Objective:To investigate the immunophenotypic and molecular biological characteristics of patients with elevated serum alpha-fetoprotein (AFP) and enteroblastic differentiated gastric adenocarcinoma (GAED).Methods:The clinicopathological data of 13 patients with elevated serum AFP and GAED admitted to Shanxi Cancer Hospital from 2018 to 2020 were collected. Immunohistochemistry (IHC) and next-generation sequencing (NGS) were used to analyze the immune markers and molecular biological characteristics of the pathological tissues of the patients. Kaplan-Meier method and log rank test were used for survival analysis. Results:Among the 13 patients with GAED, 12 were male and 1 was female, aged 41-70 years, with a median age of 64 years. The lesions were mainly located in the gastric antrum (5 cases) and gastric body (4 cases). IHC results showed that the tumor embryonic protein (AFP, SALL4, GPC3), intestinal epithelial differentiation protein (CDX-2, CD10), and some original intestinal epithelial phenotype markers (OCT3/4, Claudin6) were expressed in the tumor tissues. Combined application of multiple markers can reduce the rate of missed diagnosis. Among the 13 patients, 12 had at least one mutation (1 mutation: 1 case, 2-5 mutations: 3 cases, 6-15 mutations: 8 cases), and 1 case was not detected. The gene with the highest mutation frequency was TP53 (10 cases), and other mutant genes included EPHB1 (3 cases), ATRX (2 cases), EPHA5 (2 cases), GATA3 (2 cases), LRP1B (2 cases) and MAP2K4 (2 cases) were also detected. Three of the 13 patients had structural variations, which were C14orf177- GNAS, AIM1- FGFR3, and EPHA6- ROS1 gene rearrangements. All 13 patients had copy number variation, and 11 patients had copy number variation of more than 2 genes. The common amplification genes were IRS2 (5 cases), PTEN (5 cases), GNAS (4 cases), CCNE1 (3 cases), CEBPA (3 cases), PCK1 (3 cases) and ERBB2 (2 cases). The common deletion genes were SOX2 (5 cases) and MYC (5 cases). Among the 13 patients, 4 died, and 2 of the dead patients had liver metastasis. There were 4 patients with disease-free survival and 5 patients with disease progression, including 3 cases of abdominal metastasis and 2 cases of liver metastasis. The 3-year survival rate of patients was 65.9 %, and the 3-year progression-free survival rate was 30.7 %. Gene LRP1B point mutation was associated with poor prognosis ( P<0.001). There was no significant improvement in the prognosis of patients treated with immunotherapy compared with those treated with chemotherapy alone ( P=0.595), but the prognosis of patients treated with postoperative chemotherapy or postoperative chemotherapy plus immunotherapy was better than that of patients treated with surgery alone ( P<0.05). Conclusions:Elevated serum AFP with GAED is a highly invasive tumor with unique molecular characteristics, often accompanied by multiple molecular events. TP53 mutation is the most common type of gene mutation. In addition, some cases are accompanied by HER2 amplification and gene rearrangement.

【Objective】 To investigate the efficacy of low intensity pulsed ultrasound (LIPUS) in the treatment of erectile dysfunction (ED). 【Methods】 A total of 121 ED patients treated during June 2020 and June 2022 were selected as the research objects. According to the International Erectile Index Score (IIEF-EF), the patients were divided into three subgroups:mild (17-25 points), moderate (11-16 points), and severe (0-10 points). The total effective rate, erectile hardness scale (EHS), sex life log questions (SEP), general assessment questionnaire (GAQ), peak systolic velocity (PSV), end diastolic velocity (EDV), and adverse reactions of the three groups before treatment, 4 weeks and 12 weeks after treatment were analyzed. 【Results】 A total of 119 patients completed the follow-up. There were significant increases in IIEF-EF and EHS at week 4 and 12 (P<0.05), and the total effective rate was 69.75% and 76.47%, respectively. The total effective rate was significantly higher in the mild and moderate groups than in the severe group at week 4 and 12 (P<0.05). The patients who answered "yes" to SEP2 and SEP3 accounted for 91.60% and 71.43%, respectively at week 4, and 92.44% and 78.15% at week 12, both significantly higher than the rates before treatment (52.10% and 27.73%, P<0.05). The proportion of patients who answered "yes" to GAQl and GAQ2 at week 4 were 84.87% and 71.43%, respectively, and were 82.35% and 70.59% respectively at week 12, with no significant difference. The PSV level significantly increased at week 12 compared to that before treatment [(48.85±14.11) cm/s vs. (41.42±14.90) cm/s] (P<0.05), while the EDV level significantly decreased [(-0.57±7.01) cm/s vs. (2.25± 5.68)cm/s] (P<0.05). 【Conclusion】 LIPUS can improve erectile function in ED patients without obvious adverse reactions.

The fall armyworm (FAW), Spodoptera frugiperda, is a destructive pest native to America and has recently become an invasive insect pest in China. Because of its rapid spread and great risks in China, understanding of FAW genetic background and pesticide resistance is urgent and essential to develop effective management strategies. Here, we assembled a chromosome-level genome of a male FAW (SFynMstLFR) and compared re-sequencing results of the populations from America, Africa, and China. Strain identification of 163 individuals collected from America, Africa and China showed that both C and R strains were found in the American populations, while only C strain was found in the Chinese and African populations. Moreover, population genomics analysis showed that populations from Africa and China have close relationship with significantly genetic differentiation from American populations. Taken together, FAWs invaded into China were most likely originated from Africa. Comparative genomics analysis displayed that the cytochrome p450 gene family is extremely expanded to 425 members in FAW, of which 283 genes are specific to FAW. Treatments of Chinese populations with twenty-three pesticides showed the variant patterns of transcriptome profiles, and several detoxification genes such as AOX, UGT and GST specially responded to the pesticides. These findings will be useful in developing effective strategies for management of FAW in China and other invaded areas.

Objective To evaluate the clinical effect of minimally invasive treatment of clavicle fracture with a hollow screw. Methods Data of 65 patients, who had undergone minimally invasive treatment with a hollow screw for clavicle fracture from April 2009 to October 2010,were retrospectively analyzed. There were 41 males and 24 females, aged from 19 to 67 years (average,35.8 years).According to the Craig's classification, there were 29 of group I and 36 of group Ⅱ-Ⅱ.A 4-5 cm transverse incision was made to expose the clavicle fracture.A guide pin was inserted into the marrow cavity, and then moved towards the acromion, after the fracture was reduced the pin was moved back to the proximal clavicle, finally a hollow screw was implanted to fix the fracture. The Neer score was used to evaluate the function of the shoulder.Radiographs were taken to observe the condition of the fracture union. The operative duration,intra-operative blood loss, fracture healing time,Neer score, and complications were compared with those of 65 patients with clavicle fracture who were treated by the same surgeons with plates. Results The incision length was 4-5 cm in hollow screw group and 10-11 cm in plate group.X-rays showed bone union was achieved in both groups, and the average bone healing time was 13.2±6.9 weeks in hollow screw group and 16.3±8.7 weeks in plate group. All patients were followed up for 6 to 20 months (average,10.6 months). The average Neer score was 96.6±3.4 in hollow screw group and 94.2±5.8 in plate group. There was no infection, local skin necrosis and fracture nonunion in both groups. In hollow screw group, screw loosing occurred in 5,and fracture displacement in 3,fortunately,the fracture healed by controlling activities. There was a significant difference in fracture healing time between two groups. However,no significant difference was observed between two groups in Neer score. Conclusion Minimally invasive treatment of clavicle fracture with a hollow screw has several advantages, such as mini-invasion,short bone healing time, good clinical outcomes, and lower expense.
8.Mechanism and expression of KLF6 tumor suppressor gene in related tumors
Journal of International Oncology 2011;38(2):99-102
It has been revealed that the major mechanism of KLF6 is to upregulate p21 in an p53-independent manner leading to inhibition of cell proliferation. Lacking of KLF6 activity or its abnormal expression may be related to multiple tumors'development and prognosis such as prostate cancer, liver cancer, gastric cancer and other tumors. Furthermore, KLF6 may become a novel candidate for molecular target therapy.

Objective To explore the brain's metabolism about the hippocampus, anterior cingulate and thalamus of first episode depressive patients. Methods Using proton magnetic resonance spectroscopy and 1 HMRS analyzing technology to detect multiple metabolic indices of the hippocampus, anterior cingulate and thalamus of 20 patients and 20 healthy persons, including N-acetylaspartate ( NAA), choline ( Cho ), myoinositol ( MI ), creatine(Cr). Results The ratios of MI/Cr of the bilateral thalamus of the depressive patients group decreased ( Left (0.23 ±0.13) ,(0.25 ±0.09), t=2.25, P=0.03,Right(0.33 ±0.15,0.34 ±0.04), t=2.42, P=0.02).The ratios of NAA/Cr and Cho/Cr of the bilateral hippocampus of the depressive patients group were both lower (NAA/Cr: Left t=3.06, P=0.004,Right t=3.54, P=0.001. Cho/Cr: Left t=2.94, P=0.006,Right t=4.02,P=0.001 ). The ratios of Cho/Cr and MI/Cr of the anterior cingulate of the depressive patients group were both significantly higher (NAA/Cr: t=3.12, P=0. 003;Cho/Cr: t=2.62, P=0.01). Conclusion The metabolic abnormalities of hippocampus, anterior cingulate and thalamus maybe constitute the nervous biochemical basis of first episode depression.
