1.Characteristics of Fragmented QRS Waves and Their Correlations with Ventricular Arrhythmias in Patients with CHD During Treadmill Exercise Test

Objective To investigate the characteristics of fragmented QRS waves and their correlation with ventricular ar-rhythmias in coronary heart disease patients during treadmill exercise tests.Methods A total of 216 patients suspected of hav-ing coronary heart disease or clinically diagnosed at our hospital from November 2021 to October 2022 were selected for the study.The general clinical data,cardiac ultrasound indices,laboratory indices and ECG parameters of the subjects were collect-ed.The patients were divided into two groups according to the results of coronary angiography:coronary heart disease(CHD)group(n=152)and non-CHD group(n=64).According to the presence of the fQRS wave in the treadmill exercise test,the CHD group and non-CHD group were divided into the QRS group(without the fQRS wave)and the fQRS group(with the fQRS wave).For patients with confirmed CHD,the occurrence of ventricular arrhythmia within 12 months was observed,and the CHD group was divided into a ventricular arrhythmia group(n=80)and non-ventricular arrhythmia group(n=72).ROC curves were used to analyze the diagnostic efficacy of the fQRS wave in the exercise treadmill test for CHD.A logistic regres-sion model adjusted for confounding variables was used to explore the relationship between fQRS wave duration and the risk of ventricular arrhythmias in patients with CHD.Random forest algorithm was used to screen the possible influencing factors of ventricular arrhythmias,and the formula for predicting ventricular arrhythmia events was obtained according to the regression e-quation y=1-1/(1+e-x).Bootstrap method was used to verify the prediction model.Results In the exercise treadmill test,the incidence of fQRS wave in the CHD group(116/152,76.32%)was significantly higher than that in the non-CHD group(21/64,32.81%).In the exercise treadmill test,the maximum area under the ROC curve(AUC)of the fQRS wave for CHD was 0.899(P<0.05),the sensitivity and specificity were 94.3%and 78.2%,respectively,and the Youden index was 0.725.In the CHD group,the duration of the QRS wave and fQRS wave after exercise was significantly longer than that before exercise,and the time limit of the fQRS wave before and after exercise was significantly longer than that of the QRS wave(P<0.05).Compared with those of the QRS group,the proportions of two-vessel and multi-vessel lesions of the fQRS group were greater,and the proportions of severe stenosis and complete occlusion were greater than those of the QRS group.The Gensini score of the fQRS group was greater than that of the QRS group.In the exercise treadmill test,the duration of the fQRS group before and after ex-ercise was positively correlated with the Gensini score.In the CHD group,the incidence of ventricular arrhythmias in the fQRS group(77/116,66.38%)was significantly higher than that in the QRS group(3/36,8.33%).The duration of the fQRS wave be-fore and after exercise was positively correlated with the risk of ventricular arrhythmia in patients with CHD.Conclusion The incidence of fQRS waves is significantly increased,and the duration of fQRS waves is significantly prolonged in patients with CHD during the treadmill exercise test.The fQRS wave has a certain value in predicting the degree of CHD and CAD.The fQRS wave and its duration in the exercise treadmill test are positively correlated with the risk of ventricular arrhythmia in pa-tients with CHD.
2.Value of 3D printed polyether ether ketone prosthesis in the treatment of scapular tumors

Polyetheretherketone (PEEK) as a new type of thermoplastic engineering plastic, has good biological activity, elastic modulus close to human cortical bone and radiation permeability, and has been widely used in medical field. This study aims to explore the safety and clinical efficacy of using 3D printing personalized PEEK materials to repaire scapular bone defects after bone tumor resection. A total of 6 patients who underwent the implantation of 3D printed PEEK scapular prosthesis from January 2020 to December 2021 in Yunnan Cancer Hospital were retrospectively analyzed. There were 3 males and 3 females, with age ranged from 14 to 52 years. There were 1 case of synovial sarcoma, 1 of Ewing's sarcoma, and 4 of chondrosarcoma. PEEK prosthesis was designed and fabricated based on CT data before surgery. Tumor resection and prosthesis replacement were performed under the premise of ensuring safe surgical boundaries, including 2 cases of total scapular prosthesis replacement and 4 cases of partial scapular prosthesis replacement. The operation time was 90-170 min, and the intraoperative blood loss was 100-400 ml. All 6 patients received satisfactory follow-up, with a tumor progression free survival time of 16-28 months. No tumor recurrence or metastasis was observed, and all patients survived tumor free. At last follow-up, the Constant-Murley shoulder joint score was a minimum of 62 points and a maximum of 68 points. The Japanese Orthopaedic Association's shoulder joint score was 63 points minimum and 78 points maximum. Computer-aided design 3D printing PEEK material prosthesis has certain advantages in the treatment of scapular tumor limb salvage. It has light weight, well adapted, relatively simple installation, good histocompatibility, and can obtain a better appearance and function of the shoulder joint after operation. It can become one of the options for limb salvage treatment of scapular tumor.
3.Application of bilateral lateral pre-decompression of distal and proximal digits in replant of degloving injury

Objective:To explore the clinical application of distal and proximal bilateral lateral pre-decompression in replant of digit degloving injury.Methods:From March 2012 to May 2021, 14 patients with 29 digits had replantation surgery of degloved soft tissue and severed digits in Section II, Department of Orthopaedics, Changzhou Medical District of 904th Hospital of PLA Joint Logistic Support Force. There were 2 types of injuries: degloving injury of soft tissue but with intact digital tip, and digital tip degloving injury with intact distal phalanx and nail bed. With the technique of distal and proximal bilateral lateral pre-decompression, pre-decompression incisions were made to the subdermal on both sides of the degloved skin over the proper palmar digital arteries. From where, the distal stump of the proper palmar digital arteries and nerves for anastomosis were found and had them anastomosed with the proximal proper palmar digital arteries and proper palmar digital nerves, then anastomosed digital pulp and digital dorsal veins (11 cases were direct anastomosis and 3 cases were bridged anastomosis). Follow-up was carried out by outpatient consultation, telephone and WeChat APP. The appearance of the digit body, the shape of digital pulp, the nails grow and the motions of the digits were observed, and the Evaluation Standard of Replantation of Severed Fingers by the Society of Hand surgery of Chinese Medical Association was used to evaluate the recovery of function.Results:All 29 segments of degloving digit survived. Small necrotic areas was found in 4 digits and healed after the change of dressing. The followed-up time was lasted for 3-36 months. There was no obvious atrophy found in all the digits. Appearances of digit pulp and nails were satisfactory with good sensational recovery for TPD at 6-10 mm, 7 mm in average. Motions of all the repaired digits were good. According to the Evaluation Standard of Replantation of Severed Fingers by the Society of Hand surgery of Chinese Medical Association, 23 digits were in excellent, 4 in good and 2 in poor, with a satisfactory rate at 93.1%.Conclusion:For a distal digit injury with intact distal soft tissue and relatively mild injury of blood vessels and nerves, using the method of distal and proximal bilateral lateral pre-decompression can help to achieve good appearance in survived digital pulp, good sensational recovery and good nail growth. An individualised treatment intra-and-after the surgery could offer an ideal therapeutic effect.
4.Mental health status in close contacts of COVID-19 infections undergoing centralized quarantine and observation

ObjectiveTo explore the prevalence rate and related risk factors of anxiety, depression and insomnia symptoms in close contacts of COVID-19 patients undergoing centralized quarantine and observation, so as to provide references for follow-up targeted psychological crisis intervention. MethodsFrom February 6 to 8, 2020, a total of 136 close contacts at designated quarantine sites in four districts and counties of Ya'an city were enrolled, and assessed using the general information questionnaire, Hospital Anxiety and Depression Scale (HAD) and Insomnia Severity Index (ISI). ResultsA total of 26 quarantined individuals (19.12%) had various degrees of anxiety, depression or insomnia, including 16 cases (11.76%) with anxiety, 9 cases (6.62%) with depression and 16 cases (11.76%) with insomnia. Correlation analysis showed that HAD (A) score was positively correlated with HAD (D) score (r=0.642, P<0.01). HAD (A) score was positively correlated with ISI score (r=0.590, P<0.01). HAD (D) score was positively correlated with ISI score (r=0.329, P<0.01). Multivariate Logistic regression analysis showed that the risk factor of anxiety was the first week of isolation (OR=3.290, P<0.05), meantime, the risk factors of insomnia included the old age (OR=7.990, P<0.05) and the first week of isolation (OR=4.659, P<0.05). ConclusionThe close contacts of COVID-19 infections undergoing centralized quarantine and observation in Ya’an have high prevalence rate of anxiety and insomnia, and the symptoms are most severe during the first week of quarantine.
5.Robot-assisted gait training improves the walking ability of stroke survivors

Objective:To explore the effectiveness of the Exowalk gait training robot in improving the walking ability of stroke survivors.Methods:Forty stroke survivors were randomly divided into a control group and an experimental group, each of 20. In their rehabilitation, the control group was given routine walking training, while the experimental group′s training was assisted with the Exowalk robot. Both groups trained for 60 minutes a day, five days a week for four weeks. Before as well as after 2 and 4 weeks of training functional ambulatory categories (FACs), the Berg balance scale (BBS), the 6-minute walking test (6MWT), the 10-minute walking test (10MWT), the Rivermead mobility index and an exercise index were used to evaluate those in both groups.Results:After 2 weeks significant improvement was observed in the average FAC, BBS, 6MWT and 10MWT results of both groups, without significant differences between them. After 4 weeks there was still no significant difference in the groups′ average BBS scores. However, the average FAC rating in the experimental group had improved significantly while there was no significant increase in the control group′s average score.Conclusions:The Exowalk robot can help to improve the balance and walking ability of hemiplegic stroke survivors.
6. Review on the etiology and complications of hand, foot and mouth disease, using data from the national sentinel surveillance program, in China, 2015-2016

Objective:
To understand the characteristics relating to the etiology and complications of hand, foot and mouth disease (HFMD) based on data from the pilot National Sentinel Surveillance (NSS) program so as to explore the feasibility, advantages and disadvantages of the NSS.
Methods:
Data were extracted from the NSS system, conducted in 11 provinces of China from November 2015 to October 2016. Characteristics regarding the etiology, complications of HFMD and factors related to the positive rates of HFMD specimens were analyzed under the logistic regression method by SPSS 20.0 software.
Results:
A total of 4 783 specimens were collected, including 3 390 from mild, 1 390 from severe and 3 from death cases. The overall positive rate was 81.43% (3 895/4 783). Other enteroviruses (non EV71/Cox A16 enteroviruses) appeared the major serotype (52.68%, 1 482/2 813) for mild infection of the disease while EV71 was for the severe cases (65.31%, 706/1 081). The serotype spectrum revealed by the pilot NSS was almost identical with the existing surveillance system. Other enteroviruses tended to infect younger children (
7. Bisphosphonate-related osteonecrosis of the jaw in malignant tumor patients with bone metastases: clinical analysis of six cases

Objective:
To analyze the clinical manifestation, imaging characteristics and prevention of medication-related osteonecrosis of the jaw (MRONJ) after intravenous bisphosphonate (BP) for cancer patients with bone metastases.
Methods:
The clinical data and radiographic findings of 6 primary breast cancer patients with bone metastases diagnosed as MRONJ from January 2014 to April 2018 in Shanxi Dayi Hospital were retrospectively analyzed.
Results:
All 6 patients were female, with the median age of 65.5 years old. All patients had no history of systemic application of hormone therapy, no history of diabetes, no history of radiation therapy, no history of metastasis of the jaw, and no history of infection. The average usage time of BP was 28 months. MRONJ occurred in 2 cases on maxilla and 4 cases on mandible. There were 2 patients with tooth extractions history in BP treatment. Clinical symptoms included maxillofacial pain, loosened teeth, fistula suppuration, and exposed sequestrum. Radiographic findings included osteolysis and bone sclerosis or the mixed manifestation of both, with or without periosteal reaction. In addition, nonhealing tooth sockets and sequestrum separation imaging were also included.
Conclusions
Tooth extraction is considered as an increased risk for MRONJ in patients with malignant bone metastases after BP therapy. MRONJ is more likely to appear in the mandible, but it can also appear in the maxilla. Early screening and initiation of appropriate dental care are necessary for the patients before using BP therapy.

Clinical teaching round is one of main teaching methods during standardized training for residents. However, the particularity of standardized resident training in clinical anesthesia determines that it is difficult to apply the teaching round model of other disciplines. In this study, seven core contents of Mini-Clinical Evaluation Exercise (Mini-CEX) were modified after considering the characteristics of anesthesia specialty and applied to the teaching rounds of standardized training for residents of anesthesia , thus promoting the standardization and improving the quality of anesthesia teaching rounds.
9.Severe cases with hand, foot and mouth disease: data based on national pilot hand, foot and mouth disease surveillance system

Objective To investigate the clinical severity,etiological classification and risk factors of severe cases with hand,foot and mouth disease (HFMD).Methods A total of 1 489 records on severe and fatal HFMD cases reported to the national pilot surveillance system of HFMD were used to analyze the demographic,medical treatment,etiological classification of the cases.Treatment outcome related risk factors were also studied with multi-variable stepwise logistic regression method.Results Seven out of the 1 489 severe HFMD cases died of this disease.A total of 960 (72.9%) were under three years old and 62.9% were male and most of the cases (937,62.9%) resided in rural areas.Among all the cases,494 (33.2%) went to seek the first medical assistance at the institutions of village or township level.Durations between disease onset and first medical attendance,being diagnosed as the disease or diagnosed as severe cases were 0(0-1) d,1 (0-2) d and 2 (1-4) d,respectively.In total,773 (51.9%) of the severe HFMD cases were diagnosed as with aseptic meningitis,260 (17.5%) with brainstem encephalitis,377 (25.3 %) with non-brainstem encephalitis,6 (0.4%) with encephalomyelitis,1 (0.1%) with acute flaccid paralysis,4 (0.3%) with pulmonary hemorrhage/pulmonary edema and 68 (4.6%) with cardiopulmonary failure.Of the etiologically diagnosed 1 217 severe and fatal HFMD cases,642 (52.8%) were with EV71,other enterovirus 261 (21.5%),Cox A16 36 (3.0%),1 (0.1%) with both EV71 and Cox A16.However,277 (22.8%) showed negative on any pathogenic virus.Complication (Z=3.15,P=0.002) and duration between onset and diagnosed as severe cases (Z=3.95,P<0.001) were shown as key factors related to treatment outcomes.Conclusions Most severe HFMD cases appeared in boys,especially living in the rural areas.Frequently seen complications would include aseptic meningitis,non-brainstem encephalitis and brainstem encephalitis.EV71 was the dominant etiology for severe and fatal cases.Early diagnosis and complication control were crucial,related to the treatment outcome of HFMD.
10.Multiple measures to improve perioperative antimicrobial use in typeⅠin-cision operation

Objective To evaluate the effect of multiple intervention measures on perioperative antimicrobial use in pa-tients undergoing typeⅠincision operation in a hospital,and provide basis for rational use of antimicrobial agents. Methods 9 823 patients with type I incision operation in April and October of 2005-2012 were surveyed retrospectively, data of 2005 was as baseline,from 2006 to 2012,multiple measures,including training,examination,supervision,feed-back,and cooperation of relevant departments were conducted,antimicrobial use before and after intervention was com-pared.Results The qualified rate of perioperative antimicrobial use in patients undergoing typeⅠ incision operation in-creased from 14.20% in 2006 to 92.30% in 2012;the rate of combined use of antimicrobial agents was relatively higher (7.00%-9.00%)in 2006-2009,had a downward trend in 2010- 2012,and decreased to 3.20% in 2012.Types of an-timicrobial agents for prophylactic use in typeⅠincision operation in 2006 and 2007 were similar to that of 2005,the main used antimicrobial agents were cephalosporins,penicillin and it’s compounds,and aminoglycosides;the major antimicrobial prophylaxis in 2008-2012 were the first and second generation cephalosporins,penicillin and it’s compounds.Multivariate non-conditional logistic regression analysis showed that age (40 - 59 years old),departments (orthopedics,general surgery,and ophthalmology),and years (2011 and 2012)were the main influencing factors for the qualified use of antimicrobial agents(all P<0.05).Conclusion Implementation of multiple intervention measures can improve the qualified rate of perioperative antimicrobial use in typeⅠ incision operation,reduce types of antimicrobial use and rate of combined antimicrobial use.
