1.Invasion and Metastasis in Colorectal Cancer Mediated by Traditional Chinese Medicine via Cell Signaling Pathway: A Review

Colorectal cancer is a common and malignant tumor in the digestive tract. Invasion and metastasis of cancer cells are key factors leading to the high mortality rate and postoperative recurrence of colorectal cancer. Chemotherapy is the main treatment method for preventing recurrence of this disease. However, there are many toxic side effects in clinical application, which seriously hinder the treatment process. Therefore, it is imperative to search for efficient and low-toxicity drugs. Traditional Chinese medicine (TCM) has a long history of treating colorectal cancer and offers advantages such as safety, effectiveness, multiple targets, multiple pathways and minimal toxic side effects, which have made it increasingly popular worldwide. According to TCM, the pathogenesis of colorectal cancer is rooted in both deficiency and excess. TCM formulas mainly focus on tonifying the body to address the invasion and metastasis of colorectal cancer, such as Jianpi compound, Jianpi Xiaoai decoction, and Bushen Jiedu Sanjie decoction. TCM monomers, such as emodin, berberine, and tanshinone, mainly focus on clearing heat and removing toxin, circulating blood and transforming stasis, and resolving swelling and dispersing nodules. Signaling pathways play a crucial role for analyzing invasion and metastasis, and research has shown that pathways such as Wnt/β-catenin, phosphatidylinositol-3 kinase/protein kinase (PI3K/Akt), Janus kinase 2/signal transduction and transcription activating factor 3 (JAK2/STAT3), nuclear factors-κB (NF-κB), vascular endothelial growth factor (VEGF) play important roles in the invasion and metastasis of colorectal cancer. The invasion and metastasis of colorectal cancer can be inhibited via regulating the key proteins and related factors in these pathways. In this review, we searched various literature databases, such as PubMed, China National Knowledge Infrastructure (CNKI), and VIP, using keywords such as "colorectal cancer", "signaling pathway", "invasion and metastasis", and "traditional Chinese medicine", to summarize and analyze the relevant pathways of TCM compounds and monomers against invasion and metastasis of colorectal cancer published in the past five years. The review aims to provide new insights and references for in-depth research on the therapy for invasion and metastasis of colorectal cancer and new drug development.

Diabetes is a very complex endocrine disease whose common feature is the increase in blood glucose concentration. Persistent hyperglycemia can lead to blindness, kidney and heart disease, neurodegeneration, and many other serious complications that have a significant impact on human health and quality of life. The number of people with diabetes is increasing yearly. The global diabetes prevalence in 20-79 year olds in 2021 was estimated to be 10.5% (536.6 million), and it will rise to 12.2% (783.2 million) in 2045. The main modes of intervention for diabetes include medication, dietary management, and exercise conditioning. Medication is the mainstay of treatment. Marketed diabetes drugs such as metformin and insulin, as well as GLP-1 receptor agonists, are effective in controlling blood sugar levels to some extent, but the preventive and therapeutic effects are still unsatisfactory. Peptide drugs have many advantages such as low toxicity, high target specificity, and good biocompatibility, which opens up new avenues for the treatment of diabetes and other diseases. Currently, insulin and its analogs are by far the main life-saving drugs in clinical diabetes treatment, enabling effective control of blood glucose levels, but the risk of hypoglycemia is relatively high and treatment is limited by the route of delivery. New and oral anti-diabetic drugs have always been a market demand and research hotspot. Inhibitor cystine knot (ICK) peptides are a class of multifunctional cyclic peptides. In structure, they contain three conserved disulfide bonds (C3-C20, C7-C22, and C15-C32) form a compact “knot” structure, which can resist degradation of digestive protease. Recent studies have shown that ICK peptides derived from legume, such as PA1b, Aglycin, Vglycin, Iglycin, Dglycin, and aM1, exhibit excellent regulatory activities on glucose and lipid metabolism at the cellular and animal levels. Mechanistically, ICK peptides promote glucose utilization by muscle and liver through activation of IR/AKT signaling pathway, which also improves insulin resistance. They can repair the damaged pancrease through activation of PI3K/AKT/Erk signaling pathway, thus lowering blood glucose. The biostability and hypoglycemic efficacy of the ICK peptides meet the requirements for commercialization of oral drugs, and in theory, they can be developed into natural oral anti-diabetes peptide drugs. In this review, the structural properties, activity and mechanism of ICK pattern peptides in regulating glucose and lipid metabolism were summaried, which provided a reference for the development of new oral peptides for diabetes.
3.Exploring Mechanism of Breast Cancer Prevention by Liuwei Dihuangwan, Erzhiwan and Zuoguiwan Based on Macrophage Phagocytosis

ObjectiveTo investigate the effects of three traditional Chinese medicine formulas for nourishing kidney Yin on the occurrence of breast cancer, including Zuoguiwan(ZGW), Liuwei Dihuangwan(LWDHW) and Erzhiwan(EZW), and to explore their preventive mechanisms from the phagocytic function of macrophages. MethodsTen-month-old post-breeding female mice were randomly divided into the blank group, soy isoflavone group(0.13 g·kg-1·d-1, mixed with feed), ZGW group(2.34 g·kg-1·d-1), LWDHW group(0.56 g·kg-1·d-1), and EZW group(4.68 g·kg-1·d-1). The mice were palpated every 3 d until the age of 18 months, and those with detected lumps were confirmed for tumor development through hematoxylin-eosin(HE) staining, and the incidence of breast cancer in mice was calculated. A total of 40 SPF-grade female SD rats were randomly divided into the blank serum group(physiological saline), ZGW drug-containing serum group(16.20 g·kg-1·d-1of ZGW), LWDHW drug-containing serum group(3.89 g·kg-1·d-1 of LWDHW) and EZW drug-containing serum group(32.40 g·kg-1·d-1 of EZW), each group was orally administered 3 times a day for 5 consecutive days. On the 5th day, blood was collected from the abdominal aorta 1 h after the last gavage to prepare drug-containing serum. The effect of drug-containing serum with different concentrations on the viability of RAW264.7 cells was assessed by cell counting kit-8(CCK-8). Taking 20% of drug-containing serum concentration as the research object, its effect on the expression levels of major histocompatibility complex-Ⅱ(MHC-Ⅱ), CD80 and CD86 on the surface of RAW264.7 cells was detected by immunofluorescence(IF), the phagocytic function of RAW264.7 cells was examined by flow cytometry, and the levels of interleukin-6(IL-6) and nitric oxide(NO) in RAW264.7 cells in the conditioned medium(CM) co culture system of 4T1 cells were detected by enzyme-linked immunosorbent assay(ELISA) and Griess assay. ResultsAfter prophylactic administration, the tumor incidence rates in the soy isoflavone group(4%), ZGW group(4%), LWDHW group(4%), and EZW group(6%) were lower than that in the blank group(8%). Compared with the blank serum group, the drug-containing serum of the three nourishing kidney Yin formulas could enhance the expression levels of MHC-Ⅱ, CD80 and CD86 on the surface of RAW264.7 cells, enhance phagocytic ability towards tumor cells, and reduce the content of IL-6 and increase the level of NO(P<0.05, P<0.01). ConclusionThe three nourishing kidney Yin formulas can reduce the incidence of tumor in mice, the mechanism may be related to activating RAW264.7 cells, increasing their phagocytosis to breast cancer cells, and regulating the secretion of IL-6 and NO.
7.Prognostic Value of MELD 3.0 Based Model for Survival Outcomes in Alcoholic Cirrhosis Patients

ObjectiveTo explore the value of the Model for End-Stage Liver Disease (MELD) 3.0 in predicting survival outcomes for patients with alcoholic cirrhosis and to establish an effective mortality prediction model. MethodsClinical data of 788 hospitalized patients who were first diagnosed with alcoholic cirrhosis at the Third Affiliated Hospital of Sun Yat-sen University between January 1, 2011 and December 31, 2019 were analyzed. Patients were followed up until December 31, 2023 and divided into survival and mortality groups based on the survival outcomes at 30 days, 90 days, 1 year, and 3 years after admission. The prognostic values of the MELD 3.0, MELD, MELD-Sodium (MELD-Na) for survival in alcoholic cirrhosis patients were assessed and compared by using the receiver operating characteristic (ROC) curve and the area under the curve (AUC). Additional risk factors associated with mortality in alcoholic cirrhosis patients were identified, and a novel mortality prediction model based on MELD 3.0 was developed. ResultsThe AUC of the MELD 3.0 score in predicting 30-day, 90-day, 1-year, and 3-year survival was 0.823, 0.730, 0.686, and 0.658, respectively, which were superior to those of the MELD-Na (0.802, 0.708, 0.666, and 0.645, respectively) and MELD scores (0.698, 0.668, 0.654, and 0.633, respectively) (all P < 0.05). MELD 3.0 demonstrated better performance at 30 and 90 days (AVC=0.823,0.730; both P < 0.05) than at 1 year and 3 years (AVC=0.686,0.658; both P < 0.05). Binary logistic regression combined with LASSO regression indicated that the independent risk factors associated with the 1-year outcome included MELD 3.0, baseline ascites and hepatocellular carcinoma. A survival prediction model was then established with AUC of 0.748, sensitivity of 0.695, and specificity of 0.775. ConclusionsMELD 3.0 has a superior predictive ability for 30-day, 90-day, 1-year, and 3-year survival in patients with alcoholic cirrhosis than MELD-Na and MELD. The prediction model incorporating MELD 3.0, ascites and hepatocellular carcinoma improves the prediction of 1-year survival outcomes for alcoholic cirrhosis patients.

Objective To investigate the incidence and the types of gene mutations of α-thalassemia in the child-bearing pop-ulation of Conghua District,Guangzhou.Methods Blood samples from 24 083 people of childbearing age were screened by blood cell analysis and hemoglobin electrophoresis,α-globin gene variation was detected by GAP-PCR and PCR reverse dot blot in the positive cases,and 17 common β-globin gene mutations were detected by PCR reverse Dot blot.Results A total of 2 596 cases of α-thalassemia gene abnormality were detected by gene identification,and the abnormal rate was 10.78%.A sum of 170 cases(0.71%)had a compound mutation of α-β gene.There were 2 550 cases(98.23%)of deletion and 46 cases(1.77%)of non-deletion in the mutant genes.There were 14 types of gene mutation,including 5 types of HbH disease(with--SEA/-α3.7 primarily),4 mild types(with 68.61%of--SEA/αα genotype),and 5 quiescent types(the top two genotypes were-α3.7/αα and-α4.2/αα).A total of 23 types of αβ complex gene mutation were detected,and the top six types were--SEA/βCD41-42,-α3.7/βCD41-42,--SEA/β654,--SEA/-28,-α3.7/β654 and-α3.7/βCD17,which accounted for 75.27%of all the complex types.Conclusion The gene abnormality rate of α-thalassemia in Conghua District of Guangzhou City was high.The gene mutation type and constitu-ent ratio,which have their own characteristics,is a special region of α-thalassemia.
10.Mutual facilitation of Alzheimer disease and sarcopenia:roles of myokines,amyloid proteins,and other factors

Increasing age is the most important factor for cognitive impairment.Alzheimer disease(AD)and sarcopenia are significant causes of frailty and disability in older adults.It is important to have an in-depth understanding of the relationship between sarcopenia and AD.Studies have reported that sarcopenia often disturbs the secretion of muscle factors,which may increase the risk of developing dementia.In turn,the pathological feature of dementia,such as the de-position of amyloid β-protein(Aβ),amyloid precursor protein(APP)and tau protein in peripheral neurons,may be related to a decline in muscle function.In particular,the deposition of Aβ and APP may eventually lead to movement disorders and disability.Therefore,we hypothesize that AD and sarcopenia may mutually promote each other's pathological develop-ment.This results in exacerbation of clinical and pathological damage,in which myokine and amyloid proteins play impor-tant roles.However,the interrelationship based on amyloid protein and myokine production has not been discussed in de-tail in other reviews.In this paper,we reference and discuss the studies on this topic,and review the common risk factors for sarcopenia and AD and the potential and mechanisms for mutual improvement.
