In recent years, the corona virus disease 2019 (COVID-19) pandemic has had a huge impact on the global medical, political and economic fields. Since the beginning of the COVID-19 epidemic, our understanding of the impact of COVID-19 has grown exponentially. Recently, the COVID-19 epidemic has changed rapidly in China, and there has been controversy over how to carry out surgical operations for patients with lung neoplastic lesions. Some studies have shown that lung cancer patients undergoing surgery are more likely to experience respiratory failure and perioperative death after contracting COVID-19 than the general population, however, delays in cancer treatment are also associated with increased mortality among these patients. In particular, the novel coronavirus Omikron variant has a higher transmissibility and may escape the immunity obtained through the previous novel coronavirus infection and vaccination. In order to minimize the risk of novel coronavirus infection in surgical patients, it is necessary to develop new treatment guidelines, expert consensus and preventive measures. However, the current rapid change of the epidemic situation has led to insufficient time and evidence to develop guidelines and consensus. Therefore, thoracic surgeons need to evaluate specific patient populations at higher risk of severe complications before surgery and weigh the benefit of surgical treatment against the risk of novel coronavirus infection. We try to give some recommendations on lung surgery during the current domestic epidemic situation based on the guidelines and consensus of oncology and thoracic surgery organizations in different regions on lung surgery.

Early screening based on computed tomography (CT) pulmonary nodule detection is an important means to reduce lung cancer mortality, and in recent years three dimensional convolutional neural network (3D CNN) has achieved success and continuous development in the field of lung nodule detection. We proposed a pulmonary nodule detection algorithm by using 3D CNN based on a multi-scale attention mechanism. Aiming at the characteristics of different sizes and shapes of lung nodules, we designed a multi-scale feature extraction module to extract the corresponding features of different scales. Through the attention module, the correlation information between the features was mined from both spatial and channel perspectives to strengthen the features. The extracted features entered into a pyramid-similar fusion mechanism, so that the features would contain both deep semantic information and shallow location information, which is more conducive to target positioning and bounding box regression. On representative LUNA16 datasets, compared with other advanced methods, this method significantly improved the detection sensitivity, which can provide theoretical reference for clinical medicine.

Objective:To explore the relationship between human papilloma virus (HPV)-DNA and cytomegalovirus (CMV)-DNA virus content and the risk factors of endometrial cancer (EC) and their prediction models.Methods:Fifty-eight patients with EC from January 2017 to June 2020 in Suzhou Hospital Affiliated to Anhui Medical University were selected as the observation group, and 58 patients with endometrial hyperplasia (EH) during the same period were selected as the control group according to the 1∶1 matching case control principle. The clinical data, HPV-DNA, CMV-DNA positive rate and virus content of the two groups were compared, and the independent variables with P<0.05 were included in the Logistic regression model through single factor analysis. The risk factors of EC were analyzed, and the correlation between HPV-DNA, CMV-DNA and EC risk factors were evaluated. Logistic regression model was evaluated using the likelihood ratio chi-square and goodness-of-fit test, and the receiver operating characteristic (ROC) curve was used to analyze the predictive value of the model to EC. Results:The proportion of patients with age ≥ 55 years old, body mass index (BMI)≥ 24 kg/m 2, a history of hypertension and diabetes, malignant tumor family history and hormone replacement therapy history in the observation group were higher than those in the control group: 72.41%(42/58) vs. 39.66%(23/58),70.69% (41/58) vs. 43.10%(25/58), 36.21%(21/58) vs. 10.34%(6/58), 31.03%(18/58) vs. 8.62%(5/58),29.31%(17/58) vs. 5.17%(3/58), 27.93%(17/58) vs. 8.62%(5/58), and the differences were statistically significant ( P<0.05). The positive rates of HPV and CMV, the contents of HPV-DNA and CMV-DNA in the observation group were higher than those in the control group: 62.07%(36/58) vs. 29.31%(17/58),81.03%(47/58) vs. 41.38%(24/58), (471.16 ± 33.58) copies/ml vs. (240.08 ± 17.41) copies/ml, (256.19 ± 24.77) copies/ml vs. (132.27 ± 13.20) copies/ml, the differences were statistically significant ( P<0.05). Age ≥ 55 years, BMI ≥ 24 kg/m 2, history of hypertension and diabetes, family history of malignant tumors, history of hormone replacement therapy, HPV positive, CMV positive were risk factors for EC ( P<0.05). HPV-DNA virus content was positively correlated with the age of EC patients, family history of malignant tumors, and history of diabetes ( P<0.05), the viral content of CMV-DNA was positively correlated with age, family history of malignant tumors, and history of hypertension ( P<0.05). According to the ROC curve drawn by the predicted value and the true value of the Logistic regression model, the area under the curve was 0.930 (95% CI 0.885 - 0.976), when the predictive probability of individual nosocomial infection>0.528, the prediction sensitivity was 84.48%, and the specificity was 89.66%. Conclusions:The occurrence of EC is related to many factors, and the content of HPV-DNA and CMV-DNA viruses are the risk factors of its occurrence. We constructed the CAP2NC prokaryotic expression vector of HIV-1 NL4-3 strain and obtained relatively pure CAP2NC protein by optimizing its purification conditions to explore its in vitro self-assembly conditions. Primers were designed according to the CAP2NC DNA sequence of HIV-1 NL4-3 strain. The target gene was amplified by PCR and cloned into prokaryotic expression vector pTO-T7. Then the recombinant strain was transformed into Escherichia coli BL21 (DE3). IPTG induced protein expression, then the protein was purified by hydrophobic chromatography. SDS-PAGE and Western blotting were performed to analyze the target protein, and the biological activity of the antigen was identified through ELISA. The self-assembly of CAP2NC protein was analyzed by transmission electron microscopy and gel filtration chromatography. The protein had good reaction with the specific antibodies of p24 and formed different structures in various conditions. When 10% yeast RNA was added to the protein complex, the recombinant protein only formed into a tubular structure, which was similar to the self-assembled structure of the HIV-1 virus capsid. The results showed that the HIV-1 CAP2NC protein had in vitro self-assembly activity, and the RNA affected the structure of CAP2NC protein assembly. The protein can be used as a simple and effective molecular model to study its structure, and then it can provide a reference for the study of HIV immature virus particles.
