1.A cohort study of infant development and health in Tianjin

Objective:
To investigate the current situation of obesity and related metabolic abnormalities among preschool children, so as to provide theoretical support for future intervention.
Methods:
A cohort of 3 952 children, born in Tianjin and enrolled in the kindergarten from September 2017 to October 2018, were selected to conduct a baseline survey and a three-year follow-up (questionnaire survey, physical examination and laboratory testing). At the same time, a two-way cohort study was conducted to retrospectively collect maternal prenatal examination, delivery and regular physical examination information of children from birth to preschool age from Tianjin Maternal and Child Health Information System.
Results:
A total of 3 935, 3 654 and 2 739 children completed the follow up in the primary, middle and senior classes of kindergarten respectively. The height and weight of pre-school children increased with age, while the percentage of body fat decreased with age ( β-trend =-0.74, P <0.01). During three-year follow up, height, weight and body mass index of boys were higher than girls (P<0.05), while the percentage of body fat was lower than girls (primary class: 17.5%,18.5%; middle class: 16.4%,17.2%; senior class: 16.1%,17.1%, P <0.05). The detection rate of overweight (including obesity) and obesity increased with age( χ 2 trend were 15.51,38.72, P <0.05). The total detection rate of obesity increased from 5.4% at the baseline level to 9.6%. Laboratory test results showed that the detection rates of fasting blood glucose of boys were higher than that of girls in primary class, but blood lipid abnormalities were in the opposite (glucose: 7.7%, 4.8 %; lipid: 23.8% , 27.7%)( χ 2=12.01, 6.63, P <0.05).
Conclusion
The study has established a large growth cohort starting from the early embryonic stage, which will help to establish the strategies to promote children s health and prevent obesity and chronic diseases from multidimensional perspectives.
2.General population cohort study in Beijing-Tianjin-Hebei area: past and future

The general population cohort study in Beijing-Tianjin-Hebei area is a large-scale prospective longitudinal study conducted since 2017, which covers over 114 850 diverse populations from early life to senior age. Up to December 2021, 106 031 people had completed at least one follow-up, with an overall follow-up rate of 92.3%. Considering of the characteristics of the environmental and health problems in Beijing-Tianjin-Hebei area, in this cohort study we have focused on health problems of children and adolescents' growth and development, cardiometabolic diseases and their risk factors, aging and comorbidity, health hazards caused by air pollution. The long-term follow up of the general population cohort study in Beijing-Tianjin-Hebei area will contribute to establishing unique and invaluable data and biobank resource for scientific research. This paper aims to comprehensively describe the background, significance, current status and outcomes, limitations and challenges, and future plan and development of general population cohort study in Beijing-Tianjin-Hebei area, thus to provide reference for professionals both at home and abroad to carry out related research.
3.Relationship between prepregnancy body mass index weight gain during pregnancy and offspring body composition during preschool age

Objective:
To explore the relationship between prepregnancy body mass index, weight gain during pregnancy with preschool obesity and body composition in offspring, so as to provide evidence for gestational weight gain and childhood obesity prevention.
Methods:
A total of 1 333 preschool children were recruited from 3 kindergartens in Tianjin from September to December 2020. Structured questionnaire was used to collect children s lifestyle information. Height, weight and body fat mass of children were assessed, and body fat percentage (FM%), fat mass index (FMI) and non fat mass index (FFMI) were calculated. Maternal medical records were collected and the mothers were grouped according to their prepregnancy weight status and weight gain during pregnancy. χ 2 test, t test, linear regression model and Logistic regression were used to analyze the differences of obesity and body composition among different groups.
Results:
The prevalence of overweight and obesity in preschoolers was 12.7% and 7.7%. After adjusting maternal age and delivery, gestational age, gender, age and lifestyle of children, the correlation between maternal pre pregnancy BMI,gestational weight gain with obesity and body composition indexes of children in preschool age was statistically significant ( P <0.05). For mothers with normal weight before pregnancy, excessive weight gain during pregnancy increased risk of high FM% and high FMI in offspring ( OR=1.81, 1.68, P <0.05). There was no significant correlation between maternal weight gain during pregnancy with offspring obesity and body composition among mothers with prepregnant overweight or obesity.
Conclusion
Maternal weight status before pregnancy and weight gain during pregnancy are correlated with obesity and body composition in the preschool age of offspring. It is suggested that mothers should maintain appropriate weight status before and during pregnancy.
4.Validation of bioelectrical impedance analysis in measuring body composition of children aged from 3 to 6

Objective:To examine the concordance between bioelectrical impedance analysis (BIA) and dual energy X-ray absorptiometry (DXA) in determining body composition of children aged between 3 and 6.Methods:A total of 230 children aged 3-6 from a kindergarten in Jinnan, Tianjin were enrolled in this study from November 16 th to December 8 th, 2017.The soft lean mass (SLM) and the body fat mass (BFM) of the children were measured by using BIA and DXA.The children were divided into several groups by sex, age and body mass index (BMI). The consistency of the body composition results between BIA and DXA in each group was analyzed. Results:The SLM and BFM assessed by BIA were significantly correlated with those measured by DXA ( R2=0.951, 0.947, all P<0.001). The Bland-Altman plots suggested that the SLM measured by BIA was 0.70 kg(95% CI: -1.78-0.38) higher in that by DXA, and the BFM measured by BIA was 1.36 kg (95% CI: 0.19-2.52) lower in that by DXA.A similar trend could be seen in children stratified by age, sex and BMI.The SLM of boys and girls measured by BIA was 0.57 kg (95% CI: -1.63-0.49) and 0.84 kg (95% CI: -1.88-0.20) higher than those by DXA, respectively.The BFM of boys and girls measured by BIA was 1.23 kg (95% CI: 0.11-2.36) and 1.49 kg (95% CI: 0.34-2.64) lower than that by DXA, respectively.The SLM measured by BIA was 0.67 kg (95% CI: -1.74-0.41), 0.76 kg (95% CI: -1.65-0.13), 0.69 kg (95% CI: -1.85-0.47) and 0.67 kg (95% CI: -1.75-0.41) higher than those by DXA in the 3 to 6-year-old groups, respectively.The BFM measured BIA was 1.09 kg (95% CI: -0.12-2.30), 1.44 kg (95% CI: 0.60-2.28), 1.39 kg (95% CI: 0.15-2.64)and 1.43 kg(95% CI: 0.38-2.48)lower than that by DXA in the 3 to 6-year-old groups, respectively.Moreover, the smallest difference of the SLM and BFM were observed between BIA and DXA in the obese child.The SLM deviation between BIA and DXA was estimated within ± 0.39 kg and the BFM deviation was within ± 0.93 kg. Conclusions:There is high consistency between BIA and DXA in the assessment of the SLM and BFM of children aged from 3 to 6.
5.Post-stroke constipation treated with acupoint embedding therapy: a multi-center randomized controlled trial.

OBJECTIVE:
To verify the clinical effect of acupoint embedding therapy on post-stroke constipation.
METHODS:
The multi-central randomized controlled trial was adopted. 210 patients of post-stroke constipation were divided into an acupoint embedding group (105 cases, 4 cases dropped off) and a sham-embedding group (105 cases, 6 cases dropped off). In the acupoint embedding group, the acupoint embedding therapy was used at Tianshu (ST 25), Daheng (SP 15), Xiawan (CV 10), Zhongwan (CV 12), Qihai (CV 6), Guanyuan (CV 4) and Daju (ST 27). In the sham-embedding group, the sham-embedding therapy was given, in which, the acupoint selection, needle devices and manipulation were the same as the acupoint embedding group. But, no absorbable surgical suture was used in the needle tube. The treatment was given once every two weeks and 4 treatments were required in either group. It was to compare the weekly average complete spontaneous bowel movements (CSBMs) during treatment (from the 3rd to the 8th week) between the two groups, the weekly average spontaneous bowel movements (SBMs), Bristol stool form score (BSFS), the score of the patient assessment of constipation quality of life questionnaire (PAC-QOL) and the score of defecation difficulty before and after treatment.
RESULTS:
The percentage of the cases with weekly average CSBMs ≥ 3 times in the patients of the acupoint embedding group was higher markedly than the sham-embedding group [91.1% (92/101) vs 43.4% (43/99), <0.01]. Compared with the values before treatment, the weekly average SBMs and BSFS scores after treatment were all increased obviously in the two groups (<0.01), and PAC-QOL score and the score of defecation difficulty were reduced remarkably (<0.01). After treatment, the increase range of SBMs and BSFS scores, as well as the decrease range of PAC-QOL score and the defecation difficulty score in the acupoint embedding group were all higher than the sham-embedding group respectively (<0.05).
CONCLUSION
The acupoint embedding therapy remarkably increases the spontaneous bowel movements, improves in feces form and defecation difficulty and strengthens the quality of life in the patients of post-stroke constipation.

6.Analysis of dietary fiber intake in breakfast of primary school students in Gongshu district of Hangzhou

Objective:
To evaluate the daily average intake of dietary fiber of primary school students in Gongshu District, Hangzhou, in order to strengthen the diet guidance and health education.
Methods:
The method of stratified random cluster sampling was employed to carry out a dietary survey for 887 pupils from 2-6th grade selected from 4 primary schools in Gongshu District, Hangzhou City. The consumption of various kinds of breakfast in pupils’ diet was obtained through dietary survey. The content of dietary fibers in various kinds of food was determined by enzymatic-gravimetric method, and based on this, the daily average intake of dietary fiber of pupils in Gongshu District was calculated.
Results:
A total of 434 samples of 12 kinds of food in Gongshu District of Hangzhou were determined. The results showed that the dietary fiber content ranged from 0.31 to 2.17 g/100 g in all kinds of breakfast. The difference of dietary fiber content was statistically significant(F=76.50, P=0.00). Among them, noodles and soybean milk were found of more dietary fiber content. However, pupils’ dietary fiber intake for breakfast ranged from 1.82 to 3.04 g/100 g, and the intake was generally low, showing an increasing trend with the increase of grade. The difference of dietary fiber intake among the second to sixth grades was statistically significant (F=18.72, P=0.01). After comparison by SLD method, it was found that the difference of dietary fiber intake between the second and third grade (P=0.22), and between the fifth and sixth grade(P=0.30) were not statistically significant, but the others were statistically significant(P<0.05).
Conclusion
The unreasonable dietary structure and poor breakfast quality are common phenomena. Well-tailored education programs slaping of adequate and blanced dietary behaviors are in great needs.

In this study, retrospective multi-center study on medical records of Uyghur medicine diagnosis and treatment program of osteoporosis was done and demographic, diagnostic, therapeutic, efficacy evaluation and nursing data and other information were collected. The main and secondary disease manifestations of osteoporosis were analyzed inductively. Osteoporosis was divided into 2 types in Uyghur medicine field, which were abnormal Savda type and abnormal Balgham type. Uighur medicine syndrome type standards of osteoporosis were established. Treatment efficacy was evaluated according to bone pain, bone density, life quality, and improvement of clinical symptoms and Uyghur medical symptoms. On this basis, in accordance with the technical requirements of the national drug review, the study developed clinical research guidelines for the treatment of osteoporosis with new Uyghur medicine (draft).

The main and secondary disease manifestations of hyperlipidemia were analyzed inductively and hyperlipidemia were divided into 4 types in Uyghur Medicine field, which were surplus and sticky Kan type, Chuchumal Balgham type, Gaisiman Balgham type, and Koygan Savda type. Therefore, Uighur Medicine syndrome type standards of hyperlipidemia were established. Treatment efficacy was evaluated with the combination of laboratory observation parameters and main and secondary disease manifestations. This research conducted retrospective multi-center study on medical records of Uyghur Medicine diagnosis and treatment program of shingles, and collected information about demography, diagnostics, and therapeutics, efficacy evaluation and nursing to conclude and analyze the main syndromes and sub-disease performance. Shingles was divided into 3 types in Uyghur Medicine field: abnormal blood type shingles, abnormal bile liquid type shingles, and abnormal black choledochal typeshingles. Uighur Medicine syndrome type standards of shingles were established. Rash suspended time, rash dry scab time, the pain start to ease time interval, time completely pain, pain bounce rate, the incidence of postherpetic neuralgia, and PHN recovery time were evaluated. Disease and syndrome clinical criteria of shingles were established by evaluating Uygur Medical symptoms improvement, improvement of patients’ life quality and the changes in patients’ internal environment. On this basis, in accordance with the technical requirements of the national medicine review, the research developed clinical research guidelines for the treatment of shingles with new Uyghur Medicine (draft).

In this article, retrospective multi-center study on medical records of Uyghur Medicine diagnosis and treatment program of acne vulgaris was done and demographic, diagnostic, therapeutic, efficacy evaluation and nursing data and other information were collated and analyzed. The main and secondary disease manifestations of acne vulgaris were analyzed inductively and acne vulgaris was divided into 3 types in Uyghur Medicine field, which are blood-type acne vulgaris, yellow bile-type acne vulgaris, and astringent mucus-type acne vulgaris. Uyghur Medicine syndrome type standards of acne vulgaris were established. At the same time, through evaluating skin integrity and morphology of lesions as well as changes in patients’ symptoms and quality of life and observing changes in patients’ internal environment respectively before and after treatment, disease and syndrome clinical criteria of acne vulgaris were established. On this basis, in accordance with the technical requirements of the national medicine review, this study developed clinical research guidelines for the treatment of acne vulgaris with new Uyghur Medicine.
