1.Changes in the Non-targeted Metabolomic Profile of Three-year-old Toddlers with Elevated Exposure to Polycyclic Aromatic Hydrocarbons

Objective To investigate changes in the urinary metabolite profiles of children exposed to polycyclic aromatic hydrocarbons(PAHs)during critical brain development and explore their potential link with the intestinal microbiota. Methods Liquid chromatography-tandem mass spectrometry was used to determine ten hydroxyl metabolites of PAHs(OH-PAHs)in 36-month-old children.Subsequently,37 children were categorized into low-and high-exposure groups based on the sum of the ten OH-PAHs.Ultra-high-performance liquid chromatography coupled with quadrupole time-of-flight mass spectrometry was used to identify non-targeted metabolites in the urine samples.Furthermore,fecal flora abundance was assessed by 16S rRNA gene sequencing using Illumina MiSeq. Results The concentrations of 21 metabolites were significantly higher in the high exposure group than in the low exposure group(variable importance for projection>1,P<0.05).Most of these metabolites were positively correlated with the hydroxyl metabolites of naphthalene,fluorine,and phenanthrene(r=0.336-0.531).The identified differential metabolites primarily belonged to pathways associated with inflammation or proinflammatory states,including amino acid,lipid,and nucleotide metabolism.Additionally,these distinct metabolites were significantly associated with specific intestinal flora abundances(r=0.34-0.55),which were mainly involved in neurodevelopment. Conclusion Higher PAH exposure in young children affected metabolic homeostasis,particularly that of certain gut microbiota-derived metabolites.Further investigation is needed to explore the potential influence of PAHs on the gut microbiota and their possible association with neurodevelopmental outcomes.
2.Clinical trial of sindillimab combined with TN chemotherapy regimen in patients with advanced non-small cell lung cancer

Objective To analyze the efficacy of sintilizumab combined with paclitaxel(albumin-bound)and nedaplatin(TN chemotherapy regimen)in patients with advanced non-small cell lung cancer(NSCLC)and its effects on serum tumor markers and immune cell levels.Methods The patients were divided into control group and treatment group according to random number table method.In the control group,80 mg·m-2 nedaplatin and 260 mg·m-2 paclitaxel(albumin-bound type)were injected intravenally on the 1st day for 21 days.The etreatment group was given sindilizumab 200 mg intravenously on the basis of the control group,21 days a cycle for 4 cycles.The anti-tumor efficacy,tumor markers,lung function,immune cytokines and adverse drug reactions of the two groups were compared after 4 cycles of treatment,and the 1-year survival of the two groups was statistically analyzed.Results A total of 7 cases fell off during treatment.Finally,98 patients in the treatment group and 96 patients in the control group were included in the analysis.After treatment,the objective response rate(ORR)of treatment group and control group were 43.88%(43 cases/98 cases)and 29.17%(28 cases/96 cases);the disease control rate(DCR)were 77.55%(76 cases/98 cases)and 60.42%(58 cases/96 cases),the difference was statistically significant(all P<0.05).After treatment,carbohydrate antigen 125(CA125)in treatment group and control group were(39.03±5.97)and(42.15±6.35)U·mL-1;squamous cell carcinoma antigen(SCCA)were(4.58±0.63)and(5.29±0.84)ng·mL-1;forced vital capacity(FVC)were(2.96±0.52)and(2.71±0.49)L;forced expiratory volume in the first second(FEV1)/FVC were(68.47±11.39)%and(64.92±10.43)%;Th1/Th2 were 5.01±0.63 and 5.36±0.74;Th17/Treg were 1.04±0.15 and 1.20±0.19;CD4+/CD8+were 1.36±0.19 and 1.23±0.17,respectively.The differences were statistically significant(all P<0.05).The total incidence of adverse drug reactions in treatment group and control group were 25.51%(25 cases/98 cases)and 22.92%(22 cases/96 cases),respectively,the difference was not statistically significant(P>0.05).There was no significant difference in 1-year overall survival curves between the two groups(P>0.05).Conclusion Sindilizumab combined with TN chemotherapy regimen in the treatment of advanced NSCLC can enhance the anti-tumor efficacy,reduce the level of tumor markers,improve lung function and immune function,and has good safety.
3.Relationship Between Mindfulness and Affiliate Stigma in Parents of Children with Autism Spectrum Disorder in China: The Mediating Role of Coping Styles

Purpose:
This study aims to investigate the current status of affiliate stigma among parents of autistic children, analyze the influencing factors, explore the relationship among mindfulness, coping styles, and affiliate stigma, and verify the mediating role of coping styles between mindfulness and affiliate stigma in parents of children with autism in China.MethodBetween February and April 2023, the Child Development Behaviour Centre of a public hospital in China recruited 345 parents of children with autism. These parents completed the general information questionnaire, the Mindful Attention Awareness Scale, the Affiliate Stigma Scale, and the Simple Coping Style Questionnaire. We then adapted the Hayes Process Macro and Bootstrap methods to examine the mediating effects of coping styles between mindfulness and affiliate stigma.
Results:
(1) The total affiliate stigma score of parents of children with autism was 48.53 (standard deviation:: 10.74). Parents' age, monthly family income, duration of care, mindfulness, and coping styles were the influencing factors of parental affiliate stigma. (2) Mindfulness was positively correlated with positive coping style (r = 0.33, p < .01) and negatively correlated with negative coping style, affiliate stigma (r = −0.38, −0.39, p < .01), whereas affiliate stigma was negatively correlated with positive coping style (r = −0.34, p < .01) and positively correlated with negative coping style (r = 0.41, p < .01). (3) Positive coping style and negative coping style play a parallel mediating role between mindfulness and affiliate stigma of parents of autistic children.
Conclusions
Parents of children with autism experience significant levels of affiliate stigma. Mindfulness has a direct impact on associated stigma in parents of children with autism and also indirectly predicts associated stigma through the intermediary influence of positive and negative coping styles. Healthcare professionals could perform mindfulness interventions from an optimistic psychology viewpoint to boost parents' mindfulness and coping abilities, thereby accomplishing the objective of mitigating affiliate stigma.

Objective: To analyze and compare therapy responses, outcomes, and incidence of severe hematologic adverse events of flumatinib and imatinib in patients newly diagnosed with chronic phase chronic myeloid leukemia (CML) . Methods: Data of patients with chronic phase CML diagnosed between January 2006 and November 2022 from 76 centers, aged ≥18 years, and received initial flumatinib or imatinib therapy within 6 months after diagnosis in China were retrospectively interrogated. Propensity score matching (PSM) analysis was performed to reduce the bias of the initial TKI selection, and the therapy responses and outcomes of patients receiving initial flumatinib or imatinib therapy were compared. Results: A total of 4 833 adult patients with CML receiving initial imatinib (n=4 380) or flumatinib (n=453) therapy were included in the study. In the imatinib cohort, the median follow-up time was 54 [interquartile range (IQR), 31-85] months, and the 7-year cumulative incidences of CCyR, MMR, MR(4), and MR(4.5) were 95.2%, 88.4%, 78.3%, and 63.0%, respectively. The 7-year FFS, PFS, and OS rates were 71.8%, 93.0%, and 96.9%, respectively. With the median follow-up of 18 (IQR, 13-25) months in the flumatinib cohort, the 2-year cumulative incidences of CCyR, MMR, MR(4), and MR(4.5) were 95.4%, 86.5%, 58.4%, and 46.6%, respectively. The 2-year FFS, PFS, and OS rates were 80.1%, 95.0%, and 99.5%, respectively. The PSM analysis indicated that patients receiving initial flumatinib therapy had significantly higher cumulative incidences of CCyR, MMR, MR(4), and MR(4.5) and higher probabilities of FFS than those receiving the initial imatinib therapy (all P<0.001), whereas the PFS (P=0.230) and OS (P=0.268) were comparable between the two cohorts. The incidence of severe hematologic adverse events (grade≥Ⅲ) was comparable in the two cohorts. Conclusion: Patients receiving initial flumatinib therapy had higher cumulative incidences of therapy responses and higher probability of FFS than those receiving initial imatinib therapy, whereas the incidence of severe hematologic adverse events was comparable between the two cohorts.

6.Risk analysis and suggestions of 570 drug wholesale enterprises in Hunan province

OBJECTIVE To provi de reference for standardizing the distribution behavior of drug wholesale enterprises and improving the supervision level of drug supervision departments. METHODS The defective items found in the on-site inspection of 570 drug wholesale enterprises in Hunan province in 2020 were summarized ,the main risks were analyzed to put forward countermeasures and suggestions. RESULTS & CONCLUSIONS The provincial drug regulatory departments conducted daily supervision and inspection among 192 drug wholesale enterprises ,of which 168 enterprises(87.50%)were ordered to make corrections;a total of 1 804 defective items were found ,involving 11 serious defects ,806 major defects and 987 general defects. The provincial drug regulatory departments conducted special inspections among 20 wholesale enterprises of narcotic drugs and psychotropic drugs ,of which 18 enterprises(90.00%)were ordered to make corrections ;a total of 48 defective items were found , involving 33 major defects and 15 general defects ,and no serious defective items were found. The drug regulatory departments of all cities (autonomous prefectures )inspected 358 drug wholesale enterprises ,of which 290 enterprises(81.00%)were ordered to make corrections ;a total of 1 499 defective items were found ,including 665 main defects and 834 general defects ,and no serious defective items were found. According to the statistics of occurrence frequency ,defective items of drug wholesale enterprises in Hunan province were mainly concentrated in storage and maintenance (687 items,20.50%),facilities and equipments (608 items, 18.14%),personnel and training (579 items,17.28%),receipt and acceptance (272 items,8.12%)and quality management system documents (260 items,7.76%),which together accounted for 71.80% of the total defects. The main risks of drug wholesale enterprises included weak enterprise risk awareness ,high frequency of quality risks in drug marketing ,imperfect enterprise quality management system documents ,and fraud in enterprise business behavior. It is recommended that drug wholesale enterprises should establish quality risk management institutions ,evaluation standards and management systems ,improve the operation quality management system ,strengthen the management of drug storage ,standardize the operation and management of enterprises,strengthen personnel training and standardize procurement behavior ,so as to j ointly maintain the safety and efficacy of drugs and controllable quality.

OBJECTIVES:
To explore the optimal maintenance dose of caffeine citrate for preterm infants requiring assisted ventilation and caffeine citrate treatment.
METHODS:
A retrospective analysis was performed on the medical data of 566 preterm infants (gestational age ≤34 weeks) who were treated and required assisted ventilation and caffeine citrate treatment in the neonatal intensive care unit of 30 tertiary hospitals in Jiangsu Province of China between January 1 and December 31, 2019. The 405 preterm infants receiving high-dose (10 mg/kg per day) caffeine citrate after a loading dose of 20 mg/kg within 24 hours after birth were enrolled as the high-dose group. The 161 preterm infants receiving low-dose (5 mg/kg per day) caffeine citrate were enrolled as the low-dose group.
RESULTS:
Compared with the low-dose group, the high-dose group had significant reductions in the need for high-concentration oxygen during assisted ventilation (P=0.044), the duration of oxygen inhalation after weaning from noninvasive ventilation (P<0.01), total oxygen inhalation time during hospitalization (P<0.01), the proportion of preterm infants requiring noninvasive ventilation again (P<0.01), the rate of use of pulmonary surfactant and budesonide (P<0.05), and the incidence rates of apnea and bronchopulmonary dysplasia (P<0.01), but the high-dose group had a significantly increased incidence rate of feeding intolerance (P=0.032). There were no significant differences between the two groups in the body weight change, the incidence rates of retinopathy of prematurity, intraventricular hemorrhage or necrotizing enterocolitis, the mortality rate, and the duration of caffeine use (P>0.05).
CONCLUSIONS
This pilot multicenter study shows that the high maintenance dose (10 mg/kg per day) is generally beneficial to preterm infants in China and does not increase the incidence rate of common adverse reactions. For the risk of feeding intolerance, further research is needed to eliminate the interference of confounding factors as far as possible.

8.Relationship between mental health status and vision-related quality of life in the students with moderate and high myopia

Objective:
To understand the relationship between mental health status and vision-related quality of life( VRQL ) of students with moderate and high myopia, and to provide basis for the optimization of VRQL.
Methods:
Using convenient sampling method, the 8-18 years old students with moderate and high myopia were selected from Sichuan, Chongqing and Zhejiang. The mental health status and VRQL of students with moderate and high myopia were evaluated with Hospital Anxiety and Depression Scale and Quality of Life Scale for Ametropia. The multiple linear regression model was used to analyze the influencing factors for VRQL.
Results:
A total of 360 questionnaires were sent out and 354 were effectively recovered, with an effective rate of 98.33%. There were 116 ( 32.77% ) boys and 238 ( 67.23% ) girls; the median age was 16.65 years old. There were 211 ( 59.60% ) cases of moderate myopia and 143 (40.40%) cases of high myopia.There were 141 ( 39.83% ) found to be anxious and 176 ( 49.72% ) depressed. The median score of Quality of Life Scale for Ametropia was 64. The results of multiple linear regression analysis showed that sex ( β'= -0.179 ), diopter ( β'= 0.208 ), eyesight with glasses ( β'= -0.229 ) and anxiety ( β'= 0.439 ) were influencing factors for VRQL.
Conclusion
The mental health problems of the students with medium and high myopia are prominent; anxiety has a significant impact on the VRQL of the students.
9.The Diagnostic Value of Peripheral Blood Cell Parameters for Early Recognition of Myelodysplastic Syndrome.

OBJECTIVE:
To evaluate the diagnostic value of peripheral blood cell parameters for early recognition of myelodysplastic syndrome (MDS) patients.
METHODS:
The clinical and laboratory data of 86 patients with MDS and 72 patients with non-malignant clonal anemia treated in first diagnosed in the Second Hospital of Hebei Medical University from January 1, 2015 to December 31, 2017 was retrospectively analyzed. The peripheral blood cell parameters of the patients in two groups were analyzed, generated the receiver operator characteristic curve (ROC curve) from the statistically significant parameters, the binary logistic model was build to calculate and compare the area under the ROC curve (AUC) combined with multiple indicators and individual indicators, sensitivity, specificity, positive and negative likelihood ratio, and diagnostic accuracy, the diagnostic efficacy of the patients was analyzed.
RESULTS:
Compared with patients in the non-malignant clonal anemia group ,white blood cell count (WBC), neutrophil percentage (NE%), eosinophil percentage (E%), eosinophil absolute value (E#), platelet count (PLT), platelet specific volume (PCT%) in the MDS patients were significantly reduced; while percentage of lymphocytes (LY%), basophilic percentage (B%), and the width of platelet distribution (PDW) significantly increased. The several ROC curves with the above indicators were established, which showed that AUC
CONCLUSION
PDW, B% and LY% in peripheral blood cell parameters have certain diagnostic value for early recognition of MDS.

10.Frequency-dependent Alterations in Amplitude of Low-frequency Fluctuations in Resting-state Functional Magnetic Resonance Imaging of Post Stroke Aphasia

Objective:To explore the characteristics of local brain neural activity in post stroke aphasia (PSA) patients in different frequency bands and the relationship between Western Aphasia Battery (WAB) scores and specific frequency bands. Methods:From March, 2015 to May, 2018, 15 PSA patients, and 15 healthy adults as controls matched for age, gender and education were recruited. They were assessed with WAB and scaned with resting-state functional magnetic resonance imaging. The amplitude of low-frequency fluctuation (ALFF) and fractional amplitude of low-frequency fluctuation (fALFF) on the frequency bands of 0.01-0.08 Hz, 0.027-0.073 Hz, and 0.01-0.027 Hz were calculated. The ALFF and fALFF in different frequency bands were extracted and the correlation with the WAB scores in the patients were analyzed. Results:On 0.01-0.08 Hz, ALFF increased in the right precentral gyrus in the patients. On 0.027-0.073 Hz, ALFF increased values in the right precentral gyrus, and fALFF decreased in the right cerebellar Crus2 region; fALFF in the right cerebellar Crus2 region negatively correlated with the scores of information content (
