Colorectal cancer is one of the most common malignant tumors, and its morbidity and mortality increase year by year. In recent years, some patients with colorectal cancer have benefited from precision targeted therapies, but the overall prognosis is still unsatisfactory. Treatment of homologous recombination deficiency represented by BRCA1/2 has become a hot research direction at present. With the wide application and exact curative effect of poly (adenosine diphosphate-ribose) polymerase (PARP) inhibitors in ovarian cancer, breast cancer, pancreatic cancer and other malignant tumors, PARP inhibitors are also gradually being used in colorectal cancer. This review summarizes the current research progress of PARP inhibitors in treatment of colorectal cancer.

Sepsis is a common cause of acute kidney injury (AKI), and the pathogenesis of sepsis-related AKI is very complicated. Recent studies have shown that oxidative stress in septic patients damages mitochondria in renal tubular epithelial cells, and causes cell death. Meanwhile, mitochondrial quality control is inhibited, including imbalance of division and fusion, excessive autophagy, and synthesis disorders, which aggravates kidney injury. Therefore, mitochondria play an important role in the pathogenesis of sepsis-related AKI, and can serve as a potential therapeutic target for sepsis-related AKI. This article reviews the mechanism of mitochondria in the pathogenesis of sepsis-related AKI and explores the treatment strategy by targeting mitochondria.

Objective:To explore the demand and mode of palliative care for emergency dying patients by analyzing the case data of emergency death and cardiopulmonary resuscitation. Methods:The data of 776 cases of emergency clinical death and cardiopulmonary resuscitation in the Second Affiliated Hospital of Soochow University from 2017 to 2020 were retrospectively analyzed. Results:A total of 687 patients were included with (70.38 ± 16.57) years old, and 49.8% (342/687) of them were 75 years old and above; among them, 36.0% (247/687) patients or their families chose not to give cardiopulmonary resuscitation (DNR) in the last stage of their lives, and 63.2%(156/247) of DNR patients were 75 years old and above. The top four etiology of DNR were cerebral hemorrhage, respiratory failure, multiple organ dysfunction syndrome and out of hospital cardiac and respiratory arrest. After successful cardiopulmonary resuscitation, 37.5% (45/120) of the patients' family members chose to give up treatment again. The median stay time of DNR patients in the emergency room was 738.7 minutes. Conclusions:The patients who choosed DNR were mainly 75 years old and above, with cerebral hemorrhage, respiratory failure, multiple organ failure and cardiac and respiratory arrest. The detention of these patients in the emergency room increases the congestion of the emergency room, and at the same time, they can not get a peaceful palliative care environment. It is suggested that emergency medical staff should strengthen the awareness and improve the ability of palliative care. A relative independent area and corresponding soothing palliative treatment and nursing should be given to the DNR patients.

Acute ischemic stroke has ischemic penumbra and actual infarct core, and when there is a bigger difference in the volume of the two, it is called " mismatch". It is not only manifested as a mismatch between the clinical manifestations and the infarct core, but also as a mismatch between the infarct core and the perfusion area. The advancement of neuroimaging technology enables this " mismatch" phenomenon to be manifested through different imaging methods or different sequences of the same imaging method, thereby providing more guidance for the further diagnosis and treatment process.

Objective To investigate the effect of the active ingredients of Tianma preparation on the expression of calcitonin gene-related peptide(CGRP) and adenosine A1 receptor in migraine model rats. Methods Eighty-four SPF rats were randomly divided into 7 groups (n=12):sham operation group (group A),electrical stimulation of the trigeminal ganglion group(ESTG group),Sumatriptan group (group C),gastrodin group (group D),4-hydroxybenzyl alcohol group (group E),vanilanol group (group F),β-sitosterol group (group G). The enzyme-linked immuno sorbent assay (ELISA),immunofluorescence and western-blot techniques were used to dected the effect of the active components of Tianma preparation on the expression of CGRP and adenosine A1 receptor. Results Compared with group A,the expression of CGRP in trigeminal ganglia (TG) and trigeminal nucleus caudalis (TNC) in group B rats was significantly increased, while the expression of adenosine A1 receptor was significantly decreased, with statistically significant differences (P<0.01). Compared with group B,the expression of CGRP in TG and TNC in group C and D was significantly decreased, and the expression of adenosine A1 receptor was significantly increased, with statistically significant differences (P<0.01). There was no significant difference between group E,F,G and group B (P>0.05). Compared with group C,the expression of CGRP and adenosine A1 receptor in TG and TNC of rats in group D was not statistically significant (P>0.05). Conclusion Like sumatriptan,The preventive application of gastrodin can alleviate the migraine attack, while the effects of vanillin,4-hydroxybenzyl alcoholand β-sitosterol were not good enough. In addition, gastrodin can inhibit the occurrence of migraine by activating adenosine A1 receptor and inhibiting the release of CGRP.
