1.Content Determination of Alkaloids and Catalpol in Different Proportions of Qianjin Huanglian Pills

Objective To establish an HPLC method for the determination of alkaloids(epiberberine,coptisine,palma-tine,berberine)and catalpol in different ratios(1∶1,1∶10)of ancient and modern Qianjin Huanglian Pills,and to compare the differences in their contents.The content differences were compared to preliminarily evaluate the differences in the efficacy of Qianjin Huanglian Pills in the treatment of diabetes under different preparation processes and different ratios.Methods The alkaloid solvent was methanol∶ hydrochloric acid(100∶1).The detection conditions were as follows:C18 column,acetonitrile-0.05 mol·L-1 potassium dihydrogen phosphate solution(50∶50),detection wavelength 345 nm,column temperature 30℃,flow rate 1 mL·min-1,injection volume 10 μL.The catalpol solution was methanol∶ water(20∶80).The detection conditions were as follows:chromatographic column C18 column,methanol-0.1%phosphoric acid solution(1∶ 99),detection wavelength 210 nm,column temperature 30℃,flow rate 1 mL·min-1,injection volume 10 μL.Results The established method was spe-cific,and the separation effect of the five components was good.It exhibited a good linear relationship(R2>0.999)in their respec-tive linear ranges.The repeatability,precision,stability,and sample recovery rate all met the requirements.The content of four alka-loids in the ancient method 1∶1 was the highest,and the content of catalpol was the lowest.The content of four alkaloids in the ancient method 1∶10 was the lowest;the content of 1∶1 in the present method was higher than that in the ancient method 1∶10,and the content of berberine in the present method 1∶10 was slightly lower than that in the present method 1∶1,and the rest were higher than that in the present method 1∶1.The PCA results showed that the chemical composition contents of the four kinds of Qianjin Huanglian pills were very different.Conclusion The method is simple,accurate,and reproducible,making it suitable for the quality control of Qianjin Huanglian Pills.It provides a theoretical basis for exploring the difference in efficacy of Qianjin Huanglian Pills.
2.Value of a risk assessment model in predicting venous thromboembolism in patients with liver failure after artificial liver support therapy

Objective To investigate the value of a risk assessment model in predicting venous thromboembolism (VTE) in patients with liver failure after artificial liver support therapy. Methods A retrospective analysis was performed for the clinical data of 124 patients with liver failure who received artificial liver support therapy in Affiliated Drum Tower Hospital of Nanjing University Medical School from March 2019 to December 2021, among whom there were 41 patients with VTE (observation group) and 143 patients without VTE (control group). Related clinical data were compared between the two groups, and the Caprini risk assessment model was used for scoring and risk classification of the patients in both groups. The t -test was used for comparison of continuous data between two groups; the chi-square test was used for comparison of categorical data between two groups; the Mann-Whitney U rank sum test was used for comparison of ranked data between two groups. The logistic regression analysis was used to investigate the independent risk factors for VTE in patients with liver failure after artificial liver support therapy. The receiver operating characteristic (ROC) curve was used to investigate the value of Caprini score and the multivariate predictive model used alone or in combination in predicting VTE. Results The observation group had a significantly higher Caprini score than the control group (4.39±1.10 vs 3.12±1.04, t =6.805, P < 0.001). There was a significant difference between the two groups in risk classification based on Caprini scale ( P < 0.05), and the patients with high risk or extremely high risk accounted for a higher proportion among the patients with VTE. The univariate analysis showed that there were significant differences between the two groups in age ( t =6.400, P < 0.001), catheterization method ( χ 2 =14.413, P < 0.001), number of times of artificial liver support therapy ( Z =-4.720, P < 0.001), activity ( Z =-6.282, P < 0.001), infection ( χ 2 =33.071, P < 0.001), D-dimer ( t =8.746, P < 0.001), 28-day mortality rate ( χ 2 =5.524, P =0.022). The multivariate analysis showed that number of times of artificial liver support therapy (X 1 ) (odds ratio [ OR ]=0.251, 95% confidence interval [ CI ]: 0.111-0.566, P =0.001), activity (X 2 ) ( OR =0.122, 95% CI : 0.056-0.264, P < 0.001), D-dimer (X 3 ) ( OR =2.921, 95% CI : 1.114-7.662, P =0.029) were independent risk factors for VTE in patients with liver failure after artificial liver support therapy. The equation for individual predicted probability was P =1/[1+e -(7.425-1.384X 1 -2.103X 2 +1.072X 3 ) ]. The ROC curve analysis showed that Caprini score had an area under the ROC curve of 0.802 (95% CI : 0.721-0.882, P < 0.001), and the multivariate model had an area under the ROC curve of 0.768 (95% CI : 0.685-0.851, P < 0.001), while the combination of Caprini score and the multivariate model had an area under the ROC curve of 0.957 (95% CI : 0.930-0.984, P < 0.001). Conclusion The Caprini risk assessment model has a high predictive efficiency for the risk of VTE in patients with liver failure after artificial liver support therapy, and its combination with the multivariate predictive model can significantly improve the prediction of VTE.
3.Effect of Modified Huangqi Gancaotang on Proliferation, Apoptosis, Invasion, Migration, and Epithelial-mesenchymal Transition of Non-small Cell Lung Cancer Cells Based on Wnt/β-catenin Pathway

ObjectiveTo investigate effect of lyophilized powder of modified Huangqi Gancaotang on proliferation, apoptosis, invasion, migration, and epithelial-mesenchymal transition of human non-small cell lung cancer cells (A549, PC9) and possible mechanism. MethodEffect of 1.0, 2.0, 4.0, 8.0, 12.0 g·L-1 modified Huangqi Gancaotang on the proliferation of non-small cell lung cancer cells was detected by cell counting kit-8 (CCK-8) assay. A549 and PC9 cells were classified into the blank group and the low-, medium-, and high-dose Huangqi Gancaotang groups (2.0, 4.0, 8.0 g·L-1). Plate cloning assay was used to examine the effect of modified Huangqi Gancaotang on cell cloning ability. Hoechst 33342 staining and flow cytometry were employed to detect the apoptosis, and scratch assay and Transwell migration assay were applied to examine cell migration and invasion abilities, respectively. Mammosphere assay was used to examine the sphere-forming ability of tumor cells, and real-time polymerase chain reaction (Real-time PCR) to detect the mRNA expression of stemness-related molecules octamer-binding transcription factor 4 (Oct-4), human sex-determining region Y-box 2 (Sox2), and homeobox transcription factor (Nanog) to assess cancer stem cell activity. The protein expression of B-cell lymphoma 2 (Bcl-2), Bcl-2-associated death promoter (Bad), Bcl-2-associated X protein (Bax), cleaved Caspase-3, Caspase-3, E-cadherin, N-cadherin, vimentin, matrix metalloproteinase-2 (MMP-2), β-catenin, c-Myc, Cyclin D1, and zinc-finger transcription factor (Slug) was determined by Western blot. ResultThe proliferation ability of A549 and PC9 cells was significantly inhibited after 24 h and 48 h treatment with 1.0, 2.0, 4.0, 8.0, and 12.0 g·L-1 lyophilized powder of modified Huangqi Gancaotang compared with that in the blank group and the inhibition was dose- and time-dependent (P<0.05, P<0.01). Compared with the blank group, the low-, medium-, and high-dose modified Huangqi Gancaotang suppressed the cloning ability of A549 and PC9 cells (P< 0.05, P<0.01), and high-dose modified Huangqi Gancaotang induced apoptosis of A549 and PC9 cells (P<0.01). In comparison with the blank group, the low-, medium-, and high-dose modified Huangqi Gancaotang inhibited the invasion and migration of A549 and PC9 cells (P<0.05, P<0.01). Compared with the blank group, the low-, medium-, and high-dose modified Huangqi Gancaotang significantly decreased volume of the microspheres of A549 cells and the mRNA expression of Oct-4, Sox2, and Nanog in A549 and PC9 cells (P<0.05, P<0.01). Compared with the blank group, the medium- and high-dose modified Huangqi Gancaotang down-regulated the expression of the anti-apoptotic protein Bcl-2 (P<0.05, P<0.01), up-regulated the expression of pro-apoptotic proteins Bad, Bax, and cleaved Caspase-3/Caspase-3 (P<0.05, P<0.01) in A549 and PC9 cells, decreased the expression of MMP-2, N-cadherin, and vimentin (P<0.05, P< 0.01), and raised the E-cadherin expression (P<0.05, P<0.01). Moreover, the medium-dose and high-dose modified Huangqi Gancaotang all reduced the expression of β-catenin, c-Myc, Cyclin D1, and Slug in A549 and PC9 cells (P<0.01). ConclusionModified Huangqi Gancaotang can inhibit the proliferation, invasion, migration, activity of cancer stem cells, and epithelial-mesenchymal transition of human non-small cell lung cancer (A549, PC9) cells and induce apoptosis, and the mechanism is the likelihood that it regulates Wnt/β-catenin signaling pathway.
4.Reoperation and perioperative management of residual cyst wall with perineal intractable sinus after resection of presacral cyst tumors

Objective:To investigate the reoperation and perioperative management of residual cyst wall with perineal intractable sinus after resection of presacral cyst tumors.Methods:The clinical data of 29 patients with residual cyst wall and perineal intractable sinus after resection of presacral cyst tumors in Affiliated Cancer Hospital of Zhengzhou University from January 2014 to August 2019 were reviewed, including the characteristics of the residual cyst wall with perineal intractable sinus after resection of presacral cyst tumors, surgical method, and perioperative management.Results:Twenty-nine patients with residual cyst wall and perineal intractable sinus after resection of presacral cyst tumors, including 9 cases of epidermoid cysts, 7 cases of dermoid cysts, 10 cases of mature teratomas and 3 cases of malignant cysts (including malignant transformation of caudate cyst and teratoma); The 29 patients underwent posterior approaches for cyst resection in other hospital before, of whom 1 patient underwent posterior combined with transabdominal approach. All of thes patients underwent resection of residual presacral cyst wall and perineal intractable sinus in our hospital, of whom 25 patients underwent a transperineal approach through an arc-shaped incision anterior to the apex of the coccyx, and the other 4 patients underwent transperineal arc-shaped incision combined with transabdominal approach. All of the patients were cured without serious complications occurring, postoperative pathological and the magnetic resonance imaging diagnosis showed that the residual cyst wall and perineal intractable sinus were all completely removed.Conclusion:Appropriate surgical approache and perioperative treatment for the patients with residual cyst wall and perineal intractable sinus are very important to promote the resection of residual cyst wall and the healing of perineal intractable sinus.

6.Research progress on non-alcoholic fatty liver disease animal models

In recent years, with the changes in living standards and dietary structure, the incidence of non-alcoholic fatty liver disease has been increasing year by year in China, and the incidence rate in the general population is as high as 29.81%. An increasingly epidemiological evidence suggests that non-alcoholic fatty liver disease has become one of the causes of increasing liver cirrhosis and liver cancer. However, its etiology and pathogenesis are complex and have not yet been fully elucidated. Therefore, establishing an appropriate non-alcoholic fatty liver disease animal models for pre-clinical research is essential to elucidate its pathogenesis. This article summarizes the latest research progress of non-alcoholic fatty liver disease animal models, which are common at home and abroad in recent years.
7. Clinical features of 30 cases with novel coronavirus pneumonia

Objective:
To analyze the epidemiological and clinical characteristics of patients with 2019 novel coronavirus (2019-nCoV) infection in Shenyang.
Methods:
The epidemiological and clinical characteristics of 30 patients diagnosed with 2019-nCoV infection admitted to Shenyang sixth people's hospital on January 22, 2020 and February 8, 2020 were retrospectively analyzed.
Results:
Among the 30 cases, 21 were imported, including 17 from Hubei Province and four from other provinces. Nine cases were local infections. There were 18 men and 12 women, aging from 21 to 72 years with the median of 43 years. Eight cases had underlying diseases including hypertension, diabetes, coronary heart disease and bronchitis. On admission, two (7%) cases were mild, 19 (63%) cases were ordinary, eight (27%) cases were severe, and one (3%) case was critical. Clinical manifestations mainly include fever, with or without upper respiratory tract symptoms, normal, decreased or slightly increased white blood cell counts, mainly decreased lymphocyte counts, normal or increased c-reactive protein, and normal procalcitonin. The computed tomography (CT) of the early stage of the lungs showed that multiple patchy ground glass shadows were mainly accompanied by consolidation, which often involved both lungs or multiple lobes of one lung. At the moment, the clinical treatment mainly included respiratory support, symptomatic treatment, antiviral treatment adn anti-bacterial treatment. By February 15, a total of nine cases were cured and discharged, including one mild case, six ordinary cases, and two severe cases. 8.Study of isolation efficiency of new islet isolation solution for mouse islets

Objective To investigate the isolation and protective effect of a new islet purification solution (IPS)-Optiprep solution on the islet in mouse models. Methods The digested pancreatic islets were divided into the IPS and UW groups according to the islet volume. The pancreatic islets were isolated by the continuous gradient density centrifugation using IPS-Optiprep or UW-Optiprep solutions. The purification efficiency and isolated islet activity of purification solution were compared between two groups. The diabetic mouse models were successfully induced and randomly assigned into three groups. In the experimental group (n=10), the mice received pancreatic islet transplantation using islets isolated and purified by the IPS-Optiprep solution. In the control group (n=10), the mice underwent pancreatic islet transplantation using islets isolated and purified by the UW-Optiprep solution. In the sham surgery group (n=5), the mice merely underwent surgery without pancreatic islet transplantation. Postoperative blood glucose levels were detected and compared among three groups. The blood glucose levels of intraperitoneal glucose tolerance test at postoperative 21 d were statistically compared between the experimental and control groups. The cost of the preparation of two isolation solutions was also compared. Results Compared with the UW group, the islet equivalent (IEQ), islet purity, recovery rate and islet integrity were significantly higher in the IPS group. Islet morphological observation revealed that the islet membrane was complete and the islet diameter in the IPS group was considerably larger than that in the UW group. The activity of purified islets in the UW group was significantly higher than that in the IPS group [(88±5)% vs. (84±3)%, P<0.01]. Compared with the UW-Optiprep solution, identical in vivo islet function was obtained in the IPS-Optiprep solution.The cost of IPS-Optiprep solution was significantly less than that of the UW-Optiprep solution. Conclusions The new IPS-Optiprep solution yields higher islet isolation efficiency, purification, integrity and recovery rate and significantly reduces the purification cost compared with the UW-Optiprep solution. Nevertheless, IPS-Optiprep solution exerts a less protective effect on the activity of islet cells, which is probably correlated with the high islet integrity and the endotoxin in the IPS-Optiprep solution.
9. Effects of hydrotherapy with traditional Chinese medicine and magnetotherapy on treatment of scars after healing of deep partial-thickness burn wounds in children

Objective:
To study the effects of hydrotherapy with traditional Chinese medicine combined with magnetotherapy on treatment of scars after healing of deep partial-thickness burn wounds in children.
Methods:
Forty-eight children with scars after healing of deep partial-thickness burn wounds admitted to the Burn Department of Air Force General Hospital from January to December in 2016 met the criteria for inclusion in this study, and their medical records were analyzed retrospectively. Within one month after the wound was completely healed, 24 children were treated with silicone cream for 2 times per day and wore elastic sleeves all day long according to the wishes of the children and their families, and they were enrolled in routine treatment group. The other 24 children were treated by hydrotherapy with traditional Chinese medicine Fuchunsan No. 2 once every 2 days for 30 min each time on the basis of treatment methods of routine treatment group, plus magnetotherapy using pulse magnetotherapy apparatus in the morning and evening of each day for 30-60 min each time, and they were enrolled in hydrotherapy+ magnetotherapy group. Before treatment and 12 weeks after treatment, the Vancouver Scar Scale was used to assess the scar condition of children, the Wong-Baker Facial Expression Scale was used to assess the degree of scar pain of children, the Numerical Rating Scale was used to assess the degree of scar itching of children, the modified Barthel Index was used to assess the activity of daily living (ADL) of children, and the Motor Function Evaluation Scale was used to assess the dysfunction condition of limbs with scar. 10.Effect of bone mesenchymal stem cell transplantation on accelerating the vascularization surrounding transplant pancreatic islet

Objective To investigate whether pancreatic islet transplantation in combination with bone mesenchymal stem cells (MSC) transplantation can promote the vascularization surrounding the transplant pancreatic islet.Methods The non-obese diabetic (NOD) mice were utilized as the recipients and randomly divided into pancreatic islet transplantation combined with MSC transplantation group (co-transplantation group,n=6),pancreatic islet transplantation group(n=6),MSC transplantation group(n=6) and sham transplantation group (n=3).The variation in blood glucose level and survival rate post-transplantation of NOD mice in each group was observed.The proliferation and apoptosis of the transplant pancreatic islet in the pancreatic islet transplantation group and co-transplantation group at 1,2 and 4 weeks after pancreatic islet transplantation were analyzed by 5-ethynyl-2'-deoxyufidine (EdU) and terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling (TUNEL).The vascular density surrounding the transplanted pancreatic islet in the pancreatic islet transplantation group and co-transplantation group at postoperative 2,4 and 8 weeks were observed under light microscope and quantitatively analyzed by histochemical and immunohistochemical staining.Results Both MSC combined with pancreatic islet transplantation and pancreatic islet transplantation significantly improved the blood glucose level and enhanced the survival rate of NOD mouse models after transplantation.In addition,it could accelerate the regeneration of pancreatic islet cells and decrease cell apoptosis.MSC combined with pancreatic islet transplantation significantly enhanced the vascular density surrounding the transplant pancreatic islet compared with pancreatic islet transplantation alone.Conclusions MSC transplantation can accelerate the vascularization surrounding the transplant pancreatic islet,increase the blood supply and protect the function and activity of the transplant pancreatic islet.
