1.Quality evaluation of Euscaphis japonica from different habitats using chemometrics combined with weighted TOPSIS model

OBJECTIVE To evaluate the quality of Euscaphis japonica from different habitats. METHODS The relative correction factors of gallic acid, protocatechuic acid, ellagic acid, isoquercitrin, astragalin and apigenin were calculated with quercetin as the internal reference; the relative correction factors of euscaphic acid, oleanolic acid, stigmasterol and β-sitosterol were also calculated with ursolic acid as the internal reference. The contents of 12 components in 18 batches of samples were calculated by QAMS method and were compared with external standard method. At the same time, the contents of water-soluble extract, alcohol-soluble extract, total ash and acid-insoluble ash were detected. The quality of E. japonica was evaluated by principal component analysis (PCA), orthogonal partial least squares-discriminant analysis (OPLS-DA), and weighted technique for order preference by similarity to ideal solution (TOPSIS) method. RESULTS There was no significant difference between the results of QAMS method and external standard method for the 12 components in the 18 batches of samples. However, notable content variations were observed among different batches of samples. The results of PCA and OPLS-DA showed that S1-S7, S8- S12, and S13-S18 were clustered into one category respectively. Seven key characteristic components variable importance in projection values >1, euscaphic acid, ursolic acid, protocatechuic acid, apigenin, β-sitosterol, isoquercitrin, and oleanolic acid, respectively. The analysis results of the weighted TOPSIS method revealed that the relative closeness for evaluating the quality of 18 batches of samples ranged from 0.283 5 to 0.644 1, with the samples of E. japonica from Fengjie, Chongqing, demonstrating the highest quality. CONCLUSIONS The established method is accurate and feasible, which can be used for the quality evaluation of E. japonica combined with chemometrics and weighted TOPSIS model.
2.Differential Analysis of Erythrocyte Flexibility of PbK173 Artemisinin-sensitive Strains

ObjectiveTo detect the flexibility differences of Plasmodium berghei K173 (PbK173)-infected red blood cells with varying degrees of sensitivity to artemisinin-based drugs and to preliminarily explore the underlying mechanisms of the differences. MethodA total of 102 specific-pathogen-free (SPF) male C57BL/6 mice were randomly divided into three groups, with 30 mice each in the control group and PbK173-resistant (PbK173-R) group, and 42 mice in the PbK173-sensitive (PbK173-S) group. Except for the control group, the rest groups were vaccinated with 1×107 PbK173-S/PbK173-R infected red blood cells to establish a mouse malaria model. During the administration and recovery periods (control group, PbK173-R/PbK173-S), dihydroartemisinin (DHA, 40 mg·kg-1) and malaridine (MD, 6 mg·kg-1) were administered continuously for four days. Peripheral blood was taken from the PbK173-S/PbK173-R groups with an infection rate equal to or greater than 20%. Peripheral blood and each organ were taken on the first day at the end of administration (dosing period) and on the fifth day at the end of administration (recovery period), and blood parameters and organ indices of each group were examined. The osmotic fragility of peripheral blood red blood cells in each group was detected using the red blood cell osmotic fragility test. Western blot was applied to determine the levels of Piezo1 and Band3 proteins in the red blood cell membrane. ResultDuring the administration and recovery periods, there were no significant differences between the PbK173-S MD group and the DHA group. During the administration period, there were no significant differences in hematological parameters between PbK173-S and PbK173-R in the MD group. However, during the recovery period, the red blood cell count, hemoglobin concentration and hematocrit of the PbK173-R group were significantly higher than those of the PbK173-S group (P<0.05) in the MD group. Compared with that of the control group, the osmotic fragility of the PbK173-S/PbK173-R groups was significantly enhanced (P<0.01), and the osmotic fragility of the PbK173-S group was significantly stronger than that of the PbK173-R group (P<0.01). The osmotic fragility of red blood cells in the PbK173-S group during the administration period was significantly stronger than that in the control group and PbK173-R group during the administration period (P<0.01). The osmotic fragility of red blood cells in the PbK173-R group during the recovery period was significantly higher than that in the control group during the administration period and the PbK173-S group during the recovery period (P<0.05). Compared with those in the control group, the Piezo1 protein and Band3 protein in the red blood cell membrane of the PbK173-S group were significantly reduced (P<0.01). Compared with those in the PbK173-R group, the Piezo1 protein and Band 3 protein in the red blood cell membrane of the PbK173-S group were significantly reduced. ConclusionThe flexibility of PbK173-infected red blood cells with different sensitivities to artemisinins differed. Plasmodium-infected red blood cells significantly reduced the levels of Piezo1 and Band3 proteins in the red blood cell membrane, and the erythrocyte flexibility exhibited a decreasing trend in the following order: normal group, PbK173-R group, and PbK173-S group.
3.Effect of structured therapy and education based on personal strength on ischemic stroke:a randomized con-trolled trial

Objective To evaluate the effect of structured therapy and education based on personal strength on ischemic stroke. Methods From March,2021 to September,2023,a total of 114 patients with ischemic stroke from the First People's Hos-pital of Kunshan were randomly divided into control group(n=57)and experimental group(n=57).The control group received routine medicine,rehabilitation training and standard health education,while the experimental group received routine medicine,rehabilitation training and structured therapy and education based on personal strength,for six weeks.They were compared with the scores of modified Rankin Scale(mRS),motor-evoked po-tential(MEP)latency and amplitude,modified Barthel Index and Self-Recovery Efficacy Scale score and recur-rence within three months after the start of the study.The scores of Morisky Medication Adherence Scale(MMAS)and Five Facet Mindfulness Questionaire(FFMQ)were also compared. Results After treatment,the score of mRS(t=5.002),MEP latency period(t=9.739)and amplitude(t=4.394),modi-fied Barthel Index(t=11.261),the score of Self-Recovery Efficacy Scale(Z=-2.638),and the scores of MMAS(t=19.521)and FFMQ(t=15.381)were better in the experimental group than in the control group(P<0.05). Conclusion Structured therapy and education based on personal strength could faciliate to improve the recovery and self-management of ischemic stroke patients.
4.Study on the feasibility of low-dose chest CT combined with deep learning reconstruction algorithm in the diagnosis of pediatric mycoplasma pneumonia

Objective:To explore the feasibility of 60 kV low-dose scanning technique on chest combined with ClearInfinity deep learning reconstruction algorithm in the diagnosis of pediatric mycoplasma pneumonia.Methods:A total of 132 pediatric patients,who admitted to Taihe Hospital Affiliated to Hubei Medical College and were diagnosed as mycoplasma pneumonia,were selected,and all of them underwent computed tomography(CT)scans on chest.They were randomly divided into routine dose group(66 cases),low dose ClearView and ClearInfinity group(66 cases).In the routine dose group,the tube voltage of CT scan on chest was 100 kV,and 50%ClearView iterative algorithm was adopted in this group.The tube voltage of CT scan on chest was 60kV in low dose ClearView and ClearInfinity group,and 50%ClearView iterative algorithm and 50%ClearInfinity deep learning reconstruction algorithm were used respectively to conduct reconstruction.The difference of radiation dose among the three groups was compared.The CT values and standard deviation(SD)values,signal-to-noise ratio(SNR)and contrast-to-noise ratio(CNR)of the region of interest(ROI)of the images of 3 groups were measured and calculated.The images were subjectively evaluated by two diagnostic attending physicians with 10 years of work experience,and the Kappa test was adopted to analyze the consistency of the score results.Results:Compared with the routine dose group,the volume CT dose index(CTDIvol)values,dose-length product(DLP)values and effective radiation dose(ED)values of the low dose ClearView group and ClearInfinity group reduced respectively by 87.58%,87.24%and 88.00%,and the differences were statistically significant(t=4 584.07,63.73,61.27,P<0.01).The noise values of left and right lung of the routine dose group were significantly lower than those of the low dose ClearView group,while were significantly higher than those of ClearInfinity group,and the differences were significant(Z=-9.912,-7.013,P<0.01),and the difference of them between low dose ClearView group and ClearInfinity group was significant(Z=-9.912,P<0.01).The SNR and CNR of left and right lung of low dose ClearView group were significantly lower than those of the routine dose group,with statistically significant(t=-34.810,5.522,P<0.01),while these of the low dose ClearInfinity group were significant higher than them of the routine dose group(t=3.544,-8.674,P<0.05),respectively.The two attending physicians had favorable consistency in the subjective evaluation for images(Kappa>0.75,P<0.01).The subjective score of the routine dose group was not significantly different with that of the low dose ClearInfinity group(P>0.05),but was significantly better than that of the low dose ClearView group(Z=-6.425,P<0.01).Conclusion:For pediatric patients with mycoplasma pneumonia,the 60 kV low dose CT on chest combined with ClearInfinity deep learning reconstruction algorithm can ensure image quality on the premise of reducing radiation,which can ensure the diagnostic effect.
5.Analysis of clinical characteristics of food allergy in children

Objective To investigate the food allergy situation of children in Changping District of Beijing,and to explore the influence of allergic family history,gender and mode of delivery on food allergy in children,the distribution of food allergy in different age groups,the types of food allergy that are easy to cause in this area,and the comorbidities of food allergy.Methods A total of 515 children aged 0 to 14 years who were admitted to the general pediatric outpatient and emergency department and inpatient of Beijing Changping Hospital from April to November 2023.Using immunoblotting to detect specific immunoglobulin E in the serum of pediatric patients,and using SPSS 26.0 statistical software to perform binomial tests on gender and delivery mode non parameters;Using custom Excel functions to statistically analyze the family history of allergies,the number of people in different age groups,and the frequency of allergic foods in each group;Use a self-made mini program to statistically analyze the combination of comorbid allergic diseases.Results ① The number of male children with food allergies(306 cases)was higher than that of female children(209 cases),and the difference was statistically significant(P<0.05).(2)There were 109 cases of pediatric patients with parents who had no history of allergic diseases,accounting for 21.17%of the total cases;There were a total of 406 cases where at least one parent had a family history of allergies,accounting for 78.83%of the total cases.Among them,228 cases(44.27%)had one parent with a history of allergies,and 178 cases(34.56%)had both parents with a history of allergies.③ Among 515 children with positive food allergens,there were 10 cases(1.94%)in infancy,37 cases(7.19%)in early childhood,235 cases(45.63%)in preschool,192 cases(37.28%)in school age,and 41 cases(7.96%)in adolescence.The highest positivity rate for food allergens is in milk(444 times),followed by egg white(70 times),cashew nuts(57 times),crab(37 times),beef(26 times),mango(24 times),shrimp(21 times),pineapple(6 times),and shellfish(1 time).Milk protein is the most common allergen in all age groups.(4)Among 515 children with food allergies,399 cases were single food allergies,accounting for 77.47%;116 cases of multiple food allergies(2 or more types of food allergies),accounting for 22.53%.⑤ The most common comorbidity of food allergies is food allergy related gastrointestinal diseases combined with allergic rhinitis,with a total of 267 cases;Secondly,there were 192 cases of allergic rhinitis combined with chronic cough,and 124 cases of food allergy related gastrointestinal diseases combined with chronic cough.Conclusion Milk is the main allergen of food allergy in people of age 14 and under,and gastrointestinal symptoms are the most common in children with food allergy.
6.Establishment of Poly (I∶C)-induced Cytokine Storm Model in Mice and Intervention Effect of Artesunate and Qingfei Paidu Decoction

ObjectiveTo observe the intervention effect of artesunate (ART) and Qingfei Paidu decoction (QFPD) on the mouse model of cytokine storm (CS) induced by viral mimic Poly (I∶C). MethodEighty-four SPF male BALB/c mice were randomly divided into seven groups, with 12 mice in each group. Mice, except for those in the blank group (n=12), were subjected to CS model induction by tail vein injection of Poly (I∶C) at 15 mg·kg-1, followed by drug treatments of low-dose ART (ART-l, 10 mg·kg-1), medium-dose ART (ART-m, 20 mg·kg-1), high-dose ART (ART-h, 40 mg·kg-1), Qingfei Paidu Decoction (QFPD, 2.4 g·kg-1), and dexamethasone (DXM, 10 mg·kg-1). After 6 hours, lung tissues, bronchoalveolar lavage fluid (BALF), spleen, lung, and peripheral blood were collected. The lung and spleen indexes were calculated and the number of inflammatory cells in BALF was detected. The pathological changes in lung tissues were observed by hematoxylin-eosin (HE) staining and the levels of tumor necrosis factor-α (TNF-α), interferon-γ (IFN-γ), interleukin-1β (IL-1β), and IL-6 in BALF were detected by enzyme-linked immunosorbent assay (ELISA). The expression of immune cells in BALF and peripheral blood was detected by flow cytometry. ResultThe analysis of lung and spleen indexes showed that compared with the blank group, the model group showed increased lung and spleen indexes to varying degrees (P<0.05). Compared with the model group, the ART groups showed reduced spleen index (P<0.05) and the ART-l group showed reduced lung index (P<0.05). Additionally, the QFPD group showed reduced lung and spleen indexes (P<0.05). ELISA results showed that except for TNF-α, the levels of IFN-γ, IL-1β, and IL-6 in the model group increased compared with those in the blank group (P<0.05). Compared with the model group, the ART-l group and the QFPD group showed reduced content of TNF-α (P<0.05), and all groups with drug intervention showed reduced content of IFN-γ, IL-1β, and IL-6 (P<0.05). The number of inflammatory cells in BALF showed a downward trend in the model group, and the number of cells increased in the groups with drug intervention except for the DXM group (P<0.05). Flow cytometry showed that compared with the blank group, the model group showed decreased number of CD3 in the peripheral blood (P<0.05), increased Ly-6G and F4/80 (P<0.05), decreased expression of CD45, CD3, and F4/80 in BALF (P<0.05), and increased expressions of Ly-6G (P<0.05). Compared with the model group, the ART groups and QFPD group showed increased CD45 content in peripheral blood (P<0.05), decreased Ly-6G and F4/80 content (P<0.05), increased CD45 and F4/80 content in BALF (P<0.05), and decreased expression of Ly-6G (P<0.05). ConclusionART and QFPD have a good protective effect on Poly (I∶C)-induced CS in mice, and the mechanism is related to the effective intervention in immune cell disorder.
7.Method Establishment for Content Determination of 8 Nucleosides in Wild and Cultivated Pinellia ternata and the Difference Analysis

OBJECTIVE:To establish the metho d for the content determination of 8 nucleosides in wild and cultivated Pinellia ternate,and to conduct the difference analysis. METHODS :The contents of uracil ,uridine,inosine,xanthine,adenine, guanosine,β-thymidine and adenosine in 20 batches of P. ternate (wild product YS 1-YS8,cultivated product ZP 1-ZP12)were determined by HPLC method. Based on the contents of the above 8 nucleosides,cluster analysis ,principal component analysis (PCA),partial least squares analysis (OPLS-DA)were used to comprehensively evaluate the wild and cultivated P. ternata . RESULTS:The contents of uracil ,uridine,inosine,xanthine,adenine,guanosine,β-thymidine and adenosine in 20 batches of P. ternate were 0.02-0.24,0.01-0.24,0.06-0.37,0.02-0.14,0.04-0.22,0.14-0.42,0.01-0.09,0-0.32 mg/g,respectively. Cluster analysis,PCA and OPLS-DA showed that 8 batches of wild P. ternate (YS1-YS8)were clustered into one category ,and 12 batches of cultivated P. ternate (ZP1-ZP12)were clustered into one category. Main characteristic markers of wild P. ternate were guanosine,uridine,adenosine and adenine ,while the main characteristic markers of cultivated P. ternate were urinine ,xanthine, inosine,and β-thymidine. CONCLUSIONS:The method for the content determination of 8 nucleosides in P. ternate is established. Nucleosides as quality markers can effectively distinguish wild and cultivated P. ternata ,and the quality of the wild P. ternate was better than that of cultivated P. ternate .
8.Efficacy and safety of dual antiplatelet therapy guided by thromboelastography

Objective:To analyze the efficacy and safety of dual antiplatelet therapy guided by thromboelastography (TEG).Methods:One hundred and nine patients with ischemic cerebrovascular diseases, admitted to and accepted dual antiplatelet therapy in our hospital from August 2020 to April 2021, were enrolled in this study. TEG test was completed 3-5 d after treatment, and clopidogrel gene test was completed during hospitalization. According to the results of adenosine diphosphate (ADP) inhibition rate in TEG, they were divided into high-efficiency group, moderate efficiency group and low efficiency group. No medication change was recommended for patients in the high-efficiency group, but medication changes were recommended for patients in the moderate efficiency group and low efficiency group; differences of National Institutes of Health Stroke Scale (NIHSS) scores, efficacies and safety were recorded in these 3 groups after drug adjustment.Results:The proportion of patients with coronary heart disease in moderate efficiency group was statistically higher than that in high-efficiency group and low efficiency group ( P<0.05). Six patients in the high-efficiency group switched to use aspirin and cilostazol; 28 in the moderate efficiency group and 14 patients in the low efficiency group were disinclined to change their medications and continued to use aspirin+clopidogrel. In patients from the high-efficiency group, the overall incidence of new stroke and in-stent restenosis in patients without changing medications (1/39) was lower than that in patients with changing medications (1/6); in patients from the moderate efficiency group, that in patients without changing medications (6/28) was higher than that in patients with changing medications (1/9); in patients from the low efficiency group, that in patients without changing medications (3/14) was higher than that in patients with changing medications (0/13). In patients with slow metabolism from high-efficiency group, the overall incidence of new stroke and in-stent restenosis in patients without changing medications (2/3) was higher than that in patients with changing medications (1/2). Conclusions:Dual antiplatelet therapy guided by TEG can reduce clinical adverse events without increasing the risk of bleeding. For patients with ADP inhibition rate>70% and clopidogrel genotype (slow metabolism), clopidogrel replacement can further improve efficacy.

10. Characteristics of patients visiting emergency department by different ways of transportation

Objective:
To explore the characteristics of patients visiting emergency department by different ways of transportation.
Methods:
The medical information of 23 039 patients visiting emergency department from January 2017 to December 2017 was obtained through the Hospital Information System (HIS). There were 20 856 patients visiting emergency arranged by themselves (self-visiting group), and 2 183 patients sent by ambulance (ambulance group). The characteristics of two groups were compared.
Results:
The proportion of young patients in the self-visit group was the highest (45.97%, 9 587/20 856), and the proportion of elderly patients in the ambulance group was the highest (40.04%, 874/2 183). Throughout the year, the number of patients who came to the hospital by themselves was the highest in the summer, that by ambulance was the highest in the winter, while that was the lowest in spring for both groups. There was no significant fluctuation in the number of patients who came to the hospital during the week. In the self-visited group, the daily peak hours were 18:00-02:00 at night and the ambulance group were 10:00-18:00. The number of visits for top 5 system diseases were trauma, digestive system, other diseases, respiratory system and urinary system diseases in self-visiting group; and were nervous system, trauma, digestive system, respiratory system and circulatory system diseases in ambulance group. The hospitalization rates of self-visiting group and ambulance group were 14.41% (3 006/20 856) and 76.64% (1 564/2 183), respectively.
Conclusion
The emergency patients with different visiting modes have different characteristics. The information would be useful for rationally allocate emergency resources, optimize the treatment process, improve the emergency capacity and ensure the safety of patients.
