1.Differentiation and Treatment of Vascular Dementia Applying the Method of Promoting Yang

It is believed that the root cause of vascular dementia lies in insufficient primal yang and weakness in the circulation of blood. The key pathological factors are the mutual stagnation of phlegm and blood stasis, and the obstruction of blood vessels. The general treatment principle is to tonify qi and promote yang, expel pathogenic factors, and unblock yang. This involves supplementing the innate and acquired yang qi of the spleen and kidneys, removing pathogenic factors like phlegm, turbidity, and blood stasis, in order to restore the function of yang qi and promote the circulation of blood and qi. The basic prescription for tonifying qi and promoting yang includes Heishunpian (Lycium Ruthenicum), Zhichuanwu (Aconitum Carmichaelii), Guizhi (Cinnamomum Cassia), Renshen (Panax Ginseng), and Huangqi (Astragalus Membranaceus). Depending on the type of pathogenic excess, the treatment can be modified with Tongqiao Huoxue Decoction (通窍活血汤), adding Yujin (Curcuma Longa) and Xiangfu (Cyperus Rotundus) to invigorate blood and promote yang, or with Fabanxia (Pinellia Ternata), Chenpi (Citrus Reticulata), Zhishi (Citrus Aurantium) plus Sijunzi Decoction (四君子汤) to resolve phlegm and promote yang. Both the root and branch are treated simultaneously. When yang qi is activated, blood and qi are nourished, and when pathogenic excess is expelled, the blood vessels are unblocked. This approach aims to provide a treatment strategy for vascular dementia.
2.Hypertension risk stratification prediction model based on frequency-domain pulse wave Mel-scale frequency cepstral coefficient features

Objective To propose a frequency-domain pulse wave prediction model based on fusion attention mechanism,improving the low classification accuracy and poor generalization performance of hypertension time-domain pulse wave classification based on artificial intelligence technology.Methods Firstly,the time-domain pulse wave was transformed into frequency-domain Mel-scale frequency cepstral coefficient features to enhance its discriminability.Then,temporal convolutional network and Transformer structures were employed to extract the deep features of pulse waves,and self-attention mechanism and selective kernel attention were combined for decision fusion to extract relevant features.Floodings regularization method was used to indirectly control the training loss and prevent overfitting.A 5-fold cross-verification experiment was conducted based on 527 clinical pulse diagnosis data provided by Longhua Hospital,Shanghai University of Traditional Chinese Medicine and Shanghai Traditional Chinese Medicine-Integrated Hospital.Additionally,the extreme gradient boosting algorithm was employed to calculate the contribution rate ranking of frequency-domain pulse wave features,and the key factors affecting the classification accuracy of the model were analyzed to provide reference for the clinical auxiliary diagnosis of traditional Chinese medicine.Results The evaluation metrics accuracy,F1 score,precision,recall rate and area under curve value of the model proposed in this study were 0.939 6,0.924 9,0.940 9,0.929 5,and 0.993 4,respectively.The static characteristics of the pulse wave,the contribution rate of the first-order difference and the second-order difference coefficients were relatively balanced,indicating that the degree of hypertension risk was not only related to the static characteristics of the pulse wave,but also to the dynamic characteristics of the pulse wave.Conclusion The proposed model has higher classification accuracy and generalization performance compared to typical pulse wave classification models.
3.Association of Perceived Stress With Depression Among Vaccinated Healthcare Workers During the SARS-CoV-2 Variant Outbreak: The Mediating Role of Compassion Fatigue

Objective:
Depression was common during coronavirus disease-2019 (COVID-19) pandemic, while the association of perceived stress with depression among vaccinated healthcare workers has not been investigated. This study aimed to address this issue.
Methods:
We included a total of 898 fully vaccinated healthcare workers during the outbreak of severe acute respiratory syndrome coronavirus 2 Delta variant in Nanjing, 2021. Depression was ascertained by Patient Health Questionnaire-9, with a cut-off score of ≥5 indicative of mild-to-severe depression. Perceived stress, resilience, and compassion fatigue were assessed by Perceived Stress Scale-10, Resilience Scale-25, and Professional Quality of Life Scale version-5, respectively. Logistic regression analyses were used to estimate the odds ratio (OR) and 95% confidence interval (CI), along with subgroup and mediation analyses.
Results:
The prevalence of mild-to-severe depression was 41.1% in vaccinated healthcare workers. The odd of mild-to-severe depression was increased with higher perceived stress. Compared with vaccinated healthcare workers with the lowest tertile of perceived stress, those with the highest tertile had increased odds of mild-to-severe depression by 120% (OR 2.20, 95% CI 1.46 to 3.31) after multivariable-adjustment. However, perceived stress was not associated with mild-to-severe depression in vaccinated healthcare workers with strong resilience, but was in those with weak resilience (pinteraction=0.004). Further analysis showed that compassion fatigue mediated the relationship between perceived stress and mild-to-severe depression, with a mediating effect of 49.7%.
Conclusion
Perceived stress was related to an increased odd of mild-to-severe depression in vaccinated healthcare workers during COVID-19 pandemic, and this relationship might be explained by compassion fatigue.
4.Heavy metal pollution of soil and chronic diseases among residents around an abandoned rare earth ore in Jiangxi

ObjectiveTo understand the distribution characteristics of soil metal pollution around an abandoned rare earth ore in Jiangxi and the health impact on the surrounding residents. MethodsAccording to the distribution of abandoned rare earth ore, the village was divided into mining and non-mining areas. The prevalence of chronic diseases among residents over 15 years old in the village was collected through a self-designed questionnaire. Twenty-three soil samples were collected. The contents of rare earth metals (including lanthanum, cerium, praseodymium, and neodymium) and heavy metals (including arsenic (metalloids), cadmium, and lead) in the soil samples were determined by inductively coupled plasma mass spectrometry (ICP-MS). ResultsThe metal content showed a cumulative increasing trend. One of the23 sampling sites showed mild cadmium pollution. Compared with non-mining farmland, the metal content of farmland soil around the abandoned rare earth mine was relatively higher. The residents' top six self-reported chronic diseases were hypertension, chronic bronchitis, diabetes, stroke, hyperlipidemia, and cataract. The prevalence of hypertension in mining area was higher than non-mining area (χ2=4.141, P=0.042). The main related factors for hypertension in residents were the increase in age (OR=14.576, 95%CI: 2.773‒76.605) and body mass index (OR=3.147, 95%CI: 1.121‒8.835). ConclusionAbandoned rare earth ore may have a potential impact on the health of surrounding residents.
5.Predictive factors of poor prognosis in children with acute kidney injury treated with renal replacement therapy

Objective:To investigate the predictive factors of poor prognosis in children with acute kidney injury (AKI) treated with renal replacement therapy (RRT).Methods:In this retrospective case-control study, the clinical data were collected from 134 pediatric patients (82 male, 52 female) with AKI treated with RRT in six tertiary hospitals from May 2015 to June 2018. According to the serum creatinine level at discharge, the patients were divided into the favorable outcome group and unfavorable outcome group. The data of sex, age, primary diseases, AKI stage, time from diagnosis of AKI to start of RRT (h) and whether to start RRT within 24 hours, urine volume and complications between the two groups were compared. Continuous variables were compared by t test and Mann-Whitney U test, and percentage or proportions were compared by Chi square test. The predictive factors of adverse prognosis were analyzed by using univariate and unconditional binary logistic regression analysis. Results:The average age of the 134 AKI patients was (6±4) years. There were 114 patients (85.0%) in the favorable outcome group and 20 patients (15.0%) in the unfavorable outcome group. No statistically significant differences were found between the two groups in terms of sex (χ 2=2.596, P=0.107), age ( t=0.718, P=0.474), primary disease (χ 2=2.076, P=0.722), AKI stage (χ 2=0.004, P=0.998), time from diagnosis of AKI to start RRT (h) ( P=0.745), whether to start RRT within 24 hours (χ 2=0.016, P=0.899), urine volume (χ 2=3.118, P=0.374), fluid overload (χ 2=0.014, P=0.905), multiple organ dysfunction syndrome (MODS) (χ 2=2.972, P=0.085), acidosis (χ 2=3.204, P=0.073), hyperkalemia (χ 2=2.829, P=0.093), the level of blood urea nitrogen ( t=1.351, P=0.179) and serum creatinine ( P=0.901) at the beginning of RRT. In the unfavorable outcome group, the proportion of patients with mechanical ventilation (45.0% (9/20) vs. 12.3% (14/114), χ 2=12.811, P<0.01) and the incidence of extra organ injury (≥3) (30.0% (6/20) vs. 10.5% (12/114), χ 2=6.365, P=0.041) were higher than those in the favorable outcome group. Logistic regression analysis showed that mechanical ventilation ( OR=12.540, 95 %CI: 3.376-46.577, P<0.01) and hyperkalemia ( OR=4.611, 95 %CI: 1.265-16.805, P=0.021) were the predictive factors of poor prognosis in patients with AKI treated with RRT. Conclusion:Mechanical ventilation and hyperkalemia may predict a poor prognosis in AKI patients treated with RRT.
6. Clinical study of salvage strategy for patients with locally recurrent esophageal cancer after definitive radiochemotherapy

Objective:
To investigate the salvage strategy and efficacy for patients with locally recurrent esophageal squamous cell carcinoma after definitive radiochemotherapy.
Methods:
A total of 126 patients who met the inclusion criteria were enrolled in this study and divided into the salvage surgery, salvage radiochemotherapy and best supportive care.
Results:
Fifty-eight of 126 patients received salvage esophagectomy, 52 underwent salvage radiochemotherapy and the remaining 16 patients received best supportive care. The 1-, 3-, 5-year overall survival rates of patients receiving salvage therapy were 51%, 16% and 4% for the three groups, whereas all patients in the best supportive care group died within 12.0 months (
7.Effects of nutritional status investigation combined with WeChat platform intervention in ulcerative colitis children

Objective To explore the effects of nutritional status investigation combined with WeChat platform intervention on nutritional quality, weight and disease recurrence rate of ulcerative colitis (UC) children. Methods A total of 96 UC children were selected in Affiliated Hospital of Jining Medical University. From January to December 2017, 48 children were included in control group. From January to December 2018, 48 children were included in observation group. Control group carried out routine follow-up. On the basis of that in control group, observation group implemented nutritional status investigation combined with WeChat platform intervention. We compared the disease cognition, children's nutrition cognition, ratio of strictly implementing doctor's advice and correcting the habit of doting on children, family members' nursing satisfaction among children's patients between two groups before intervention and 6 months after intervention. Besides, we observed the nutritional biochemical indexes before intervention and three weeks after intervention, and dietary intake as well as nutritional status, body mass index (BMI), bone mineral density (BMD) before intervention and 6 months after intervention, and disease recurrence rate of children within 6 months. Results A total of 6 months after intervention, the disease cognition, children's nutrition cognition, ratio of strictly implementing doctor's advice and correcting the habit of doting on children, nursing satisfaction among children's patients, and daily protein, fat, sugar as well as energy intake, BMI and BMD in observation group were higher than those in control group with statistical differences (P< 0.05). Three weeks after intervention, serum albumin and prealbumin of children in observation group were higher than those in control group with statistical differences (P<0.05). Conclusions Nutritional status investigation combined with WeChat platform intervention can improve the disease and children's nutrition cognition of UC children's patients, promote them to strictly implement doctor's advice, and improve UC children's nutritional status, weight and growth, and reduce the risk of disease recurrence, and enhance nursing satisfaction which is worth to be used widely in clinical application.
8.Lentiviral vector mediated up?regulation of miR?126 promotes migration and invasion on AGS cells

Objective To establish an AGS cell line that stably expressing miR?126 and to study the effect of miR?126 on the proliferation and metastatic abilities of the AGS cell line in vitro. Methods AGS cells were infected by lentivirus with Lv?has?mir?126. After confirmation by RT?PCR ,CCK?8 and clone formation assays were used to evaluate the effect of miR?126 on AGS cell growth. Transwell migration and invasion assays were used to evaluate the effect of miR?126 on metastasis of AGS cells. Results We verified correct construction of recombinant AGS cells. RT?PCR confirmed mRNA levels of miR?126 existed significantly differences among the recombinant cell lines (P< 0.05). Proliferation assays and clone formation assays did not show a remarkable growth suppression in AGS?mir?126 cell line. However,transwell assay showed a notable acceleration in AGS?mir?126(P< 0.05). Conclusions We successfully constructed recombinant AGS cell line with stably high miR?126 expression level. MiR?126 could facilitate the metastasis of AGS cell in vitro.
9.Reliability and validity of the Stroke and Aphasia Quality of Life scale (Chinese version)

Objective To develop a quality of life scale suitable for aphasic Chinese stroke patients by translating and evaluating the psychometric properties of the original 39-generic version of the Stroke and Aphasia Quality of Life scale (SAQOL-39g).Methods The SAQOL-39g was translated into Chinese and edited.The well edited and translated self-rated and non-self-rated scales were used to test 86 aphasia patients and their caretakers to evaluate the feasibility,internal consistency,test-retest reliability,content validity,and construct validity of the scale.Results The feasibility of the Chinese version of the SAQOL-39g scale was sufficient,with 97% of the ques tionnaires completed.The average time taken to complete the self-report version was (21.4±4.37) minutes,with (13.25±5.61) minutes needed for the proxy version.The Cronbach's alpha values for the overall survey ranged from 0.879 to 0.950 and for the subdomains from 0.863 to 0943 for both forms,suggesting satisfactory internal reliability.The test-retest coefficients for the two forms ranged from 0.804 to 0.974 and from 0.861 to 0.987.A total of 3 common factors were extracted using factor analysis,and the cumulative contribution rate was 59.7%.The consistency between the self-reports and the proxy-reports was good.Conclusion The Chinese version of the SAQOL-39g scale demonstrates good feasibility,reliability and validity,and good consistency between the self-reported and proxy-reported versions.It seems suitable for assessing the quality of life of Chinese stroke patients with aphasia.
10. A retrospective analysis of prognosis in favorable-risk acute myeloid leukemia patients with different consolidation regimens

Objective:
To explore the impact on prognosis in favorable-risk acute myeloid leukemia (AML) patients with different consolidation regimens after first complete remission (CR1).
Methods:
A total of 107 cases of non-refractory adult AML from January 2010 to June 2015 in single center were enrolled in the study. HD-Ara-C group (38 cases) as the control group, we explore the prognosis in three consolidation regimens, including micro-transplantation (16 cases) , autologous transplantation (auto-PBSCT, 14 cases) , allogeneic transplantation (allo-HSCT, 39 cases).
Results:
Of 107 patients (59 males and 48 females) , with a median age of 33 (16-59) years old and a median follow-up of 36.5 (5.3-79.1) months, the overall relapse rate was 20.6% (22/107) , and overall mortality rate was 18.7% (20/107). The 5 years cumulative relapse rate (CIR) of HD-Ara-C, micro-transplantation, auto-PBSCT and allo-HSCT group were 39.7%, 6.2%, 14.3% and 5.6%, respectively (
