1.Effects of arctigenin on the proliferation, apoptosis, and invasion of oral squamous cell carcinoma HSC-3 cells by regulating the Notch/Hes-1 signaling pathway

[摘 要] 目的:探究牛蒡子苷元(ARC)通过调控Notch/Hes-1信号通路对口腔鳞状细胞癌(OSCC)HSC-3细胞增殖、凋亡和侵袭的影响及其机制。方法:使用不同质量浓度的ARC处理人HSC-3细胞,CCK-8法检测ARC对细胞增殖活力的影响,以选择适宜的药物浓度。将HSC-3细胞分为对照组、ARC-L组(10 mg/L ARC)、ARC-M组(20 mg/L ARC)、ARC-H组(40 mg/L ARC)和ARC-H+Jagged1/FC组(40 mg/L ARC+1.2 μg/mL Jagged1/FC)。采用EdU法检测细胞增殖能力,划痕愈合实验、Transwell实验和流式细胞术分别检测细胞的迁移、侵袭能力及细胞周期和细胞凋亡率,WB法检测增殖(c-Myc、cyclin D1)、凋亡(BAX、Bcl-2、survivin)、EMT(E-cadherin、vimentin、Snail)及Notch/Hes-1通路(Notch 1、Hes-1、NICD)相关蛋白的表达水平。结果:与0 mg/L相比,10~80 mg/L的ARC均能显著降低HSC-3细胞增殖活力(均P<0.05)。与对照组相比,ARC-L组、ARC-M组和ARC-H组HSC-3细胞EdU阳性率、划痕愈合率、侵袭细胞数、S期和G2/M期细胞占比及c-Myc、cyclin D1、Bcl-2、survivin、vimentin、Snail、Notch 1、Hes-1和NICD蛋白表达均显著降低(均P<0.05),细胞凋亡率、G0/G1期细胞占比及BAX、E-cadherin的蛋白表达均显著升高(均P<0.05),且呈浓度梯度依赖性。同时使用Notch激动剂Jagged1/FC,则可部分逆转ARC对HSC-3细胞增殖、迁移、侵袭、凋亡及相关蛋白表达的作用(均P<0.05)。结论:ARC可能通过抑制Notch/Hes-1信号通路抑制OSCC细胞HSC-3增殖和侵袭并促进细胞凋亡。
2.Research progress on epilepsy after reperfusion therapy in ischemic stroke

There is no unified international guidelines or consensus on seizures and epilepsy following acute stroke reperfusion therapy so far.In this review,we briefly summarize its definitions and mechanisms.Post stroke epilepsy after reperfusion treatment is defined as patients with ischemic stroke who have received intravenous thrombolysis and/or endovascular therapy,without other definitive causes or epilepsy history before stroke,have at least two epileptic seizures occurred within 7 days of stroke onset,or at least one epileptic seizures occurred within 30 days of stroke onset.The incidence rate of epilepsy after intravenous thrombolysis is about 6.4%-20.6%,and arterial thrombectomy is about 5%.The pathophysiological mechanism of post stroke epilepsy after reperfusion treatment may be related to local hyperfusion,epileptogenic properties of tPA and hemorrhagic transformation.Higher stroke severity,cortical involvement,middle cerebral artery infarction,and early post-stroke seizures may be predictive factors for post-stroke epilepsy after reperfusion therapy.Levetiracetam and lamotrigine may be effective drugs for post-stroke epilepsy after reperfusion therapy.Sustained seizures after thrombolysis may increase the risk of death.
3.Feasibility study on deep learning image reconstruction algorithm to improve the quality of low-dose CT images of the brain

Objective:To explore the effectiveness of deep learning image reconstruction (DLIR) algorithm compared to adaptive statistical iterative reconstruction (ASIR-V) algorithm in improving the quality of low-dose brain CT images.Methods:Retrospective inclusion of patients who underwent brain CT examination in the People's Liberation Army General Hospital from November 2021 to August 2022. Four different algorithms were used to reconstruct low-dose CT scans of all patients to obtain 30% intensity ASIR-V (ASIR-V-30%) images, low intensity DLIR (DLIR-L) images, medium intensity DLIR (DLIR-M) images, and high intensity DLIR (DLIR-H) images. The regions of interest were selected from four sets of images, including superficial white matter, superficial gray matter, deep white matter, and deep gray matter, and their CT values and standard deviations were measured for calculating signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR).Subjective evaluation of image quality was conducted by three neuroimaging physicians based on the Likert 5-component scale. The objective and subjective scores of the 4 groups of images were analyzed using ANOVA or Kruskal Wallis. If there are overall differences, pairwise comparisons were conducted within the group.Results:A total of 109 patients were enrolled, including 104 males and 5 females, aged 65-110 years (89.16 ± 9.53) years. The radiation exposure of brain CT low-dose scanning was (0.93 ± 0.01)mSv, significantly lower than that of conventional scanning (2.92 ± 0.01) mSv ( t = 56.15, P < 0.05). The differences in objective image quality analysis of ASIR-V-30%, DLIR-L, DLIR-M, and DLIR-H images of low-dose CT in SNR deep gray matter, SNR deep white matter, SNR superficial gray matter, SNR superficial white matter, CNR deep gray white matter, and CNR superficial gray white matter were statistically significant( F =98.23, 72.95, 68.43, 58.24, 241.13, 289.91, P < 0.05). Among them, DLIR-H images had the lowest noise in deep gray matter, deep white matter, superficial gray matter, and superficial white matter, and had statistically significant differences compared to other image groups ( t = 167.43, 275.46, 182.32, 361.54, P < 0.05). The subjective score of DLIR-H image quality was superior to ASIR-V-30%, DLIR-L, and DLIR-M, with the statistically significant difference ( t = 7.25, 8.32, 9.63, P < 0.05). Conclusions:Compared with ASIR-V, DLIR algorithm can effectively reduce image noise and artifacts in low-dose brain CT, and improve SNR and CNR. The subjective and objective image quality evaluation of DLIR-H is the best.
4.Advances in the application of radiomics in predicting outcomes after transarterial chemoembolization for hepatocellular carcinoma

Hepatocellular carcinoma is one of the common malignant tumors, and most patients with hepatocellular carcinoma are already in the middle stage at the time of clinical detection, transarterial chemoembolization is the treatment of choice for mid-stage hepatocellular carcinoma.Due to the high degree of tumor heterogeneity, accurately predicting the outcome of patients with hepatocellular carcinoma after transarterial chemoembolization remains one of the difficulties in clinical practice.As an emerging technology, radiomics can not only reflect tumor heterogeneity non-invasively, but also monitor, evaluate and predict tumor progression by analyzing changes in the tumor microenvironment to guide patients′ personalized treatment and prolong their survival time.This article reviews the progress of the application of radiomics in predicting the efficacy of transarterial chemoembolization for hepatocellular carcinoma.
5.Progress in the diagnosis and differential diagnosis of small renal cell carcinoma by CT texture analysis

Small renal cell carcinoma refers to a renal malignant tumor with a maximum diameter of 4 cm.Due to the small size, its diagnosis and differential diagnosis have been difficult points in clinical work. CT texture analysis is an emerging technique, it determines the tumor heterogeneity by analyzing the distribution and relationship of pixel or voxel gray-scale levels in the CT images, it acts to more accurately predict the benign and malignant tumors and the classification of tumors.This paper reviews CT texture analysis on the diagnosis and differential diagnosis of small renal cell carcinoma, in order to guide the correct diagnosis of doctors and effectively clinical treatment.
6.Application of delayed-enhancement MRI in lumpectomy cavity delineation for patients after breast-conserving surgery in prone radiotherapy using deformable image registration

Objective:To assess the feasibility of delayed-enhancement MRI in contouring the lumpectomy cavity (LC) for patients with invisible seroma or a low cavity visualization score (CVS≤2) in the excision cavity after breast-conserving surgery (BCS).Methods:Twenty-six patients with stage T 1-2N 0M 0 who underwent prone radiotherapy after BCS were recruited. The LC delineated on CT simulation images was denoted as LC CT. The LCs delineated on T 2WI, as well as on different delayed phases (2-, 5-and 10-minute) of delayed-enhancement T 1WI were defined as LC T2, LC 2T1, LC 5T1 and LC 10T1, respectively. Subsequently, the volumes and locations of the LCs were compared between CT simulation images and different sequences of MR simulation images using deformable image registration. Results:The volumes of LC T2, LC 2T1, LC 5T1 and LC 10T1 were all larger than that of LC CT. A statistical significance was found between the volume of LC CT and those of LC 2T1 or LC 5T1, respectively (both P<0.05). The conformal index (CI), degree of inclusion (DI), dice similarity coefficient (DSC) and the distance between the center of mass of the targets (COM) of LC CT-LC 10T1 were better than those of LC CT-LC T2, LC CT-LC 2T1 and LC CT-LC 5T1, however, there was no statistical difference among them (all P>0.05). Conclusions:It is feasible to delineate the LC based on prone delayed-enhancement MR simulation images in patients with low CVS after BCS. Meanwhile, the LCs derived from prone delayed-enhancement T 1WI of 10-minute are the most similar with those derived from prone CT simulation scans using titanium clips, regardless of the volumes and locations of LCs.
7.Analysis of the evaluation errors of dose-volume parameters for left ventricular myocardium in esophageal cancer radiotherapy

Objective To evaluate the dynamic variation of the dose-volume parameters of the left ventricular myocardium following heart beat in radiotherapy for esophageal cancer. Methods The left ventricular myocardium of 22 patients was contoured on 20 phases (0%-95%) of electrocardiography (ECG) gated heart 4DCT images. The radiotherapy plan was designed on the simulation CT images,and then the dose distribution of radiotherapy plan was imported into MIM Maestro system and 4D dose-volume histogram (DVH) was reconstructed. The variations of position,volume and dice similarity coefficient (DSC) of the left ventricular myocardium were analyzed. The changing ranges of Dmean ,V10,V20 ,V30 and V40 of the left ventricular myocardium during different phases were statistically compared. Results ( 1 ) The biggest displacement of the left ventricular myocardium was in Y axes. The maximum variation rate of volume and DSC of the left ventricular myocardium were (24.23±11. 35)% and (184.33±128. 61)% in different phases with statistical significance (both P<0. 05).(2) The maximum variation rate of Dmean of the left ventricular myocardium was (87.05± 38. 34)% in different phases with the highest rate of 163. 52% with statistical significance (P<0. 05).(3) The maximum variation values of V10,V20,V30 and V40 of the left ventricular myocardium were (13.64±4. 33)%,(12.84±4. 55)%,(11.62±4. 85)% and (3.63±2. 56)% with statistical significance (all P<0. 05). Conclusions The impact of heart beat on the dose-volume parameters of the left ventricular myocardium should be considered during esophageal cancer radiotherapy. Traditional static 3DCT-based assessment of the dose-volume parameters of the left ventricular myocardium can yield relatively large errors, which is probably reduces the prediction efficiency of the dose-volume parameters for radiation-induced heart injury.
8.Pathological and radiological characteristics of cystic lesions accompanied with pancreatic ductal adenocarcinoma

Objective To explore the correlation between pathologic findings of pancreatic ductal adenocarcinoma accompanied with cystic lesion and their radiological images .Methods The pathological and radiological data of 97 cases with cystic lesion of 560 patients who were diagnosed as pancreatic ductal adenocarcinoma based on postoperative pathology were retrospectively analyzed .All the patients underwent CT and MRI examinations .Results Postoperative pathological report showed that solid masses and cysts coexisted in all the lesions .Solid masses had compact structures and consisted of innumerable fibrous collagen mixed with tumorous tubular or acinar gland tissue but lack of vessels and inflammatory cells were infiltrated in tissues surrounding the tumors .As for the accompanied cystic lesions , 54 cases ( 56%) were pseudocysts , 22 cases (23%) were retention cysts, 12 cases (12%) were pancreatic intraductal papillary mucinous neoplasms ( IPMN) , and 9 cases (9%) were the necrosis and cystic changes of the tumors .Solid masses were hypodense or iso-hypodense in CT, and hypodense on T1 WI and isodense or iso-hyperdense on T2 WI in MR.The focal calcification was detected in only one mass .Seventy-eight cases displayed slight enhancement and 19 cases displayed no enhancement after enhancement .The tumor′s boundary were unclear in all cases .The cystic lesions which were located in masses were only observed in patients with tumors′necrosis and cystic degeneration, and other cystic lesions were adjacent to the solid masses .Single cyst was found in 70 cases (49 with pseudocysts , 12 with retention cysts , 9 tumors′necrosis and cystic degeneration ,) and multiple cysts were in 27 cases (5 with pseudocysts, 10 with retention cysts ,12 with IPMN).Exudation appeared surrounding the tumors in all cases accompanied with pseudocysts , and the cystic wall was attached to the spleen in 10 cases and the kidney in 5 cases.Hemorrhage appeared in 16 cases with pseudocysts .Pancreatic ducts were blocked by tumor masses and the proximal duct was dilated in 80 cases.Common bile ducts were obstructed with the proximal duct dilated in 18 patients.Splenic veins were infiltrated in 11 cases.Wall nodular lesions were seen in 5 cases with cystic degeneration .Conclusions Pancreatic ductal adenocarcinoma is a highly invasive tumor with compact structures , and the tumors′cystic degeneration is uncommon in such patients .Most of the cystic lesions accompanied with the tumors are pseudocysts or retention cysts , which are secondary changes caused by the obstruction in pancreatic ducts , and these pathological features are correlated the imaging findings .
9.Application of 4D-CT and deformable registration in assessment of dose accumulation in radiotherapy for hepatocellular carcinoma

Objective To explore the effect of respiration on dose accumulation for target volume and normal liver in radiotherapy for hepatocellular carcinoma (HCC) while applying 4D-CT and deformable registration.Methods Nineteen HCC patients who had received transcatheter arterial chemoembolization were enrolled in this study.All patients underwent 3D-and 4D-CT simulation in free breathing.The 3D dose (Dose-3D) was calculated from the treatment planning designed on the 3D-CT image.The Dose-3D then was recalculated on ten phases of 4D-CT images respectively,and the end-inspiration and end-expiration doses were defined as Dose El and Dose-EE.The 4D dose (Dose-4D) was obtained by deforming and accumulating ten-phase doses of 4D-CT images on the end-expiration phase image.The dosimetric differences of planning target volume and normal liver were compared among Dose-3D,Dose-4D,Dose-EI and Dose-EE.Results The D99 and D95of planning target volume (PTV) in Dose-3D were higher than those of Dose-4D,Dose-Fl and Dose EE (x2 =32.75,26.31,P < 0.05).The conformal index (CI) and homogeneity index (HI) in Dose-3D were better than those of Dose-4D,Dose-E1 and Dose-EE,in which CI decreased from 0.78 to0.63,0.60 and 0.57,while HI increased from 0.08 to 0.15,0.16 and 0.19 (x2 =37.80,31.86,P <0.05).No statistically significant differences were found in dosimetric indices of PTV between Dose-4D and Dose EI,Dose-EE,and between Dose-El and Dose-EE (P > 0.05).The mean dose (D),V5,V10,V20,V30 and V40 of normal liver were similar among four dose distributions (P > 0.05).Conclusions More objective and precise dose distribution for target volume and normal liver could be obtained by applying both 4D-CT and deformable registration,which is beneficial to accurately predicting the dosevolume indices of radiation-induced liver injury and offering more reliable evidence of escalation for target dose.

Objective To study the feasibility of defining the internal gross tumor volume (IGTV) of hepatocellular carcinoma applying the enhanced four-dimensional computed tomography (4DCT) images with deformable registration technology.Methods Ten HCC patients who accepted radiation therapy were selected in this study.The 4DCT in free breathing,non-enhanced 3DCT and arterial phase enhanced 3DCT in end inspiration breath holding associated with active breathing coordinator were acquired sequentially.4DCT were sorted into ten series CT images according to breath phase,and named CT00,CT10..…CT90.Gross tumor volume (GTV) were contoured on different CT series and the IGTV1 was merged by ten phases GTVs of 4DCT.The GTV of enhanced 3DCT was registered to different CT series of 4DCT and the IGTVDR was obtained by merging the GTVs after deformable registration.The target volumes differences were compared by paired t-test.Results The edge of tumor was difficult to define on 4DCT and non-enhanced 3DCT images.The enhanced 3DCT image showed clearer tumor edge,and the GTV increased by mean 37.99% compared to GTV on 4DCT different series images and non-enhanced 3DCT image (P =0.002).The GTV after deformable registration on 4DCT different phase images increased by mean 36.34% (P =0.011),which were similar to GTV on enhanced 3DCT image (P =0.632).The IGTVDR increased by 19.91% (P =0.017),compared to IGTV1.Conclusions The contrast-enhanced 4DCT image which was obtained by combining enhanced 3DCT and 4DCT images with deformable registration technology could raise the position precision of the HCC IGTV effectively.
