1.Associations of cholecystectomy with the risk of colorectal cancer: a Mendelian randomization study.

BACKGROUND:
Cholecystectomy is a standard surgery for patients suffering from gallbladder diseases, while the causal effects of cholecystectomy on colorectal cancer (CRC) and other complications are still unknown.
METHODS:
We obtained genetic variants associated with cholecystectomy at a genome-wide significant level ( P value <5 × 10 -8 ) as instrumental variables (IVs) and performed Mendelian randomization (MR) to identify the complications of cholecystectomy. Furthermore, the cholelithiasis was also treated as the exposure to compare its causal effects to those of cholecystectomy, and multivariable MR analysis was carried out to judge whether the effect of cholecystectomy was independent of cholelithiasis. The study was reported based on Strengthening the Reporting of Observational Studies in Epidemiology Using Mendelian Randomization guidelines.
RESULTS:
The selected IVs explained 1.76% variance of cholecystectomy. Our MR analysis suggested that cholecystectomy cannot elevate the risk of CRC (odds ratio [OR] =1.543, 95% confidence interval [CI]: 0.607-3.924). Also, it was not significant in either colon or rectum cancer. Intriguingly, cholecystectomy might decrease the risk of Crohn's disease (OR = 0.078, 95% CI: 0.016-0.368) and coronary heart disease (OR = 0.352, 95% CI: 0.164-0.756). However, it might increase the risk of irritable bowel syndrome (IBS) (OR = 7.573, 95% CI: 1.096-52.318). Cholelithiasis could increase the risk of CRC in the largest population (OR = 1.041, 95% CI: 1.010-1.073). The multivariable MR analysis suggested that genetic liability to cholelithiasis could increase the risk of CRC in the largest population (OR = 1.061, 95% CI: 1.002-1.125) after adjustment of cholecystectomy.
CONCLUSIONS
The study indicated that cholecystectomy might not increase the risk of CRC, but such a conclusion needs further proving by clinical equivalence. Additionally, it might increase the risk of IBS, which should be paid attention to in clinical practice.

2.Bone Grafting Can Promote the Prognosis of Displaced Femoral Neck Fractures: A Follow-up of the Clinical Significance of Bone Defects

Background:
Femoral neck fractures (FNFs) comprise a large proportion of osteoporotic fractures in Asia. However, the full range of prognostic variables that affect prognosis remains unclear. Here, we aimed to determine whether the severity of bone defects at the fracture site and other variables impact the prognosis of displaced FNFs.
Methods:
We evaluated the incidence of FNF internal fixation failures at regular intervals after surgery in data collected retrospectively. Digital Imaging and Communications in Medicine (DICOM) magnetic resonance imaging data of the displaced FNFs of 204 patients (> 20 years old; mean age, 52.3 years; men, 55.4%) who underwent internal fixation were used to construct threedimensional (3D) virtual models of the femoral neck region. We calculated the position and volume of bone defect (VBD) using our independently developed algorithm and Mimics software. Each participant was followed up for at least 24 months; complications were noted and correlated with VBD and demographic and clinical variables.
Results:
On the basis of VBD values calculated from virtual reduction models, 57 patients were categorized as having a mild defect, 100 as having a moderate defect, and 47 as having a severe defect. Age (p = 0.046) and VBD (p < 0.001) were significantly correlated with internal fixation failure. Multivariate analysis revealed that severe bone defects were associated with internal fixation failure (adjusted odds ratio [aOR], 23.073; 95% confidence interval [CI], 2.791–190.732) and complications (aOR, 8.945; 95% CI, 1.829–43.749). In patients with a severe defect, bone grafting was inversely associated with internal fixation failure (aOR, 0.022; 95% CI, 0.002–0.268) and complications (aOR, 0.023; 95% CI, 0.002–0.299).
Conclusions
Bone defect severity was associated with internal fixation failure and other complications. For young adults with large VBDs, bone grafting of the defect can reduce the risk of internal fixation failure. These results provide useful new quantitative information for precisely classifying displaced FNFs and guiding subsequent optimal treatments.

The fall armyworm (FAW), Spodoptera frugiperda, is a destructive pest native to America and has recently become an invasive insect pest in China. Because of its rapid spread and great risks in China, understanding of FAW genetic background and pesticide resistance is urgent and essential to develop effective management strategies. Here, we assembled a chromosome-level genome of a male FAW (SFynMstLFR) and compared re-sequencing results of the populations from America, Africa, and China. Strain identification of 163 individuals collected from America, Africa and China showed that both C and R strains were found in the American populations, while only C strain was found in the Chinese and African populations. Moreover, population genomics analysis showed that populations from Africa and China have close relationship with significantly genetic differentiation from American populations. Taken together, FAWs invaded into China were most likely originated from Africa. Comparative genomics analysis displayed that the cytochrome p450 gene family is extremely expanded to 425 members in FAW, of which 283 genes are specific to FAW. Treatments of Chinese populations with twenty-three pesticides showed the variant patterns of transcriptome profiles, and several detoxification genes such as AOX, UGT and GST specially responded to the pesticides. These findings will be useful in developing effective strategies for management of FAW in China and other invaded areas.

6.Simultaneous determination of five constituents in Jiangzhi Huoxue Tablets by UPLC-MS/MS

AIM To establish an ultra-performance liquid-chromatography tandem mass spectrometry (UPLC-MS/MS) method for the simultaneous content determination of five constituents in Jiangzhi Huoxue Tablets (Polygoni multiflori Radix,Astragali Radix,Chuanxiong Rhizoma,etc.).METHODS The 50% methanol extract of this drug was performed on a 40 ℃ thermostatic Restek UItra BiPh column (100 mm × 2.1 mm,5 μm),with the mobile phase comprising of acetonitrile (containing 0.1% formic acid)-0.1% formic acid flowing at 0.4 mL/min in a gradient elution manner.RESULTS Stilbene glycoside,tanshinone Ⅱ A,emodin,ferulic acid and puerarin showed good linear relationships within the ranges of 4.01-1 027,0.7-187,1.48-380,3.98-1 020 and4.285-1 097 ng/mL (r >0.994 0),whose average recoveries were 98.57%-101.0% with the RSDs (n =6) of less than 4.79%.CONCLUSION This specific and sensitive method can be used for the quality control of Jiangzhi Huoxue Tablets.
7.Advances in Research on Toxic Side Effects and Mechanism of Drugs Induced by Enterohepatic Cycle

The enterohepatic circulation is an important form of drug absorption and excretion.Drugs with enterothepatic circulation can induce toxic side effects,and elucidate the mechanism of induced side effects is a necessary understanding of the phenomenon of adverse drug reaction.Enterohepatic circulation of drugs involves in two ways:one is based on the drug prototype for enterohepatic circulation;the other is phase Ⅱ metabolic pathway for enterohepatic circulation.In this paper,two kinds of enterohepatic circulation were reviewed.The toxic side effects of six kinds of drugs with enterohepatic circulation were introduced,and the possible mechanism were described and discussed,so as to have contributed to more rational use of drugs in clinical practice.Furthermore,it can promote the research and development of new drugs.
8.Emergency laparoscopic splenectomy for the treatment of traumatic splenic rupture

Objective To investigate the indications and clinical effect of emergency laparoscopic splenectomy for traumatic splenic rupture.Me,ods Clinical data of 30 traumatic splenic rupture patients undergoing emergency laparoscopic splenectomy from Aug 2011 to Mar 2016 in our hospital were analyzed retrospectively.Results.All patients were of simple traumatic spleen rupture without combined injuries.There were 2 cases of grade Ⅱ (6.7%),17 cases of grade Ⅲ (56.7%),10 cases of grade Ⅳ (33.3%),1 case of grade Ⅴ (3.3%).28 cases underwent successful laparoscopic splenectomy and 2 cases were converted to open surgery.22 cases used intraoperative autologous blood transfusion (73.3%),19 cases received blood transfusion (63.3 %),average perioperative blood loss were (1 791 ± 643) ml,operation time were (135 ± 51) min,postoperative fasting time was (2.7 ± 1.8) d,postoperative hospital stay was (8 ± 5) d.The overall postoperative morbidity rate was 20% (6/30),including pancreatic leakage in one,pleural effusions in 2,infections in 3.There were no operative mortality and other serious complications.Conclusions Emergency laparoscopic splenectomy is minimally invasive with faster recovery,less complication and shorter hospital stay.
9.Triptolide Attenuates Cardiac Remodeling in Isoprenaline-induced Chronic Heart Failure Rats via Upregulating PTEN Pathway

Objective]To assess the effects and mechanism of triptolide(TPL)on cardiac remodeling in chronic heart failure (CHF)rats.[Methods]Thirty-two Wistar rats were divided into four groups of eight:Control group,isoprenaline(Iso)group,TPL group(Iso+TPL)and captopril group(Iso+Cap). CHF rat model was induced by Iso. In TPL and Cap group,TPL(20μg/kg·d)and Cap(15 mg/kg·d)were administrated to CHF rats for six weeks. Left ventricular internal diameter at end-diastole(LVIDd)and left ventricular internal diameter at end-systole (LVIDs)were detected. Histopathological changes of myocardial tissues of rats were evaluated byMasson staining and immunohistochemical staining of type Ⅰcollagen. Ventricular weight/body weight ratio(VW/BW), collagen volume fraction(CVF),perivascular collagen volume area(PVCA)of myocardial tissues were calculated. With real-time polymerase chain reaction and Western blot,the protein and mRNA levels of phosphatase and tensin homologue deleted on chromosome ten(PTEN)were detected.[Results]Compared to the Iso group,the levels of LVIDs,LVIDd,VW/BW,CVF and PVCA reduced in TPL and Cap group. TPL and Cap can alleviate the myocardial fibrosis in CHF rats. The expression of PTEN protein and mRNA increased markedly in the TPL or Cap treated group.[Conclusion]TPL can attenuate cardiac remodeling in Iso-induced CHF rats. The potential mechanism may be highly associated with the up-regulating of PTEN signaling pathway.

Paris polyphylla, the dried rhizome of Paris polyphylla Smith Var.yunnanensis (Franch.) Hand-Mazz.or Paris polyphylla Smith Var.chinensis (Franch.) Hara, has been commonly used as a traditional Chinese folk medicine for thousands of years.The extensive pharmacological studies indicated that steroidal saponins had multiple pharmacological activities, such as potential anticancer activity, antibacterial, antivirus, hemostasis, sedative activity, protecting cardiovascular and liver tissues.Obviously, the studies on steroidal saponins activity are very important in the development and clinical application of the potential new drug.The recent research advances in the pharmacological activities of Paris polyphylla reported in the paper could provide reference for the further utilization of Paris polyphylla.
