1.Monitoring results of mosquito-ovitraps placed in different orientations in multi-storey residential areas

ObjectiveTo find out whether there is any difference in the monitoring results of mosq-ovitraps placed in different orientations in multi-storey residential areas, so as to provide a scientific basis for routine and emergency monitoring of Aedes albopictus with mosq-ovitraps in residential areas. MethodsFrom July 6th to October 26th 2023, one mosquito ovitrap was set up in each of the 4 orientations of east, south, west and north around the buildings in a multi-storey residential area in Jinhui Town, Fengxian District, Shanghai. Data was collected and recorded 72 hours after placement. The chi-square test was used to compare the mosquito ovitrap indices (MOIs) of two independent samples, and the Kruskal⁃Wallis H test was used to compare the MOIs of multiple independent samples. ResultsAfter 16 weeks of surveillance, 997 mosquito ovitraps were recovered, of which 211 were positive, with the mosquito ovitrap index (MOI) of 21.16% and the Aedes albopictus density index of 1.03 mosquitoes·ovitrap-1. The MOIs were higher in September (24.22%) and October (23.96%), and the MOIs in the west, south and north within the two months were all above 20.00%. From July to October, the MOIs in the east, west, south and north were 20.70%, 22.20%, 25.50% and 16.20%, respectively, and the difference in MOIs among the 4 orientations was not statistically significant (χ2=6.647, P=0.084). Stratified analysis by month showed that in August, the south side of the multi-storey residential areas had the highest MOI (31.30%), the north side had the lowest MOI (1.30%), and there was a statistically significant difference in MOI in the east, west, south and north (χ2=25.986, P<0.001). In October, the MOI in the west was the highest (33.30%) and the MOI in the east was the lowest (6.30%), the difference in MOIs of the 4 orientations was statistically significant (χ2=12.007, P=0.007). The MOIs in the south side of the building in the outskirts of the residential area from the 1st week in July to the 4th week in October was lower (19.20%) than that in the south side of the inner building (31.70%), and the difference in MOI was statistically significant (χ2=5.118, P=0.024). ConclusionThe study of MOI in different orientations in a multi-storey residential area is a preliminary exploration based on field work, and the results show that there is a difference in MOIs in different orientations during the peak breeding period of mosquitoes. Further indicators such as temperature, humidity and wind speed in different orientations can be collected to explore the influencing factors of MOIs.
2.Clinical efficacy of single branch stent-graft treatment for retrograde type A intramural hematoma: A retrospective cohort study

Objective To explore the efficacy of using a single branch stent-graft to treat primary intramural hematoma located at the distal arch or descending aorta in Stanford A type aortic intramural hematoma. Methods From July 2020 to November 2022, 10 patients with primary intramural hematoma of Stanford A type aortic intramural hematoma were treated with endovascular repair using a single branch stent-graft in the Department of Cardiovascular Surgery at The University of Hong Kong-Shenzhen Hospital. There were 9 males and 1 female, aged from 32 to 66 years, with a mean age of (47.0±10.4) years. All patients had intramural hematoma involving the ascending aorta and aortic arch, diagnosed as type A intramural hematoma, with the tear located in the descending aorta. Among them, 6 patients were complicated by ulceration of the descending aorta with intramural hematoma, and 4 patients had changes of the descending aortic dissection. All patients underwent endovascular stent repair, with 8 patients undergoing emergency surgery (≤14 days) and 2 patients undergoing subacute surgery (15 days to 3 months). Results There were no neurological complications, paraplegia, stent fracture or displacement, or limb or visceral ischemia during the perioperative period in all patients. One patient had continuous chest pain after surgery, and the stent had a new tear at the proximal end, requiring ascending aorta and partial arch replacement. As of the latest follow-up, all patients had obvious absorption or complete absorption of the intramural hematoma in the ascending aorta and aortic arch compared with before the operation. Conclusion Single branch stent-graft treatment of retrograde ascending aortic intramural hematoma is safe and effective, with good short-term results.
3.Knowledge, attitude, practice and influencing factors on influenza and influenza vaccine among pregnant women in Suzhou

Objective:To understand the current knowledge, attitude, and practice (KAP) on influenza and influenza vaccine among pregnant women in Suzhou and to analyze its influencing factors to provide technical support data for public health strategies for promoting influenza vaccination in pregnant women.Methods:A questionnaire was designed, and a stratified sampling method was used to conduct a face-to-face survey among pregnant women at different stages of pregnancy who received antenatal examinations at different levels of medical institutions in Suzhou, Jiangsu Province, in 2023. KAP status and influencing factors were analyzed by χ2 test and logistic regression analysis. Results:A total of 2 195 valid questionnaires were collected in this survey. The M( Q1, Q3) of knowledge about influenza and influenza vaccine among pregnant women in Suzhou was 7.60 (5.23, 9.80) points, and the score range was 0.20-14.71 points, the passing rate was 34.12%, the awareness rate of influenza vaccine was 57.45%, and the vaccination rate of influenza vaccine was 1.91% within one year before the survey. The willingness to receive influenza vaccine during pregnancy was only 3.57%. Multivariate analysis of influenza and influenza vaccine-related knowledge scores showed that the passing rate was positively correlated with education level and gestational age. In contrast, family income was negatively correlated with living in rural areas, working as migrant workers, and having no medical insurance. Multivariate analysis of vaccination intention showed that decreased effectiveness of influenza vaccine and increased adverse reactions decreased vaccination intention during pregnancy. Conclusions:The pregnant women in Suzhou pay more attention to influenza, and vaccination rates and intentions are generally low. Pregnant women with early and second trimester of pregnancy, low education, living in rural areas, working as migrant workers, and not purchasing medical insurance are the key groups to popularize the knowledge about influenza and influenza vaccine.
4.A retrospective study on the evolution of TCM syndrome and TCM syndrome elements in the course of disease in 1,049 patients with psoriasis vulgaris

Objective The study aimed to elucidate the evolution of the syndromes in Traditional Chinese Medicine(TCM)and TCM syndrome elements in different chronic stages of psoriasis vulgaris.Methods A database was constructed using electronic medical records collected from July 2019 to March 2024 from 1,049 patients with psoriasis vulgaris.The study used Sankey diagrams and network association graphs to analyze the evolution of TCM syndromes and their elements in patients at the different stages:initial diagnosis,progressive stage(Week 2-3),progressive stage(Week 4-5),skin lesion improvement stage(Week 6-7),and remission stage.The syndrome elements network was constructed using community detection algorithms,and the association rules between local skin lesion syndrome differentiation and overall syndrome differentiation were displayed using heatmaps.Results(ⅰ)Initial diagnosis.In the syndrome differentiation of local skin lesions,blood heat syndrome was the most common(79.79%);among the disease location of TCM syndrome elements(called"disease location"),liver was the most prevalent(35.62%);and among the pathological factors of TCM syndrome elements(called"pathological factors"),fire(heat)was the most common(75.48%).(ⅱ)Active stage(Week 2-3).In the syndrome differentiation of local skin lesions,blood heat syndrome remained the most prevalent(73.13%);among the disease location,liver was still the most prevalent(31.71%);and among the pathological factors,fire(heat)continued to be the most common(82.11%),while dampness(22.26%)and qi stagnation(8.39%)began to increase.(ⅲ)Active stage(Week 4-5).The syndrome differentiation of local skin lesions was dominated by blood heat syndrome(45.91%)and blood dryness syndrome(37.19%);among disease location,the interior was the most prevalent(15.25%);and among the pathological factors,fire(heat)remained the most common(50.66%),with an increase in yin deficiency(34.26%).(ⅳ)Skin lesion improvement stage(Week 6-7).In the syndrome differentiation of local skin lesions,both blood dryness syndrome(49.44%)and blood stasis syndrome(33.33%)increased;among the disease location,meridians increased most significantly and became the most prevalent(13.44%);and among the pathological factors,blood stasis increased most significantly and became the most prevalent(28.20%).(ⅴ)Remission stage.In the syndrome differentiation of local skin lesions,blood stasis syndrome became the primary(55.69%),while the percentage of blood dryness syndrome decreased(21.16%);meridians(25.71%)and blood stasis(62.34%)remained the most predominant syndrome elements related to disease location or pathological factors.Conclusion The overall pattern of TCM syndromes in psoriasis vulgaris evolved from excess to deficiency.From the initial diagnosis to the active phase(Week 2-3),heat syndrome dominated;during the active phase(Week 4-5),heat syndrome coexisted with damp syndrome or yin deficiency syndrome;changes in the syndrome element network were the most significant during the lesion improvement phase,with blood stasis gradually increasing and peaking during the remission phase.Blood stasis,dampness,and qi stagnation were pervasive throughout psoriasis vulgaris;qi stagnation and blood stasis may be the main elements causing further deterioration and prolonged course of the disease during the active phase in patients.
5.Analysis of complete genome sequence characteristics of coxsackievirus A2, A4 and A5 in Suzhou city

Objective:To understand the genomic characteristics and evolutionary trends of three coxsackievirus (CV) subtypes, including CV-A2, CV-A4 and CV-A5, in Suzhou city.Methods:Totally 248 samples were collected during routine monitoring for hand, foot, and mouth disease in Suzhou city from January to June in 2022. The specimens were detected for CV genotypes using Real-time fluorescence-reverse transcription polymerase chain reaction (Real-time RT-PCR). The CV positive samples were sequenced using the Miseq high-throughput sequencing platform. After obtaining the whole genome sequences of CV strains, homology analysis was performed. An evolutionary tree was constructed using molecular evolutionary genetics analysis (MEGA) X. Sequence recombination regions were analyzed using SimPlot and BootScan.Results:During routine monitoring for hand, foot and mouth disease in Suzhou city from January to June in 2022, one strain each of CV-A2, CV-A4, and CV-A5 viruses were identified, and their complete genomes were sequenced. CV-A2 and CV-A4 were clustered with the current prevalent strains in China, and no new amino acid site mutations were found in the whole genome of CV-A2; CV-A4 had several new amino acid site mutations in the non-structural region compared with the prototype strain; CV-A5 belonged to the C1 subgenotype 3 lineage, and the genome of CV-A5 had several new amino acid mutation sites in the non-structural protein region and underwent recombination.Conclusions:The CV-A2 and CV-A4 strains are stable epidemic strains, the CV-A5 strain is recombinant strain in Suzhou city.
6.Effect of Sangqiao Qingfei Formula combined with western medicine conventional therapy combined with mechanical ventilation on pulmonary function and serum PCT in patients with AECOPD respiratory failure

Objective:To explore the effects of Sangqiao Qingfei Prescription combined with Western medicine conventional therapy with mechanical ventilation on lung function and airway inflammation in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) respiratory failure; To evaluate clinical efficacy.Methods:A randomized controlled trial study was conducted. Totally 90 AECOPD patients with respiratory failure in our hospital from January 2020 to December 2022 were selected as the observation subjects. They were divided into two groups using a random number table method, with 45 cases in each group. The control group received mechanical ventilation treatment, while the observation group received Sangqiao Qingfei Prescription on the basis of the control group. Both groups were treated for 2 weeks. TCM syndrome scoring was performed before and after treatment, and the time of successful withdrawal from the machine was recorded; a blood gas analyzer was used to detect PaO 2, PaCO 2, blood oxygen saturation (SaO 2) and pH values; the plateau pressure (Pplat), peak airway pressure (Ppeak), airway resistance (Raw) and dynamic lung compliance (Cdyn) were recorded during the ventilator; a pulmonary function meter was used to measure respiratory rate (RR), maximum expiratory flow (PEF), FVC, FEV1, and the percentage of FEV1 to the estimated value (FEV1% estimated value); serum CRP, TNF-α, and Procalcitonin (PCT) were detected using ELISA method. Clinical efficacy was evaluated. Results:During the treatment period, there were no cases of detachment in both groups. The mechanical ventilation time in the observation group was (7.16 ± 0.69) d, while in the control group it was (9.88 ± 1.04) d, with statistical significance ( t=14.62, P<0.001); after treatment, the main symptom, secondary symptom scores, and total scores of the observation group were lower than those in the control group ( t values of 13.43, 18.53, 31.21, P<0.001); the PaO 2 [(79.16 ± 7.42) mmHg vs. (67.49 ± 6.88) mmHg, t=8.24], SaO 2 [(95.15 ± 9.93)% vs. (84.59 ± 9.48)%, t=5.16], and pH value (7.35 ± 0.23 vs. 7.26 ± 0.16, t=2.16) in the observation group were higher than those in the control group ( P<0.01 or P<0.05), while PaCO 2 [(49.89 ± 3.65) mmHg vs. (62.39 ± 4.27) mmHg, t=14.93] was lower than that of the control group ( P<0.01); after treatment in the observation group, Pplat [(15.31 ± 2.51) cmH 2O vs. (17.53 ± 2.02) cmH 2O, t=4.62], Ppeak [(22.43 ± 3.16) cmH 2O vs. (25.78 ± 3.17) cmH 2O, t=5.02], Raw [(18.96 ± 3.86) cmH 2O/(S?L) vs. (24.29 ± 4.29) cmH 2O/(S?L), t=6.20] were lower than those in the control group ( P<0.01), Cdyn [(34.53 ± 3.35) cmH 2O/(S?L) vs. (30.27 ± 3.87) cmH 2O/(S?L), t=5.58] was higher than the control group ( P<0.01); the RR [(19.25 ± 2.43) times/min vs. (23.49 ± 3.07) times/min, t=7.26] in the observation group was lower than that of the control group ( P<0.01), PEF [(4.99 ± 0.40) L/s vs. (4.03 ± 0.34) L/s, t=12.27], FVC [(3.04 ± 0.20) L vs. (2.14 ± 0.22) L, t=20.31], FEV1 [(2.83 ± 0.20) L vs. (2.16 ± 0.13) L, t=18.84], FEV1% estimated value [(42.23 ± 4.66)% vs. (36.43 ± 5.09)%, t=5.64] were higher than those in the control group ( P<0.01); serum CRP, IL-6, TNF-α and PCT in the observation group were lower than those in the control group ( t values were 18.13, 13.36, 15.97, 30.67, P<0.01). The total effective rate of the observation group was 93.33% (42/45), while that of the control group was 77.78% (35/45), with statistical significance ( χ2=4.41, P=0.036). Conclusion:The combination of Sangqiao Qingfei Prescription and conventional Western medicine treatment with mechanical ventilation can effectively improve lung ventilation function, reduce inflammatory cytokine levels, alleviate inflammatory reactions, and improve clinical efficacy in AECOPD patients with respiratory failure.
7.Summary of the best evidence for cosmetic beauty in breast cancer patients with self-image disorder

Objective:To retrieve, evaluate and integrate evidence related to cosmetic beauty in breast cancer patients with self-image disorder, so as to provide a basis for clinical practice.Methods:According to the evidence-based resource "6S" model, the guideline websites and Chinese and English databases were systematically searched from top to bottom for clinical decisions, guidelines, systematic evaluation, expert consensus and original researches on cosmetic beauty in breast cancer patients with self-image disorder. The search period was from establishment of the databases to November 2023. The included literature was evaluated by two researchers and evidence was extracted from the literature that met the quality criteria.Results:A total of nine articles were included, including one guideline, two systematic reviews, three expert consensus and three original studies. A total of 24 best pieces of evidence were formed from eight aspects, including team building and educational guidance, body image assessment and referral, basic skin care and precautions, daily makeup, hair loss care, clothing matching, prosthetic breast selection and health education.Conclusions:The best evidence summarized in this study is comprehensive and can be used by clinical medical staff in practice, but the evidence should be personalized according to patients' wishes and clinical practical problems to improve the self-image of breast cancer patients.
8.Research Developments of Classic Famous Prescription:Wendan Decoction and its Prediction Analysis of Q-marker

The Wendan Decoction,a classic prescription first recorded in Yao Shengyuan's"Ji Yan Fang"during the Southern and Northern Dynasties,has been a mainstay in Traditional Chinese Medicine(TCM)clinical applications.Recently included in the first batch of the"Catalogue of Ancient Classical Famous Prescriptions"and highlighted in the"Key Information List of Ancient Classical Famous Prescriptions(25 Prescriptions)",this paper summarizes the research progress on Wendan Decoction.The decoction is recognized for its lipid-lowering,blood pressure-reducing,anti-inflammatory,and anti-schizophrenic properties,and is extensively used in modern clinical practice,especially for neurological,cardiovascular,digestive,and endocrine system disorders.In TCM,Wendan Decoction is often tailored and applied in cases diagnosed with phlegm and stasis.Referencing the"Q-marker"quality standard proposed by Academician Liu Changxiao in 2016,this study predicts the quality markers of Wendan Decoction based on the"five principles",identifying β-sitosterol,succinic acid,quercetin,tricin,luteolin,naringin,nobiletin,hesperidin,naringenin,6-gingerol,glycyrrhizin,and glycyrrhetinic acid as key constituents.These findings are intended to facilitate the refinement of Wendan Decoction's prescription and enhance its clinical effectiveness.
9.Distribution and health risk of residual trihalomethane in regional drinking water

ObjectiveTo explore the variation rules and health risks of trihalomethane in regional drinking water, and to provide evidence for the innovative water processing technology and the optimization of drinking water quality. MethodsBased on regional drinking water sanitation monitoring, non-parametric rank sum test was used to analyze the effects of residual trihalomethane production in different periods and with disinfection methods. The United States environmental protection agency (USEPA) classic "four-step" health risk assessment model was used to evaluate the carcinogenic risk and non-carcinogenic risk of trihalomethane through drinking water exposure. ResultsThe yield of trichloromethane in wet season was 6.3 μg·L-1, which was higher than that in dry season. Compared with chlorination pretreatment, ozone pretreatment reduced the content of bromomethane dichloromethane. Compared to liquid chlorine disinfection, sodium hypochlorite treatment incresed the levels of trichloromethane and bromomethane chloride. Although the total carcinogenic and non-carcinogenic risks of trihalomethane in drinking water in the region were at safe levels, they were above the acceptable limits occasionally. The highest carcinogenic risk of trihalomethane were dichlorobromomethane and chlorodibromomethane,and the highest non-carcinogenic risk was trichloromethane. The health risk of children was 1.2 times higher than that for adults. ConclusionThe production of residual trihalomethane in drinking water in this area is relatively low, which is less harmful to the health of adults and children. Monitoring, including other disinfection byproducts, should continue and appropriate disinfection techniques for drinking water should be explored.
10.Application of radial artery in total arterial coronary revascularization in elderly patients.

OBJECTIVE:
To summarize the application experience and clinical effect of radial artery in total arterial coronary revascularization (TAR) in elderly patients.
METHODS:
Retrospectively analyzed the clinical data of patients who underwent TAR at the University of Hong Kong Shenzhen Hospital from July 1, 2020 to May 30, 2022. Patients were divided into ≥ 65-year-old group and < 65-year-old group according to age. The radial artery blood flow, diameter, intimal integrity and Allen test were evaluated by ultrasound before operation. The distal ends of radial artery were collected for pathological examination during operation. Coronary artery CT angiography (CTA) was examined postoperatively and follow up. The safety and reliability of ultrasonic assessment of radial artery and application of radial artery in elderly patients with TAR were summarized and analyzed.
RESULTS:
A total of 101 patients received TAR, including 35 cases aged ≥ 65 years old, 66 cases aged < 65 years old; 78 cases used bilateral radial arteries, and 23 cases used unilateral radial arteries. 4 cases of bilateral internal mammary arteries. All the proximal ends of the radial artery were anastomosed to the proximal end of the ascending aorta, 34 cases were performed of "Y" grafts, and 4 cases were sequential anastomoses. There was no in-hospital death and perioperative cardiovascular events. Perioperative cerebral infarction occurred in 3 patients. 1 patients was reoperated for bleeding. Intra-aortic balloon pump (IABP) assistance was used in 21 patients. Poor wound healing occurred in 2 cases and healed well after debridement. Follow-up of 2 to 20 months after discharge showed no internal mammary artery occlusion and 4 radial artery occlusions; no major adverse cardiovascular and cerebrovascular event (MACCE) occurred, and the survival rate was 100%. There was no significant difference in the above perioperative complications and follow-up endpoints between the two age groups.
CONCLUSIONS
By adjusting the order of bypass anastomosis and optimizing the preoperative evaluation method, radial artery combined with internal mammary artery can obtain better outcome early in TAR, and can be safely and reliably applied to elderly patients.
