1.Application of distal radius coronal bone structure matching in the treatment of distal radius fractures

		                        			Objective To investigate the clinical effect of coronal bone structure matching(CBSM)in the treatment of dis-tal radius fracture.Methods A total of 39 cases of distal radius fracture between Jannary 2018 and Jannary 2022 were included in this study.Among them there were 22 males and 17 females with an average age of(48.9±16.3)years old,ranged from 22 to 65 years old.All patients were treated with open reduction and internal fixation with plates.Based on the measurement of CB-SM value on the X-ray film the next day after surgery.All patients were divided into matched group and mismatched group ac-cording to the coronal bone structure matching in the normal range or not.There were 27 patients in the matched group,includ-ing 15 males and 12 females,the age ranged from 22 to 64 years old with an average of(48.0±16.2)years old.AO classifica-tion of fracture was C1 in 6 cases and C2 in 21 cases;the operation time ranged from 1 to 6 days after injury;9 cases were com-plicated with ulnar styloid process fracture.There were 12 patients in the mismatched group,including 7 males and 5 females;the age ranged from 22 to 65 years old with an average of(48.8±15.8)years old.AO classification of fracture was C1 in 4 cases and C2 in 8 cases;the time from injury to operation ranged from 1 to 5 days;4 cases were complicated with ulnar styloid pro-cess fracture.The X-ray films were used to evaluate fracture healing,humeral height,ulnar angle and palm tilt angle at 3 months after operation.The range of wrist motion(pronation,supination,palmar inclination and dorsiflexion),function out-comes(Gartland-Werley score)and pain levels(visual analogue scale,VAS)were compared between the two groups at the last follow-up.Results The average follow-up time of 39 patients were(9.5±4.3)months,ranged from 6 to 14 months.All pa-tients healed in one stage without postoperative infection,fracture nonunion and fracture displacement occurred.Compared with match group at the last follow-up,the VAS in the mismatch group was increased[(2.5±1.3)points vs(1.6±1.0)points],the wrist pronation were decreased[(70.5±12.6)° vs(80.5±9.4)°],with statistically significant difference(P<0.05).There was no sig-nificant difference in the range of motion(supination,palmar inclination,dorsiflexion)and excellent good rate between the two groups at last follow-up after operation(P>0.05).Conclusion Wrist dysfunction,limited pronation,and wrist pain may occur when the postoperative matching degree of the distal radius fracture is not within the normal range.

		                        			We wished to establish an expert consensus on late stage of critical care (CC) management. The panel comprised 13 experts in CC medicine. Each statement was assessed based on the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) principle. Then, the Delphi method was adopted by 17 experts to reassess the following 28 statements. (1) ESCAPE has evolved from a strategy of delirium management to a strategy of late stage of CC management. (2) The new version of ESCAPE is a strategy for optimizing treatment and comprehensive care of critically ill patients (CIPs) after the rescue period, including early mobilization, early rehabilitation, nutritional support, sleep management, mental assessment, cognitive-function training, emotional support, and optimizing sedation and analgesia. (3) Disease assessment to determine the starting point of early mobilization, early rehabilitation, and early enteral nutrition. (4) Early mobilization has synergistic effects upon the recovery of organ function. (5) Early functional exercise and rehabilitation are important means to promote CIP recovery, and gives them a sense of future prospects. (6) Timely start of enteral nutrition is conducive to early mobilization and early rehabilitation. (7) The spontaneous breathing test should be started as soon as possible, and a weaning plan should be selected step-by-step. (8) The waking process of CIPs should be realized in a planned and purposeful way. (9) Establishment of a sleep-wake rhythm is the key to sleep management in post-CC management. (10) The spontaneous awakening trial, spontaneous breathing trial, and sleep management should be carried out together. (11) The depth of sedation should be adjusted dynamically in the late stage of CC period. (12) Standardized sedation assessment is the premise of rational sedation. (13) Appropriate sedative drugs should be selected according to the objectives of sedation and drug characteristics. (14) A goal-directed minimization strategy for sedation should be implemented. (15) The principle of analgesia must be mastered first. (16) Subjective assessment is preferred for analgesia assessment. (17) Opioid-based analgesic strategies should be selected step-by-step according to the characteristics of different drugs. (18) There must be rational use of non-opioid analgesics and non-drug-based analgesic measures. (19) Pay attention to evaluation of the psychological status of CIPs. (20) Cognitive function in CIPs cannot be ignored. (21) Delirium management should be based on non-drug-based measures and rational use of drugs. (22) Reset treatment can be considered for severe delirium. (23) Psychological assessment should be conducted as early as possible to screen-out high-risk groups with post-traumatic stress disorder. (24) Emotional support, flexible visiting, and environment management are important components of humanistic management in the intensive care unit (ICU). (25) Emotional support from medical teams and families should be promoted through"ICU diaries"and other forms. (26) Environmental management should be carried out by enriching environmental content, limiting environmental interference, and optimizing the environmental atmosphere. (27) Reasonable promotion of flexible visitation should be done on the basis of prevention of nosocomial infection. 3.Bis (2-butoxyethyl) Phthalate Delays Puberty Onset by Increasing Oxidative Stress and Apoptosis in Leydig Cells in Rats.

		                        			OBJECTIVE:
		                        			This study investigated the effects of bis (2-butoxyethyl) phthalate (BBOP) on the onset of male puberty by affecting Leydig cell development in rats.
		                        		
		                        			METHODS:
		                        			Thirty 35-day-old male Sprague-Dawley rats were randomly allocated to five groups mg/kg bw per day that were gavaged for 21 days with BBOP at 0, 10, 100, 250, or 500 mg/kg bw per day. The hormone profiles; Leydig cell morphological metrics; mRNA and protein levels; oxidative stress; and AKT, mTOR, ERK1/2, and GSK3β pathways were assessed.
		                        		
		                        			RESULTS:
		                        			BBOP at 250 and/or 500 mg/kg bw per day decreased serum testosterone, luteinizing hormone, and follicle-stimulating hormone levels mg/kg bw per day (P < 0.05). BBOP at 500 mg/kg bw per day decreased Leydig cell number mg/kg bw per day and downregulated Cyp11a1, Insl3, Hsd11b1, and Dhh in the testes, and Lhb and Fshb mRNAs in the pituitary gland (P < 0.05). The malondialdehyde content in the testis significantly increased, while Sod1 and Sod2 mRNAs were markedly down-regulated, by BBOP treatment at 250-500 mg/kg bw per day (P < 0.05). Furthermore, BBOP at 500 mg/kg bw per day decreased AKT1/AKT2, mTOR, and ERK1/2 phosphorylation, and GSK3β and SIRT1 levels mg/kg bw per day (P < 0.05). Finally, BBOP at 100 or 500 μmol/L induced ROS and apoptosis in Leydig cells after 24 h of treatment in vitro (P < 0.05).
		                        		
		                        			CONCLUSION:
		                        			BBOP delays puberty onset by increasing oxidative stress and apoptosis in Leydig cells in rats.

		                        			OBJECTIVES:
		                        			To investigate the clinical treatment outcomes and the changes of the outcomes over time in extremely preterm twins in Guangdong Province, China.
		                        		
		                        			METHODS:
		                        			A retrospective analysis was performed for 269 pairs of extremely preterm twins with a gestational age of <28 weeks who were admitted to the department of neonatology in 26 grade A tertiary hospitals in Guangdong Province from January 2008 to December 2017. According to the admission time, they were divided into two groups: 2008-2012 and 2013-2017. Besides, each pair of twins was divided into the heavier infant and the lighter infant subgroups according to birth weight. The perinatal data of mothers and hospitalization data of neonates were collected. The survival rate of twins and the incidence rate of complications were compared between the 2008-2012 and 2013-2017 groups.
		                        		
		                        			RESULTS:
		                        			Compared with the 2008-2012 group, the 2013-2017 group (both the heavier infant and lighter infant subgroups) had lower incidence rates of severe asphyxia and smaller head circumference at birth (P<0.05). The mortality rates of both of the twins, the heavier infant of the twins, and the lighter infant of the twins were lower in the 2013-2017 group compared with the 2008-2012 group (P<0.05). Compared with the 2008-2012 group, the 2013-2017 group (both the heavier infant and lighter infant subgroups) had lower incidence rates of pulmonary hemorrhage, patent ductus arteriosus (PDA), periventricular-intraventricular hemorrhage (P-IVH), and neonatal respiratory distress syndrome (NRDS) and a higher incidence rate of bronchopulmonary dysplasia (P<0.05).
		                        		
		                        			CONCLUSIONS
		                        			There is a significant increase in the survival rate over time in extremely preterm twins with a gestational age of <28 weeks in the 26 grade A tertiary hospitals in Guangdong Province. The incidences of severe asphyxia, pulmonary hemorrhage, PDA, P-IVH, and NRDS decrease in both the heavier and lighter infants of the twins, but the incidence of bronchopulmonary dysplasia increases. With the improvement of diagnosis and treatment, the multidisciplinary collaboration between different fields of fetal medicine including prenatal diagnosis, obstetrics, and neonatology is needed in the future to jointly develop management strategies for twin pregnancy.

		                        			Astragali Radix-Angelicae Sinensis Radix (AA) is a basic pair of drugs mainly targeting the syndrome characteristics of Qi and blood diseases. LI Dong-yuan's Danggui Buxuetang (DBT) is composed of AA, which is mainly used to tonify Qi and generate blood, with main indications of Qi deficiency and blood deficiency, blood heat and so on. It is favored by doctors because of its refined prescription and remarkable curative effect. However, there are many compatibility ratios of AA in different prescriptions in ancient books, and their efficacy and indications are also slightly different. This research showed that DBT also had the effect of invigorating Qi and activating blood, and the previous study of the group showed that 3∶1 compatibility ratio of the two herbs in the total amount of 36 g had more obvious effect of invigorating Qi and activating blood. By consulting the relevant literature, it was found that the drug pair had a certain effect of invigorating Qi and activating blood in various compatibility ratios such as 1∶1, 3∶1, 1∶5, 3∶2, 2∶1, 5∶1. The corresponding pharmacological effect mainly included regulating the energy metabolism of substances, regulating immune function, reducing blood viscosity, anti-oxidation stress, anti-inflammation, lowering blood lipids, lowering blood sugar, protecting heart function, protecting blood vessel wall, intervening angiogenesis, fighting against organ tissue fibrosis and so on. Regardless of the AA single-medicine's activating blood effect and the theory that "Qi circulation leads to blood circulation" or the drug pair's manifestation in modern pharmacological effects, all of these have confirmed that AA's effect of invigorating Qi and activating blood does exist, and the difference of action performance caused by different ratios of AA is closely related to dosage and proportion, which needs further study. Based on the study focusing on the effect of tonifying Qi and generating blood, it is easy to ignore the effect of invigorating Qi and activating blood, which limits the clinical application of the latter. Therefore, the tonifying Qi and activating blood circulation effect of the drug pair is reviewed in this paper, so as to provide a theoretical basis for its clinical rational drug use and related research.

10.Effect of An-pressing manipulation on post-stroke muscle spasticity in rats and its mechanism study

		                        			Objective: To explore the mechanism of An-pressing manipulation in improving post-stroke muscle spasticity, by observing the changes of γ-aminobutyric acid (GABA) and glycine (Gly) in plasma and gray matter of L1-L3 spinal cord anterior horn in post-stroke rats with muscle spasticity after An-pressing manipulation intervention. Methods: Ten of 80 adult male Sprague-Dawley (SD) rats were randomly selected as the blank group, and the remaining 70 were used for modeling. The middle cerebral artery occlusion (MCAO) rat model was established by insertion suture occlusion method in the left external carotid artery. Thirty rats with a Longa neurological score of 2-3 points and a modified Ashworth spasticity scale score of 1-, 1+, or 2 were included in the experiment. Using the random number table method, the 30 successfully modeled rats were randomly divided into a model group, an An-pressing tendon group and an An-pressing muscle belly group. Two days after modeling, rats in the An-pressing tendon group and An-pressing muscle belly group received An-pressing manipulation on the tendon and belly of quadriceps femoris muscle respectively, with the pressure of (350±50) g and the frequency of 5 s/time, 15 min per session, once a day for 5 continuous days. After the 5th treatment, the tension of the rat quadriceps femoris muscle was evaluated using the modified Ashworth spasticity scale. The Gly levels in rat plasma and L1-L3 segments of spinal cord were determined by enzyme-linked immunosorbent assay (ELISA). The GABA levels in rat plasma and L1-L3 segments of spinal cord were measured by high performance liquid chromatography (HPLC). Results: The decrease in rat muscle tension scored by the modified Ashworth spasticity scale in the An-pressing tendon group was more significant than that in the An-pressing muscle belly group (P<0.01); the increases in Gly and GABA levels in the rat plasma and L1-L3 segments of spinal cord were more significant in the An-pressing tendon group than those in the An-pressing muscle belly group (all P<0.01). Conclusion: Based on the theory of 'anti-stretch reflex' of tendon organs, the use of An-pressing manipulation to induce the 'anti-stretch reflex' by stimulating the tendon organs can improve the muscle spasticity of rats, which is better than An-pressing the muscle belly. Increased levels of Gly and GABA in rat plasma and L1-L3 segments of spinalcord may be one mechanism of An-pressing manipulation to improve muscle spasticity by stimulating tendon organs.
