1.Importance of a Diversity Committee in Advancing the Korean Society of Gastroenterology: A Survey Analysis

BACKGROUND/AIMS: The numbers of women, young doctors, and foreigners in the medical field have increased continuously. On the other hand, the environment for these minority groups has not improved, particularly in Eastern countries. The authors aimed to increase the awareness of the importance of a Diversity Committee in the Korean Society of Gastroenterology (KSG) by an analysis of a survey. METHODS: From January to February in 2019, a survey was conducted on physicians and a few medical students by googling. The questionnaire consisted of the target doctors of the Diversity Committee, purpose, specific activities, and expected effects of Diversity Committee to the KSG. The participants requested to respond with yes/no or a 5-point scale. RESULTS: A total of 202 participants completed the questionnaire, and 93.5% (189/202) were medical specialists. The proportion of males was 61.9% (125/202), and 39.6% (80/202) and 36.1% (73/202) participants were in their 30s and 40s, respectively. A total of 174 participants (86.1%) agreed with the necessity of a Diversity Committee, and 180 participants (89.1%) answered this committee would help advance the KSG with significant differences between males and females (80.8% vs. 94.8%, p=0.006; 84.8% vs. 96.1%, p=0.011). Similarly, there were significant differences in the responses according to sex in most questions. CONCLUSIONS: Most participants of the survey expected a contribution of the Diversity Committee to the advancement of the KSG. On the other hand, in most of the priorities of the target, purpose, specific activities, and expected effects of the Diversity Committee, there was a difference in the perceptions between males and females. Therefore, continuous efforts are needed to reduce the differences within the KSG.

3.Development of a Self-Care Performance Scale for Patients with Stomach Cancer after Gastrectomy.

PURPOSE: This study was to develop a valid and reliable self-care performance scale for stomach cancer patients with gastrectomies. METHODS: The initial items for the self-care performance scale were derived by a literature review and in-depth interviews with stomach cancer patients with gastrectomies and a conceptual framework based on Orem's theory was used. The identified items were evaluated for content validity by experts. The main survey with refined items was performed with a sample of 232 patients. The data were analyzed using item analysis, Pearson correlation coefficients, Cronbach's alpha and factor analysis. RESULTS: A 22 item instrument with 7 factors was developed and the total variance explained was 66.9%. The factors were 'manage dietary restriction', 'avoid hazards', 'rest and elimination', 'manage social support and stress', 'regulation diet', 'meal portion control' and 'seek information and follow-up care'. The internal consistency, Cronbach's alpha was .86 and reliability of the subscale ranged from .65 to .82. CONCLUSION: This study suggested that the self-care performance scale is a reliable and valid instrument to measure self-care performance for patients with stomach cancer after gastrectomy.

4.Effects of Frozen Gauze with Normal Saline on Thirst and Oral Health of the Patients with Nasal Surgery.

PURPOSE: The purpose of this study was to examine the effects of frozen gauze with normal saline on thirst and the oral health of patients with nasal surgery. METHODS: A quasi-experimental, nonequivalent control group pretest-post test design was used. Participants (n=52) received either gauze frozen with normal saline (n=26), or wet gauze (n=26). The subjective thirst level and oral health of the participants were assessed before the intervention, 30 minutes after the first intervention, 30 minutes after the second intervention, and 30 minutes after the third intervention. RESULTS: After oral hygiene was provided twice, the thirst level was improved in patients receiving the gauze frozen with normal saline. After oral hygiene was provided a third time, the thirst level was improved in patients receiving the gauze frozen with normal saline. CONCLUSION: Gauze frozen with normal saline can be effective for oral hygiene in reducing the thirst level and improving the oral health in nasal surgery patients.

5.A survey on the appropriate level of premiums for National Health Insurance.

As the National Health Insurance Law was established in 2000, the finances of workplace health insurance and regional health insurance were integrated. However, since employee health insurance (workplace insurance) and self-employed health insurance (community insurance) still use different contribution rating systems, the problem of inequity of premiums has caused ongoing controversy. Therefore, to examine the public's satisfaction with premiums calculation methods, and collect opinions on solving the problem of inequity of premiums, this survey was performed. A computer-assisted telephone interview with multi-stage stratified sampling of the population in Korea was performed. Responses were obtained from 1,016 people aged 25 to 64 years. The analyses showed that, to the question about the premiums calculation methods, more respondents answered that they were unsatisfied (41.4%) than satisfied (36.6%). The responses differed depending on the type of health insurance. The proportion of those who were satisfied with their insurance was relatively higher among those with employee health insurance. On the other hand, among those with self-employed health insurance, the proportion of those who were dissatisfied with insurance was 3 times more common than satisfaction with it, showing that those who belong to the self-employed health insurance group are less satisfied with the premiums calculation methods. It was also found that the majority people perceive that the current single payer health insurance system should be divided into mutually separate employee and self-employed health insurance systems or a multiple payer health insurance system (60.3%). To achieve premium equity in the future, the Health Insurance Corporation should make a greater effort to determine income levels correctly. In the institutional improvement process of the premiums contribution rating system in the future, it seems necessary to carry out further research on a multiple payer insurance system reflecting the attitudes and demands of the people.

6.A survey of the medical practitioners' offices in Korea 2011.

Because of various current problems and issues which are low health insurance medical fee of National Health Insurance Corporation, extreme competition with very large general hospitals, new supply of more than of 3,000 new doctors, many medical practitioner's offices who are suffering management difficulties are increasing. In order to investigate their current overall management situations, we made this study through statistical extraction procedures with the sample population of the medical practitioners which are registered in Korean Medical Association and subsequently we made the study with 1,031 selected cases. The average number of patients is 53.6 patients per day. 44.4% of the medical practitioners' offices are working at night and legal holidays and 8.5% are working at Sunday. Average working hours is 50.1 hours per week, which greatly exceeds 40 hours per week that is regulated in the Labor Standards Act. According to the management performance analysis of those medical clinics through profit and loss statements, average total annual sales revenue in 2010 fiscal year was 444,167,867 KRW, the expenses were 314,217,081 KRW and the earnings before taxes was 129,940,786 KRW. The average net profit (earnings before taxes) of the director of the medical practitioners' offices was 122,337,868 KRW per year and 10,194,822 KRW per month. According to the study results, we have found that we need to increase doctor's bill for outpatient, and establishment and its improvement of medical service delivery systems and classification standards of medical services for first and succeeding outpatients. Considering overall results of the study, readjustment of outpatients' treatment fees and reestablishment of more efficient medical service delivery systems which require the first medical service is to be provided properly should be realized in order to improve the management performance of the medical practitioners' offices.

7.Financial state of primary care physicians under the Korean insurance system.

Greater than its influence on the medical practitioner's individual ability is the National Health Insurance System's influence on the management of medical practitioners' offices in Korea. However, despite the important effect health insurance exerts on the income of medical clinics, recently, the financial difficulties of medical clinics have often become an issue, and financial difficulty has been aggravated as much as a solution has been sought. The current state of the overall management of medical clinics was investigated to understand the factors influencing the sales and expenses in their management. A questionnaire was completed by 1,009 physicians registered in the Korean Medical Association who were participating in a statistical extraction course. As a result of the study, the factors influencing the total revenue and total expenditures of medical clinics, such as increases in the total number of doctors, increasing numbers of outpatients, the size of the medical office, medical disputes, and clinical specialties (based on the first medical treatment) showed statistical significance. In conclusion, in order to improve medical clinic management, a health insurance medical fee should be more reasonably fixed, a medical transfer system should be reestablished, and a cooperative strategy should be created for medical clinics and general hospitals in order to attract patients. As a result, low cost and highly efficient medical services could be provided and the satisfaction of patients improved.

8.Effects of Self-Efficacy Promotion Program on Self-Efficacy, Self-Care Behavior, and Quality of Life in Breast Cancer Patients Receiving Radiotherapy.

PURPOSE: The purpose of this study was to examine the effects of self-efficacy promotion programs on self-efficacy, self-care behavior, and quality of life in breast cancer patients receiving radiation therapy. METHODS: This study was conducted from November 1, 2009 to December 10, 2010. A total of 39 breast cancer patients, who received radiotherapy in a University hospital located in city B, participated in the study and were divided into two groups (experimental group 17 and control group 22). The data were analyzed with the chi2-test and ANCOVA using the SPSS/Win 17.0 program. RESULTS: Self-efficacy promotion programs aimed at self-management were effective in enhancing concrete self-efficacy but not effective in promoting general self-efficacy, self-care behavior, and quality of life among breast cancer patients who have received radiotherapy. CONCLUSION: Organizing a support group for the breast cancer patients seems to be highly necessary to help themselves obtain higher level of specific self-efficacy, self-care behavior, and quality of life in general. It will also be beneficial for the breast cancer patients to understand their unique situations and improve their health problems for themselves.

9.Factors Influencing Organizational Commitment among Hospital Nurses.

PURPOSE: The purpose of this study is to examine the effective factors of organizational commitment among hospital nurses. Method: The subjects were 250 registered nurses working in 3 hospitals in Busan. Data were obtained by self-reported questionnaires from the 10th of June to 30th of June, 2009. Data were analyzed by frequency and percentage, t-test, ANOVA and Sheffe's test and stepwise multiple regression. Result: Mean of job satisfaction was 60.56+/-9.25 (range of scale: 20-100), internal marketing was 69.49 (range of scale: 25-125), job stress obtained 100.60+/-17.93 (range of scale: 45-180), and organizational commitment got 47.78+/-6.84 (range of scale: 15-75). The extent of the organizational commitment had differences according to the age (F=3.300, p=.039), educational level (F=3.21, p=.042), religion (F=3.30, p=.021), position (F=5.837, p=.003), and career length (F=2.642, p=.035). The influencing factors in organizational commitment of hospital nurses were job satisfaction, internal marketing, and age. R2=.503. The job satisfaction has the highest significant predictor of organizational commitment. CONCLUSION: The influencing factors on organizational commitment were job satisfaction, internal marketing and age. Thus it is needed to improve the job satisfaction and internal marketing of hospitals' nurses and to provide care and support to younger nurses.

10.Belief, Knowledge, and Practice about Oral Health Care of Middle-aged Women.

PURPOSE: This study was performed to identify the relationship between belief, knowledge, and practice about oral health care of middle-aged women, and to prepare baseline data for developing a dental health education and promotion program. METHOD: For the survey, 120 individuals(middle-aged women) were chosen by convenience sampling and agreed to participate in the study. Their belief, knowledge, and practice about oral health care were measured. For descriptive statistics, t-test, ANOVA, Tukey and Pearson's correlation coefficient were used with SPSS Win 14.0. RESULT: The level of belief and practice about oral health care was middle for the subjects. However, the level of knowledge about oral health care was relatively high. The level of practice about oral health care related to characteristics of subjects showed significant differences according to level of education and oral health status. There was no significant correlation between the level of practice and the belief about oral health care. The practice about oral health care showed a significant positive correlation with knowledge. CONCLUSION: These findings demonstrate a need for the development of oral health care programs that is effective in improving belief, knowledge, and practice about oral health care reported by middle-aged women.
