointestinal ulcer in Lhasa region

Objective:To investigate the clinical characteristics,and diagnostic and curative performance of patients with peptic ulcer disease(PUD)in Lhasa region.Methods:A retrospective analysis was conducted on 237 PUD patients who received diagnosis and treatment at Lhasa People's Hospital between September 2017 and December 2019.The general information of demographics,clinical presentations,treatment and outcomes of patients were investigated and analyzed.Results:In the PUD patients of this region,180 cases were male patients(75.9%),which was more than 57 female patients(24.1%),and 89 cases(37.6%)had smoking history with long term and 70 cases(29.5%)had history of heavy drinking.In the common comorbidities,39 cases(16.5%)were chronic liver disease,24 cases(10.1%)were hypertension,and 8 cases(3.4%)were arthritis(gout).Clinical symptoms presented in 234 PUD patients(98.7%),and the most common symptoms were respectively abdominal pain(200 cases,84.4%),hematemesis or melena(122 cases,51.5%),heartburn(90 cases,38.0%),and abdominal distension(84 cases,35.4%).The number of patients with multiple ulcers was 131 cases(55.3%),which was more than that of patients with single ulcers(106 cases,44.7%).The distribution of ulcer locations,from most to least frequent,was as follows:gastric angle(124 cases,52.3%),gastric body(72 cases,30.4%),antrum(69 cases,29.1%),pylorus(48 cases,20.3%),duodenal bulb(29 cases,12.2%)and gastric fundus(14 cases,5.9%).In the 235 patients whose treatment plans could be tracked,229 cases(97.4%)received standard acid-suppressive therapy and gastric mucosal protectant.In the 237 PUD patients,166 cases received follow-up,and 161 PUD patients provided feedbacks for symptom.The result showed that the symptoms of 158(98.1%)patients were alleviated or disappeared.In the 166 patients who received follow-up,49 cases underwent reexamination of endoscopy,and 11 cases(22.4%)of them appeared recurrence of ulcer.During the period of study,7 patients(4.2%)of 166 patients who received follow-up re-admitted to hospital due to PUD,and 7 patients(4.2%)died,and the main reasons of death involved to gastrointestinal bleeding,gastric cancer,stomach-related diseases,liver cancer,and coronary heart disease.Conclusion:The result of this study provided important information of clinical characteristics of PUD in the Lhasa region.The prognostic research of that is helpful to better understand the epidemiological characteristics and the status of clinical management for PUD in this region.However,this study still need larger sample size and further research to confirm and expand these results.
10.Optimization of immobilization and positioning methods in hypofractionated stereotactic radiotherapy for brain metastases

Objective:To compare translational and rotational setup errors between immobilization with open masks combined with positioning with the optical surface monitoring system (OSMS) and immobilization with full masks combined with positioning with laser lights and mask markers, find the advantages of open masks combined with OSMS in hypofractionated stereotactic radiotherapy (HSRT) for brain metastases, and calculate planning target volume (PTV) expansions with different immobilization and positioning method for patients with brain metastases.Methods:The setup data of 55 patients with brain metastases who received HSRT were analyzed retrospectively. According to immobilization and positioning method, the patients were divided into group A (OSMS + open masks), group B1 (full head-neck-shoulders masks + polyurethane foam cushions), and group B2 (full head-neck-shoulders masks + standard headrests). Positioning was directed by automatic couch motion in OSMS in group A and by laser lights and mask markers in groups B1 and B2. Cone beam computed tomography (CBCT) scans were registered using the bone registration method to obtain setup errors in six directions ( x, y, z, roll, pitch, yaw). PTV expansions were calculated according to the van Herk formula. Results:A total of 288 sets of CBCT registration data were acquired. Among three groups, group A showed the smallest mean setup errors and 3D vector error, which were (0.47±0.33) mm, (0.49±0.31) mm, (0.44±0.31) mm, (0.42±0.32)°, (0.48±0.31)°, (0.42±0.22)°, and (0.90±0.39) mm, respectively. Group A differed significantly from group B1 and group B2 in the errors at all directions ( P < 0.05) except for the yaw direction compared with group B1 ( P > 0.05). Group A had no setup error ≥2 mm in translational directions or ≥2° in rotational directions. Group B1 showed significantly smaller setup errors in the y, z, and yaw directions and 3D vector error than group B2 ( P < 0.05). In group A, PTV expansions in three directions ( x, y, and z) were 1.32, 1.19, and 1.22 mm, respectively, which were smaller than those of the other two groups. Conclusions:In HSRT for patient with brain metastases, compared with full head-neck-shoulders masks combined with laser lights and mask markers, open masks combined with OSMS can significantly improve setup precision in six directions and reduce repeated setup and PTV expansions, which shows promise for clinical application.
