1.Vascular endothelial growth factor combined with basic fibroblast growth factor improves replicative senescence of bone marrow mesenchymal stem cells

BACKGROUND:Mesenchymal stem cells are susceptible to senescence during in vitro expansion,which greatly hinders their application in vivo and in vitro.How to improve the replicative senescence of mesenchymal stem cells is an urgent problem to be solved in tissue engineering. OBJECTIVE:To determine whether vascular endothelial growth factor combined with basic fibroblast growth factor can improve the aging of bone marrow mesenchymal stem cells caused by replicative passage. METHODS:Rat bone marrow mesenchymal stem cells were extracted by whole bone marrow adhesion method.Passage 2 cells were selected as normal control group.Passage 7 and later algebraic cells were selected as aging model group.Vascular endothelial growth factor(50 μg/L),basic fibroblast growth factor(10 μg/L),and their combination were administered.Cell proliferation was detected by CCK-8 assay.Cell senescence was observed by β-galactosidase activity staining.Cytoskeleton size and colony formation ability were observed by phalloidine staining and Giemsa staining,respectively,and the levels of senescence-related genes P16,P21,and P53 were detected by qRT-PCR.Gene expression levels of P16,P21,and P53 were tested by qRT-PCR. RESULTS AND CONCLUSION:(1)Vascular endothelial growth factor combined with basic fibroblast growth factor could promote the proliferation of aged bone marrow mesenchymal stem cells,which began to enter the plateau stage on day 9,and the absorbance value of the combined intervention group was significantly higher than that of the model group on day 9(P<0.05).(2)The phenotypic markers of the cells in the combined intervention group did not change,and the cell morphology changed from broad to slender.(3)Compared with the model group,the positive rate of β-galactosidase was significantly decreased(P<0.01);the number of nuclei increased(P<0.001);the total area of cytoskeleton increased(P<0.01);colony formation ability was enhanced(P<0.05);expression level of P16 was decreased(P<0.01)in the combined intervention group.These results indicate that vascular endothelial growth factor combined with basic fibroblast growth factor can improve the senescence of bone marrow mesenchymal stem cells caused by replicative passage without changing the cell phenotype.
2.A case of familial dysalbuminemic hyperthyroxinemia with growth retardation

A retrospective analysis was made on clinical data of a case of familial dysalbuminemic hyperthyroxinemia (FDH) treated at Hebei Children′s Hospital in August 2021.The patient, female, 4 years and 2 months old, was diagnosed with growth retardation.Genetic sequencing of the child and her immediate family revealed a heterozygous mutation, c.725G>A(p.R242H) in exon 7 of the albumin gene, which confirmed the diagnosis of FDH.The growth and development of the girl were monitored regularly.Early identification and diagnosis of FDH can prevent misdiagnosis or improper antithyroid medication from affecting children′s growth and development, and growth hormone therapy is effective for children with FDH who are slow growing.
3.Development of Cognitive Assessment Scale for Spinal Cord Injury Rehabilitation and its reliability and validity

Objective:To develop a Cognitive Assessment Scale for Spinal Cord Injury (SCI) Rehabilitation and conduct reliability and validity tests in community-dwelling patients with SCI.Methods:Based on expectation value theory, social cognition theory, and goal setting theory, a Cognitive Assessment Scale for SCI Rehabilitation was developed through literature review, group discussions, patient trials, and expert verification. From February to December 2021, convenience sampling was used to select 231 community-dwelling patients with SCI as research subjects, including 67 community-dwelling patients with SCI who participated in rehabilitation training at Shanghai Sunshine Rehabilitation Center and 164 patients with SCI in the "Hope Home" WeChat group of Shanghai Sunshine Rehabilitation Center. Research subjects were surveyed using the Cognitive Assessment Scale for SCI Rehabilitation (patient version), 9-item depression scale of Patient Health Questionnaire, 7-item Generalized Anxiety Disorder Scale, EuroQol 5 Dimension-Visual Analogue Scale (EQ-VAS), General Self-Efficacy Scale, and general information questionnaire. SPSS 16.0 software and Amos 21.0 software were used for correlation analysis and reliability and validity testing.Results:The Cognitive Assessment Scale for SCI Rehabilitation (patient version) included two primary dimensions, eight secondary dimensions, and 24 items. The trial showed good results among patients with SCI and their caregivers, and experts generally agreed. Exploratory factor analysis found that the scale were divided into recognition dimension and understanding dimension. Cronbach's α coefficient of the scale was 0.98, the correlation coefficient between each item and its corresponding dimension was 0.75 to 0.88, and our results indicated good test-retest reliability. Correlation analysis showed that patient anxiety and depression scores were negatively correlated with rehabilitation cognitive scores ( P<0.05), and self-efficacy, quality of life were positively correlated with rehabilitation cognitive scores ( P<0.05) . Conclusions:The Cognitive Assessment Scale for SCI Rehabilitation is scientific and feasible, with good reliability and validity, and can be used to evaluate the rehabilitation cognition of community-dwelling patients with SCI.
4.Summary of best evidence for the application of massage in meibomian gland dysfunction-related dry eye

Objective:To summarize the best evidence for the application of massage in the management of meibomian gland dysfunction (MGD) -related dry eye.Methods:A systematic search was conducted in databases and websites including UpToDate, BMJ Best Clinical Practice, Joanna Briggs Institute Evidence-Based Healthcare Center database, National Guideline Clearinghouse, Medlive, PubMed, China National Knowledge Infrastructure, and WanFang data. The search covered literature on massage for MGD-related dry eye, including clinical decisions, best practices, evidence summaries, guidelines, expert consensus, systematic reviews, and randomized controlled trials. The search period extended to December 2023. Two researchers independently evaluated the quality of the literature and extracted and summarized the evidence.Results:A total of nine articles were included: three guidelines, four expert consensus papers, and two systematic reviews. Twenty-five pieces of best evidence were summarized from six aspects: massage practitioners, massage methods and indications, massage instruments and devices, massage duration and frequency, combination therapy, and the safety and efficacy of massage.Conclusions:This study summarizes the best evidence for applying massage in MGD-related dry eye. It is recommended that healthcare professionals apply this evidence in combination with clinical context and patient preferences.
5.Protective effects of total saponins from Panax japonicus against high-fat diet-induced testicular Sertoli cell junction damage in mice.

OBJECTIVE:
To investigate the protective effects of total saponins from Panax japonicus (TSPJ) against high-fat dietinduced testicular Sertoli cell junction damage in mice.
METHODS:
Forty male C57BL/6J mice were randomized into normal diet group, high-fat diet group, and low-dose (25 mg/kg) and high-dose (75 mg/kg) TSPJ treatment groups (n=10). The mice in the normal diet group were fed a normal diet, while the mice in the other groups were fed a high-fat diet. After TSPJ treatment via intragastric administration for 5 months, the testes and epididymis of the mice were collected for measurement of weight, testicular and epididymal indices and sperm parameters. HE staining was used for histological evaluation of the testicular tissues and measurement of seminiferous tubule diameter and seminiferous epithelium height. The expression levels of ZO-1, occludin, claudin11, N-cadherin, E-cadherin and β-catenin in Sertoli cells were detected with Western blot, and the localization and expression levels of ZO-1 and β-catenin in the testicular tissues were detected with immunofluorescence assay. The protein expressions of LC3B, p-AKT and p-mTOR in testicular Sertoli cells were detected using double immunofluorescence assay.
RESULTS:
Treatment with TSPJ significantly improved high-fat diet-induced testicular dysfunction by reducing body weight (P < 0.001), increasing testicular and epididymal indices (P < 0.05), and improving sperm concentration and sperm viability (P < 0.05). TSPJ ameliorated testicular pathologies and increased seminiferous epithelium height of the mice with high-fat diet feeding (P < 0.05) without affecting the seminiferous tubule diameter. TSPJ significantly increased the expression levels of ZO-1, occludin, N-cadherin, E-cadherin and β-catenin (P < 0.05) but did not affect claudin11 expression in the testicular tissues. Immunofluorescence assay showed that TSPJ significantly increased ZO-1 and β-catenin expression in the testicular tissues (P < 0.001), downregulated LC3B expression and upregulated p-AKT and p-mTOR expressions in testicular Sertoli cells.
CONCLUSION
TSPJ alleviates high-fat diet-induced damages of testicular Sertoli cell junctions and spermatogenesis possibly by activating the AKT/mTOR signaling pathway and inhibiting autophagy of testicular Sertoli cells.

6.Biological Connotation of Pathogenesis of Ulcerative Colitis with Liver Depression and Spleen Deficiency Based on Bile Acid-intestinal Flora Axis

Ulcerative colitis (UC) is one of the chronic refractory inflammatory bowel diseases characterized by abdominal pain, diarrhea, and mucus, pus and blood in the stool. In recent years, with changes in human life style and improvements of the diagnosis, the incidence and prevalence of UC have been increasing. The pathogenesis of UC is closely related to intestinal mucosal immune dysfunction, intestinal flora disturbance, and abnormal bile acid secretion. Patients with UC have abnormal bile acid secretion and intestinal flora imbalance. A large number of studies have found that abnormal bile acid secretion inhibits immune function, affects signal transduction, and destroys the intestinal mucosal barrier. Intestinal flora disturbance has an important impact on the occurrence and development of inflammation, immune homeostasis, and stress. Bile acids indirectly or directly affect the structure and function of intestinal flora, and at the same time, they produce secondary bile acids under the modification of intestinal flora, entering the liver through enterohepatic circulation. Therefore, the complex dialogue mechanism of bile acid-intestinal flora axis is closely related to the occurrence and development of UC. Based on the basic theory of traditional Chinese medicine(TCM) and clinical research, it is found that emotion is an important factor that induces this disease, spleen and stomach weakness is the root of the disease, and liver depression and spleen deficiency are the key pathogenesis of UC. Combined with modern medicine and molecular biology research, it is believed that abnormal secretion of bile acids is a microscopic manifestation of liver depression in TCM, and intestinal flora disturbance is the biological basis of spleen deficiency. In the pathogenesis of UC, the imbalanced bile acid-intestinal flora axis is consistent with the pathogenesis of liver depression and spleen in TCM. The exploration of the biological connotation of the pathogenesis of UC with liver depression and spleen deficiency from the perspective of bile acid-intestinal flora axis can better explain the scientific nature of its pathogenesis, which provides new clinical solutions and reliable references for studying the pathogenesis of UC with liver depression and spleen deficiency and finding representative prescriptions to prevent and treat this disease.
7.Switching between eltrombopag and recombinant human thrombopoietin in patients with immune thrombocytopenia: an observational study.

BACKGROUND:
Recombinant human thrombopoietin (rh-TPO) and eltrombopag are two distinct TPO receptor agonists (TPO-RAs) with different mechanisms. During the pandemic, when immunosuppressive medications are controversial, switching to another TPO-RA may be worth exploring in patients who do not benefit from their first TPO-RA. We investigated the outcomes of switching from rh-TPO to eltrombopag or vice versa in immune thrombocytopenia (ITP) patients.
METHODS:
This prospective, open-label, observational investigation included 96 adult ITP patients who needed to switch between rh-TPO and eltrombopag between January 2020 and January 2021 at Peking University People's Hospital in China. The study evaluated response rates and platelet counts at different time points after the switch, bleeding events, time to response, duration of response, and adverse events.
RESULTS:
At 6 weeks after switching, response was observed in 21/49 patients (43%) who switched for inefficacy and 34/47 patients (72%) who switched for non-efficacy-related issues. In the inefficacy group, 9/27 patients (33%) responded to eltrombopag, and 12/22 patients (55%) responded to rh-TPO. In the non-efficacy-related group, 21/26 (81%) and 13/21 (62%) patients in the eltrombopag and rh-TPO groups maintained their response rates at 6 weeks after switching, respectively. Response at 6 months was achieved in 24/49 patients (49%) switching for inefficacy and 37/47 patients (79%) switching for non-efficacy issues. In the inefficacy group, 13/27 patients (48%) responded to eltrombopag, and 11/22 patients (50%) responded to rh-TPO. In the non-efficacy-related group, 22/26 patients (85%) and 15/21 patients (71%) in the eltrombopag and rh-TPO groups maintained their response rates at 6 months after switching, respectively. Both eltrombopag and rh-TPO were well tolerated.
CONCLUSIONS:
Our study confirmed the safety and effectiveness of switching between rh-TPO and eltrombopag for ITP patients who had no response to or experienced adverse events with their first TPO-RA. When the switch was motivated by other reasons, including patient preference and platelet count fluctuations, the probability of response was high.

8.Application of Mini-CEX combined with simulation teaching + case-based learning in pediatric clinical teaching

Objective:To explore the effect of mini-clinical evaluation exercise (Mini-CEX) combined with simulation teaching + case-based learning (CBL) in pediatric clinical teaching.Methods:A total of 60 medical students who practiced in Department of Pediatrics, Affiliated Hospital of Jining Medical College from August 2019 to August 2020 were selected as the research objects. According to different teaching methods, they were divided into control group ( n=30) and observation group ( n=30). The control group received traditional teaching, and the observation group received Mini-CEX combined with simulated teaching + CBL. The assessment results, comprehensive ability of medical interview, critical thinking ability and teaching satisfaction of the two groups were compared. SPSS 20.0 was performed for t-test and Chi-square test. Results:The test scores and total scores of the observation group were significantly higher than those of the control group ( P<0.05). After teaching, the Mini-CEX scores, the CTDI-CV (critical thinking disposition inventory Chinese version) scores and the total scores of the two groups increased significantly, and the above scores were significantly higher in the observation group than the control group ( P<0.05). The scores of the teaching program satisfaction questionnaire in the observation group were significantly higher than those in the control group ( P<0.05). Conclusion:The application of Mini-CEX combined with simulation teaching + CBL in pediatric clinical teaching can help to cultivate the medical interview ability, clinical practice ability and critical thinking ability of interns, improve the examination results, and improve the teaching satisfaction.
9.Analysis of the status quo and influencing factors of spiritual care needs of patients with gynecological malignant tumors

Objective:To explore the status quo of spiritual care needs of patients with gynecological malignant tumors, and analyze their influencing factors.Methods:Totally 100 patients with gynecological tumors hospitalized by the Fifth Affiliated Hospital of Xinjiang Medical University from March 2018 to March 2021 were selected by convenient sampling and investigated with the general information questionnaire, the Chinese version of the Death Attitude Profile-Revised (DAP-R) , and the Chinese version of the Nurse Spiritual Therapeutics Scale (NSTS) . Multiple linear regression was used to analyze the related factors affecting the spiritual care needs of patients with gynecological tumors. A total of 100 questionnaires were distributed in this study, and 100 valid questionnaires were returned.Results:The total score of spiritual care needs of 100 patients with gynecological malignant tumors was (65.14±11.37) , at a medium level. The results of multiple linear regression analysis showed that the duration of disease, religious belief, tumor stage, and fear of death were the influencing factors for the spiritual care needs of patients with gynecological malignant tumors ( P<0.05) . Conclusions:The spiritual care needs of patients with gynecological malignant tumors are at a moderate level, which is affected by the duration of disease, religious beliefs, tumor stage, and fear of death. Targeted nursing interventions should be given to patients with gynecological malignant tumors in clinical practice.
10.Effects of security risk self-examination mode on nursing quality in Operating Room

Objective:To explore the effects of security risk self-examination mode on nursing quality in Operating Room.Methods:From January to December 2018, we selected 60 surgery patients from Operating Room of the Fifth Affiliated Hospital of Xinjiang Medical University in control group with routine nursing of Operating Room. From January to December 2019, we selected 60 surgery patients in intervention group and implemented the security risk self-examination mode on the basis of routine nursing of Operating Room. We compared the nursing quality scores, nursing adverse events and incidences of postoperative infection between two groups.Results:All nursing quality scores of intervention group were higher than those of control group with statistical differences ( P<0.05) . The incidences of nursing adverse events and postoperative infections in intervention group were lower than those in control group also with statistical differences ( P<0.05) . Conclusions:The security risk self-examination mode can improve the nursing quality of Operating Room and reduce the incidence of nursing adverse events.
