1.Value of different assessment scales in the diagnosis of drug-induced liver injury

Objective To determine the scores of patients with a confirmed diagnosis of drug-induced liver injury(DILI)using Roussel Uclaf Causality Assessment Method(RUCAM),Maria&Victorino assessment scale,and Revised Electronic Causality Assessment Method(RECAM),to compare the accuracy of the three scales in diagnosis,and to investigate their clinical significance in the diagnosis of DILI.Methods A total of 98 patients with a confirmed diagnosis of DILI who were hospitalized in Peking University First Hospital from January 2011 to December 2022 were enrolled,with liver biopsy results supporting DILI and a clear history of medication.Clinical data were collected from all subjects,and the above causality assessment scales were used for scoring.The chi-square test was used to analyze the diagnostic accuracy of the causality assessment scales,and the weighted kappa coefficient was used to analyze the consistency between the three scales.Results For all patients with DILI enrolled,RECAM had the highest accuracy,with a significant difference compared with RUCAM(χ2=5.667,P=0.017).RUCAM and RECAM had moderate consistency in diagnosis(κw=0.469),while RECAM and Maria&Victorino scale had poor consistency(κw=0.156).For the patients with acute DILI,RECAM,RUCAM,and Maria&Victorino scales had a diagnostic inconsistency rate of 3.7%,11.1%,and 42.6%,respectively;for the patients with hepatocellular type DILI,the three scales of a diagnostic inconsistency rate of 8.9%,21.4%,and 62.5%,respectively;for the patients with cholestasis type or mixed type DILI,the three scales of a diagnostic inconsistency rate of 10.0%,22.5%,and 47.5%,respectively.Conclusion The use of RECAM and RUCAM scales in acute DILI can improve diagnostic rate,and for hepatocellular type DILI and DILI with the clinical manifestation of cholestasis(cholestasis type DILI and mixed type DILI),the use of RECAM and RUCAM scales can also improve diagnostic rate.The selection of causality assessment scales with a relatively high accuracy based on the course and clinical classification of the disease may help to further improve clinical diagnostic rate.
2.Effects of thoracic X-ray irradiation on the spermatogenesis of adult male mice

Objective:To investigate the effect of thoracic X-ray irradiation on the spermatogenesis of adult male mice.Methods:A total of 24 healthy adult male C57BL/6 mice (6-8 weeks old) were randomly divided into radiation group (Radiation) and sham-radiation group (Sham), 12 mice in each group. The area of thoracic irradiation was 1.5 cm× 2 cm, and the dose rate was 3.04 Gy/min, 8 Gy/d for 3 consecutive days, 24 Gy in total. At 7 d and 21 d after thoracic irradiation, the bilateral testes and epididymal tails were stripped and the testicular index was calculated. The morphology of testis was examined by haematoxylin-eosin (HE) staining, then the diameter of seminiferous tubules and the thickness of seminiferous epithelium were measured. The sperms were collected from the bilateral epididymal tails for sperm counting. The level of apoptosis in testis and levels of apoptosis-related proteins were detected by TUNEL and Western blot, respectively.Results:Compared with Sham group, the morphology of testis and epididymis was seriously damaged, the diameter of seminiferous tubules significantly decreased at 21 d after irradiation ( t = 8.93, P < 0.05), and the seminiferous epithelium significantly decreased at 7 d and 21 d after irradiation ( t = 4.24, 12.77, P < 0.05). In addition, the number of sperms significantly decreased ( t = 4.30, 2.98, P < 0.05). The number of TUNEL positive cells in the seminiferous epithelium significantly increased at 7 d and 21 d after irradiation ( t = -2.73, -3.74, P < 0.05). Meanwhile, the level of cleaved Caspase-3 protein significantly increased at 7 d and 21 d after irradiation ( t = -2.96, -2.46, P < 0.05). The concentrations of SCF and GDNF did not change at 7 d after irradiation, but were significantly increased at 21 d after irradiation ( t = -10.46, -5.42, P < 0.05). Conclusions:The thoracic X-ray irradiation could lead to spermatogenesis disorder in male adult mice, and the induction of spermatogenic cell apoptosis and the secretory dysfunction of sertoli cells may be involved.
3.Study on the Effects of Oleanolic Acid on the Proliferation ,Invasion and Metastasis of Human Ovarian Cancer SKOV3 Cells and Its Mechanism

OBJECTIVE:To investigate the effects and m echanism of oleanolic acid on inhibiting the proliferation ,invasion and metastasis of human ovarian cancer SKOV 3 cells. METHODS :CCK-8 assay was used to detect the effects of different concentrations of oleanolic acid (10,20,40,60,80,100 μmol/L)on the proliferation of ovarian cancer SKOV 3 cells at 12,24, 36 and 48 h. The effects of low-dose and high-dose of oleanolic acid (20,40 μmol/L)on the metastasis and invasion ability of SKOV3 cells for 24 h were observed in Transwell assay. Western blotting assay was used to detect the effects of low-dose and high-dose of oleanolic acid on the protein expression of NF-κB p65,PRL-3,TNF-α,IL-6 and E-cadherin in SKOV 3 cells. Through LPS induction and NF-κB p65 plasmid transfection ,Western blotting and RT-qPCR assay were used to investigate the effects of low-dose and high-dose oleanolic acid on the expression of NF-κB/PRL-3 pathway related proteins and their mRNA. RESULTS : With the increase of the concentration and action time of oleanolic acid ,the proliferation capacity of ovarian cancer SKOV 3 cells was decreased ,the surval rates of administration groups were significantly lower than that of the control group (P<0.05 or P< 0.01). Low-dose and high-dose of oleanolic acid could significantly reduce the number of migrating and invading cells (P<0.05 or P<0.01). The protein relative expression of NF-κB p65,PRL-3,TNF-α and IL-6 in SKOV 3 cells were significantly decreased , while the protein relative expression of E-cadherin was significantly increased (P<0.05 or P<0.01). After LPS induction ,protein and mRNA relative expression of NF-κB p65,PRL-3,TNF-α and IL-6 were increased significantly in LPS model group ,while protein and mRNA relative expression of E-cadherin were significantly decreased (P<0.05 or P<0.01). The protein and mRNA relative expression of NF-κ B p65,PRL-3,TNF-α and IL-6 were significantly decreased ,and protein and mRNA relative expression of E-cadherin were significantly increased in low-dose and high-dose of oleanolic acid group (P<0.05 or P<0.01). In SKOV3 cells with over-expressed NF-κB p65,low-dose and high-dose of oleanolic acid c ould significantly down-regulat the proteinexpression of NF-κ B p65,PRL-3,TNF-α and IL-6,while upregult the protein relative expression of E-cadherin (P<0.05 E-mail:122821905@qq.com or P<0.01). CONCLUSIONS : Oleanolic acid can inhibit SKOV3 cells proliferation,invasion and metastasis by regulating NF-κB/PRL-3 signaling pathway.
4.Retrospective analysis on clinical consultations participated by clinical blood transfusion physicians in 5 224 cases of blood transfusion

Objective To investigate the effect of blood transfusion physicians for participating in clinical consultation to provide a reference for blood transfusion physicians participating in clinical practice.Methods The clinical data in 5 224 inpatients receiving blood transfusion before the blood transfusion physicians participating in clinical consultation (from January 2010 to December 2015) and after participating in clinical consultation (from January 2013 to December 2015) were retrospectively analyzed.Results The occurrence rates of blood transfusion adverse reactions before and after the blood transfusion physicians participating in clinical consultation were 2.39 % and 1.48 % respectively (x2 =5.674,P=0.021),the blood transfusion ratios of inpatients were 4.85 % and 3.55 % respectively (x2 =135.5,P<0.01),the average allogeneic blood transfusion volumes were 0.563 U and 0.420 U (t=3.986,P=0.016),and the constituent ratios of mild,moderate andsevere anemia were 5.40% and 0.23 %,29.60% and 17.90%,and 65.00% and 81.80% respectively (x2 =237.6,P<0.01).Conclusion Participating in clinical consultation by the blood transfusion physicians is conducive to clinical treatment.
5.The experimental study of the effect of Thyroid hormone on the bone metabolism in New Zealand rabbits

Objective To explore the effect of thyroid hormone on bone metabolism.Methods The thyroid in the rabbits had been cut off and the physics and histology of the bone in the rabbits were observed.The therapeutic effect of exterofection thyroid hormone was also observed.Results The lacking or reducing of thyroid hormone could lead to osteoporosis.The substitutive treatment of T_4 could partially inhibit the oesteopenia.Conclusion When the T_3 level equals to the level of T_4,the oesteopenia of the rabbits with thyroid being cut off may be caused by the lack of calcitonin.
