1.Effect of ANAs on hormone response in patients with AIH-PBC overlap syndrome and AIH-only

		                        			Objective To investigate the effect of antinuclear antibodies(ANAs)on hormone response in patients with autoimmune hepatitis(AIH)-primary biliary cholangitis(PBC)overlap syndrome(AIH-PBC OS)and AIH-only within half a year.Methods A retrospective analysis of 77 patients with autoimmune liver disease(AILD)admitted to First Clinical Medical College of Lanzhou University from January 2018 to December 2021,all of whom were confirmed by liver biopsy and receiving glucocorticoid treatment.Among them,46 patients were in AIH-PBC OS group and 31 were in AIH-only group.The general clinical characteristics,liver puncture-related indexes,autoantibodies and immunoglobulin indexes of patients in each group at the time of diagnosis were collected and compared,and the biochemical and immunoglobulin indexes of patients at the time of hormone use and at the time of review within 6 months were also collected,and the hormone response within 6 months was evaluated according to the levels of glutamic transaminase(AST),glutamic alanine transaminase(ALT)and immunoglobulin G(IgG),and the effect of ANAs on hormone response outcomes in both groups over a six-month period was analyzed.Multifactorial ordered logistic analysis was performed to evaluate the effect of ANAs on hormone response between two groups.Results There was no statistically significant difference in the percentage of AIH-PBC OS and AIH-only patients among both ANAs-positive and-negative AILD patients(55.6%vs.44.4%and 65.6%vs.34.4%,P>0.05).Among 46 patients with AIH-PBC OS,there were 25 in ANAs-positive group and 21 in ANAs-negative group.The rate of complete hormone response within 6 months was lower than that of ANAs-negative group(44.0%vs.76.2%),while the rate of hormone non-response was higher than that of ANAs-negative group(20.0%vs.0),and the difference was statistically significant(P<0.05).There were 20 cases of ANAs-positive and 11 cases of ANAs-negative in the 31 AIH-only patients.There was no statistically significant difference in the results of hormone response within 6 months between the two groups(P>0.05).Multifactorial ordered logistic analysis showed that AIH-PBC OS patients were more likely to have a higher likelihood of 6-month hormone non-response rate in ANAs-positive patients,and the difference was statistically different(P<0.05).And there was no significant effect of ANAs type on hormone response outcome in AIH-only patients(P>0.05).Conclusion AIH-PBC OS ANAs-positive patients have a poor hormone response within half a year.In AIH-only patients,ANAs have no significant effect on hormone response results.
		                        		
		                        		
		                        		
		                        	
2.Hepatitis C virus infection:surveillance report from China Healthcare-as-sociated Infection Surveillance System in 2020

		                        			Objective To investigate the infection status and changing trend of hepatitis C virus(HCV)infection in hospitalized patients in medical institutions,and provide reference for formulating HCV infection prevention and control strategies.Methods HCV infection surveillance results from cross-sectional survey data reported to China Healthcare-associated Infection(HAI)Surveillance System in 2020 were summarized and analyzed,HCV positive was serum anti-HCV positive or HCV RNA positive,survey result was compared with the survey results from 2003.Results In 2020,1 071 368 inpatients in 1 573 hospitals were surveyed,738 535 of whom underwent HCV test,4 014 patients were infected with HCV,with a detection rate of 68.93%and a HCV positive rate of 0.54%.The positive rate of HCV in male and female patients were 0.60%and 0.48%,respectively,with a statistically sig-nificant difference(x2=47.18,P<0.001).The HCV positive rate in the 50-<60 age group was the highest(0.76%),followed by the 40-<50 age group(0.71%).Difference among all age groups was statistically signifi-cant(x2=696.74,P<0.001).In 2003,91 113 inpatients were surveyed.35 145 of whom underwent HCV test,resulting in a detection rate of 38.57%;775 patients were infected with HCV,with a positive rate of 2.21%.In 2020,HCV positive rates in hospitals of different scales were 0.46%-0.63%,with the highest in hospital with bed numbers ranging 600-899.Patients'HCV positive rates in hospitals of different scales was statistically signifi-cant(X2=35.34,P<0.001).In 2020,12 provinces/municipalities had over 10 000 patients underwent HCV-rela-ted test,and HCV positive rates ranged 0.19%-0.81%,with the highest rate from Hainan Province.HCV posi-tive rates in different departments were 0.06%-0.82%,with the lowest positive rate in the department of pedia-trics and the highest in the department of internal medicine.In 2003 and 2020,HCV positive rates in the depart-ment of infectious diseases were the highest,being 7.95%and 3.48%,respectively.Followed by departments of orthopedics(7.72%),gastroenterology(3.77%),nephrology(3.57%)and general intensive care unit(ICU,3.10%)in 2003,as well as departments of gastroenterology(1.35%),nephrology(1.18%),endocrinology(0.91%),and general intensive care unit(ICU,0.79%)in 2020.Conclusion Compared with 2003,HCV positive rate decreased significantly in 2020.HCV infected patients were mainly from the department of infectious diseases,followed by departments of gastroenterology,nephrology and general ICU.HCV infection positive rate varies with gender,age,and region.
		                        		
		                        		
		                        		
		                        	
3.Relationship between occupational coping self-efficacy and health-related productivity loss in ICU nurses: path analysis of perceived social support

		                        			BackgroundCompared with absenteeism, health-related productivity loss has a long-lasting negative effect, and poses a greater harm and loss. The health-related productivity loss is mediated by self-efficacy, and perceived social support has been shown to have an impact on health-related productivity loss, whereas the interaction mechanism among the three remains unclear. ObjectiveTo investigate the status of perceived social support, occupational coping self-efficacy and health-related productivity loss among ICU nurses, and to test the mediation role of perceived social support in the relationship between occupational coping self-efficacy and health-related productivity loss. MethodsFrom September to November, 2021, purposive sampling technique was adopted to select 468 ICU nurses in 8 tertiary hospitals in Sichuan Province. Subjects were assessed using self-made general information questionnaire, Perceived Social Support Scale (PSSS), Occupational Coping Self Efficacy Scale for Nurses (OCSE-N) and Stanford Presenteeism Scale-6 (SPS-6). Pearson correlation analysis was used to examine the correlation among variables. Amos 24.0 was utilized to test the mediation role of perceived social support in the relationship between occupational coping self-efficacy and health-related productivity loss. ResultsA total of 415 ICU nurses completed the valid questionnaire survey. ICU nurses scored (63.13±11.62) on PSSS, (22.24±6.15) on OCSE-N, and (16.83±4.24) on SPS-6. Health-related productivity loss was detected in 279 ICU nurses (67.23%). Correlation analysis denoted that PSSS total score was positively correlated with OCSE-N total score (r=0.348, P<0.05), and negatively correlated with SPS-6 total score (r=-0.274, P<0.05). OCSE-N total score was negatively correlated with SPS-6 total score (r=-0.421, P<0.05). The direct effect value of occupational coping self-efficacy on health-related productivity loss was -0.401, and perceived social support showed a mediation role in the relationship between occupational coping self-efficacy and health-related productivity loss (the indirect effect value was -0.052, accounting for 11.48% of the total effect). 		                        			Objectives It was a preliminary exploration on rare disease management of comprehensive hospital based on the"Center-Multidisciplinary Team"structure.Methods By applying interrupted time series analysis,the characteristics and related medical service of rare disease patients discharged from sample hospitals from 2021 to 2023 were measured.Results The results showed that the hospital served 19 825 rare disease patients from 2021 to 2023.The proportion of patients whose main diagnosis were rare diseases was approximately 27%.The interrupted time series technique indicated that the expected proportion of rare disease patients increased instantly by 1.89 thousand points.The capability to diagnose has significantly improved,with a growing trend on volume of bioinformatics analysis services for rare diseases patients.Conclusion The rare disease management based on the"Center-Team"structure can increase the attention of medical staff to rare diseases,strengthen the diagnostic capacity of rare diseases,and make patient management more continuous and disciplinary co-operation more efficient without significantly increasing the input burden on hospitals.

		                        			With the increase in the number of patients with heart failure and the improvement of medical care,the prevalence of patients with end-stage heart failure is increasing.Due to the limited donor supply for heart transplantation,the implantation of left ventricular assist devices has become a major alternative treatment option for patients with end-stage heart failure.Patients with left ventricular assist device have to face potential heart failure and long-term self-care related to left ventricular assist devices.It is of great significance to comprehensively evaluate the level of self-care behavior of patients with left ventricular assist devices and to formulate targeted self-care guidance programs to improve their prognosis.This article reviewed the concept and connotation,assessment tools,current status,influencing factors and intervention of self-care of patients with left ventricular assist devices,in order to provide theoretical basis and practical guidance for improving the self-care level of patients with left ventricular assist devices,so as to improve the quality of life and improve the prognosis of patients.
		                        		
		                        		
		                        		
		                        	
6.Expert consensus on postoperative care of patients with a left ventricular assistant device

		                        			Objective To standardize nursing management on postoperative patients with a left ventricular assist-ant device(LVAD).Methods The first draft of the Consensus was formed on the basis of literature review.2 rounds of expert consultations and a round of online meeting discussion were held for adjustments and modifications the draft of the Consensus.Results The recovery rate of the inquiry questionnaire was 93.75%.The authority coefficients of the 2 rounds of inquiry experts were 0.927 and 0.920.The concentration degree of expert opinions for each indicator was greater than 3.5 score,and the coefficient of variation was less than 0.25.The Kendall harmony coefficients for 2 rounds of correspondence were 0.402 and 0.407(P<0.01).The final Consensus formed through expert consultations and meetings includes 7 themes:hemodynamic monitoring,LVAD function monitoring,coagulation function monitoring,percutaneous cable and wound care,exercise rehabilitation care,health education and guidance,and pre-discharge assessment.Conclusion The Consensus is scientific,rigorous,and authoritative.The Consensus covers all aspects of postoperative care for patients with LVAD,and it will benefit to clinical practice.
		                        		
		                        		
		                        		
		                        	
7.Real-life experience of implantable venous access ports for cancer patients: a Meta-synthesis of qualitative research

		                        			Objective To analyze and summarize the real-life experiences of cancer patients using infusion ports both domestically and internationally. Methods Literatures were searched in the Cochrane Library, Embase, CINAHL, PubMed, Web of Science, ProQuest Dissertations & Theses Global (PQDT), CNKI, Wanfang Database, VIP, and China Biology Medicine up to May 1, 2023. After rigorous evaluation of the retrieved literature, the themes related to the real-life experiences of cancer patients with infusion ports were analyzed and synthesized. Results A total of 13 articles were included, with 6 in Chinese and 7 in English. From these, 64 findings were extracted, comprehensively categorized into 7 groups, and further consolidated into 3 main results: diverse needs across different stages of cancer patients with infusion ports, dynamically intertwined positive and negative psychological states in response to encountered events, and the establishment of a new normal in life experiences for cancer patients with infusion ports. Conclusion Clinical nurses should recognize and address the varying needs of cancer patients with implantable venous access ports at different stages, strengthen their positive experiences, alleviate their negative experiences, assist in adapting to life with implantable venous access ports, improve the quality of nursing services, and enhance patients' sense of happiness.
