1.Results of Proficiency Tests for HBsAg and Anti-HCV as the Blood Donor Screening Testing (2012~2015).

BACKGROUND: Donor screening test is one of the most important processes for blood safety management. Korea Center for Disease Control and Prevention (KCDC) has been conducting an annual proficiency test program that includes the distribution of specially manufactured panels for hepatitis B surface antigen (HBsAg) and antibody to hepatitis C virus (anti-HCV) to blood centers. Here, KCDC reports the results of these proficiency tests for HBsAg and anti-HCV blood donor screening for all licensed blood centers in Korea between 2012 and 2015. METHODS: Panels for the proficiency tests were manufactured and distributed to blood centers by Chung-Ang University Hospital, which has been participating in the Korea Blood Safety Commission. Well-proven reactive sera and healthy donor's sera acquired from the Human Serum Bank in Chung-Ang University were used to make the panels. To identify the S/CO ratio of the panel, three medical institutes triple-checked the results of each panel. RESULTS: Most blood centers reported correct answers for the proficiency test with six panels. The average percentages (year) of correct answers were as follows: 98.7% (2012), 98.5% (2013), 99.1% (2014) and 99.6% (2015) for the HBsAg proficiency tests; and 97.7% (2012), 99.5% (2013), 99.1% (2014), and 99.8% (2015) for the anti-HCV proficiency tests. CONCLUSION: To improve the blood center's ability for donor screening tests, KCDC will continue the proficiency test program by managing specialized panels for HBsAg and Anti-HCV tests. Furthermore, we will investigate the level of satisfaction to improve the quality of the program.

The Ael subgroup expresses the least amount of A antigens and could only be detected by performing the adsorption-elution test. The frequency of the Ael subgroup is about 0.001% in Koreans, and the Ael02 allele, which originates from A102, is the most frequently identified allele in the Korean population. We report a Korean family with the Ael03 allele identified by molecular genetic analysis. To the best of our knowledge, this is the first such report in Korea to date.

3.Pelvic Ultrasonography Findings in Girls with Precocious Puberty.

PURPOSE: We analyzed pelvic ultrasonography (USG) findings in girls with central precocious puberty (CPP) and assessed the role of uterine and ovarian measurements in discriminating between CPP and other pubertal conditions. METHODS: Seventy-four girls (chronological age 7.8 +/- 0.5 years, bone age 9.9 +/- 0.8 years) with precocious pubertal signs were enrolled. Measurements of uterine and ovarian parameters by pelvic USG included antero-posterior diameters of the uterine fundus and cervix, diameter of each ovary, number of follicles, and maximal diameter of the largest follicle. The pelvic USG parameters were compared between girls with CPP (n = 49) and girls with atypical premature thelarche (PT) (n = 25). RESULTS: Antero-posterior diameter of uterine fundus (1.05 +/- 0.34 vs. 0.74 +/- 0.78 cm, P = 0.001), maximal ovarian diameter (2.13 +/- 0.48 vs. 1.84 +/- 0.74 cm, P = 0.048) and mean ovarian area (2.31 +/- 0.79 vs. 1.69 +/- 0.71 cm, P = 0.002) were significantly greater in girls with CPP than in girls with atypical PT. For the diagnosis of CPP, the sensitivity and specificity of A-P diameter of uterine fundus (> 0.9 cm) was 65.3% and 84.0%, the sensitivity and specificity of maximal ovarian diameter (> 2.0 cm) was 55.1% and 76%, and the sensitivity and specificity of mean ovarian area (> 2.0 cm2) was 62.9% and 80.0%. CONCLUSION: Girls with CPP had significantly higher dimensions of the uterus and ovary measurements compared to girls with atypical PT, but sensitivity and specificity were not high enough to differentiate CPP from atypical PT. Pelvic USG may help the diagnosis of CPP in girls.

The average sodium intake of Koreans was reported to be 5,279.9 mg/day, which is one of the highest intake levels worldwide. The average Koreans intake 19.6% of sodium from kimchi, showing kimchi as the main contributor of sodium in this country (Ministry of Health and Welfare, 2005). The sodium content of dishes that are frequently chosen by workers, and which were served by foodservice cafeterias were chemically analyzed. The average sodium content of one meal provided by 10 foodservice cafeterias was 2,777.7 mg. Twenty-one, one-dish-meals, frequently chosen by workers for a lunch menu, were collected at 4 different restaurants for each menu by one male, aged in the twenties and analyzed chemically also. Workers who eat lunch at a workplace cafeteria everyday could intake about 8 g of salt at a one-time meal and those who eat out for a one-dish-meal would intake 3-8 g of salt without counting sodium content from the side dishes. From these study results, one could estimate that over 10 g of salt could be possible for a single meal for workers who eat out everyday. A nationwide nutrition campaign and education for low salt diets for restaurant owners and foodservice providers should be seriously considered.

5.A Randomized, Prospective, Comparative, Multicenter Study of Rabeprazole and Ranitidine in the Treatment of Reflux Esophagitis.

BACKGROUND/AIMS: This study was done to evaluate the efficacy of rabeprazole (proton-pump-inhibitor) and ranitidine (H2-receptor antagonist) in the symptom relief and treatment of erosive esophagitis diagnosed by endoscopy. METHODS: A total of 110 patients with typical gastroesophageal reflux disease (GERD) symptoms were enrolled in this multicenter study. They were randomized into rabeprazole group (53 patients) and ranitidine group (57 patients) respectively. The patients in rabeprazole group were given 10 mg of rabeprazole and ranitidine group received 300 mg of ranitidine before breakfast and dinner for 8 weeks. After the end of treatment, we evaluated the endoscopic healing rate of reflux esophagitis and symptomatic improvement. RESULTS: After 8 weeks of treatment, rabeprazole group showed significantly higher complete endoscopic cure rate than ranitidine group (86.8% [46/53] vs. 57.9% [33/57], p=0.001) and higher symptomatic improvement of heartburn (91.2% [31/34] vs. 76.2% [32/42], p=0.085), especially in the first 7 days (76.7% vs. 45.3%, p=0.008). Also, rabeprazole group showed significantly higher improvement of regurgitation symptom than ranitidine group (100% [35/35] vs. 83% [39/47], p=0.009). Both group showed no differences in the improvement of chest pain and globus sensation. All the adverse events (rabeprazole group 4 events vs. ranitidine group 3 events) were mild and there was no abnormality in laboratory test. CONCLUSIONS: In patients with GERD, rabeprazole 10 mg b.i.d. is superior to ranitidine 300 mg b.i.d. in healing of reflux esophagitis and resolving typical GERD symptoms. Rabeprazole is an effective and well-tolerated drug for GERD treatment.

OBJECTIVE: To know the effect of adenosine 5'-triphosphate (ATP) on intracellular calcium level and cell proliferation in cervical cancer cells. METHODS: Study design: Four different human cervical cancer cell lines (Caski, C33A, HeLaS3 and SiHa) were used in this study. The change of intracellular calcium level, cell proliferation and the activity of proliferation- and calcium-related transcription factors by extracellular ATP were examined in these cell lines. RESULTS: Extracellular ATP induced calcium mobilization, cell proliferation and the activation of NF-kappa B in all cell lines used. CONCLUSION: These results suggest that calcium mobilization and NF-kappa B dependent signaling pathway play an important role in the cell proliferation by ATP in cervical cancer.

OBJECTIVE: Comparison of protein expressions by two-dimensional gel electrophoresis (2-DE) in normal cervix and squamous cell carcinoma tissues in Korean women. METHODS: Normal cervix and squamous cell carcinoma tissues were solubilized with 2-DE buffer and the first dimension of PROTEAN IEF CELL, isoelectric focusing (IEF), was performed using pH3-10 linear IPG strips of 17 cm. And then running 12% sodium dodecyl sulfate polyacrylamide gel electrophoresis (SDS-PAGE) and sliver stain. Scanned image was analyzed using PDQuest 2-D softwareTM. Protein spot spectrum was identified by assisted laser desorption/ionization-time of fighting (MALDI-TOF) and the protein mass spectrum identifications were performed by searching protein databases of Swiss-prot/TrEMBL, Mascot and MS-FIT. RESULTS: We found 9 up-regulation proteins (Alpha enolase, Keratin 19 type I, Keratin 20 type I, Keratin 13 type I, beta-actin, Aflatoxin B1 aldehyde reductase 1, Annexin A2, Squamous cell carcinoma antigen 2, unknown), 7 down-reguation proteins (Annexin 1, Myosin regulatory light chain 2, 14-3-3 protein epsilon, Heat shock 27 kDa protein, Hypothetical protein (DKFZP434C1715), Tumor necrosis factor receptor superfamily member 13B, Smoth muscle protein 22-alpha) and 6 up and down-regulation proteins (Tropomyosin 1, Tropomyosin 2, Tropomyosin 3, Serine (or cysteine) proteinase inhibitor, Phosphatidylinositol transfer protein alpha isoform, Src homology 3 domain-containing protein HIP-55) between normal cervix and squamous cell carcinoma cell tissues. CONCLUSION: 2-DE offers total protein expressions between normal cervix and squamous cell carcinoma cell tissues, and searching of differently expressed protein for the diagnostic markers of squamous cell carcinoma tissue.

OBJECTIVE: The electric analog circuit model can be used to analyse the various function of the cardio vascular system and the dynamic characteristics on each part of human body. According to the previous studies, viscous resistance, flow inertia, and vascular compliance in the cardiovascular system are analogous to resistance, inductance, and capacitance in electric circuit, so the cardiovascular system models have been represented by the electric circuit models. These approaches were to propose suitable models for the interested part of body and to simulate the various characteristics on the cardiovascular system. METHODS: This paper tried to put the characteristics of morphologic structure into the suggested electric circuit model. After setting parameter values of the model the dynamic characteristics of the cardio vascular system is simulated using VisSim, which is one of the simulation tools. RESULTS: The derived simulation results have represented major cardiovascular functions of normal adults. Also simulation results reflect the variation due to the pathophysiological state. CONCLUSION: The controller by baroreceptor, which is one of controllers to control the cardiovascular system, is appended in the model. The dynamic response characteristics and the procedure to restore blood pressure to normal state was observed by simulation lasted a certain period of bleeding.

9.Development of Bluetooth based Integrated Home Monitoring System for the Aged.

The remote surveillance system based on the wireless home sensor network, wireless internet connection, and the portable, hand-held device has been designed for the patron to get the emergency alarm for immediate care for the Dementia aged, when the patron is not at home. It consists of transmitting server and the receiving terminal. The transmitting server gathers the information through bluetooth connection regarding to environment threatening the safety of the Dementia aged from five sensors, including fire, gas, conductivity, photo, and motion sensors, and cameras up to the number of 16, fixated within home. It also determines the emergency situation automatically from sensors' environmental condition and the motion detected from the camera output targeted to the aged people, and sends the alarm message with the picture taken from home to the patrol for comprehending the emergency situation remotely. Either the PDA or the cellular phone is used as the receiving terminal device through wireless connectivity without the location limitation for the patrol. The functional evaluation via the implemented prototype system has been performed to initially demonstrate the efficacy, the usefulness, and ease to operate for later use for either solitarily living old people or nursery schools and infantile shelters.

10.A Study of the Use of EEG Analysis to Evaluate the Depth of Anesthesia Using Bispectrum Analysis Method.

BACKGROUND:In this study, a distribute characteristics on the bispectral coupling of EEG were examined according to the depth of anesthesia by using bispectum analysis a type of nonlinear signal processing. METHODS: 25 patients during the general anesthesia were studied. In the preprocess, base line correction and linear detrend was used. During the awakening state, preoperatively and postoperatively, appearance rate of bispectral coupling was observed that a strong appearance rate was represented in major frequency zone such as (15, 15) Hz but weak appearance rate investigated in the other minor frequency zone. Whilst anesthetized, a strong appearance rate revealed the low frequency range below (10, 10) Hz and a weak appearance rate viewed in the other frequency range. As such was compared with the delta ratio and spectrum edge frequency (SEF). RESULTS: The delta ratio showed significantly lower values at induction (3.60 +/- 0.98) and during the maintenance (5.0+/-1.74) of anaesthesia than preoperatively (10.30 +/- 2.44) and postoperatively (13.03 +/- 2.29)(P < 0.05). These results were consistent with the trend of delta ratio, namely that the index of evaluation for the depth of anesthesia, known as the reflex of the conscious level of patient's during anesthesia. While these results were not consistent with SEF analysis results that the index of evaluation for suppression level of patient's during recovery stage of anesthesia. SEF shows lower values at maintenance (17.00 +/- 1.22) and postoperatively (12.20 +/- 1.53) than preoperatively (22.50 +/- 1.75) and at the induction (22.40 +/- 1.18) of anesthesia(P < 0.05). CONCLUSIONS: The bispectrum analysis method provided useful information about the index of evaluation according to the level of unconsciousness and suppression among other evaluation factor on depth of anesthesia.
