ObjectiveTo investigate the recurrence of ischemic stroke (IS) and to analyze the association between four indices of total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), and triglycerides (TG) and the risk of IS recurrence by analyzing the follow-up data related to IS in the community-based natural population of Songjiang District, Shanghai, so as to provide a scientific basis for improving the prognosis of stroke patients in the community and controlling IS recurrence. MethodsA prospective follow-up study was conducted among the IS patients in the community-based cohort population, collecting data about patient’s age, gender, disease history, biochemical indicators, and etc. Cox regression model and restricted cubic spline model were used to analyze the relationship between different levels of plasma lipids and the recurrence of IS in these patients. ResultsA total of 1 368 patients with IS were included. The total follow-up duration was 7 171.46 person-years, with a median follow-up time of 6.24 years. There were 420 cases of IS recurrence, resulting in a cumulative recurrence rate of 30.70%. The results of multivariate Cox regression analysis showed that the recurrence risk of IS was reduced when the baseline TC and LDL-C levels of IS patients were in the ranges of 4.65‒5.67 mmol·L-1 and 2.52‒3.46 mmol·L-1, respectively. The results of restricted cubic spline analysis showed a U-shaped relationship between baseline TC and LDL-C levels and the recurrence risk in IS patients. ConclusionThe cumulative recurrence rate of patients with IS in the community of Songjiang District in Shanghai is high, and the levels of TC and LDL-C at baseline survey are correlated with the recurrence of IS in these patients. It is suggested to pay more attention to the levels of LDL-C and TC in patients with IS, so as to improve the prognosis.

BACKGROUND:With the aging of the global population, the incidence rate of osteoporosis is also increasing. It is very important to further understand its pathogenesis and propose new therapeutic targets.Recent studies have shown that ferroptosis is closely related to the pathogenesis of some bone diseases, such as inflammatory arthritis, osteoporosis and osteoarthritis. OBJECTIVE:To summarize the previous studies on the mechanism of ferroptosis in osteoporosis,so as to provide new therapeutic ideas and potential therapeutic targets for osteoporosis. METHODS:The first author used the computer to search the documents published from 2000 to 2022 in CNKI,WanFang,VIP,PubMed and Web of Science with the key words of"ferroptosis, osteoporosis, osteoblasts, osteoclasts, iron chelators, reactive oxygen species, nuclear factor erythroid 2-related factor 2,heme oxygenase-1,glutathione peroxidase 4,review"in Chinese and English.A total of 70 articles were finally included according to the inclusion criteria. RESULTS AND CONCLUSION:Ferroptosis is significantly different from necrosis, apoptosis and autophagy. In terms of cell morphology and function,it does not have the morphological characteristics of typical necrosis, nor does it have the characteristics of traditional apoptosis, such as cell contraction, chromatin condensation, the formation of apoptotic bodies and the disintegration of cytoskeleton.Contrary to autophagy, ferroptosis does not form a classical closed bilayer membrane structure(autophagic vacuole).Morphologically, ferroptosis is mainly manifested by obvious contraction of mitochondria, increased membrane density, and reduction or disappearance of mitochondrial cristae, which are different from other cell death modes.Iron overload can destroy bone homeostasis by significantly inhibiting osteogenic differentiation and stimulating osteoclast formation, leading to osteoporosis.Iron overload interferes with the differentiation of stem cells to osteoblasts, leading to a weakened osteoblast function and further imbalance of bone metabolism in the body, which eventually leads to osteoporosis.Stimulated by iron overload, osteoclast bone resorption is enhanced and bone loss exceeds new bone formation.Iron chelators have been proved to have osteoprotective effects by inhibiting osteoclast activity and stimulating osteogenic differentiation of osteoblasts. Its potential mechanism is related to inhibiting osteoclast differentiation and promoting osteoblast differentiation.Antioxidants can prevent reactive oxygen species production and inhibit bone absorption, thus improving bone metabolism and effectively preventing osteoporosis.

BACKGROUND:Minipigs are often used in research fields such as skin injury, vascular trauma and cosmetic medicine because they are highly similar to human beings in terms of skin tissue structure and cardiovascular system.Hydrogel as a wound repair drug possesses a variety of excellent physicochemical properties such as strong water retention and adhesion, which can provide isolation moisturization and drug release for wounds. OBJECTIVE:To summarize and conclude the progress of the application of trauma models for different experimental purposes of hydrogel therapy for minipigs,to reveal the development status of various types of minipig trauma models,to analyze the deficiencies of minipig trauma models at the present stage. METHODS:The relevant articles published in Web of Science database and CNKI database from the establishment of each database to 2023 were checked, using"piglet, miniature pig, minipig, miniature pig;gel, hydrogel;trauma, injury, wound, lesion, incision"as Chinese search terms and"Miniature Swine,Miniature pig, minipig;gel, hydrogel;injury, wound, lesion, incision"as English search terms.A total of 438 Chinese and English documents were retrieved, and 59 documents were included in the study through the inclusion and exclusion criteria. RESULTS AND CONCLUSION:(1)At present, the main models used clinically for trauma repair are large animal species(dogs and pigs),rabbits, and rodents(rats and mice).Because the skin structure of the minipig is more like that of humans, the minipig is the most ideal animal model for trauma repair.(2)In the in-vitro skin injury model, skin defect model is the basic wound model, which can be divided into full skin defect model and medium-thickness skin defect model according to the depth of the wound defect.Burn wound model and infected wound model are multidimensional models with hot metal scald and bacterial culture imposed on the basis of the skin defect model, which have the advantages of high safety coefficient and low operation difficulty.(3)In the in-vivo trauma repair model,mini-pigs are used as esophageal cricothyrotomy model which is more in line with the pathological state of clinical diseases.Mini-pigs are used in the gastric perforation and vascular hemostasis model, which can visually demonstrate the stronger organ adhesion, hemostatic properties and tissue regeneration-promoting effects of the hydrogel.(4)The specific parts of the pig also has the corresponding mode of use:pig ear is usually used to evaluate the hydrogel drug delayed-release effect.Porcine cellular proteins and pig skin collagen are mostly used to prepare composite hydrogels of tissue origin.

OBJECTIVE:As one of the serious complications after total hip arthroplasty or total knee arthroplasty, periprosthetic joint infection has always been the focus of joint surgeons.Albumin/globulin ratio is emerging as a promising biomarker for the diagnosis of periprosthetic joint infection. This study aimed to investigate the diagnostic value of albumin/globulin ratio in diagnosing periprosthetic joint infection. METHODS:A systematic search was conducted in PubMed,Embase, and Cochrane Library databases on July 29,2023.The literature was screened according to the inclusion and exclusion criteria, and the literature related to diagnostic trials of albumin/globulin ratio was included in this study.QUADAS-2 method was used to evaluate the quality of the included articles.Bivariate mixed-effect model was applied to combine the sensitivity, specificity, likelihood ratio, and diagnostic odds ratio of the included articles, and the receiver operating characteristic curve and area under the curve were integrated to specifically evaluate the accuracy of D-dimer in the diagnosis of periprosthetic joint infection.Subgroup analysis was used to explore the sources of heterogeneity. RESULTS:We included eight eligible diagnostic studies, all of which were of medium to high quality. The sensitivity and specificity of the combined diagnosis were 0.78(95%CI:0.66-0.86)and 0.83(95%CI:0.78-0.88),respectively.The combined positive and negative likelihood ratios were 4.63(95%CI:3.43-6.22)and 0.27(95%CI:0.17-0.42),respectively.The combined diagnostic scores and diagnostic odds ratio were 2.85(95%CI:2.23-3.48)and 17.35(95%CI:9.29-32.45),respectively.The area under the summary receiver operating characteristic curve was 0.88(95%CI:0.85-0.90). CONCLUSION:The albumin/globulin ratio is of guiding significance in the diagnosis of periprosthetic joint infection. However,it is not universal in clinical practice,so it should be combined with specific clinical practice. In addition, there are relatively few studies on the threshold of albumin/globulin ratio, different sampling types, different laboratory detection methods, and different races,so more prospective clinical trials with large samples,multi-centers,and scientific design should be carried out for verification in the future.
