1.Levels and clinical significance of serum miR-651 and miR-630 in patients with cervical cancer

		                        			Objective To investigate the levels and clinical significance of serum microRNA(miR)-651 and miR-630 in patients with cervical cancer.Methods From June 2017 to May 2020,108 cervical cancer patients accepted by our hospital were collected as the cervical cancer group.Meantime,100 cervical intraepithelial neoplasia(CIN)patients treated in our hospital were regarded as the CIN group,and 110 healthy individuals who underwent physical examination were regarded as the control group.Real-time fluorescence quantitative PCR(qRT-PCR)was applied to detect serum levels of miR-651 and miR-630,while analyzing the relationship between serum miR-651 and miR-630 as well as clinical features and prognosis of patients.Results The serum levels of miR-651 and miR-630 in the cervical cancer group and CIN group were obviously lower than those in the control group(P<0.05),while the serum levels of miR-651 and miR-630 in the cervical cancer group were obviously lower than those in the CIN group(P<0.05).The expression levels of serum miR-651 and miR-630 were positively corre-lated(r = 0.542,P<0.05).The serum levels of miR-651 and miR-630 were related to HPV infection,differentia-tion,lymph node metastasis,and FIGO staging(P<0.05).The overall survival rate of patients with low levels of miR-651 and miR-630 was lower than that of patients with high levels.Cox regression analysis showed that,HPV infection,degree of differentiation,lymph node metastasis,FIGO staging,miR-651,and miR-630 were all influ-encing factors for the prognosis of cervical cancer patients(P<0.05).Conclusion The serum levels of miR-651 and miR-630 in cervical cancer patients decrease,which are related to HPV infection,differentiation,lymph node metastasis,FIGO staging,and prognosis.
		                        		
		                        		
		                        		
		                        	
2.Value of brain 18F-FDG PET/CT in the preoperative localization of epileptogenic foci in refractory epilepsy patients with negative MRI

		                        			Objective:To evaluate the value of 18F-FDG PET/CT for preoperative localization of epileptogenic foci in refractory epilepsy patients with negative MRI. Methods:Clinical data (550 lobes) of 55 epilepsy patients (38 males, 17 females, age (20.0±8.1) years) with negative MRI who underwent preoperative 18F-FDG PET/CT-MRI between January 2014 and June 2020 at the First Affiliated Hospital of Jinan University were retrospectively analyzed. The sensitivity, specificity, accuracy, positive predictive value (PPV) and negative predictive value (NPV) of 18F-FDG PET/CT, video electroencephalogram (VEEG), PET/CT+ VEEG and PET/CT-VEEG for localizing epileptogenic foci were calculated using stereoelectroencephalography (SEEG) and the outcomes of at least 1 year of postoperative follow-up as reference standards. χ2 test was used to compare the efficiencies of different examination modalities for unilobar, multilobar and all patients. Results:The correct lateralization rate of epileptogenic foci was 92.6%(25/27) using PET/CT. The sensitivity, specificity, accuracy, PPV and NPV of PET/CT for localization of epileptogenic foci were 65.1%(54/83), 77.9%(364/467), 76.0%(418/550), 34.4%(54/157) and 92.6%(364/393), respectively. The sensitivities of PET/CT-VEEG for localization of epileptogenic foci in all patients and patients with multilobar epilepsy were higher than those of VEEG alone (75.9%(63/83) vs 45.8%(38/83), 68.6%(35/51) vs 31.4%(16/51); χ2 values: 15.80, 14.16, both P<0.001). The specificities of PET/CT+ VEEG for localization of epileptogenic foci in all patients and patients with unilobar epilepsy were higher than those of VEEG alone (97.6%(456/467) vs 94.6%(442/467), 97.9%(282/288) vs 94.1%(271/288); χ2 values: 5.66, 5.48; P values: 0.017, 0.019). The sensitivity of PET/CT-VEEG (PET/CT and VEEG concordance) for localization of epileptogenic foci was higher than that of PET/CT+ VEEG (PET/CT and VEEG discordance) (8/9 vs 28.4%(21/74); χ2=10.40, P=0.001), and its specificity and accuracy were higher than those of PET/CT-VEEG (PET/CT and VEEG discordance) (93.4%(57/61) vs 71.7%(291/406), 92.9%(65/70) vs 72.1%(346/480); χ2 values: 13.23, 13.96; both P<0.001). Conclusions:18F-FDG PET/CT can localize and lateralize epileptogenic foci in patients with negative MRI. The combination of 18F-FDG PET/CT and VEEG improves the sensitivity, specificity, and accuracy for epileptogenic foci detection. 18F-FDG PET/CT is more accurate in detecting epileptogenic foci when it is concordant with VEEG.
		                        		
		                        		
		                        		
		                        	
3.Deep neural networks analysis of 18F-FDG PET imaging in postoperative patients with temporal lobe epilepsy

		                        			Objective:To predict the short-term postoperative recurrence status of patients with refractory temporal lobe epilepsy (TLE) by analyzing preoperative 18F-FDG PET images and patients′ clinical characteristics based on deep residual neural network (ResNet). Methods:Retrospective analysis was conducted on preoperative 18F-FDG PET images and clinical data of 220 patients with refractory TLE (132 males and 88 females, age 23.0(20.0, 30.2) years)) in the First Affiliated Hospital of Jinan University between January 2014 and June 2020. ResNet was used to perform high-throughput feature extraction on preprocessed PET images and clinical features, and to perform a postoperative recurrence prediction task for differentiating patients with TLE. The predictive performance of ResNet model was evaluated by ROC curve analysis, and the AUC was compared with that of classical Cox proportional risk model using Delong test. Results:Based on PET images combined with clinical feature training, AUCs of the ResNet in predicting 12-, 24-, and 36-month postoperative recurrence were 0.895±0.073, 0.861±0.058 and 0.754±0.111, respectively, which were 0.717±0.093, 0.697±0.081 and 0.645±0.087 for Cox proportional hazards model respectively ( z values: -3.00, -2.98, -1.09, P values: 0.011, 0.018, 0.310). The ResNet showed best predictive effect for recurrence events within 12 months after surgery. Conclusion:The ResNet model is expected to be used in clinical practice for postoperative follow-up of patients with TLE, helping for risk stratification and individualized management of postoperative patients.
		                        		
		                        		
		                        		
		                        	
4.Allogeneic hematopoietic stem cell transplantation for children with severe aplastic anemia: a retrospective analysis from a single center

		                        			Objective:To evaluate the clinical efficacy of allogeneic hematopoietic stem cell transplantation(allo-HSCT)in children with severe aplastic anemia(SAA).Methods:Twenty-seven cases with SAA who had been treated with allo-HSCT from January 2020 to December 2022 were retrospectively analyzed and reviewed.Results:(1)A total of 27 SAA patients were enrolled,including 18 males and 9 females,with a median age of 8 (2-15) years.There were 20 cases of SAA-Ⅰ type,7 cases of SAA-Ⅱ type.Based upon donor sources,three cases of matched sibling donors hematopoietic stem cell transplantation,and 24 cases of haploidentical hematopoietic stem cell transplantation were adopted.(2)Hematopoietic reconstruction was achieved in all 27 cases.The median implantation time of neutrophils and platelets was 10(9-20)days and 12(7-26)days respectively.The cumulative incidence of acute graft-versus-host disease(GVHD)was 66.67%(18/27).The incidence of grade Ⅰ-Ⅱ was 55.56%(15/27)and that of grade Ⅲ-Ⅳ was 11.11%(3/27).The incidence of chronic GVHD was 7.41%(2/27).Transplant-associated thrombotic microangiopathy (TA-TMA) occurred in 7.41%(2/27)patients,cytomegalovirus viremia in 62.96%(17/27)patients,epstein-barr virus infection in 33.33%(9/27)patients,and 14.81%(4/27)patients progressed to post-transplant lymphoproliferative disorder (PTLD).(3)The median follow-up time was 12 (2-28) months.The overall survival rate was 96.29%.Twenty-six patients survived,and one patient died due to multiple complications of severe acute GVHD,TA-TMA,cytomegalovirus infection,PTLD and secondary epilepsy.Conclusion:Allo-HSCT is an effective therapy for SAA in children.The effective rate of this research is 96.29%.Acute GVHD is still the key to therapy.The incidence rate of acute GVHD is 66.67% in this study.The blood incompatibility of donor and recipient may affect the incidence of GVHD.The intensity of GVHD prevention should be reduced after HLA-matched sibling donor-hematopoietic stem cell transplantation so as to avoid the complications of virus recurrence and PTLD.
		                        		
		                        		
		                        		
		                        	
5.Optimization of anesthesia for gastrointestinal endoscopy: effect of TEAS combined with moderate sedation with propofol

		                        			Objective:To evaluate the optimization effect of transcutaneous electrical acupoint stimulation (TEAS) combined with moderate sedation with propofol (TEAS-propofol balanced anesthesia) for gastrointestinal endoscopy.Methods:This was a single-blind randomized controlled trial. American Society of Anesthesiologists Physical Status classification Ⅰ-Ⅲ patients, aged 18-64 yr, undergoing elective gastrointestinal endoscopy at the Endoscopic Diagnosis and Treatment Center of the Affiliated Hospital of Qingdao University from May to August 2022, were divided into 2 groups using the block random allocation method: conventional anesthesia group (group C) and TEAS-propofol balanced anesthesia group (group TPB). Patients received moderate sedation with propofol plus routine anesthesia with fentanyl 50 μg in group C. In TPB group, TEAS was performed at bilateral Neiguan, Hegu and Zusanli acupoints before surgery until the end of surgery, and patients received propofol for moderate sedation (Modified Observer′s Assessment of Alertness/Sedation scale score was 3). The efficacy and safety of anesthesia and parameters related to outcomes were observed and recorded.Results:In this study, 66 patients were recruited, with 33 in each group, the failure rate of anesthesia in both groups was 3%, and no reflux or aspiration was found. Compared with group C, no significant changes were found in the patients′ satisfaction on the same day, intraoperative pain response score, incidence of intraoperative adverse reactions (tachycardia, hypertension, bucking and body movement), awake time, consumption of propofol, rate of intraoperative awareness and rate of patients hoping to receive the same anesthesia method again postoperatively ( P>0.05), the patients′ satisfaction was significantly increased on the next day ( P<0.05), the incidence of intraoperative respiratory depression, physician satisfaction, and degree of postoperative dizziness and nausea were significantly reduced ( P<0.05), and the discharge time and time to the complete recovery of normal behavior function was significantly shortened in group TPB ( P<0.05). Conclusions:The combination of TEAS at bilateral Neiguan, Hegu and Zusanli acupoints with moderate sedation using propofol for gastrointestinal endoscopy is not only safe and effective, but also beneficial to the postoperative outcome of patients, and the effect is better than that of conventional anesthesia with propofol and fentanyl.
		                        		
		                        		
		                        		
		                        	
6.Evaluation Value of Serum DcR3 Combined With COSSH-ACLF Score in Short-term Prognosis of Patients With HBV-ACLF

		                        			Background:HBV-related acute-on-chronic liver failure(ACLF)has a high mortality rate,early and accurate prediction is helpful for formulating the best treatment options.Aims:To investigate the value of serum decoy receptor 3(DcR3)combined with COSSH-ACLF scoring model in predicting short-term(90-day)prognosis in patients with HBV-ACLF.Methods:A total of 205 patients with HBV-ACLF,50 patients with chronic hepatitis B(CHB)and 50 healthy people(control group)from January 2020 to August 2022 at the Third Affiliated Hospital of Nantong University were enrolled.Serum DcR3 level was detected by ELISA.Patients with HBV-ACLF were divided into improvement group and deterioration group according to the survival status at 90 days after admission.Scores of iMELD,CLIF-C ACLF and COSSH-ACLF were calculated.Multivariate Logistic regression analysis was used to evaluate the risk factors affecting short-term prognosis of patients with HBV-ACLF.ROC curve was used to assess the predictive value of serum DcR3 and COSSH-ACLF score.Results:Serum DcR3 level in HBV-ACLF group was significantly higher than that in CHB group and control group(P<0.05).Serum DcR3,iMELD score,CLIF-C ACLF score and COSSH-ACLF score in HBV-ACLF deterioration group were significantly higher than those in improvement group(P<0.001).Multivariate Logistic regression analysis showed that serum DcR3 and COSSH-ACLF score were independent risk factors affecting short-term prognosis of patients with HBV-ACLF(P<0.001).Pearson correlation analysis showed that serum DcR3 was positively correlated with COSSH-ACLF score(r=0.477,P<0.001).ROC curve results showed that AUC of serum DcR3,COSSH-ACLF score for predicting short-term prognosis of patients with HBV-ACLF were 0.869 and 0.897,respectively.AUC of combined prediction model was 0.939.Conclusions:Serum DcR3 and COSSH-ACLF score have a good value in predicting short-term prognosis of patients with HBV-ACLF,and the combination of serum DcR3 and COSSH-ACLF score has higher predictive value.
		                        		
		                        		
		                        		
		                        	
7.Virulence associated characteristics and drug resistance of Klebsiella pneumoniae isolated from patients living in Zhongjiang county of Sichuan province

		                        			Objective:To investigate the distribution characteristics of virulence-related phenotypes/genotype, capsular serotype, drug resistance phenotypes, and sequence typing (ST) of Klebsiella pneumoniae in patients living in Zhongjiang county, improve clinical understanding, and provide evidence for the prevention and control of bacterial drug resistance and clinical rational drug use. Methods:The data of 135 strains of Klebsiella pneumoniae isolated from patients who received treatment in Zhongjiang County People's Hospital from July to December 2019 were retrospectively analyzed. Bacterial identification and drug sensitivity testing were performed using the WalkAway-40Plus automated microbiology system. Strains with a high viscosity phenotype were identified using wire drawing experiments. Hypervirulence-associated capsular serotype and virulence genes were verified by polymerase chain reaction. ST of Klebsiella pneumoniae strain was identified using multilocus sequence typing. Results:Strains with a high viscosity phenotype were identified in 50.4% of the 135 strains. 54.1%, 54.8%, and 54.1% of the strains were positive for virulence genes iucA, iroN, rmpA. The proportion of strains with capsular Serotype K1 or K2 was 11.9% and 15.6%, respectively. A total of 65 kinds of ST were identified, with ST23 and ST37 being the most common, accounting for 11.1% and 6.7%, respectively. The resistance rate of the strains to 16 kinds of antibiotics was 0.0%-25.2%, and the resistance rate to Carbapenem antibiotics, Amikacin, and Tigecycline was less than 1%. The positive rate of virulence gene of strains with a high viscosity phenotype was significantly higher than that of strains without a high viscosity phenotype ( P < 0.001), and its resistance rate to Cephalosporin was significantly lower in strains with a high viscosity phenotype than that in strains without a high viscosity phenotype ( P < 0.001). Conclusion:Klebsiella pneumoniae in Zhongjiang County is characterized by "high virulence and low drug resistance". It is necessary to continuously monitor the changes in the virulence and drug resistance of Klebsiella pneumoniae in Zhongjiang County, Sichuan Province, and be alert to the rapid dissemination of highly virulent strains.

9.Disrupted tenogenesis in masseter as a potential cause of micrognathia.

		                        			Micrognathia is a severe craniofacial deformity affecting appearance and survival. Previous studies revealed that multiple factors involved in the osteogenesis of mandibular bone have contributed to micrognathia, but concerned little on factors other than osteogenesis. In the current study, we found that ectopic activation of Fgf8 by Osr2-cre in the presumptive mesenchyme for masseter tendon in mice led to micrognathia, masseter regression, and the disrupted patterning and differentiation of masseter tendon. Since Myf5-cre;Rosa26R-Fgf8 mice exhibited the normal masseter and mandibular bone, the possibility that the micrognathia and masseter regression resulted directly from the over-expressed Fgf8 was excluded. Further investigation disclosed that a series of chondrogenic markers were ectopically activated in the developing Osr2-cre;Rosa26R-Fgf8 masseter tendon, while the mechanical sensing in the masseter and mandibular bone was obviously reduced. Thus, it suggested that the micrognathia in Osr2-cre;Rosa26R-Fgf8 mice resulted secondarily from the reduced mechanical force transmitted to mandibular bone. Consistently, when tenogenic or myogenic components were deleted from the developing mandibles, both the micrognathia and masseter degeneration took place with the decreased mechanical sensing in mandibular bone, which verified that the loss of mechanical force transmitted by masseter tendon could result in micrognathia. Furthermore, it appeared that the micrognathia resulting from the disrupted tenogenesis was attributed to the impaired osteogenic specification, instead of the differentiation in the periosteal progenitors. Our findings disclose a novel mechanism for mandibular morphogenesis, and shed light on the prevention and treatment for micrognathia.

10.The mediating effect of nurses′self-efficacy in palliative care between the past behavioral experience and core competence

		                        			Objective:To explore the mediating effect of nurses′ self-efficacy in palliative care between the past behavioral experience of end-of-life care and core competence, and provide theoretical reference for improving the core competence of clinical nurses in palliative care.Methods:579 clinical nurses from 2 tertiary general hospitals in Shandong Province were investigated by convenience sampling method using general information questionnaire, nurses′ self-efficacy questionnaire for palliative care and palliative care nurses′ core competency questionnaire. The data were analyzed by SPSS 25.0.Results:The total score of core competence of hospice care of 579 nurses was (71.41 ± 22.74), nurses′ self-efficacy of palliative care was positively correlated with their core competence ( r = 0.648, P<0.01), past behavior experience was positively correlated with self-efficacy ( r = 0.479, P<0.01), positively correlated with core competence ( r = 0.427, P<0.01). Nurses′ self-efficacy of palliative care played a partial mediating role between the pastbehavioral experience and core competence, which accounted for 64.67% of the total effect. Conclusions:The past behavioral experience of end-stage nursing can directly or indirectly affect the core competence of nurses in palliative care through self-efficacy. It can improve the core competence of palliative care by taking active measures to enrich the past behavioral experience of end-of-life care and improve self-efficacy.
