BACKGROUND:Myocardial patches are used as an effective way to repair damaged myocardium, and there is controversy over which cells to use to make myocardial patches and how to maximize the therapeutic effect of myocardial patches in vivo. OBJECTIVE:To find out the best way to make myocardial patches by overviewing the cellular sources of myocardial patches and strategies for perfecting them. METHODS:The first author searched PubMed and Web of Science databases by using"cell sheet, cell patch, cardiomyocytes, cardiac progenitor cells, fibroblasts, embryonic stem cell, mesenchymal stem cells"as English search terms, and searched CNKI and Wanfang databases by using"myocardial patch, biological 3D printing, myocardial"as Chinese search terms. After enrollment screening,94 articles were ultimately included in the result analysis. RESULTS AND CONCLUSION:(1)The cellular sources of myocardial patches are mainly divided into three categories:somatic cells, monoenergetic stem cells, and pluripotent stem cells, respectively. There are rich sources of cells for myocardial patches, but not all of them are suitable for making myocardial patches,e.g.,myocardial patches made from fibroblasts and skeletal myoblasts carry a risk of arrhythmogenicity, and mesenchymal stem cells have a short in vivo duration of action and ethical concerns. With the discovery of induced multifunctional stem cells,a reliable source of cells for making myocardial patches is available.(2)There are two methods of making myocardial patches. One is using cell sheet technology. The other is using biological 3D printing technology.Cell sheet technology can preserve the extracellular matrix components intact and can maximally mimic the cell growth ring in vivo. However,it is still difficult to obtain myocardial patches with three-dimensional structure by cell sheet technology.Biologicasl 3D printing technology, however, can be used to obtain myocardial patches with three-dimensional structures through computerized personalized design.(3)The strategies for perfecting myocardial patches mainly include:making myocardial patches after co-cultivation of multiple cells, improving the ink formulation and scaffold composition in biological 3D printing technology, improving the therapeutic effect of myocardial patches, suppressing immune rejection after transplantation, and perfecting the differentiation and cultivation protocols of stem cells.(4)There is no optimal cell source or method for making myocardial patches, and myocardial patches obtained from a particular cell or technique alone often do not achieve the desired therapeutic effect. Therefore, researchers need to choose the appropriate strategy for making myocardial patches based on the desired therapeutic effect before making them.

This paper summarized professor WANG Shouchuan's experience in differentiating and treating children epistaxis from the perspective of "four excess and three deficiency". It is believed that the pathogenesis of children epistaxis is concluded as "four excess and three deficiency", of which the four excess syndromes are exuberant heat in the lung channel, intense stomach fire, heart fire hyperactivity, and liver fire flaming upward, while the three deficiency syndromes include qi, yin and yang deficiency. Seven methods for treating children epistaxis are summarized. For exuberant heat in the lung channel syndrome, it is recommended to clear lung and direct qi downward, using self-made Xiebai Zhiniu Decoction (泻白止衄汤). For intense stomach fire syndrome, the method of clearing stomach and draining fire can be used with self-made Qingwei Zhiniu Decoction (清胃止衄汤). In terms of heart fire hyperactivity syndrome, it is better to clear heart and drain fire, using self-made Daochi Zhiniu Decoction (导赤止衄汤). For liver fire flaming upward syndrome, it is advised to clear liver and drain fire, using self-made Yimu Zhiniu Decoction (抑木止衄汤). In terms of qi deficiency syndrome, the method of fortifying spleen and boosting qi and containing blood should be used with self-made Futu Zhiniu Decoction (扶土止衄汤). If there is yin deficiency syndrome, it is advised to supplement kidney, enrich yin and clear heat, using self-made Zishui Zhiniu Decoction (滋水止衄汤). If there is yang deficiency syndrome, the method of boosting qi, warming yang and nourishing blood can be used, using self-made Wenpi Zhiniu Decoction (温脾止衄汤).

Objective To investigate the perioperative clinical effects and follow-up results of minimally invasive coronary artery bypass grafting (MICS CABG) versus conventional coronary artery bypass grafting (CABG) in thoracotomy. Methods The patients who received off-pump CABG in Beijing Anzhen Hospital from January 2017 to October 2021 were collected. Among them, the patients receiving MICS CABG performed by the same surgeon were divided into a minimally invasive group, and the patients receiving median thoracotomy were into a conventional group. By propensity score matching, preoperative data were balanced. Perioperative and postoperative follow-up data of the two groups were compared. Results A total of 890 patients were collected. There were 211 males and 28 females, aged 60.54±9.40 years in the minimally invasive group, and 487 males and 164 females, aged 62.31±8.64 years in the conventional group. After propensity score matching, there were 239 patients in each group. Compared with the conventional group, patients in the minimally invasive group had longer operation time, shorter drainage duration, less drainage volume on the first postoperative day, shorter postoperative hospital stay, and lower rate of positive inotropenic drugs use, while there was no statistical difference in the mean number of bypass grafts, ICU stay, ventilator-assisted time, blood transfusion rate or perioperative complications (P>0.05). During the median follow-up of 2.25 years, there was no statistical difference in major adverse cardiovascular and cerebrovascular events, including all-cause death, stroke or revascularization between the two groups (P>0.05). Conclusion Reasonable clinical strategies can ensure perioperative and mid-term surgical outcomes of MICS CABG not inferior to conventional CABG. In addition, MICS CABG has the advantages in terms of postoperative hospital stay, postoperative drainage volume, and rate of positive inotropic drugs use.

Objective:To investigate the effect of on-demand glucocorticoid strategy on the occurrence and outcome of porcine anti-lymphocyte globulin (p-ALG) -associated serum sickness in aplastic anemia (AA) .Methods:The data of AA patients who received in the Anemia Diagnosis and Treatment Center of Haematology Hospital, CAMS & PUMC from January 2019 to January 2022 were collected. Among them, 35 patients were enrolled in the on-demand group, with the glucocorticoid strategy adjusted based on the occurrence and severity of serum sickness; 105 patients were recruited in the usual group by matching the age and disease diagnosis according to 1∶3 ratio in patients who received a conventional glucocorticoid strategy in the same period. The incidences, clinical manifestations, treatment outcomes of serum sickness, and glucocorticoid dosage between the two groups were analyzed. Results:The incidences of serum sickness in the on-demand group and the usual group were 65.7% and 54.3% ( P=0.237) , respectively. The median onset of serum sickness was the same [12 (9, 13) d vs the 12 (10, 13) d, P=0.552], and clinical symptoms and signs, primarily joint, and/or muscle pain, fever, and rash were similar. Severity grades were both dominated by Grades 1-2 (62.8% vs 51.4%) , with only a few Grade 3 (2.9% vs 2.9%) , and no Grades 4-5. No significant difference in the serum sickness distribution ( P=0.530) . The median duration of serum sickness was the same [5 (3, 7) d vs 5 (3, 6) d, P=0.529], and all patients were completely cured after glucocorticoid therapy. In patients without serum sickness, the average dosage of prophylactic glucocorticoid per patient in the usual group was (469.48 ±193.57) mg (0 in the on-demand group) . When compared to the usual group, the average therapeutic glucocorticoid dosage per patient in the on-demand group was significantly lower [ (125.91±77.70) mg vs (653.90±285.56) mg, P<0.001]. Conclusions:In comparison to the usual glucocorticoid strategy, the on-demand treatment strategy could significantly reduce glucocorticoid dosage without increasing the incidence of serum sickness; in addition, the duration of serum sickness and the incidence of above Grade 2-serum sickness were similar.

ObjectiveTo observe the behavioral and pain threshold alterations, as well as the changes in indexes related to depression and pain in the serum and central system in mice stressed by maternal separation and chronic neuropathic pain, and explore the underlying mechanism of Wenyang prescription (WY), Jieyu prescription (JY), and Wenyang Jieyu prescription (WYJY) in improving depression and pain sensitivity. MethodThe birth date of mice was recorded as PD0. After birth, the mice were divided into a blank group and an experimental group. The neonatal mice in the experimental group underwent maternal separation in PD5-14 at 8 h·d-1. After ablactation, the mice were divided into a maternal separation group, a WY group (Erxian decoction, 5.84 g·kg-1), a JY group (Xiaoyaosan, 12.00 g·kg-1), a WYJY group (16.68 g·kg-1), and a fluoxetine group (2.60 mg·kg-1), with 15 mice in each group. Meanwhile, 15 male mice of the same age without maternal separation were assigned to the normal control group. Mice in the blank group and the maternal separation group were fed on a regular chow diet in PD21-PD90, while the remaining groups were fed on the corresponding drugs. In PD91, sciatic nerve ligation was performed to induce a model of maternal separation and chronic neuropathic pain. The open field test was used to observe the depression-like behaviors of mice in each group, and the mechanical and temperature pain thresholds were measured to detect the pain sensitivity of mice in each group. The serum levels of corticosterone (CORT), substance P, and β-endorphin (β-EP) were determined by enzyme-linked immunosorbent assay (ELISA), and the expression of the glucocorticoid receptor (GR) in the amygdala and β-EP protein in the hypothalamus was detected by immunohistochemistry. The mRNA expression levels of amygdala GR gene (Nr3c1), FK506 binding protein 5 gene (FKBP5), metabolic glutamate receptor 5 gene (GRM5), and brain-derived neurotrophic factor (BDNF) were detected by real-time fluorescence quantitative polymerase chain reaction(Real-time PCR). ResultCompared with the blank group, the maternal separation group showed reduced stay time and total distance traveled in the 5-min open field test (P<0.01), reduced mechanical pain threshold (P<0.01), increased serum CORT and β-EP (P<0.01), declining FKBP5 mRNA expression (P<0.01), and increased hypothalamic β-EP expression (P<0.05). Compared with the maternal separation group, the groups with drug intervention showed prolonged stay time (P<0.05, P<0.01) and up-regulated pain thresholds to different degrees. The total distance traveled in the 5-min open field test increased in the WY group, the WYJY group, and the fluoxetine group (P<0.05, P<0.01). The JY group showed decreased serum CORT (P<0.01), reduced β-EP , and increased BDNF mRNA (P<0.01). Nr3c1 and GRM5 mRNA decreased in the WY group (P<0.05, P<0.01). The WYJY group showed decreased serum CORT (P<0.05)and decreased Nr3c1, GRM5, and BDNF mRNA (P<0.05, P<0.01). The levels of β-EP expression were elevated to different degrees in the groups with drug intervention, but the differences were not significant. The levels of GR expression in the WY group, the JY group, and the WYJY group increased (P<0.05). ConclusionWYJY can inhibit central pain sensitization and regulate hypothalamic-pituitary-adrenal gland (HPA) axis function by enhancing the expression of GR in the amygdala and inhibiting neuroplasticity and excitability in the amygdala to relieve depression-like behaviors and improve somatic hyperalgesia.
