1.Traditional Chinese Medicine Intervention in Depression Based on Signaling Pathway Regulation: A Review

Depression is a common psychiatric disorder characterized by persistent low mood or mental disorders. Current treatments primarily focus on regulating neurotransmitter levels, but their effectiveness is limited. The mechanisms underlying its onset are complex, and there is no unified consensus. Abnormal signaling pathway transmission plays a crucial role in the development of depression, involving multiple pathways, including Toll-like receptor 4/nucleotide-binding oligomerization domain-like receptor protein 3 (TLR4/NLRP3), nuclear factor-κB (NF-κB), Janus kinase/signal transducer and activator of transcription (JAK/STAT), mitogen-activated protein kinase/extracellular signal-regulated kinase (MAPK/ERK), brain-derived neurotrophic factor/tyrosine kinase receptor B (BDNF/TrkB), cyclic AMP/protein kinase A/cAMP response element-binding protein (cAMP/PKA/CREB), and others. Traditional Chinese medicine(TCM) is based on a holistic approach and the principle of treatment based on the differentiation of syndromes, regulating the balance of multiple systems and organ functions from a macroscopic perspective. This approach has shown unique advantages in the treatment of depression. TCM attributes the onset of depression to dysfunction of the organ systems, involving liver Qi stagnation, heart spirit deficiency, kidney essence depletion, and spleen dysfunction. TCM compound treatments focus on soothing the liver, strengthening the spleen, calming the heart, and replenishing essence, with formulas such as Xiaoyaosan, Zishui Qinggan Yin, and Chahu Jia Guizhi Longgu Muli Tang. The active components of Chinese herbs mainly aim to tonify and regulate Qi, such as salidroside, ginsenoside Rb1, astragaloside, and muscone. External TCM treatments, primarily acupuncture, aim to open the orifices and invigorate the spirit. Acupoints such as Baihui, Shenting, and Yintang are commonly used. Additionally, massage and moxibustion therapy can intervene in depression by regulating signaling pathways. This article reviews the core role of signaling pathways in the development of depression and the mechanism of TCM regulation of signaling pathways to intervene in depression, aiming to discover new therapeutic approaches that can improve the symptoms of depressed patients.

Asthma is a chronic inflammatory respiratory disease involving multiple cells and cellular components, characterized by recurrent episodes of wheezing, shortness of breath, chest tightness, and coughing, significantly impacting patients' quality of life. The phosphatidylinositol 3-kinase/protein kinase B(PI3K/Akt) signaling pathway, as a crucial hub in intracellular signaling, is widely involved in the regulation of cell growth, proliferation, survival, metabolism, and a series of pathophysiological processes. Its regulatory role in the pathological progression of asthma is particularly significant, specifically in promoting airway inflammation, mediating epithelial mesenchymal transition, accelerating airway remodeling, regulating cell autophagy, inducing mucus hypersecretion, and influencing immune response balance. This study analyzed potential molecular targets of the PI3K/Akt pathway, including activators such as cysteine proteinase inhibitor 1(CST1), found in inflammatory zone 1(FIZZ1) and free fatty acid receptor 1(FFAR1), and inhibitors such as human β-defensin-3(hBD-3), disintegrins, metalloproteinase 33(ADAM33) and interleukin-27(IL-27), and initially revealed the potential molecular mechanisms of traditional Chinese medicine(TCM) in asthma intervention. Based on this, the authors systematically summarized the efficacy and specific mechanisms of TCM monomers, compounds, and external treatments for asthma by regulating the PI3K/Akt signaling pathway through literature review and analysis, aiming at establishing a robust foundation for the wide application and advanced development of TCM in asthma treatment, offering innovative insights for clinical research and drug development of asthma.

The report of the 20th National Congress of the Communist Party of China has made a major deployment to promote the expansion of high-quality medical resources and balanced regional layout, and the construction of national regional medical centers is an important measure to implement the landing. By analyzing the " three co-construction" mode of the National Children′s Regional Medical Center of Henan Children′s Hospital (Beijing Children′s Hospital Zhengzhou Hospital), this paper systematically introduced the internal and external decision-making, management and operation of the regional medical center from four aspects: management, talent, discipline and system. The practice and effectiveness in talent introduction and training, salary distribution, two-way communication, assessment and incentive, discipline construction, technology application, sharing and evaluation of scientific and technological innovation, institutional linkage and integration, as well as the construction of a hierarchical diagnosis and treatment system, were put forward. Suggestions such as construction according to local conditions, strengthening internal coordination, and optimizing external guarantee were put forward, so as to provide practical reference for the in-depth promotion of other national regional medical center construction projects.
6.Wumeiwan in Treatment of Diseases of Large Intestine: A Review

Originating from Treatise on Cold Damage (ZHANG Zhongjing), Wumeiwan is a representative prescription for pungent dispersing and bitter descending, upper clearing and warm purging, and combining cold and warm, which can be used to treat abdominal pain, enduring diarrhea, and syncope caused by cold and heat in complexity and asthenia in origin and sthenia in superficiality. Owing to the remarkable clinical efficacy, it is widely applied to the treatment of various internal injuries and external-contraction diseases. In recent years, the incidence of diseases of large intestine such as ulcerative colitis, Crohn's disease, and colorectal cancer has been on the rise, threatening the quality of life and health of patients. Major headway has been made in the clinical research on the treatment of the above diseases with Wumeiwan and on the mechanisms. However, no relevant summary is available, leading to the failure of further excavation and utilization of the value of this prescription in the treatment of the above diseases in time. Therefore, this paper reviews the theoretical basis, clinical research, and mechanism research of Wumeiwan and its modified formulas in the treatment of diseases of large intestine in recent years. The literature research suggests that Wumeiwan can effectively relieve clinical symptoms and reduce recurrence rate of large intestine diseases (including ulcerative colitis, Crohn's disease, diarrhea-predominant irritable bowel syndrome, colorectal cancer, chemotherapy-induced intestinal mucositis, and postoperative colorectal adenoma) with little adverse reactions. At the moment, the mechanisms of Wumeiwan against the above diseases are anti-inflammatory response, regulating immune function, repairing intestinal mucosal barrier, anti-tumor, regulating intestinal flora, anti-oxidative damage, analgesia, and inhibiting colonic epithelial cell apoptosis, but the specific molecular mechanism should be further explored. Through literature research, this paper comprehensively analyzes the theoretical basis, clinical research, and mechanisms of Wumeiwan in the treatment of diseases of large intestine, which is expected to serve as a reference for the further research on Wumeiwan in the prevention and treatment of diseases of large intestine and the mechanisms.
7.Alendronate combined with Lactobacillus rhamnosus to combat bone loss in ovariectomized mice

Objective:To study the protective effect of alendronate combined with Lactobacillus rhamnosus on bone loss in ovariectomized mice.Methods:Fifty female C57BL/6 mice were divided into 5 equal groups ( n=10). Ovariotomy was performed in groups A, B, C and D while a sham operation was performed in group E. Group A was subjected to combined administration of alendronate and Lactobacillus rhamnosus, group B to administration of alendronate, group C to administration of Lactobacillus rhamnosus, and groups D and E to administration of physiological saline only. At 3 months after operation, all the mice were sacrificed to harvest their femurs. Micro CT scanning was performed to detect the bone mineral density (BMD), trabecular relative volume, bone surface area/bone volume, and trabecular thickness and number of trabecular bone. Three-point bending test was used to detect the maximum load, stiffness, ultimate load, Young's modulus, and fracture energy. Osteocalcin and alkaline phosphatase levels were measured using blood samples from the mice eyeballs. The 2 groups were compared in terms of all the above indexes. Results:The BMD [(669.87±67.87) mg/cm 3], maximum load [(14.35±0.75) N] and fracture energy [(1,497.43±38.29) J/m 2] in group A were significantly higher than those in group B [(520.07±9.01) mg/cm 3, (11.94±0.82) N and(1,277.61±35.12) J/m 2] and group C [(388.15±25.61) mg/cm 3, (11.10±0.93) N and (1,115.27±63.24) J/m 2] (all P<0.05). The osteocalcin level in group A [(22.25±1.78) ng/mL] was significantly higher than that in group B [(19.08±1.45) ng/mL] and group D [(19.33±1.66) ng/mL] (both P<0.05). The alkaline phosphatase level in group A [(83.21±9.69) ng/mL] was significantly lower than that in group C [(113.16±14.44) ng/mL] and group D [(137.96±14.01) g/mL] (both P<0.05). Conclusion:Alendronate combined with Lactobacillus rhamnosus may play a synergistic role in prevention of bone loss in ovariectomized mice, because combined administration of the two is more effective than administration of either of the two.
8.Sex differences in the link between blood cobalt concentrations and insulin resistance in adults without diabetes.

BACKGROUND:
Little is known about the effects of environmental cobalt exposure on insulin resistance (IR) in the general adult population. We investigated the association between cobalt concentration and IR.
METHODS:
A total of 1281 subjects aged more than 20 years with complete blood cobalt data were identified from the National Health and Nutrition Examination Survey (NHANES) 2015-2016 cycle. Blood cobalt levels were analyzed for their association with IR among all populations and subgroups by sex. Regression coefficients and 95% confidence intervals (CIs) of blood cobalt concentrations in association with fasting glucose, insulin and homeostatic model assessment of insulin resistance (HOMA-IR) were estimated using multivariate linear regression after adjusting for age, sex, ethnicity, alcohol consumption, body mass index, education level, and household income. A multivariate generalized linear regression analysis was further carried out to explore the association between cobalt exposure and IR.
RESULTS:
A negative association between blood cobalt concentration (coefficient = - 0.125, 95% CI - 0.234, - 0.015; P = 0.026) and HOMA-IR in female adults in the age- and sex-adjusted model was observed. However, no associations with HOMA-IR, fasting glucose, or insulin were found in the overall population. In the generalized linear models, participants with the lowest cobalt levels had a 2.74% (95% CI 0.04%, 5.50%) increase in HOMA-IR (P for trend = 0.031) compared with subjects with the highest cobalt levels. Restricted cubic spline regression suggested that a non-linear relationship may exist between blood cobalt and HOMA-IR.
CONCLUSIONS
These results provide epidemiological evidence that low levels of blood cobalt are negatively associated with HOMA-IR in female adults.

9. Clinical Significance of Serum CircNRIP1 Expression in Diagnosis and Treatment of Gastric Cancer

Background: Circular RNAs (circRNAs) is correlated with the development and progression of malignant tumor. Recent studies have shown the potentials of some circRNAs in the early diagnosis, screening of therapeutic targets and prognostic judgment of gastric cancer. Aims: To investigate the serum circNRIP1 expression and its clinical significance in the diagnosis, treatment and prediction of prognosis of gastric cancer. Methods: A total of 76 gastric cancer patients diagnosed by pathology from September 2016 to September 2019 at Dujiangyan People's Hospital were enrolled, and 63 healthy individuals were served as controls. Serum circNRIP1 expression was detected by real time fluorescent quantitative PCR, and its correlation with clinicopathological features was analyzed. ROC curve was used to analyze the diagnostic efficiency. Results: Serum circNRIP1 expression in gastric cancer group was significantly higher than that in healthy control group (P<0.01). The circNRIP1 expression in gastric cancer patients was significantly correlated with the differentiation of tumor cells, depth of infiltration, lymph node metastasis and TNM stage (P<0.05). Serum circNRIP1 expression in gastric cancer patients after operative treatment was significantly lower than that before treatment (P<0.05). After 2 years follow-up, serum circNRIP1 expression in survival group was significantly lower than that in death group (P<0.05). ROC curve analysis showed that the sensitivity of serum circNRIP1 expression for gastric cancer was 88.9%, the specificity was 88.2%, and AUC was 0.876. Conclusions: Detecting serum circNRIP1 expression has significant value in the early diagnosis and evaluation of treatment and prediction of prognosis of gastric cancer.
