1.Relationship between three-dimensional measurement of acromion coverage and degenerative full-thickness rotator cuff tears

BACKGROUND:Acromion morphology is the most important external factor of degenerative rotator cuff tear,and studies on acromion morphology and acromion coverage have been continuing. OBJECTIVE:To investigate the coverage of acromion on rotator cuff tissue in three-dimensional space,measure coronal and sagittal parameters,and explore the relationship of acromial overhang,acromioglenoid angle and acromial coverage angle with degenerative full-thickness rotator cuff tears. METHODS:A total of 56 patients diagnosed with full-thickness rotator cuff tears by shoulder arthroscopic surgery or MRI in the Affiliated Hospital of Xuzhou Medical University from January 2022 to June 2023 were retrospectively collected as the tear group,while 48 patients with non-rotator cuff tear were included as the control group.Clinical data and chest CT data of the patients of the two groups were collected,and the shoulder blades of the patients were reconstructed using Mimics software.Acromial overhang,acromioglenoid angle and acromial coverage angle were measured in 3-matic software.The differences between the measurement parameters were compared between the two groups.Binary Logistic regression analysis and receiver operating characteristic curve analysis were performed to evaluate the ability of the measurement parameters to predict rotator cuff tear.It is generally believed that the parameter had good diagnostic value when the area under the receiver operating characteristic curve was greater than 0.8. RESULTS AND CONCLUSION:(1)The mean acromial overhang in patients with rotator cuff tears was greater than that in controls(P<0.001).The mean acromial coverage angle was also greater in patients with rotator cuff tears than in controls(P<0.001).There was no significant difference in the acromioglenoid angle between the two groups(P>0.05).(2)Binary Logistic regression analysis showed that acromial overhang and acromial coverage angle were both risk factors for rotator cuff tear(P<0.05).(3)Receiver operating characteristic curve analysis indicated that the areas under the curve of acromial overhang and acromial coverage angle were 0.725 and 0.865,respectively,and the optimal cutoff values were 36.15 mm and 60.65°.(4)The results showed that the acromial overhang and acromial coverage angle were greater in patients with rotator cuff tears than in patients without rotator cuff tears.Both were associated with degenerative full-thickness rotator cuff tears,and they could be used to predict rotator cuff tears,and the acromial coverage angle was more effective in predicting rotator cuff tears.
2.Clinical Observation on the Nape Needling Combined with Swallowing Dysfunction Therapeutic Apparatus in the Treatment of Swallowing Dysfunction After Cerebral Infarction

Objective To observe the clinical effect of nape needling combined with swallowing dysfunction therapeutic apparatus in the treatment of patients with swallowing dysfunction after cerebral infarction.Methods Eighty patients with swallowing dysfunction after cerebral infarction were randomly divided into observation group and control group,40 cases in each group.On the basis of routine treatment,the control group was treated with swallowing dysfunction therapeutic apparatus,and the observation group was treated with nape needling on the basis of the control group for 4 weeks.After 1 month of treatment,the clinical efficacy of the two groups was evaluated,and the changes of the water swallowing test(WST)score and the standard swallowing assessment(SS A)score were observed before and after treatment in the two groups,as well as the changes of the vertical and horizontal movement of the hyoid bone during swallowing in the two groups.The changes of serum substance P(SP)content were compared before and after treatment between the two groups.The safety and adverse reactions of the two groups were evaluated.Results(1)The total effective rate was 90.00%(36/40)in the observation group and 67.50%(27/40)in the control group.The curative effect of the observation group was superior to that of the control group,and the difference was statistically significant(P<0.05).(2)After treatment,the WST and SSA scores of the two groups were significantly improved(P<0.05),and the improvement of WST and SSA scores in the observation group was significantly superior to that in the control group,the difference was statistically significant(P<0.05).(3)After treatment,the vertical and horizontal displacement of the hyoid bone in the two groups were significantly improved(P<0.05),and the improvement of the vertical and horizontal displacement of the hyoid bone in the observation group was significantly superior to that in the control group,the difference was statistically significant(P<0.05).(4)After treatment,the serum SP level of the two groups was significantly improved(P<0.05),and the observation group was significantly superior to the control group in improving the serum SP level,the difference was statistically significant(P<0.05).(5)The total incidence of adverse reactions in the observation group was 5.00%(2/40);the control group was 25.00%(10/40);the incidence of adverse reactions in the observation group was significantly lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion Nape needling combined with swallowing dysfunction therapeutic apparatus in the treatment of swallowing dysfunction after cerebral infarction can significantly improve the swallowing function of patients,improve the serum SP level of patients,increase the displacement distance of hyoid bone,and reduce the incidence of adverse reactions.

4.Precise application of sleeve gastrectomy.

With the improvement of the national economy and living standards, the prevalence of obesity and related metabolic diseases is increasing yearly. The treatment effect of traditional diet control, physical exercise, and drug therapy are not ideal. Metabolic and bariatric surgery is not only aimed at weight loss, but also improves or resolves a variety of metabolic comorbidities, such as type 2 diabetes, hypertension, sleep apnea syndrome, and hyperlipidemia, enhancing the quality of life of patients. Surgical methods are also updated and iterated with the continuous development of metabolic surgery for weight loss. At present, more than 50% of the mainstream procedures are laparoscopic sleeve gastrectomy (LSG). The application of LSG should focus on precise surgical operations and indications to avoid or reduce adverse reactions and complications, so as to maximize the effect of bariatric surgery. At the same time, standardized postoperative management, multidisciplinary team (MDT) and in-hospital database construction, dietary nutrition and exercise guidance after discharge, and regular follow-up review are still important guarantees for the weight loss effect.

Monkeypox is a zoonotic disease.Previous studies have shown that children are vulnerable to monkeypox and are also at high risk for severe disease or complications.In order to improve pediatricians′ understanding of monkeypox and achieve early detection, early diagnosis, early treatment and early disposal, the committee composed of more than 40 experts in the related fields of infectious diseases, pediatrics, infection control and public health formulate this expert consensus, on the basis of the latest clinical management and infection prevention and control for monkeypox released by the World Health Organization (WHO), the guidelines for diagnosis and treatment of monkeypox (version 2022) issued by National Health Commission of the People′s Republic of China and other relevant documents.During the development of this consensus, multidisciplinary experts have repeatedly demonstrated the etiology, epidemiology, transmission, clinical manifestations, laboratory examinations, diagnosis and differential diagnosis, treatment, discharge criteria, prevention, case management process and key points of prevention and control about monkeypox.

Since December 2019, severe acute respiratory syndrome coronavirus 2(SARS-CoV-2) infections have raged globally for more than 2 years.China has always adopted scientific and effective prevention and control measures to achieved some success.However, with the continuous variation of SARS-CoV-2 cases and imported cases from abroad, the prevention and control work has become more difficult and complex.With the variation of the mutant strain, the number of cases in children changed, and some new special symptoms and complications were found, which proposed a new topic for the prevention and treatment of SARS-CoV-2 infection in children in China.Based on the third edition, the present consensus according to the characteristics of the new strain, expounded the etiology, pathology, pathogenesis, and according to the clinical characteristics and experience of children′s cases, and puts forward recommendations on the diagnostic criteria, laboratory examination, treatment, prevention and control of children′s cases for providing reference for further guidance of effective prevention and treatment of SARS-CoV-2 infection in children in China.

At present, severe acute respiratory syndrome coronavirus-2(SARS-CoV-2)infection is still rampant worldwide.As of September 10, 2021, there were about 222 million confirmed cases of corona virus disease 2019(COVID-19)and more than 4.6 million deaths worldwide.With the development of COVID-19 vaccines and the gradual vaccination worldwide, the increasing number of cases in children and unvaccinated young people has drawn attention.According to World Health Organization surveillance data, the proportion of COVID-19 infection cases in children gradually increased, and the proportion of cases in the age groups of under 5 years and 5-14 years increased from 1.0% and 2.5% in January 2020 to 2.0% and 8.7% in July 2021, respectively.At present, billions of adults have been vaccinated with various COVID-19 vaccines worldwide, and their protective effects including reducing infection and transmission, reducing severe disease and hospitalization, and reducing death, as well as high safety have been confirmed.Canada, the United States, Europe and other countries have approved the emergency COVID-19 vaccination in children and adolescents aged 12 to 17 years, and China has also approved the phased vaccination of COVID-19 vaccination in children and adolescents aged 3 to 17 years. For smooth advancement and implementation of COVID-19 vaccination in children, academic institutions, including National Clinical Research Center for Respiratory Diseases, National Center for Children′s Health, and The Society of Pediatrics, Chinese Medical Association organized relevant experts to reach this consensus on COVID-19 vaccination in children.

Severe acute respiratory syndrome coronavirus-2(SARS-CoV-2)infection is still worldwide.As a vulnerable group, severe and dead pediatric cases are also reported.Under this severe epidemic situation, children should be well protected.With the widespread vaccination of SARS-CoV-2 vaccine in adults, the infection rate have decreased.Therefore, SARS-CoV-2 vaccine inoculation for children groups step by step is of great significance to the protection of children and the prevention and control of corona virus disease 2019(COVID-19) as a whole.But the safety of children vaccinated with SARS-CoV-2 vaccine is a main concern of parents.Therefore, in order to ensure the safety of vaccination and the implementation of vaccination work, National Clinical Research Center for Respiratory Diseases, National Center for Children′s Health and the Society of Pediatrics, Chinese Medical Association organized experts to interpret the main issue of parents about SARS-CoV-2 vaccine for children, in order to answer the doubts of parents.
9. Research Progress on Effect of Traditional Chinese Medicine on Blood-brain Barrier Permeability

The blood-brain barrier is located between blood vessels and brain parenchyma and is a composite tissue composed of brain capillary endothelial cell,astrocyte end foot,pericyte,basement membrane and their tight connections.The blood-brain barrier acts as an important barrier between the blood-brain. It strictly restricts the exchange of blood and brain tissue. On the one hand,it allows the nutrients required by the brain tissue to enter the barrier. On the other hand,the substance that damages the brain tissue is restricted from entering the barrier to maintain the stability of the neuron microenvironment. The barrier function of the blood-brain barrier also limits the concentration of drugs that treat certain diseases into the brain or reduces the concentration of drugs into the brain,affecting the treatment of certain diseases.Studies have found that Chinese medicine has the effect of regulating the permeability of the blood-brain barrier. For example, Chinese herbal medicines such as Moschus,borneol,Styorax,and Benzoinum can increase the permeability of the blood-brain barrier,and can help its medicine into the brain. Its role is mainly related to the reduction of the blood-brain barrier tight junction,the inhibition of the blood-brain barrier transporter,and the inhibition of the active transport of ion channels.Orifice-opening medicinal such as borneol and Moschus can reduce the permeability of the blood-brain barrier.Certain tonifying and replenishing medicinal such as Ginseng Radix et Rhizoma,Astragali Radix,and Paeoniae Radix Alba can also reduce the permeability of the blood-brain barrier to protect the blood-brain barrier and brain tissue.Its role is mainly related to the regulation of inflammatory reactions,increased expression of tight junction-associated proteins,and promotion of vascular endothelial proliferation.The two-way regulation of blood-brain barrier permeability by traditional Chinese medicine(TCM) may be the basis for the prevention and treatment of brain diseases by TCM.In this paper,we systematically sort out the relevant literatures on the study of the permeability of blood-brain barrier and its mechanism of action in Chinese medicine in recent years,and carry out in-depth combing and summary,in order to provide a scientific basis for the clinical application of TCM to prevent and treat brain diseases.
10.Clinical observation on the treatment of periprosthetic infection of coagulase-negative staphylococci by two-stage revision.

OBJECTIVETo investigate the risk factors for the failure in treating periprosthetic infection of coagulase-negative staphylococci by two-stage revision.
METHODSFrom January 2005 to June 2015, 57 patients with periprosthetic hip and knee joint infection of coagulase-negative staphylococcus by two-stage revision were retrospectively reviewed with an average age of (61.3±11.9) years old. According to the drug resistance of methicillin, the patients were divided into methicillin sensitive group(MSCoN) and methicillin resistance(MRCoN) group, 25 cases in MSCoN group(9 knees and 16 hips) included 12 males and 13 females, 32 cases in MRCoN group(11 knees and 21 hips) included 14 males and 18 females. Follow-up for at least 2 years, the inflammatory markers, incidence rate of sinus and the duration of the symptoms, reinfection or persistent infection rate after two-stage revision were compared between two groups.
RESULTSMSCoN group and MRCoN group were followed up(81.7±38.3) months and(65.9±33.8) months, respectively;23 cases and 27 cases were successfully treated;there was no significant difference between two groups(=0.643). The patients who had surgery history were 4.04 times higher of failure than the patients without a history of surgery[OR=4.04, 95%CI(0.62, 26.5)]. Patients who had sinus were 4.26 times higher of failure than the patients without sinus[OR=4.26, 95%CI(0.7, 25.9)].
CONCLUSIONSTwo-stage revision is an effective procedure in treating patients infected by MSCoN and MRCoN. There is no significant difference of treatment failure rate between MSCoN and MRCoN group by two-stage revision. Surgery history and sinus maybe the risk factors of treatment failure, while methicillin-resistance is not.
