1.Predictive value of non-enhanced CT combined with clinical indicators in severe acute pancreatitis

Objective:To establish and validate a nomogram model for early prediction of the risk of acute pancreatitis (AP) progressing to severe acute pancreatitis (SAP).Methods:CT signs and clinical laboratory parameters of 361 AP patients admitted to our Hospital from January 2016 to July 2022 were retrospectively collected. There were 221 males (61.2%) and 140 females (38.8%). According to the Atlantic score, all patients were divided into the SAP group (64 cases) and the non-SAP (NSAP) group (297 cases). Univariate analysis was used to screen out variables with statistically significant differences. Multivariate Logistic regression analysis was used to screen out the independent risk factors of SAP, and finally a nomogram prediction model was established. Receiver operating characteristic (ROC) curve, calibration curve and decision curve (DCA) were used to evaluate the predictive efficacy, accuracy and clinical practicability of the model, and Bootstrap method was used to verify the model internally.Results:Univariate analysis and multivariate Logistic regression analysis showed that pleural effusion ( OR=7.353, 95% CI: 3.344-16.170), posterior pararenal space (PPS) involvement ( OR=3.149, 95% CI: 1.314-7.527), serum creatinine concentration (Cr) ( OR=1.027, 95% CI: 1.017-1.038) and serum calcium concentration (Ca 2+) ( OR=0.038, 95% CI: 0.009-0.166) were independent risk factors for SAP ( P<0.05). A Nomogram model was established based on these four factors. The area under the ROC curve (AUC) of this model was 0.905 (95% CI: 0.869-0.933), indicating high predictive efficiency. Internal verification showed that the model had good accuracy in predicting SAP, and C-index was 0.90. DCA analysis showed that the model had high clinical practicability. Conclusions:The Nomogram model combining pleural effusion, PPS involvement, Cr and Ca 2+ had a good effect on early prediction of SAP, which could provide a new reference tool for clinical diagnosis and treatment.
2.Comparison of modified load-sharing rip-stop technique with suture bridge for repair of massive rotator cuff tears

Objective:To compare modified load-sharing rip-stop (mLSRS) technique with suture bridge (SB) for repair of massive rotator cuff tears.Methods:A retrospective analysis was conducted of the 68 patients who had been treated for massive rotator cuff tears from January 2017 to June 2019 at Department of Orthopedics, Dongyang People's Hospital. They were 26 males and 42 females, aged (63.4±1.0) years (from 45 to 84 years), with 56 right and 12 left sides affected. Of them, 32 were treated by mLSRS and 36 by double-line SB. The 2 groups were compared in terms of visual analog scale (VAS), Constant-Murley scoring, University of California Los Angeles (UCLA) scoring and American Shoulder and Elbow Surgeons (ASES) scoring at the final follow-ups, and Sugaya ultrasonic evaluation of the repair integrity at postoperative 12 months.Results:There was no statistically significant difference in preoperative general data between the 2 groups, showing they were comparable ( P>0.05). Operations were successful in all patients who were followed up for 13 to 44 months (average, 19.7 months). No patient suffered from such complications as joint infection or anchor withdrawal. At the final follow-up, the mLSRS group scored respectively 0.9±0.6, 85.3±4.8, 33.1±1.0 and 86.4±5.0 in VAS, Constant-Murley, UCLA and ASES scores, significantly improved than their preoperative values (7.2±0.8, 47.0±3.1, 15.8±3.0 and 48.5±4.5) ( P<0.05); the SB group scored respectively 1.1±0.6, 86.6±3.4, 33.2±1.1 and 86.9±4.6 in the above indicators, also significantly improved than their preoperative values (7.3±0.7, 46.5±4.7, 14.4±2.7 and 48.8±4.3) ( P<0.05); there were no significant differences between the 2 groups in preoperative or postoperative values ( P> 0.05). The Sugaya ultrasonic evaluation showed no significant difference in rotator cuff healing between the 2 groups at postoperative 12 months ( P>0.05); according to the positions of re-tears, the SB group had significantly more type Ⅱ re-tears (20%, 2/10) than the mLSRS group (0, 0/4) ( P<0.05). Conclusion:Although both mLSRS and SB techniques can achieve satisfactory and comparable clinical outcomes in arthroscopic repair of massive rotator cuff tears, mLSRS may lead to fewer postoperative type Ⅱ re-tears than SB.
3.Related factors of troublemaking among patients with mental disorders caused by amphetamine-type stimulants

ObjectiveTo explore the related factors of troublemaking behaviors among patients with mental disorders induced by amphetamine-type stimulants (ATS), and to provide references for the formulation of relevant intervention measures for ATS-induced mental disorders. MethodsA total of 105 patients who met the diagnostic criteria of International Classification of Diseases, tenth edition (ICD-10) for ATS-induced mental disorders were included, and classified into troublemaking group and non-troublemaking group. The general demographic data and clinical data of the selected individuals were collected, and all patients were assessed using Social Support Rating Scale (SSRS). Then univariate analysis and multivariate Logistic regression model were used to screen the related factors of troublemaking behaviors. ResultsThe scores of SSRS, objective support dimension and social support utilization dimension were significantly lower in troublemaking group than those in non-troublemaking group, with statistical differences [(24.10±6.59) vs. (28.94±5.59), t=3.364, P=0.001; (5.50±1.96) vs. (8.20±2.13), t=5.183, P<0.01; (4.60±2.26) vs. (6.28±1.90), t=3.435, P=0.001]. Multivariate Logistic regression analysis showed that male (OR=6.061, P=0.014) was a risk factor, while high social support level (OR=0.873, P=0.018) was the protective factor for troublemaking behaviors among patients with ATS-induced mental disorders. ConclusionPatients with ATS-induced mental disorders of the males and with low social support level are at high risk of troublemaking behaviors.
4.Application of inter-professional simulation-based clinical examination in extended nursing experimental teaching practice

Objective To explore the feasibility of inter-professional simulation-based clinical examination in nursing extended experimental teaching practice.Methods Inter-professional cooperation teams were founded, and instructional teams were included: two teachers from nursing, and clinical medicine, and one technician of teaching experiment; student practice team: 3-4 nursing students from grade 2014, 1-2 clinical medicine students from grade 2014; cases were organized and the context was designed: in the examination, every module was evaluated by two teachers at the same time, and after examination, the opinions and recommendations of students on the examination were collected, and the correlation between the examination and the results of modules in extended nursing experimental teaching practice was analyzed. Results The consistency coefficient of two teachers was 0.976; the score ranged from (7.21±1.44) to (8.32±2.13); there was no significant correlation between the ability evaluation of cooperation and other ability evaluation.Conclusions Inter-professional simulation-based clinical examination can be used in extended nursing experimental teaching practice, and it can comprehensively assess students' academic competition.
5.Analysis of C.3925_3929 deletional mutations of APC gene in pedigrees with familial adenomatous polyposis.

OBJECTIVETo analyze the characteristics of germline mutations of adenomatous polyposis coli (APC) gene in pedigrees affected with familial adenomatous polyposis (FAP).
METHODSGenomic DNA was extracted from peripheral blood samples from members of the 13 FAP pedigrees. Multiplex ligation-dependent probe amplification (MLPA) was used to detect large fragment deletions of the APC gene. Subsequently, potential mutation was screened from all exons of the APC gene with PCR amplification and direct sequencing.
RESULTSGermline mutations have been identified in 5 FAP pedigrees, which included c.3184_3187delCAAA, c.5432C>T, c.3925_3928delAAAA and c.3925_3929del AAAAG(in two pedigrees). Small deletional mutations were found primarily in the area of AAAAG tandem repeat sequences.
CONCLUSIONC.3925_3929 located in AAAAG tandem repeats is probably the hot spot for APC gene mutations, which are mostly deletional mutations, especially the 5 bp base deletion at codon 1309.

6.A new method for evaluating operation quality of air conditioning systems based on the data from IOT sys-tems and its application in practice

Objective A correlation method for evaluating operation quality of air conditioning systems is proposed , based on the outdoor and the indoor temperature history data of the terminal user from internet of things for the central air conditioning system , for lacking efficient way to evaluate . Methods A methematical model is set up based on the variance ratio of the data of the indoor and out temperture obtained from IOT within a given time interval , and theoretically induce the possibility and correctness to employ these index to evaluate the operation quality of the central air conditioning system . Results A typical central air conditioning system of a large sacal hospital is analyzed , the results shows the variance ratio of the indoor and outdoor temperature can reflect the capacity of the central air conditioning system for controlling the indoor temperature . Conclusion The method proposed for evaluating the op-eration quality of the air conditioning system based on IOT system is theotically correct and practically efficient , and can be employed to evaluate the operation quality of the air conditioning system of large scale medical buildings .
7.Relationship between blood circulation miRˉ21 expression and clinicopathological features of gastric cancer before and after gastric surgery

Objective To investigate effect of peripheral blood miR-21 expression on the clinicopathological features of gastric cancer before and after gastric surgery.Methods The peripheral blood was collected on the operative day and postoperative 7 d in 42 patients with primary gastric cancer surgery.The real-tine quantitative PCR was adopted to detect the relative expression of miR-21 in plasma with U6 as an internal reference gene.Results The plasma miR-21 expression level after gastric cancer operation was significantly down-regulated compared with before operation(P <0.01),the overall relative expression level was dropped by an average of 18.2 times;and it was found that the postoperative miR-21 expression in the patients with the tumor family history was up-regulated,the postoperative plasma miR-21 expression in the patients without the tumor family history was decreased by 22 times compared with before operation(P <0.05);the experimental results also fund that as the patient′s differentiation degree was changed from high to low,the lymph node metastasis rate was gradually increased,while postoperative average miR-21 expression was changed from low to high,showing the relative expression times of miR-21 were distributed in the four differentiated zones of high,middle,middle-low and low,which showed the correlation with the distribution of the lymph node metastasis rate in these four differentiation zones.Conclusion The peripheral blood MiR-21 detection has the clinical significance to early auxiliary diagnosis and the judgement of postoperative malignant degree.
8.Combined corpectomy in the treatment of multilevel cervical spondylotic myelopathy and ossified posterior longitudinal ligament

Objective To evaluate effect of combined corpeetomy for multilevel cervical spondylotic myelopathy (CSM) and ossified posterior longitudinal ligament (OPLL).Methods Fifteen patients with CSM or OPLL,including 9 males and 6 females,were treated with combined corpectomy which is characterized by C4 and C6 corpectomy,excision of osteophyma,protruded disc and/or ossified posterior longitudinal ligament on basis of preservation of C5 vertebral body,structural bone grafting in C3-5 and C5-7,and anterior cervical plate fixation at C3,C5,and C7.The clinical results were evaluated with Japanese Orthopaedic Association (JOA) score.X-rays and CT scans were taken to evaluate vertebral fusion,and MRI was used to access spinal canal decompression and condition of spinal cord.Results All patients were followed up for 9 to 42 months (average,26.7 months).Bony fusion was achieved in all 15 patients.The JOA score improved from preoperative 13.44±2.81 to postoperative 16.16±2.19 (P=0.0354).The cervical lordosis improved from preoperative 1.16°±11.74° to immediately postoperative 14.36°±7.85° (P=0.00217),and 12.92°+6.17° at the final follow-up (P=0.00292).The complications included temporary hoarseness in 2 cases,dysphagia in 1 case.Conclusion The combined corpectomy for treating CSM and OPLL can obtain reliable and satisfactory results.In operation,the preservation of C5 vertebral body can provide an additional screw anchoring force and strengthen stahility.
9.Exploration of the relationship between the expression level of Th1/Th2 cytokines and the familial aggregation of the hepatocellular carcinoma

Objective To investigate the effect of Th1/Th2 cytokines and immune state on the occurrence and familial aggregation of hepatocellular carcinoma(HCC).Methods Ninety-five members whose families have had two or even more HCC patients(high-occurrence families) were selected as the case group,by matching with the same nationality,gender,residential area,age±5 years old,95 members whose families had no any cancer were selected as the control.The level of peripheral blood Th1 type cytokines such as interferon-γ(IFN-γ),interleukin-2(IL-2) and Th2 type cytokines such as interleukin-4(IL-4),interleukin-10 (IL-10) were detected by enzyme linked immunosorbent assay(ELISA).Results There was a Th1/Th2 serum cytokine imbalance profile in members of HCC high-occurrence family.The levels of IFN-γ and IL-2 were significantly lower in members of HCC high-occurrence family than that of the controls.The levels of IL-4 and IL-10 were higher in members of HCC high-occurrence family than that of the controls.Conclusion There was a poor cellular immune state in members of HCC in the high-occurrence families.Th1 type cytokines was inhibited,and Th2 type cytokines was enhanced,so more susceptible to HBV chronic infection.It might be the mechanism of HCC occurrence and familial aggregation.
10.Treatment and classification of thoracic fracture accompanied with sternum fracture.

OBJECTIVE:
To determine the characteristics, classification, and treatment of thoracic fracture accompanied with sternum fracture.
METHODS:
Data of 32 patients with thoracic fractures accompanied with sternum fracture were reviewed. Patients information such as age, gender, cause of injury, site of sternum fracture, level and type of thoracic vertebral fracture, spinal cord injury and associated injuries was included in the analysis. Of the 32 patients, 13 had compressed fractures, 13 had fracture-dislocations, 5 had burst fracture and 1 had burst-dislocation. Six patients had a complete lesion of the spinal cord, 13 sustained a neurologically incomplete injury, and the other 13 were neurologically intact. Ten patients were treated nonoperatively and the other 22 surgically.
RESULTS:
All patients were followed up for 10-103 months. Road traffic accidents and falling dominated among the causes. All patients were accompanied with other injuries. None of the 6 patients with a complete paralitic lesion regained any significant function. Of the 13 neurologically intact patients, 5 had local pain although 12 of them remained normal function. One patient showed tardive paralysis. Three of the 13 patients with incomplete paraplegia returned to normal, 5 regained some function and 5 did not recover.
CONCLUSION
Thoracic fractures accompanied with sternum fracture are marked by violent force, severe fracture of the spine, severe injuries of the spinal cord, and high incidence of other injuries. The new classification method is more suitable to thoracic fractures accompanied with sternum fracture,and confirms the existence and clinical relevance of the 4th column of the thoracic spine and its role in providing spinal stability in patients with thoracic fracture.
