1.Clinical diagnosis and treatment of acute bilateral medial medullary infarction:a case report and literature review

		                        			Bilateral medial medullary infarction(MMI)was a rare subtype of stroke.The clinical manifestations of bilateral MMI were complicated,which leaded to misdiagnosis.Patients with bilateral MMI could progress rapidly,and were easy to progress to respiratory failure and even death.Patients with this type of cerebral infarction did not respond well to medication.One case data of a patient with bilateral MMI was reviewed and analyzed.The weakness on the right side of the body was presented,accompanied by slurred speech,and was finally diagnosed with atherosclerosis of the large arteries through cranial MR examination.The patient had occlusion of the left posterior inferior cerebellar artery distal to the left vertebral artery V4 segment on the left side.The patient was treated with stent thrombectomy and balloon angioplasty,and the follow-up phone call at 3 months after discharge was showed good prognosis.Through a review of the literature,combined with the clinical and imaging features of this case,a preliminary analysis suggested that atherosclerosis of the large arteries may be the most common cause of bilateral MMI.Cranial MR imaging is an important method for diagnosing this condition.For patients with bilateral MMI accompanied by occlusion of the vertebral artery V4 segment,endovascular intervention may be attempted.
		                        		
		                        		
		                        		
		                        	
2.Relationship Between Pathogenesis of "Kidney Deficiency and Blood Stasis in Collateral, Miniature Mass of Renal Collateral" and Autophagy Dysfunction in Renal Interstitial Fibrosis

		                        			Renal interstitial fibrosis (RIF) is the main pathological feature of chronic kidney disease caused by a variety of factors. "Kidney deficiency and blood stasis in collateral, miniature mass of renal collateral" is the main pathogenesis of RIF. The deficiency of healthy Qi will influence the kidney Qi, resulting in kidney deficiency and unsmooth qi transformation. As a result, phlegm, heat, stasis, toxin and other excess pathogens block the kidney collaterals, forming miniature masses. The masses accumulate in the renal collaterals, finally leading to RIF. Autophagy is a key process that keeps your body's cells in proper balance by taking aged or damaged components in a cell and recycling them. It is involved in the occurrence and development of RIF. The metabolism of excess pathogens such as phlegm, heat, stasis, and toxin in vivo is related to the degradation and reabsorption of autophagy. Autophagy is a way to eliminate phlegm, heat, stasis, toxin and other excess pathogens. Autophagy dysfunction will cause the accumulation of phlegm, heat, blood stasis, toxin and other excess pathogens, further the stasis of the kidney collaterals, miniature mass in kidney, and finally RIF. Kidney deficiency and blood stasis in collateral are the root cause of autophagy dysfunction, and the miniature mass of renal collateral is the manifestation of autophagy dysfunction. Autophagy dysfunction and miniature mass of renal collateral have the same pathological evolution. In this paper, based on the pathogenesis of "kidney deficiency and blood stasis in collateral, miniature mass of renal collateral" of RIF and RIF-autophagy relationship, this paper discusses the "kidney deficiency and blood stasis in collateral-autophagy dysfunction-miniature mass of renal collateral" relationship in RIF and comprehensively interprets the scientific connotation of the pathogenesis of "kidney deficiency and blood stasis in collateral, miniature mass of renal collateral", which is expected to lay a basis for explaining the role of autophagy in TCM theory and for the treatment of RIF and research on the mechanism. 
		                        		
		                        		
		                        		
		                        	
3.Role of Autophagy in Renal Interstitial Fibrosis and Intervention of Chinese Medicine: A Review

		                        			Renal interstitial fibrosis(RIF)is a common pathway for the progression of chronic kidney disease to renal failure,and its pathogenesis is mainly related to renal inflammatory damage,oxidative stress,apoptosis,and excessive extracellular matrix(ECM) deposition. Transforming growth factor-β1(TGF-β1) signaling pathway,mammalian target of rapamycin(mTOR) signaling pathway and other signaling pathways mediate the occurrence and development of RIF. Because of the complicated mechanism of RIF,there have been no specific prevention and treatment measures in clinical practice. Autophagy is a non-damaging response produced by eukaryotic cells. It maintains the balance of tissue homeostasis through degradation and reabsorption. At present, Chinese medicine has achieved desirable clinical effects with its unique advantages of multiple components,multiple effects,and multiple targets in the treatment of chronic kidney disease to delay the process of RIF. Scholars have found that autophagy is consistent with the Yin-Yang theory and the theory of abdominal mass in traditional Chinese medicine (TCM) to a certain extent,and it is involved in many aspects of RIF. The progression of RIF is closely related to autophagy. The targeted therapy of RIF by intervention in autophagy has become the frontier of research. However,little is known about the role of autophagy in RIF and the regulation of autophagy by Chinese medicine in the treatment of RIF. Therefore,it is necessary to further elucidate the relationship between autophagy and RIF in order to clarify the mechanism of autophagy in RIF and the mechanism of Chinese medicine regulating autophagy in targeted therapy of RIF. This article focused on the correlation between autophagy and RIF based on TCM theory,and systematically summarized the role of autophagy in RIF and the intervention of Chinese medicine by combining the effects of autophagy on inflammation damage,oxidative stress,apoptosis,and excessive ECM deposition in RIF, and the regulation mechanism of autophagy in TGF-β1 and mTOR signaling pathways in RIF. 4.Interaction Between Intestinal Flora and Chronic Renal Failure and Traditional Chinese Medicine Intervention: A Review

		                        			Chronic renal failure (CRF), a common outcome of various chronic kidney diseases, is characterized by retention of metabolites and toxins, water-electrolyte imbalance, acid-base disturbance, and various symptoms in diverse systems. The incidence and progression of this disease are influenced by many factors, particularly the change of intestinal flora. Previous research shows that the intestinal flora interacts with CRF. For CRF patients, the metabolic waste fails to be cleared in time due to the gradual decline of renal function and thus accumulates in vivo. Moreover, CRF changes the composition of intestinal flora, damages intestinal barrier, and accelerates the synthesis of intestinal uremic toxins and the accumulation in blood. As a result, the renal injury is aggravated. The imbalance of intestinal flora can induce acute kidney injury, increase cardiovascular complications, stimulate immune inflammatory responses, and thus aggravate the progression of CRF. Microbiota-targeted therapy for CRF has become the research focus. According to traditional Chinese medicine, kidney disease is related to the intestine and kidney disease should be treated from the intestine. Spleen and kidney are in the closest relationship with the pathogenesis of CRF and the intestinal flora. Chinese medicine, which features multiple targets, multiple effects, and multiple components, acts on the "gut-kidney axis". It is thus superior in the clinical treatment of CRF and the regulation of intestinal flora. To be specific, it intervenes in intestinal flora to delay the process of CRF. In this paper, based on the correlation of traditional Chinese medicine theory with intestinal flora and CRF, this paper reviewed the interaction between intestinal flora and CRF and traditional Chinese medicine intervention in the intestinal flora for the treatment of CRF, which is expected to serve as a reference for the clinical treatment of this disease and the drug development.

		                        			Membrane-disruptive peptides/peptidomimetics (MDPs) are antimicrobials or anticarcinogens that present a general killing mechanism through the physical disruption of cell membranes, in contrast to conventional chemotherapeutic drugs, which act on precise targets such as DNA or specific enzymes. Owing to their rapid action, broad-spectrum activity, and mechanisms of action that potentially hinder the development of resistance, MDPs have been increasingly considered as future therapeutics in the drug-resistant era. Recently, growing experimental evidence has demonstrated that MDPs can also be utilized as adjuvants to enhance the therapeutic effects of other agents. In this review, we evaluate the literature around the broad-spectrum antimicrobial properties and anticancer activity of MDPs, and summarize the current development and mechanisms of MDPs alone or in combination with other agents. Notably, this review highlights recent advances in the design of various MDP-based drug delivery systems that can improve the therapeutic effect of MDPs, minimize side effects, and promote the co-delivery of multiple chemotherapeutics, for more efficient antimicrobial and anticancer therapy.
		                        		
		                        		
		                        		
		                        	
6.Application of aerobic exercise combined with progressive muscle relaxation training in patients undergoing peritoneal dialysis

		                        			Objective:To explore the application effect of aerobic exercise combined with progressive muscle relaxation training in patients undergoing peritoneal dialysis.Methods:Using the convenient sampling method, a total of 90 patients undergoing peritoneal dialysis who were regularly followed up in Peritoneal Dialysis Center of Yancheng Third People's Hospital in Jiangsu Province were selected from July to December of 2020. According to the random number table method, they were divided into the intervention group and the control group, with 45 cases in each group. Patients in the control group were given routine nursing, while patients in the intervention group were given aerobic exercise combined with progressive muscle relaxation training for 12 weeks on the basis of routine nursing. Gastrointestinal Symptom Rating Scale (GSRS), Gastroesophageal Reflux Disease Questionnaire (GerdQ) and urea clearance index (Kt/V) were used to evaluate the intervention effects of the two groups.Results:After 12 weeks of intervention, the incidence of dyspepsia, constipation, reflux, diarrhea, abdominal pain, eating disorder and GerdQ score greater than 8 in the intervention group were lower than those in the control group, and the differences were statistically significant (χ 2=6.429, 5.421, 6.403, 6.559, 3.986, 7.647, 5.789; P<0.05). The total score of GSRS and the scores of each dimension of the intervention group were lower than those of the control group, and the differences were statistically significant ( Z=-5.829, -2.637, -2.434, -2.812, -2.447, -2.027, -2.9958; P<0.05). The Kt/V compliance ratio of the intervention group was higher than that of the control group, and the difference was statistically significant (χ 2=5.683, P<0.05) . Conclusions:Compared with conventional nursing methods, aerobic exercise combined with progressive muscle relaxation training can reduce the incidence of gastrointestinal symptoms in peritoneal dialysis patients, effectively alleviate gastrointestinal symptoms and improve adequacy of dialysis.
		                        		
		                        		
		                        		
		                        	
7.Analysis of the status and influencing factors of oncology nurses′ organizational silence

		                        			Objective:To explore the level of oncology nurses’ organizational silence and its influencing factors.Methods:Totally, 278 oncology nurses were recruited to fill out the General Information Questionnaire, Nurses’ Organizational Questionnaire, Perceived Social Support Scale and Connor-Davidson Resilience Scale short version.Results:The score of Nurses’ Organizational Questionnaire was (51.27±17.28), the score of acquiescent silence was (15.91±5.42), the score of defensive silence was (15.83±6.29), the score of prosocial silence was (11.03±4.16) and the score of indifferent silence was (8.50±3.46). The perceived social support and psychological reliance were negatively associated with nurses’ organizational silence ( r values were -0.364, -0.497, all P<0.01). The results of multiple linear regression analysis showed that age, degree of education, perceived social support and psychological reliance were the influencing factors of oncology nurses’ organizational silence (all P<0.05), which could explain 45.00% of the variation. Conclusions:The level of oncology nurses’ organizational silence is at a medium level, which should be improved. Nursing managers should pay attention to the phenomenon of oncology nurses’ organizational silence, and take appropriate interventions to break the phenomenon, so as to improve the job satisfaction and work efficiency of nurses’.
		                        		
		                        		
		                        		
		                        	
8. Efficacy of rituximab combined with cyclophosphamide and fludarabine in treatment of chronic lymphocytic leukemia: a Meta-analysis

		                        			To analyze the clinical efficacy and safety of fludarabine combined with cyclophosphamide (FC) and fludarabine and cyclophosphamide combined with rituximab (FCR) in the treatment of chronic lymphocytic leukemia (CLL).
		                        		
		                        			Methods:
		                        			FCR regimen was selected as the experimental group, and FC regimen was selected as the control group. The studies were retrieved from PubMed, Cochrane Library, Embase, CNKI, Wangfang and VIP databases by computer and the references listed in these studies were further searched. The randomized controlled trials (RCT) meeting inclusive criteria were extracted, and the quality was evaluated and cross-checked independently according to Cochrane Handbook for Systematic Reviews of Interventions, and then the Meta-analysis was conducted by using StataMP 14.0 software.

		                        			A total of 7 studies and 1 985 patients were included. 9.Curative Effects of Chinese Medicine Enteroelysis Combined with Antibiotics in Syndrome Differentiation Typing Treatment of Chronic Pelvic Inflammation

		                        			Objective:To observe the clinical function of Chinese medicine enema liquid combined with antibiotics in syndrome differentiation typing treatment of chronic pelvic inflammation. Methods:A total of 240 patients with chronic pelvic inflammation were randomly divided into the control group(120)and the observation groups(120)including damp-heat stasis accumulation type,qi stagnation and blood stasis type and cold-dampness accumulation type. The patients in the control group received 0. 2g levofloxacin hydrochloride with intravenous drip,twice a day for 14 days,while those in the observation groups were given 250ml Chinese medicine enteroelysis(damp-heat stasis accumulation type was given Baiyi enteroelysis,qi stagnation and blood stasis type was given Puyao enteroelysis,and cold-dampness accumulation type was given Baiyao enteroelysis )with irrigation treatment once a day additionally. The patients in the observation group were given enteroelysis three or five days after menstruation and continued for 14 days,and the treatment course was two menstrual cycles. The indices of hemorheology and clinical efficacy were compared after the treatment between the groups. Results:The total effective rate in the observation group (91. 7% )was higher than that in the control group(81. 7% )(P < 0. 05). The improvement of whole blood viscosity,plasma viscosity,fibrinogen,erythrocyte sedimentation rate and platelet aggregation rate in the observation group were better than those in the control group(P < 0. 05). 10.Preparation and Quality Evaluation of Shenhuang Vaginal Thermosensitive in situ Gels

		                        			Objective:To prepare and evaluate Shenhuang vaginal thermosensitive in situ gels. Methods:Shenhuang vaginal ther-mosensitive in situ gels( SHV-TISG)were prepared with poloxamer 407 and poloxamer 188 as the materials. The viscosity change at different temperatures and the gelation temperature of SHV-TISG and SHV-TISG diluted by stimulant vaginal fluid were measured,the in vitro drug release of the preparation was evaluated by a membraneless diffusion method,and the safety of SHV-TISG was detected by vaginal irritation test. Results:The preparation showed the vaginal injectability and gelationability in vivo without vaginal irritation. Gel erosion was the principal mechanism of the drug release from the gels.
