1.Correlation between serum miR-122-5p and FOXO3 levels and osteoporosis in postmenopausal women with non-alcoholic fatty liver disease

Objective:To investigate the correlation between serum miR-122-5p and FOXO3 levels and osteoporosis (OP) in postmenopausal women with non-alcoholic fatty liver disease (NAFLD) .Methods:The clinical data and serum of 30 postmenopausal women with NAFLD and 48 postmenopausal women with no-NAFLD were collected. The levels of miR-122-5p and FOXO3 in serum were detected by qRT-PCR. Triglycerides, high-density lipoproteins, and low-density lipoproteins were detected by biochemical autoanalyzer. The bone mineral density of lumbar vertebrae 1-4, Wards triangular bone, femoral neck, greater trochanter and total hip was detected by bone mineral density analyzer. The correlation between the above clinical indicators and OP was analyzed.Results:The expression of miR-122-5p in postmenopausal female NAFLD patients (0.76±0.28) was lower than that in non-NAFLD patients (1±0.31) ( t=3.43, P=0.001) . The downstream target gene FOXO3 of miR-122-5p was identified by bioinformatics website analysis. The expression of FOXO3 in postmenopausal female NAFLD patients (1.31±0.30) was higher than that in non-NAFLD patients (1±0.27) ( t=4.73, P<0.001) . Student’ s t test and Logistic regression analysis showed that triglyceride, miR-122-5p and FOXO3 levels were risk factors for NAFLD (all P<0.05) . Pearson correlation coefficient showed that miR-122-5p level was significantly positively correlated with BMD of femoral neck ( r=0.488, P=0.006) , greater trochanter ( r=0.367, P=0.046) and whole hip ( r=0.404, P=0.027) . FOXO3 level was negatively correlated with bone mineral density of femoral neck ( r=-0.445, P=0.014) and whole hip ( r=-0.507, P=0.004) , while other indexes were not significantly correlated (all P>0.05) . Conclusion:Decreased serum miR-122-5p level and increased FOXO3 level in postmenopausal women with NAFLD may increase the risk of OP.

Hip fractures are among the most common fractures in the elderly, presenting to be a leading cause of disability and mortality. Surgical treatment is currently the main treatment method for hip fractures. The incidence of perioperative malnutrition is increased after hip fractures in the elderly due to the comorbidities, decreased basal metabolic rate, accelerated protein breakdown, weakened anabolism and surgical stress. However, malnutrition not only increases the incidence of postoperative complications, but also leads to increased mortality, indicating an important role of perioperative nursing management of nutrition for the elderly patients with hip fractures. At present, there still lacks scientific guidance and application standards on perioperative nursing management of nutrition for the elderly patients with hip fractures. Therefore, the Orthopedic Nursing Committee of Chinese Nursing Association and the Editorial Board of Chinese Journal of Trauma organized relevant experts to formulate the Expert consensus on perioperative nursing management of nutrition for elderly patients with hip fractures ( version 2023) according to evidence-based medical evidences and their clinical experiences. Fourteen recommendations were made from aspects of nutrition screening, nutrition assessment, nutrition diagnosis, nutrition intervention and nutrition monitoring to provide guidance for perioperative nursing management of nutrition in elderly patients with hip fractures.

Pulmonary blast injury has become the main type of trauma in modern warfare, characterized by externally mild injuries but internally severe injuries, rapid disease progression, and a high rate of early death. The injury is complicated in clinical practice, often with multiple and compound injuries. Currently, there is a lack of effective protective materials, accurate injury detection instrument and portable monitoring and transportation equipment, standardized clinical treatment guidelines in various medical centers, and evidence-based guidelines at home and abroad, resulting in a high mortality in clinlcal practice. Therefore, the Trauma Branch of Chinese Medical Association and the Editorial Committee of Chinese Journal of Trauma organized military and civilian experts in related fields such as thoracic surgery and traumatic surgery to jointly develop the Clinical treatment guideline for pulmonary blast injury ( version 2023) by combining evidence for effectiveness and clinical first-line treatment experience. This guideline provided 16 recommended opinions surrounding definition, characteristics, pre-hospital diagnosis and treatment, and in-hospital treatment of pulmonary blast injury, hoping to provide a basis for the clinical treatment in hospitals at different levels.
4.Effect of non-alcoholic fatty liver disease on survival and risk factors in patients with hormone receptor-positive breast cancer

Objective:To investigate the risk factors of non-alcoholic fatty liver disease (NAFLD) in patients with hormone receptor positive (HR+), human epidermal growth factor receptor 2 negative (HER2-) breast cancer (HR+/HER2-BC) and the impact of NAFLD on the survival of patients.Methods:54 HR+BC patients were enrolled in this study. The liver fat accumulation was examined by magnetic resonance imaging (MRI). The patients were divided into two groups: non-NAFLD and NAFLD. Student's t test or Fisher's test was used to analyze the clinical indicators of the two groups. Logistic univariate and multivariate tests were used to analyze the clinical risk factors related to NAFLD. Receiver operating characteristic curve (ROC curve) was used to further analyze the sensitivity of clinical risk factors to predict the diagnosis of NAFLD. The Disease-free survival (DFS) and Overall survival (OS) of the two groups were analyzed by Log-rank (Mantel-Cox) test. Results:There were 22 NAFLD patients and 32 non-NAFLD patients diagnosed by MRI. Student's t test or Fisher's test showed that BMI, waist circumference, AST, ALT, GGT, TG, LDL and HDL were statistically different between the two groups (all P<0.05). Logistic univariate and multivariate analysis showed that AST ( OR=1.05, 95% CI: 1.02-1.10, P=0.007), GGT ( OR=1.04, 95% CI: 1.01-1.09, P=0.038), TG ( OR=1.03, 95% CI: 1.01-1.06, P=0.011) and HDL ( OR=1.06, 95% CI: 1.01-1.12, P=0.037) were the risk factors associated with NAFLD. ROC curve analysis showed that the combination of AST, GGT, TG and HDL had high sensitivity in predicting NAFLD (AUC=0.869, P<0.05). There was no difference in DFS ( HR=1.830, 95% CI: 0.983-3.409, P=0.057) or OS ( HR=2.482, 95% CI: 0.761-8.093, P=0.132) between the two groups. Conclusion:AST, GGT, TG and HDL are the independent risk factors for NAFLD in HR+BC patients during treatment, but concurrent NAFLD has no significant effect on DFS or OS.
5.Mediating effect of psychological capital between nurses' cognition of patient safety culture and career growth

Objective:To explore effects of nurses' cognition of patient safety culture and psychological capital on career growth, so as to provide a basis for promoting nurses' career growth.Methods:This research was a cross-sectional survey. From July to August 2019, a total of 480 nurses from 3 ClassⅢ Grade A hospitals in Tianjin were selected as the research objects by means of convenient sampling. General information questionnaire, Hospital Survey on Patient Safety Culture (HSOPSC) , nurses' Psychological Capital Questionnaire (PCQ) and Career Growth Scale (CGS) were used to investigate the patients. In this study, 480 questionnaires were issued and 465 valid questionnaires were collected, with an effective recovery rate of 97%. Stratified regression analysis was used to analyze the mediating effect of psychological capital, and AMOS was further used to construct the structural equation model for verification.Results:The total mean score of HSOPSC of 465 nurses was (3.91±0.30) , the total mean score of PCQ was (4.66±0.35) and the total mean score of CGS was (3.62±0.41) . Nurses' cognition of patient safety culture was positively correlated with career growth ( r=0.525, P<0.01) . Nurses' cognition of patient safety culture was positively correlated with psychological capital ( r=0.470, P<0.01) . There was a positive correlation between psychological capital and career growth ( r=0.557, P<0.01) . The direct path coefficient of patient safety culture cognition on career growth was 0.33. Psychological capital played a part of the mediating role in the relationship between patient safety cultural cognition and career growth. The mediating effect was 0.30, and the mediating effect accounted for 47.6% of the total effect. Conclusions:The career growth of nurses is at a medium level, and psychological capital plays an intermediary role between the patient's cultural awareness of safety and career growth. Nursing managers should take effective measures to improve nurses' cognition of patient safety culture, at the same time improve nurses' psychological capital level, so as to promote nurses' career growth.
6.Effect of phase I cardiopulmonary rehabilitation on patients with coronary artery bypass grafting: a Meta-analysis

Objective? To evaluate the rehabilitation effect of phase I cardiopulmonary rehabilitation on patients with coronary artery bypass grafting(CABG). Methods? Chinese and English databases were searched via computer including PubMed, Embase, Cochrane Library, Web of Science, CNKI, Wanfang, VIP, with the time set as the database establishment date to 31st January of 2019, to obtain randomized controlled trials of phase I cardiopulmonary rehabilitation in CABG. Meta-analysis of the included studies was performed by RevMan 5.3 software. Results? A total of 8 studies were included, according to the Meta analysis, 6 min walking distance increased in the phase I cardiopulmonary rehabilitation patients group(MD=56.46,95%CI 32.82-80.09,P< 0.001), the percentage of forced vital capacity(FVC) in expected value(1%) increased (MD=7.30,95%CI 2.59-12.00,P=0.002), the percentage of forced expiratory volume in first second(FEV1) in the expected value(1%) increased (MD=7.93,95%CI 3.13-12.72,P=0.001), hospital stay decreased(MD=-5.35,95%CI -9.67--1.03,P=0.002), incidence of postoperative arrhythmia decreased(MD=0.42, 95%CI -0.21-0.85),P=0.002), but the improvement effect of quality of life was not significan(t P> 0.05). Conclusions? Phase I cardiopulmonary rehabilitation can improve the activity endurance, pulmonary function, shorten the hospitalization time and reduce the incidence of postoperative arrhythmia, but it can not significantly improve the quality of life. Due to the small number of included studies and the heterogeneity of some indicators, the results of this study still need to be verified by multicenter, large sample and high-quality randomized controlled trials.
7.Study on the Correlation between Caring Perception Status of Patients and Nurse-patient Relationship Trust Degree

Objective:To investigate the perception status of inpatients to nurses' caring behavior, and analyze the correlation between them and nurse-patient relationship trust degree. Methods:Adopting general information questionnaire, Caring Behaviors Inventory ( CBI) , nurse-patient relationship trust scale, a questionnaire survey was conducted among 226 inpatients in a third class A tertiary hospital in Tianjin. Results:The total score of ca-ring behavior perception was (96. 92 ± 15. 68), and among the four dimensions, the perception of patients to nur-ses' knowledge and skills was deepest, the perception to nurse' s contact with patients was worst; total score of nurse-patient trust scale was (124. 75 ± 19. 13). There was a positive correlation between the total score of pa-tients' caring perception and nurse-patient relationship trust degree (r=0. 554);multiple linear stepwise regres-sion analysis showed respect for patients, support and assurance were the main factors influencing the nurse-pa-tient relationship trust degree. Conclusion:The patients' perception on nurses' caring behavior and nurse-pa-tient relationship trust degree is closely related. Nurses should carry out targeted care to patients with different char-acteristics to improve patients' trust to nurses and construct a harmonious nurse-patient relationship.
8. Clinical and genetic characters of 8 Chinese children with ADCK4-associated glomerulopathy

Objective:
To investigate the clinical and genetic character of Chinese children with the aarF domain containing kinase 4 (ADCK4)-associated glomerulopathy.
Methods:
Applying next generation sequencing to detect possible gene mutation(renal disease associated monogene was pooled as one panel) in 69 children with steroid-resistant nephrotic syndrome (SRNS) or persistent proteinuria of unknown origin. Sanger sequencing was used to confirm the significant mutations found in the children and to validate these mutation sites in their patients. Using online software (PolyPhen2, SIFT, Mutation Taster) to predict whether the detected missense mutations were disease causing or not. Collecting and analyzing clinical data of children with ADCK4-associated glomerulopathy, which included onset age, clinical manifestation, and renal pathology.
Results:
The ADCK4 gene mutation was detected in 8 children with a positive rate of 11.6% (8 out of 69), among which 3 patients carried homozygous c.748G>C mutation, 3 patients carried homologous c.737G>A mutation, 1 patient carried compound heterozygous mutation(c.748G>C and c.737G>A), and 1 patient carried compound heterologous mutation(c.551A>G and c.737G>A). Collectively, there were only 3 mutation sites found in total 8 patients, in which the mutation sites of c.748G>C and c.737G>A had high detection frequency in these 8 patients. These 3 mutation sites were all missense mutation which were predicted to be disease causing by online software and not reported before. The average onset age was 6.5 years (2 years-11.75 years). Four patients presented with SRNS and the other 4 presented with persistent proteinuria. All 8 patients had no extrarenal manifestation, renal biopsy revealed focal segmental glomerulosclerosis (FSGS) in most patients, among which 3 cases had gone to end-stage renal disease (ESRD) at disease onset, and 2 cases progressed to ESRD 2 and 5 years after onset respectively. Seven patients had received glucocorticoid and/or immunosuppressive drug while only one patient getting partial response. All 8 patients were treated with large amount of coenzyme Q10 (15 mg·kg-1·d-1) after definite diagnosis of ADCK4 mutation-some patients had acquired encouraging curative effect.
Conclusions
ADCK4-associated glomerulopathy is not rare especially in the children with SRNS. The onset age is relatively old and the extrarenal manifestation is less common. FSGS is a main pathology type. Patients usually have no response to immunosuppressive therapy, but may benefit from addition of large amount of coenzyme Q10. Some patients may only manifest with insidious proteinuria, causing the early diagnosis to be difficult, which deserves more attention. Three new missense mutations expand disease causing mutation repertoire of ADCK4 gene, among which the two sites of c.748G>C and c.737G>A may be mutation hotspot of ADCK4-associated glomerulopathy in Chinese population, and need further study.
9.Influence of three booster doses hepatitis B vaccine on the persistence of immune-protection among infants with normal and high antibody response to primary vaccination: a matched case-control study.

OBJECTIVETo examine the influence of three-booster-doses hepatitis B vaccines on children with normal and high antibody response to primary vaccination.
METHODSAntibody against hepatitis B surface antigen (anti-HBs) were detected after primary vaccination and children with normal or high response to hepatitis B primary vaccination at infancy, were identified. Children who were given three booster doses were selected to form the booster group and who were given no booster dose were 1∶1 matched with the same gender and residence to form the control group. Blood samples were obtained from all the participants and tested for anti-HBs and anti-HBc, 5 years after the primary vaccination.
RESULTSThe positive rates of anti-HBs response to primary vaccination were 97.39% (224/230, 95% CI: 94.41%-99.04%) in the booster group and 53.91% (124/230, 95% CI: 47.24%-60.48%) in the control group (P<0.05), 5 years after the primary vaccination. Geometric mean concentration (GMC) of anti-HBs were 1 140.02 (887.46-1 464.46) mIU/ml in the booster group and 11.53 (8.73-15.23) mIU/ml in the control group (P<0.05). The prevalence rates of breakthrough HBV infection were 0.87% (2/230) in the booster group and 2.17%(5/230) in the control group (P>0.05). RESULTS from the multivariable analysis showed that the booster doses (OR=38.75, 95%CI: 16.23-92.54) and the level of anti-HBs after the primary vaccination (OR =3.06, 95%CI:1.51-6.17) were independently associated with the positive rates of anti-HBs, 5 years after the primary vaccination (P<0.05).
CONCLUSIONPrograms with three booster doses to children that showing normal and high antibody response to primary vaccination could improve the persistence of anti-HBs but possibly would not be able to prevent the HBV infection.

10.Effects of Bushen Huoxue Prescription on Granulosa Cell Apoptosisi of Premature Ovarian Failure Mice

Objective To discuss Bushen Huoxue Prescription mechanism of action on follicular granulosa cell apoptosis of premature ovarian failure (POF) mice. Methods The mouse zona pellucida 3 as the antigen, multi-point injection of subcutaneous immunization BALB/c to female mice was used to establish autoimmune POF model. Forty clean female mice were randomly divided into blank group, model group, Estradiol Valerate group and Bushen Huoxue Prescription group. All medicine groups were given relevant medicine for gavage, while model and blank groups were given normal saline with the same amount for gavage for 15 days. After the treatment, ovary, uterus, and thymus were weighed, and the viscera index was calculated. An ovarian slice was taken and HE staining was used to observe granulosa cell apoptosis. The levels of E2, LH, and FSH were detected by using the radioimmunoassay method. RT-PCR was used to analyze Fas and Fas-L mRNA expressions in ovarian granulosa cells. Results Compared with blank group, the weight of ovary, thymus, and uterus decreased significantly (P<0.05, P<0.01);Serum E2 level dropped (P<0.01);levels of FSH and LH, and expressions of Fas and Fas-L mRNA increased significantly (P<0.01). Compared with model group, granulosa cell apoptosis significantly decreased, and ovary index, uterus index, and thymus index increased (P<0.05);serum E2 level increased (P<0.01);levels of FSH and LH, and expressions of Fas and Fas-L mRNA significantly decreased (P<0.05). Conclusion Bushen Huoxue Prescription can decrease expressions of Fas and Fas-L mRNA to inhibit cell apoptosis by adjusting the hormone level of hypothalamus-pituitary-ovarianaxis.
