1.Application of"rotation-correction loop technique"in the retrieval of complex inferior vena cava filters

Objective To discuss the application of the"rotating guidewire and correcting the filter recovery hook direction technique"("rotation-correction loop technique"for short),a technique invented by the authors in clinical practice,in the retrieval of complex inferior vena cava filter(IVCF),and to discuss its technical skills and advantages.Methods The clinical data of 417 patients carrying an IVCF,who were admitted to the Department of Vascular Surgery of Second Hospital of Shanxi Medical University of China to retrieve IVCF between January 2022 and December 2022,were retrospectively analyzed.Taking the time spent on the retrieval of IVCF and the intraoperative radiation dose as the evaluation indicators,the advantages and disadvantages of the standard filter retrieval technique,the"rotation-correction loop technique"and the other loop-assisted techniques were compared.Results Both the intraoperative radiation dose and the time spent on the retrieval of IVCF using"rotation-correction loop technique"were remarkably lower than those of other loop-assisted techniques(P<0.000 1).Conclusion For the retrieval of complex IVCF,especially for the IVCF which is heavily tilted and/or its recovered hook is attached to the vascular wall,the use of"rotation-correction loop technique"can shorten the time spent on the the retrieval of IVCF and reduce the intraoperative radiation dose.This technique carries high safety and practicability,the device is simple and it can be manipulated by single physician,which is conducive to clinical application and promotion.(J Intervent Radiol,2024,33:289-294)
2.Experience of LIN Yi in Disease-Syndrome-Syptom Differentiation and Treatment of Breast Cancer in Consolidation Phase

This article summarized the clinical experience of Professor LIN Yi in applying the idea of disease-syndrome-syptom combination to the differentiation and treatment of breast cancer in consolidation phase. It is believed that the fundamental pathogenesis of breast cancer in consolidation phase is deficiency of healthy qi, so the treatment should emphasize on reinforcing healthy qi as the main and eliminating pathogen as the supplement. Commonly, Sijunzi Decoction (四君子汤), Shenling Baizhu Powder (参苓白术散), and Liuwei Dihuang Decoction (六味地黄汤) are used as the basic formula to strengthen the spleen and kidney to reinforce healthy qi, and Baihua Sheshe Cao (Scleromitrion diffusum [Willd.] R.J.Wang), Yiyiren (Coix lacryma-jobi L.), and ezhu (Curcuma aromatica Salisb.) are used to eliminate cancer toxin. It is also believed that the biological characteristics of different molecular subtypes of breast cancer are of great significance to the syndrome differentiation and treatment in traditional Chinese medicine, therefore, the formulas used in clinical practice are often selected taking into considerations of molecular subtypes of breast cancer. For those with positive sex hormone receptor, the most important thing is to replenish the congenital and benefit kidney essence, with modified Baihua Qiqi Decoction (白花芪杞汤); for those with negative sex hormone receptor, the first step is to strengthen spleen and replenish kidneys, and it is especially important to strengthen the spleen with Baihua Qiling Decoction (白花芪苓汤). At the same time, it is suggested that the primary and the secondary symptoms of patients should be differentiated, and the diseases and symptoms, as well as the modern medical laboratory indicators should be combined to give suitable medication.
3.Chinese Medicines Treat Rheumatoid Arthritis via Gut-joint Axis: A Review

Rheumatoid arthritis (RA) is a systemic autoimmune disease with local joint pain as the main clinical manifestation. It is one of the diseases specifically responding to traditional Chinese medicine (TCM). The occurrence of RA is not only related to innate factors like genetic disorder but also associated with environmental factors, such as diets and microbial infection. The intestine, a vital human organ with digestive and immune functions, is a place where microorganisms colonize and exert intestinal metabolism-improving, barrier-protecting, and immunomodulatory effects. As the research on the onset and treatment of RA is deepening, the potential relationship of intestinal structural and functional abnormalities with the pathogenesis and progression of RA has been revealed. As clinical and experimental studies indicated, joint inflammation coexists with the impaired barrier function, imbalanced immune cells, and disordered gut microbiota. The theory of the gut-joint axis in the pathogenesis, progression, and treatment of RA is highly consistent with the holistic view in TCM. The recent pharmacological studies have shown that Chinese medicine prescriptions and active components can inhibit inflammation, protect joints, and maintain the intestinal function. This article summarizes the basic connotation of the gut-joint axis in RA and the mechanism by which TCM protect the intestinal barrier and modulate the immunity by regulating the gut microbiota structure and improving microbial metabolism in the treatment of RA. This review gives insights into the future research on the gut-joint axis in RA.
4.Application of Cardiac Magnetic Resonance in the Evaluation of Left Atrium

As the continuation of the left ventricle,the left atrium and left ventricle interact and play an important role in the function of the whole heart.At present,there are many techniques to evaluate the atrial structure and function,but the left atrial structure is complex and the myocardium is thin,which brings some challenges to the relevant evaluation.This paper introduces the parameters,precautions and relevant clinical applications in the process of left atrial evaluation from the aspects of myocardial strain and delayed enhancement.
5.Dose analysis of ciprofol combined with fentanyl in patients undergoing gastroscopy

Objective To analyze the application dose of ciprofol combined with fentanyl in patients undergoing gastroscopy.Method 99 gastroscopy patients from June 2021 to June 2023 were selected as the study objects,and divided into three groups with 33 cases each by random number table method.Group A was given 0.3 mg/kg ciprofol+1.5 μg/kg fentanyl anesthesia,group B was given 0.4 mg/kg ciprofol+1.5 μg/kg fentanyl anesthesia,group C was given 0.5 mg/kg ciprofol+1.5 μg/kg fentanyl anesthesia.Anesthesia,examination,electroencephalogram bispectral index(BIS),mean arterial pressure(MAP),heart rate(HR)and adverse reaction were compared among all groups.Results The success rate of the first anesthesia in Group A was lower than that in Group B and Group C,the gastroscopy examination time was longer than that in Group B and Group C,and the awakening time and hospital leave time were shorter than those in Group B and Group C,the awakening time and hospitalization time of Group B were shorter than those of Group C,the BIS of Group A at time points T2 to T5 was higher than that of Group B and Group C,the MAP and HR levels at time points T2 to T5 in Group A were lower than those in Group B and Group C,the incidence of body movement and coughing in Group A was higher than that in Group B and Group C,the incidence of hypotension and nausea and vomiting in Group C was higher than that in Group A and Group B,and the differences were statistically significant(P<0.05).One patient in Group A and one patient in Group C stopped gastroscopy due to hypoxemia,Group B did not experience hypoxemia,and the difference was not statistically significant(P>0.05).There were 4 cases in Group A,6 cases in Group B and 10 cases in Group C who received intervention with vasoactive drugs due to hypotension,and the difference was statistically significant(P<0.05).Conclusion Patients undergoing gastroscopy which should use 0.4 mg/kg ciprofol+1.5 μg/kg fentanyl anesthesia can achieve better anesthesia effect,shorten examination time and hospitalization time,stabilize vital signs,and reduce adverse reaction incidence rate.
6.Effects of Different Acceleration Factors of Compressed Sensing on the Imaging Quality of Cardiac Magnetic Resonance Cine Sequences

Purpose To explore the effect of compressed sensing(CS)technique with different acceleration factors on the quality of cardiac magnetic resonance cine sequences and feasibility of clinical application.Materials and Methods From January to July 2021,40 healthy volunteers were prospectively recruited for cardiac magnetic resonance cine imaging in Hefei First People's Hospital.Four scan protocols of volunteers were designed:SENSE 2 and CS-cine sequences with acceleration factors of 3,4 and 8.The imaging slices include four chamber heart,short axis heart,left ventricular two chamber heart and three chamber heart.Subjective score of image quality,left ventricular cardiac function and 16-segment myocardial thickness of SENSE 2 and CS3,CS4 and CS8 were compared and analyzed.Results Taking SENSE 2 image as the standard score of 5,the scores of CS3 and CS4 were above 3,and the scores of CS8 were below 3.There was no significant difference between the scores of four chamber heart and short axis heart images of CS3 and SENSE 2 sequence(all P>0.05).There were statistically significant differences in the subjective scores of image quality between the left ventricular two chamber heart and three chamber heart groups of each sequence(all P<0.05).There was no statistically significant difference in left ventricular function(left ventricular ejection fraction,left ventricular end systolic volume,left ventricular end diastolic volume,left ventricular stroke volume,left ventricular end-diastolic mass)(F=0.027,0.182,0.057,0.140,0.545)and myocardial thickness(F=0.052-7.366)among the four imaging schemes(all P>0.05).Conclusion Cardiac cine based on compressed sensing technology have good application prospects.With the increase of acceleration factors,the scanning time gradually decreases,and the corresponding image quality will also decrease.When the acceleration factor is 4,that is,the scanning time of the conventional cine sequence is reduced by 50%,the left ventricular function and myocardial thickness can still be accurately measured,and the image quality basically meets the diagnostic requirements.
7.Analysis of the Incidence of Lower Limb Deep Venous Thrombosis and Its Related Risk Factors in the Postoperative Patients with Lung Cancer.

BACKGROUND:
Lung cancer is the first leading cause of morbidity and mortality among the malignant tumors, which has become a hot issue in current research. Clinically, lung cancer is divided into small cell lung cancer (SCLC) and non-small cell lung cancer (NSCLC) according to the pathological types. NSCLC includes adenocarcinoma, squamous cell carcinoma and other types of lung cancer, accounting for about 80% of all lung cancer. Venous thromboembolism (VTE) includes deep venous thrombosis (DVT) and pulmonary embolism (PE), which is a recognized complication in lung cancer patients with higher morbidity and mortality. The aim of this study is to determine the incidence of DVT and reveal the risk factors for DVT in the postoperative patients with lung cancer.
METHODS:
We collected 83 postoperative patients with lung cancer admitted to the Department of Lung Cancer Surgery, Tianjin Medical University General Hospital from December 2021 to December 2022. All these patients were examined by color Doppler ultrasound of lower extremity vein upon admission and after operation to analyze the incidence of DVT. In order to explore the possible risk factors for DVT in these patients, we further analyzed the correlations between DVT and their clinical features. At the same time, the changes of coagulation function and platelet were monitored to investigate the value of blood coagulation in the patients with DVT.
RESULTS:
DVT occurred in 25 patients after lung cancer operation, and the incidence rate of DVT was 30.1%. Further analysis found that the incidences of postoperative lower limb DVT were higher in lung cancer patients of stage III+IV or over 60 years of age (P=0.031, P=0.028). D-Dimer level in patients with thrombosis was significantly higher than that in non-thrombus patients on the 1st, the 3rd, and the 5th day after operation (P<0.05), and there was no significant difference in platelets and fibrinogen (FIB) (P>0.05).
CONCLUSIONS
The overall incidence of DVT in our center after lung cancer patients operation was 30.1%. Late-stage and older postpatients were more likely to develop DVT, and these patients with higher D-Dimer values should be considered the possibility of VTE events.

8.Immunological effect and influencing factors of HAART on HIV/AIDS with baseline CD4 below 350 cells/mm3

Objective To analyze the immunological effect and its influencing factors of HAART on HIV/AIDS with a baseline CD4 of less than 350 cells/mm3 in Hubei Province during 2011-2020. Methods The treatment information of people living with HIV/AIDS (PLWH) was collected through China Disease Control and Prevention Information System, and the change of CD4 mean value was analyzed by Wilcoxon sign rank sum test. Chi square test and multivariate logistic regression were used to analyze the factors influencing the immunological effect. Results The mean value of CD4 cell count at baseline was (172.78±107.65) cells/mm3 before treatment and (398.85±206.71) cells/mm3 in the fifth year after treatment. Patients with baseline CD4<50 /mm3 had the largest increase in CD4 mean value after treatment (P<0.05). Multivariate logistic regression analysis showed that Age ≥35 years, heterosexual transmission, time interval from diagnosis to treatment≥1 month, baseline CD4<50 /mm3, treatment time 7-<18 months, and drug failure were the risk factors for the success of antiviral therapy(P<0.05). Conclusion The immunological effect of HIV/AIDS antiviral therapy in Hubei Province is affected by many factors. It is necessary to strengthen the implementation of early detection and early treatment strategy and promote treatment compliance education for patients aged ≥35 years.
9.Preliminary Recommendations on the Timing of Lung Surgery after Novel Coronavirus Infection in Patients with Pulmonary Nodules and Lung Cancer.

In recent years, the corona virus disease 2019 (COVID-19) pandemic has had a huge impact on the global medical, political and economic fields. Since the beginning of the COVID-19 epidemic, our understanding of the impact of COVID-19 has grown exponentially. Recently, the COVID-19 epidemic has changed rapidly in China, and there has been controversy over how to carry out surgical operations for patients with lung neoplastic lesions. Some studies have shown that lung cancer patients undergoing surgery are more likely to experience respiratory failure and perioperative death after contracting COVID-19 than the general population, however, delays in cancer treatment are also associated with increased mortality among these patients. In particular, the novel coronavirus Omikron variant has a higher transmissibility and may escape the immunity obtained through the previous novel coronavirus infection and vaccination. In order to minimize the risk of novel coronavirus infection in surgical patients, it is necessary to develop new treatment guidelines, expert consensus and preventive measures. However, the current rapid change of the epidemic situation has led to insufficient time and evidence to develop guidelines and consensus. Therefore, thoracic surgeons need to evaluate specific patient populations at higher risk of severe complications before surgery and weigh the benefit of surgical treatment against the risk of novel coronavirus infection. We try to give some recommendations on lung surgery during the current domestic epidemic situation based on the guidelines and consensus of oncology and thoracic surgery organizations in different regions on lung surgery.

10.Trend analysis of disease burden of gastric cancer in Chinese residents from 1990 to 2019

Objective To analyze the trend of disease burden of gastric cancer in Chinese residents from 1990 to 2019 and to provide reference for the prevention and control of gastric cancer in China. Methods The crude incidence and crude mortality rates of gastric cancer in Chinese residents of different genders from 1990 to 2019 were calculated from the Global Burden of Disease Study (GBD) database, and the standardized incidence and mortality rates were calculated to describe the trend of incidence and mortality of gastric cancer in Chinese residents. Disability Adjusted Life year (DALY) and years lived with Disability (DALY) were used. YLD, years of life lost (YLL) and other indicators were used to analyze the age and gender distribution of the disease burden of gastric cancer in Chinese residents. Joinpoint regression model was used to analyze the trend of the disease burden of gastric cancer. Results From 1990 to 2019, the incidence and crude incidence of gastric cancer increased, while the standard incidence decreased. The number of deaths was on the rise, the crude mortality rate and the standard mortality rate were both decreased, the morbidity and mortality index values of male were higher than that of female. The average annual average of the standardized DALY rate and the standardized YLL rate in China decreased by 2.30% (AAPC= -2.30%, P < 0.001), while the trend of the standardized YLD rate was not statistically significant (P=1.000). The average annual decrease rate of standardized DALY rate, standardized YLD rate and standardized YLL rate for males was smaller than that for females. In 2019, the peak rates of normalization of disease burden indicators were mainly distributed in the 55-74 age group. Conclusion From 1990 to 2019, the disease burden of gastric cancer in China showed a downward trend, and males and middle-aged and elderly people were the key prevention and treatment groups.
