Peripheral pulmonary lesions(PPL),including peripheral pulmonary nodules,are common lung problems.As the increase of patients with lung nodules,the demand for tissue sampling also increases.Safe and accurate biopsy techniques are very important for patients to identify benign and malignant lesions.Electronic bronchoscopy is one of the biopsy techniques for the diagnosis of PPL in recent decades.Various guiding techniques,such as radial probe endobronchial ultrasound and virtual navigation bronchoscope,have been proved to improve the performance of conventional bronchoscopy.This paper aims to provide an review of the available data on advanced bronchoscopic techniques and explore their application in diagnosing PPL.

3.Establishment and efficacy evaluation of deep learning model for cardiac conduction system

Objective To investigate the recognition efficiency of AI model based on deep learning for cardiac conduction system(CCS).Methods HE staining sections of cardiac muscle and CCS of 17 cases of non-sudden death were selected,and the gold standard was unanimous recognition by 2 forensic pathologists with more than 20 years of CCS diagnosis experience.Inception V3 algorithm was used to establish AI model and complete CCS identification training and testing.Confusion matrix,accuracy,precision,recall,F1 score,ROC curve and AUC value were used to evaluate the effectiveness of AI model,and accuracy,sensitivity and specificity were used to evaluate the efficiency of manual independent and AI-assisted manual recognition for CCS.Results The accuracy of AI model was 87.3%,the precision was 91.9%,the recall was 81.9%,the F1 score was 86.6%,and the AUC value was 95.3%.The accuracy of AI model was higher than that of senior forensic pathologists.There was no statistical significance in the accuracy of AI-assisted senior forensic pathologists in identifying CCS compared with manual independent detection(P>0.05),while the accuracy of AI-assisted intermediate and junior forensic pathologists in identifying CCS was increased by 8%and 14.33%,respectively,with statistical significance(P<0.05).The accuracy rate of AI-assisted junior forensic pathologists to identify CCS was higher than that of intermediate forensic pathologists in self-diagnosis.Conclusion The AI model could be used for the automatic recognition of CCS,and could improve the diagnostic efficiency of CCS and narrow the gap between the forensic pathologists with low experience and that with high experience.
4.A clinical comparative study of axillary mastoscopic mastectomy and traditional areola incision in treatment of gynecomastia

Objective:To compare the clinical effects of the two methods in surgical treatment of gynecomastia.Methods:The clinical data of 46 patients with gynecomastia were retrospectively analyzed, who received total removal of the glands through mastoscopic assistance (mastoscopic group) or traditional areola incision (traditional group) from Mar. 2017 to Mar. 2018. The two groups were compared in terms of blood loss, the mean operation duration, extubation time, the average hospitalization time, postoperative complications, the total cost and patients’ satisfaction at 6 month after operation. Chi-square test was used to compare the count data between groups, paired t test was used to compare the measurement data between groups, and SPSS18.0 statistical software was used for statistical analysis.Results:The operation was completed in both groups. There were no differences between the two group in age (0.473) , BMI (0.353) , lesion location (0.198) , Simon classification (0.683) , the mean blood loss ( P=0.999) , mean operation duration ( P=0.596) , extubation time ( P=0.755) , the average hospitalization time ( P=0.676) , postoperative complications and ( P=0.370) or the total cost ( P=0.486) . The difference of patients’ satisfaction at 6 month after operation had statistical significance ( P=0.012) . conclusion:Compared with traditional open surgery, mastoscopic surgery for gynecomastia is minimally invasive, beautiful and safe, which can be widely used in clinical practice.
5.Implementing a prevention and control of ametropia in accordance with Specification for Screening of Refractive Error in School-age Children and Adolescents

Ametropic visual impairment has become a public health problem in China and even in the world, especially the prevention and control of myopia development, and the Chinese government attaches great importances to the eye health of school-age children and adolescents.The establishment of visual health archives and the screening of refractive errors are important managements prevention and control.However, there has always been a lack of relevant domestic or international screening specifications, and which may result in screening implementation subject confusion, inconsistent screening indicators and boundary values, unscientific operation methods and feedback in the routine screening work in China.With the push and support of National Health Commission, a recommended national health standard Specification for Screening of Refractive Error in School-age Children and Adolescents (WS/T 663-2019) was officially issued on January 11, 2020 and will be implemented from June 1, 2020, which clarified the technical standardization for the specific requirement, methods, referral recommendation and management during screening of refractive error in school-age children and adolescents.Implementing an effective screening and prevention of ametropic visual impairment in school-age children and adolescents according to the standard is an important task and responsibility of medical and educational institutions.

TSCI have dyskinesia and sensory disturbance that can cause various life-threaten complications. The patients with traumatic spinal cord injury (TSCI), seriously affecting the quality of life of patients. Based on the epidemiology of TSCI and domestic and foreign literatures as well as expert investigations, this expert consensus reviews the definition, injury classification, rehabilitation assessment, rehabilitation strategies and rehabilitation measures of TSCI so as to provide early standardized rehabilitation treatment methods for TSCI.
7. CT and MR features of hepatic ischemia/necrosis after hepatosplenic surgery and upper gastrointestinal hemorrhage

Objective:
To investigate the CT and MR imaging features of hepatic ischemia/necrosis after hepatosplenic surgery and upper gastrointestinal hemorrhage.
Methods:
A total of 36 patients diagnosed with hepatic ischemia/necrosis by both medical imaging and clinical diagnosis shortly after hepatosplenic surgery and upper gastrointestinal hemorrhage were collected, including 9 patients with liver cancer resection, 5 patients with liver cancer ablation (microwave ablation/radiofrequency ablation, argon-helium knife, alcohol injection), 11 patients with spleen resection, and 11 patients with upper gastrointestinal bleeding. Conventional liver CT and / or MR plain and dynamic enhancement scan were performed to comprehensively analyze the morphology and density/signal performance of the lesions.
Results:
(1) Number of lesions: All cases had multiple lesions. (2) Distribution of lesions: scattered in the liver lobes, clustered or regional distribution, mainly in the periphery of the liver. (3) Size of lesions: the boundary of the nodular lesion was clear, and the single maximum diameter was 1.0-1.5 cm. It can be fused into a wedge-shaped patch or a segmental/sub-segmental large patch with a slight mass effect. (4) CT density or MR signal characteristics: CT plain scan showed slightly low density; MR plain scan showed slightly low signal on T1WI, high signal on T2WI, slightly high signal on DWI and no lipid/fat on dual phase imaging; 24 out of 36 cases (66.7%) showed no enhancement, while some lesions showed thin ring enhancement on the edge; emboli were found in the main and/or branches of portal vein (21/36 cases, 58.3%). (5) In the short-term review (minimum 5 days), the lesions became smaller or disappeared, and the local liver volume became smaller or the surface was depressed.
Conclusions
Hepatic ischemia/necrosis occurs after hepatosplenic surgery and upper gastrointestinal hemorrhage. The imaging manifestations are multiple nodular or flaky hypovascular foci, and the short-term review shows a markedly improvement. It needs to be differentiated from infection and metastasis of malignant tumors after operation.
8.The observation of hard and soft tissues around the small-diameter titanium-zirconium implants used in the posterior tooth area and clinical efficacy

Objective The small-diameter titanium-zirconium implants were used to repair the missing posterior teeth.The status of soft and hard tissue around the implants and the possible mechanical complications were observed and the clinical efficacy was evaluated.Methods 20 patients (24 implants) who were treated with small-diameter SLActive titanium-zirconium soft-tissue level implants (diameter 3.3 mm) in the posterior teeth area were selected as observation groups.25 patients (30 implants) were treated with standard-diameter SLA titanium soft-tissue level implants (4.1 mm) in the posterior teeth region were selected as controls.We measured and documented the implant stability quotient (ISQ);6 months after implant treatment,we recorded implant marginal bone loss (MBL),modified plaque index (mPLI),modified sulcus bleeding index (mSBI) and probing depth (PD) and documented the possible mechanical complications.Results The ISQ value in the observation group maintained an upward trend within 12 weeks after the operation;the ISQ value in the control group was low at 4 weeks and then increased steadily.There was no significant difference in MBL,mPLI,mSBI and PD between the two groups after six months of ending treatment.No mechanical complications occurred in either group.Conclusions Small-diameter titaniumzirconium implants repairing the missing posterior teeth have a good soft and hard organizational status and short-term clinical efficacy.
9.Clinical effect of surgery combined with adjuvant therapy and single surgery for resectable pancreatic cancer: a Meta analysis

Objective To systematically evaluate the clinical effect of surgery combined with adjuvant therapy (postoperative chemoradiotherapy or chemotherapy) and single surgery for resectable pancreatic cancers.Methods Literatures were researched using PubMed,Embase,Science Citation Index Expanded,Cochrane Central Register of Controlled Trials and China Biology Medicine disc with the key words including "pancreatic cancer,adjuvant therapy,chemoradiotherapy,radiochemotherapy,chemotherapy,radiotherapy,胰腺癌,辅助治疗,化学治疗and放射治疗”from the time of database building to October 2016.Two reviewers independently screened literatures,extracted data and assessed the risk of bias.All the patients undergoing surgery combined with adjuvant chemoradiotherapy,surgery combined with adjuvant chemotherapy and single surgery were respectively allocated into the first treatment group,second treatment group and control group.The inverse variance was used for mergering hazard ratio (HR) and related statistic data.HR and 95% confidence interval (CI) were used for assessing the overall survival time and disease-free survival time.The median survival time and 1-,2-,5-year survival rates were evaluated by the relative risk (RR) and 95%CI.The heterogeneity of the studies was analyzed using the I2 test.Results Eleven randomized controlled trials (RCTs) of 9 literatures were retrieved,and the total sample size was 1 482 patients,including 238 patients in the first treatment group,545 in the second treatment group and 699 in the control group.Results of Meta analysis:① overall survival time:there was no significant difference in overall survival time between the first treatment group and control group (HR =0.87,95% CI:0.56-1.17,P> 0.05).There was a significant difference in overall survival time between the second treatment group and control group (HR =0.68,95 % CI:0.55-0.80,P< 0.05).② Disease-free survival time:there was no significant difference in disease-free survival time between the first treatment group and control group (HR=0.78,95%CI:0.53-1.03,P>0.05).There was a significant difference in disease-free survival time between the second treatment group and control group (HR=0.56,95%CI:0.45-0.67,P<0.05).③ Median survival time:there were significant differences in median survival time between the first treatment group and control group (RR=1.82,95%CI:1.35-2.45,P<0.05) between the second group and control group (RR=1.32,95%CI:1.07-1.62,P<0.05).④ One-,2-,5-year survival rates:there was no significant difference in 1-,2-,5-year survival rates between the first treatment group and control group (RR=1.24,2.47,1.15,95% CI:0.72-2.12,0.82-7.41,0.71-1.84,P>0.05).One-year survival rate in the second treatment group was compared with that in the control group,with no significant difference (RR=1.15,95%CI:0.99-1.34,P>0.05).There were significant differences in 2-and 5-year survival rates between the second treatment group and control group (RR=1.24,1.73,95%CI:1.01-1.50,1.32-2.27,P<0.05).Conclusions Compared with single surgery,surgery combined with postoperative chemoradiotherapy cannot significantly improve the overall survival time and disease-free survival time of patients.However,surgery combined with adjuvant chemotherapy can prolong the overall survival time and disease-free survival time of patients.
10.Correlation between postmortem intervals and the changes of 18s rRNA degradation in liver after death under the ;condition of different temperature(

Objective To investigate the relationship between Ct value of mice liver and postmortem interval (PMI) under various ambient temperatures. Methods mice were stored at 10℃, 15℃, 20℃, 25℃ and 30℃ after execution, and total RNA was extracted from mice liver every 6 hours (PMI 6h to 72h). The levels of 18s rRNA were examined using real-time PCR. The results were expressed by cycle threshold (Ct) value to explore relationship between PMI and Ct value, and the interpolation functions were established to estimate PMI. Results In each group, Ct value increased with PMI increased. Surface equation was obtained after interpolation analysis on temperature range 10℃~30℃. The three-variable quintic surface equation was f(x, y)=-426.9+30.82x+44.48y-1.297x2-1.837xy-1.388y2+0.034 38x3+0.038 17x2y+0.038 67xy2+0.028 77y3-0.000 612 9x4-3.897e-7x3y-0.001 223x2y2+0.000 256 6xy3-0.000 537 4y4+3.606e-6x5-2.846e-6x4y+1.009e-5x3y2-3.439e-6x2y3-2.556e-7xy4+2.664e-6y5(r2=0.999 4). Conclusion The rule of Ct value changes at ambient temperature complied with three-variable quintic surface equation distribution. Measurement of interpolation function may be used for PMI estimation at ambient temperature.
