1.Multidimensional Analysis of Mechanisms of Nuciferine Against Cerebral Ischemia Based on Transcriptomic Data

ObjectiveStudies have shown that nuciferine has anti-cerebral ischemia effect, but the specific mechanism of action has not been elaborated. Based on the transcriptome results, the pharmacological mechanism of nuciferine against cerebral ischemia was analyzed from multiple dimensions including tissue, cell, pathological process, biological process and signaling pathway. MethodsThirty SD rats were randomly divided into the sham group, model group and nuciferine group(40 mg·kg-1) according to weight. Except for the sham group, the model of middle cerebral artery occlusion(MCAO) was established by thread embolization method after 30 min of administration in the other two groups. Twenty-four hours after surgery, transcriptome sequencing was used to detect the gene expression profiles in the cortex penumbra of rat cerebral tissue, and gene ontology(GO) and kyoto encyclopedia of genes and genomes(KEGG) pathway enrichment analysis were performed for differentially expressed genes. The mechanismof nuciferine against cerebral ischemia was analyzed from 5 dimensions of tissue, cell, pathological process, biological process and signaling pathway by the transcriptome-based multi-scale network pharmacology platform(TMNP). ResultsTranscriptome sequencing and gene quantitative analysis showed that 667 genes were significantly reversed by nuciferine. Further enrichment analysis of KEGG and GO suggested that the pathways of nuciferine involved regulating stress response, ion transport, cell proliferation and differentiation, and synaptic function. TMNP research found that at the tissue level, nuciferine could significantly improve the cerebral tissue injury caused by ischemia. At the cellular and pathological levels, nuciferine could play an anti-cerebral ischemia role by improving the state of various nerve cells, mobilizing immune cells, regulating inflammation. And at the level of biological processes and signaling pathways, nuciferine mainly acted on the processes such as vascular remodeling, inflammation-related signaling pathways, and synaptic signaling. ConclusionCombined with the results of transcriptome sequencing, gene quantitative analysis and TMNP, the mechanism of nuciferine against cerebral ischemia may be related to processes such as intervening in stress response and inflammation, affecting vascular remodeling and regulating synaptic function. These results can provide a basis and reference for further study of the pharmacological mechanism of nuciferine against cerebral ischemia.

Objective:To elucidate the primary elements pertaining to the perception of relapse risk and to in-vestigate the characteristics and alterations of perception of the risk of recurrence in patients with schizophrenia.Methods:By using the method of phenomenological study,semi-structured in-depth interviews were conducted among schizophrenic patients with first onset,first recurrence and multiple recurrences.Twenty-three 23 outpatient and inpatient cases that met the criteria of the Diagnostic and Statistical Manual of Mental Disorders,Fifth Edition(DSM-5)were included,and all patients had PANSS scores below 60.The interview data were collated and analyzed according to Colaizzi analysis.Results:The perception of recurrence risk in patients with schizophrenia en-compassed 4 major themes and 11 sub-themes,namely susceptibility(limited overall understanding,transition from optimism to objectivity),detection of early warning symptoms(dynamic escalation,assistance from family mem-bers in identification),understanding risk factors(lack of understanding,the relationship between psychosocial fac-tors and relapse,dynamic understanding of medication-relapse relationship),and apprehension of the consequences of recurrence(care burden and financial constraints,impaired social functioning and maladjustment,impact on pub-lic safety,personal health risks).Conclusion:The content of risk perception of recurrence in patients with schizo-phrenia is relatively substantial.The overall level of risk perception of recurrence in patients with schizophrenia is low.

Artemsia argyi Levl.et Vant is a commonly used Chinese herbal medicine and moxibustion raw material plant,so far has more than two thousand years of medicinal history,as one of the most commonly used Chinese medicinal materials.The incopat patent database was used to search the worldwide patent data of the last 20 years,and a total of 25279 argyi related patents were retrieved.The pattern of argyi patents was analyzed from the perspectives of global application trend,main technical fields,national economy composition,applicant ranking,patent value and other aspects by means of graph combination.The analysis shows that the innovation and development of argyi is in the stage of rapid development;The medical,Chinese patent medicine,cosmetics and physiotherapy of argyi are the hot research and development of current technology;There are a large number of patents related to argyi in the world,but they are mainly distributed in China and South Korea.Among them,the number of patents related to argyi in China reaches 20381,far higher than that in other countries,but the number of high-value patents is not very large,and the value and quality of patents are still insufficient compared with other countries.From the perspective of the current development trend of argyi,with the deepening of clinical application recognition and scientific research of argyi,there is a large patent space in the field of argyi.Patent applicants can formulate corresponding patent application strategies according to the global development opportunities,technological development status and existing weaknesses.
5.Effect of Suanzaoren Tang Combined with Ziwu Liuzhu Acupuncture on Vertebral Artery Hemodynamics, Inflammatory Cytokines, and Neurotrophic Factors in Patients with Cervical Insomnia

ObjectiveTo explore the effect of Suanzaoren Tang combined with Ziwu Liuzhu acupuncture on the vertebral artery hemodynamics, inflammatory cytokines, and neurotrophic factors in the patients with cervical insomnia with syndrome of deficiency of both heart and spleen. MethodThe random number table method was employed to assign 164 patients with cervical insomnia with syndrome of deficiency of both heart and spleen treated in the First Clinical Medical School of Guangzhou University of Chinese Medicine from January 2018 to June 2021 into a control group and an observation group. The control group was orally administrated with 1-2 mg estazolam tablets before bed for 4 weeks, and the observation group with Suanzaoren Tang combined with Ziwu Liuzhu acupuncture for 4 weeks. The therapeutic efficacy and safety were observed. The Pittsburgh Sleep Quality Index (PSQI) score, polysomnography monitoring results, hemodynamics parameters of vertebral artery, and serum levels of inflammatory cytokines and neurotrophic factors were compared before and after treatment. ResultExcept 4 dropouts, the remaining 160 patients were included in this study, with 80 patients in each group. The observation group had higher total effective rate than the control group [92.50% (74/80) vs. 80.00% (64/80), χ2=5.270, P<0.05]. Compared with that before treatment, the therapies in both groups decreased the PSQI score, sleep latency time, awakening time, awakening times, serum levels of interleukin-1β (IL-1β), C-reactive protein (CRP), and tumor necrosis factor-α (TNF-α) (P<0.01). Meanwhile, they increased the proportion of rapid-eye-movement (REM) sleep, the diastolic blood flow velocity (Vd), systolic blood flow velocity (Vs), and mean blood flow velocity (MFV) of vertebral artery, as well as the serum levels of brain-derived neurotrophic factor (BDNF) and glial cell-derived neurotrophic factor (GDNF) (P<0.05, P<0.01). Moreover, the observation group had lower PSQI score, sleep latency time, awakening time, awakening times, and serum IL-1β, CRP, and TNF-α levels (P<0.01) and higher proportion of REM sleep, Vd, Vs, MFV of vertebral artery, and serum BDNF and GDNF levels (P<0.05, P<0.01) than the control group. ConclusionZiwu Liuzhu acupuncture combined with Suanzaoren Tang can improve blood circulation of vertebral artery, reduce the serum levels of inflammatory cytokine, and increase the serum levels of neurotrophic factors to improve the sleep quality of the patients with cervical insomnia with syndrome of deficiency of both heart and spleen.
6.Clinical observation of the effects of Xihuang pills combined with sodium cantharidinate and vitamin B6 injection in the treatment of tumor-induced fever

Objective To study the effects of Xihuang pills combined with sodium cantharidinate and vitamin B6 injection in the treatment of tumor-induced fever. Methods A total of 100 patients with tumor-induced fever in our hospital from March 2015 to January 2017 were enrolled in this study. The subjects were divided into the control group (n=50) and the treatment group (n=50) randomly. The control group were treated with aspirin-D L-Lysine injection, and the treatment group were treated with Xihuang pills combined with sodium cantharidinate and vitamin B6 injection. The two groups were treated for 2 periods. The clinical effects of the two groups after treatment were compared. The life quality of the two groups after treatment were compared. The serum TNF-α and IL-1β of the two groups before and after treatment were compared. The adverse reaction rates of the two groups during treatment were compared. Results The total efficacy rate of defervescence of the treatment group was 88.0%(44/50) significantly higher than 56.0%(28/50) of the control group (χ2=12.698, P=0.000). After treatment, the daily life score (1.10 ± 0.18 vs. 2.47 ± 0.21, t=35.025), social communication score (1.21 ± 0.13 vs. 2.53 ± 0.25, t=33.124), mental state score (1.08 ± 0.15 vs. 2.75 ± 0.21,t=45.758), appetite score (1.13 ± 0.16 vs. 2.56 ± 0.19, t=40.708), sleep score (1.22 ± 0.17 vs. 2.71 ± 0.20, t=40.139) of the treatment group were significantly lower than those of the control group (P<0.05). After treatment, the serum TNF-α (1.98 ± 0.07 μg/L vs. 2.86 ± 0.13 μg/L, t=42.144), IL-1β (9.20 ± 1.89 μg/L vs. 13.51 ± 2.36 μg/L, t=10.080) of the treatment group were significantly lower than those of the control group (P<0.05). There was no significantly differences of the adverse reaction rates of the two groups during treatment (χ2=0.211, P=0.646). Conclusions The Xihuang pills combined with sodium cantharidinate and vitamin B6 injection for patients with tumor-induced fever has a good efficacy and low incidence of adverse reactions, can improve the quality of life and reduce the serum levels of TNF-α and IL-1β.
7.The effect of active vitamin D on monocyte chemoattratant protein-1 expression from patients with system lupus erythematosus

Objective In order to explore the effect of 25-(OH)D3 on monocyte chemoattratant protein (MCP)-1 expression from patients with system lupus erythematosus (SLE),we detected the level of active vitamin D and the expression of MCP-1 mRNA in patients with SLE,and analyzed the correlation between them.Methods The level of serum 25-(OH)D3 and mRNA expression of MCP-1 in 154 SLE patients and 31 healthy individuals were detected by enzyme-linked immuno sorbent assay (ELISA) and real time quanti-tative pol ymerase chain reaction (PCR) respectively.We also analyzed the correlation between serum 25-(OH)D3 level and the expression of MCP-1 mRNA,then analyzed the function of 25(OH)D3 on the regula-tion of MCP-1 mRNA expression in vitro.The differences between the two groups were tested by t-test and x2 test,multiple data were tested by one-way ANOVA and the correlation was analyzed by Pearson's correlation,all data were analyzed by statistical product and service solutions (SPSS) 17.0 software.Results The serum 25(OH)D3 levels in SLE group (20±11) ng/ml was significantly lower than normal control group (29±11) ng/ml (t=4.198,P<0.01),and the ratio of the serum levels of vitamin D deficiency in SLE group were significantly higher than that of normal control group [55.8%(86/154) vs 22.6%(7/31),x2=11.421,P=0.001].The expression level of MCP-1 mRNA in PBMCs from the normal control group was significantly lower than the SLE group (1.14±0.27 vs 1.44± 0.31,t=3.277,P=0.001),serum 25(OH)D3 level and MCP-1 mRNA expression in patients with SLE PBMCs were significantly negatively correlated (r=-0.289,P<0.01).Further study found that 25-(OH)D3 inhibited MCP-1 mRNA expression in PBMCs from SLE patients depending on the concentration.Conclusion The decreased 25-(OH)D3 level and up-regulated MCP-1 mRNA expression suggestthat MCP-1 may play an important role in SLE pathological process.
8.Effects of age and gender on anti-inflammatory, anti-oxidative and anti-apoptosis functions of high-density lipoprotein

Objective To investigate the effects of age and gender on anti-oxidative,antiinflammatory and anti-apoptosis functions of high-density lipoprotein (HDL).Methods Totally 120 healthy subjects aged from 20 to 60 years were randomly divided into young and middle-aged male (n=60) group and female (n =60) group,and the 120 healthy elderly aged from 60 to 78 years divided into elderly male (n =60) and elderly female (n =60) groups.Serum levels of high-and low-density lipoprotein cholesterol (HDL-C,LDL-C),total cholesterol (TC),and triacylglycerol (TG) were detected.Content of malonaldehyde (MDA) was detected to determine anti-oxidative function of HDL.Adhesion assay of endothelial cells and monocytes (THP1) was adopted to test the protective effects of HDL on endothelial cells.The expressions of endothelial cell adhesion molecules,VCAM-1 and ICAM-1,were analyzed by Western blot.MTT and flow cytometry assays were used to detect the viability and apoptosis of the cells to test anti-apoptosis function of HDL.Results The levels of low-density lipoprotein,triglycerides and total cholesterol were higher in elder female group than in other three groups (all P<0.05).The level of HDL-C was higher in young and middle-aged females than in other three groups(all P<0.05).The level of MDA was higher in elder female group than in other three groups(all P<0.05).The level of MDA was higher in elder male group than in the young and middle-aged male and female groups(all P<0.05).After protection of HDL,the number of monocytes adhesion and expression levels of VCAM-1 and ICAM-1 were higher in elder groups than in young and middle-aged male and female groups(all P< 0.05).Relative survival and viability rates of endothelial cells were higher in young and middle-aged groups than in elder groups (all P<0.05).Conclusions Ageing in both male and female induces impairments of anti-oxidative,anti-inflammatory and anti-apoptosis functions of HDL,with more evident decrease in anti-oxidative function in females.
9.Role of scaffolding protein JLP on the progression of renal interstitial fibrosis in mice model of unilateral ureteral obstruction and its underlying mechanism

Objective To observe the effect of JLP deficiency on the progression of renal interstitial fibrosis in mice model of unilateral ureteral obstruction (UUO),and to investigate the role and underlying mechanism of JLP in the development of renal fibrosis in obstructive nephropathy.Methods jlp Wild type (jlp+/+) and jlp deficient (jlp-/-) mice were divided into four groups:jlp+/+-and jlp-/--sham-operated groups(jlp-/--sham group and jlp+/+-sham group),jlp+/+-and jlp-/--unilateral ureteral obstruction (UUO)-operated groups (jlp/--UUO group and jlp+/+-UUO group).Mice were sacrificed at 7 days and 14 days after the operation respectively to evaluate the fibrosis by Masson staining.The expression of JLP in jlp +/+ renal tissue was assayed by immunohistochemistry staining,immunofluorescence and Western blotting.Immunohistochemical staining was used to detect the expression of α-smooth muscle actin (α-SMA),collagen Ⅰ (COL-Ⅰ),collagen Ⅲ (COL-Ⅲ) and transforming growth factor-β1 (TGF-β1) in sham and UUO groups.Besides,the α-SMA,COL-Ⅰ,COL-Ⅲ,TGF-β1,p-Smad2 and p-Smad3 protein levels were also analyzed by Western blotting in four groups.Results The expression of JLP was mainly demonstrated in the renal tubules of mice.A large amount of collagen deposition was observed in the renal interstitial area in jlp-/--UUO group compared to jlp+/+-UUO group.Similarly,the expression of α-SMA,COL-Ⅰ,COL-Ⅲ and TGF-β1 was significantly increased in the kidney cortices in jlp-/--UUO-operated groups.Meanwhile,Western blotting showed that the expression of α-SMA,COL-Ⅰ,COL-Ⅲ,and TGF-β1 protein was obviously higher in jlp-/--UUO group.Moreover,the expression of p-Smad2 and p-Smad3 protein was markedly higher in jlp-/--UUO group.Conclusion Scaffolding protein JLP is critical in preventing renal fibrosis through the inhibition of TGF-β1 expression and myo-fibroblast production.
10.Relationship between Injury Sites, Pathological Types and Attention in Post Stroke Cognitive Impairment

Objective To study the relationship between injury site (right or left) or pathological type (hemorrhage or infarct) and atten-tion impairment in post stroke cognitive impairment (PSCI). Methods June, 2014 to June 2015, 49 patients with PSCI were assessed with Test of Attentional Performance (TAP) within 1 week of admission. The reaction time and the numbers of correct response were recorded. Results The reaction time of alertness (with or without alarm) was shorter in the patients with left infarction (Z=-2.32, t=-3.76, P<0.05), and the correct number of auditory was more (Z=2.42, P<0.05) than those with right infarction. The reaction time of incompatibility test and Go/Nogo test was shorter in patients with left hemorrhage than those with right hemorrhage (Z>2.32, t=-3.10, P<0.05). Conclusion A trend of lateralization was found in patients with PSCI, rather than in the pathology.
