1.Analysis of Differential Compounds of Poria cocos Medicinal Materials by Integrated Qualitative Strategy Based on UPLC-Q-Orbitrap-MS

ObjectiveTo establish a rapid analytical method for identifying the differential components in Poria cocos medicinal materials based on ultra performance liquid chromatography-quadrupole-electrostatic field orbital trap high-resolution mass spectrometry(UPLC-Q-Orbitrap-MS), combined with mass defect filtering(MDF) and molecular network integration techniques. MethodsUPLC-Q-Orbitrap-MS was used for MS data acquisition and identification of P. cocos medicinal materials, with the help of MDF for the study of cleavage behavior and structural identification of triterpenoids. According to the similarity of MS/MS fragmentation patterns of each component, global natural product social molecular network(GNPS) was established, and Cytoscape 3.6.1 was used to screen molecular clusters with similar structures and the the structure of main compound classes were identified and confirmed. Multivariate statistical analyses such as principal component analysis(PCA) and orthogonal partial least squares-discriminant analysis(OPLS-DA) were used to screen the differential components of the five P. cocos medicinal materials with the variable importance in the projection(VIP) value>1 and P<0.05 as the criteria. ResultsA total of 66 compounds were identified by database comparison, 8 compounds were newly identified by MDF, 28 compounds were newly identified by GNPS, and a total of 102 chemical compounds were identified, including 43 triterpenoids, 16 saccharides, 26 amino acids and peptides, 3 nucleosides, and 14 other compounds. Triterpenoids were predominant in Poriae Cutis and wild Fushen, amino acids and peptides were the most abundant in Poria and cultivated Fushen, carbohydrates were the most abundant in Poriae Cutis. Type Ⅰ and Ⅱ triterpenoids had higher amounts in Poria and cultivated Fushen, type Ⅲ triterpenoids were more abundant in Poriae Cutis, all four types of triterpenoids were higher in Fushenmu, and type Ⅰ, Ⅱ, and Ⅳ triterpenoids were higher in wild Fushen. A total of 12 common differential chemical constituents were screened, including serine, guanosine, gallic acid, 2-octenal, maltotriose, trametenolic acid, dehydroeburicoic acid, dehydrotrametenolic acid, poricoic acid A, poricoic acid B, poricoic acid E and G, but the relative contents of them varied significantly among different medicinal materials. ConclusionAmong the five P. cocos medicinal materials, the types of constituents are generally similar, but their relative contents differed significantly among these medicinal materials, especially in the distribution of triterpenoids. The integration of UPLC-Q-Orbitrap-MS, MDF and GNPS can provide a reference for the rapid qualitative analysis of other Chinese medicines.

Morita therapy has been bom for more than 100 years.Inpatient Morita therapy is highly oper-able and easy to master.It can improve many refractory neuroses through four-stage treatment.But more neuroses are treated in outpatient clinics,and Morita therapy cannot be used in hospitalized patients.Therefore,the formula-tion of expert opinions on outpatient operations is particularly important.This paper is based on domestic and for-eign references,and after many discussions by domestic Morita therapy experts,and then drew up the first version of the expert opinions on operation of outpatient Morita therapy.Meanwhile the operation rule of Morita therapy in three stages of outpatient treatment was formulated:in the etiological analysis stage,under the theoretical guidance of Morita therapy,analyze the pathogenic factors,to improve treatment compliance and reduce resistance;during the operating stage,guide patients to engage in constructive and meaningful actions,realizing the achievement of letting nature take its course principle;in the cultivating character and enriching life stage,pay attention to positive infor-mation,expanding the scope and content of actions,improving the ability to adapt to complex life,and preventing recurrence caused by insufficient abilities.It will lay a foundation for the promotion of Morita therapy in domestic outpatient clinics,so that more patients with neurosis and other psychological diseases could receive characteristic Morita therapy treatment in outpatient clinics.
4.Differences in Chemical Composition of Polygonatum cyrtonema Rhizome Before and After Processing Based on UPLC-Q-Exactive Orbitrap MS Analysis

ObjectiveUltra performance liquid chromatography-quadrupole-electrostatic field orbitrap high resolution mass spectrometry(UPLC-Q-Exactive Orbitrap MS) was used to analyze the differences in chemical components between raw products and wine-processed products of Polygonatum cyrtonema rhizome. MethodUPLC-Q-Exactive Orbitrap MS was used to analyze the chemical compositions of P. cyrtonema rhizome before and after processing, and the effective response ions were extracted after raw data processing, and the differential compounds before and after processing were screened combined with multivariate statistical analysis and according to the conditions of variable importance in the projection(VIP) value>1, P<0.05, fold change(FC)>2 or FC<0.5, based on the retention time, quasi-molecular ions, fragment ions and other information, the components were identified in combination with the control products and the literature, and the significant difference compounds were identified by clustering thermal analysis and relative quantitative analyzed, in order to clarify the change rule of the main components in P. cyrtonema rhizome before and after processing. ResultA total of 72 differential constituents between raw products and wine-processed products were identified, including 15 alkaloids, 12 organic acids, 12 amino acids, 6 flavonoids, 4 saccharides and 23 others. There were a total of 18 significantly different components, among which 13 compounds, including L-malic acid, lactic acid and 9,12,13-trihydroxy-10-octadecenoic acid, showed an increasing trend in content after wine processing, 5 compounds such as trans-3-indoleacrylic acid, L-arginine, D-tryptophan, showed a decreasing trend after processing. ConclusionThe chemical components of P. cyrtonema rhizome are significantly different before and after processing, mainly organic acids, saccharides, amino acids, flavonoids and alkaloids, which can lay the foundation for the in-depth study of the processing mechanism of Polygonati Rhizoma.
5.Analysis of caries experience and the dental treatments under general anesthesia in 103 cases of children with autism spectrum disorders

Objective:To compare the caries experience and the kinds of dental treatment between children with autism spectrum disorders (ASD) and children without systemic disease who were all treated under general anesthesia.Methods:Totally 103 children with ASD who received dental treatments under general anesthesia in 13 professional dental hospitals around China from April to November 2016 were included in the present study. A group of 97 children without systemic disease, according to the age, gender and application propensity score matching method, were chosen as controls, who received dental treatments under general anesthesia between January 2015 to November 2018 in the same hospitals as the children with ASD. Decay missing filling tooth (DMFT/dmft, DMFT for permanent teeth and dmft for primary teeth) indices of two groups of children and the contents of the dental treatments under general anesthesia were analyzed.Results:No significant difference of DMFT/dmft index [ M( Q25, Q75)] was found between children with ASD group [0 (0, 3)/11(8, 14)] and control group [0 (0, 3)/9(7, 13)] ( P>0.05). The average number of dental treatments under general anesthesia and the average number of endodontic treatment in children with ASD were 13 (11, 15) and 3 (2, 6) teeth respectively, while those in the control group were 12 (9, 14) and 2 (1, 4) teeth respectively, the differences were statistically significant ( P<0.01, P<0.05). Conclusions:No significant difference was found between children with ASD and the normal controls who receive dental treatments under general anesthesia in DMFT/dmft index, but the treatment needs of children with ASD is relatively higher, and their tooth decay is relatively severer.
6.Health behaviors and psychosocial determinants among primary and middle school students in Henan province

Objective:
To investigate the current status and psychosocial determinants of health behavior among primary and middle school students in Henan province,and to provide a reference for the direction and priority of the health education in primary and middle schools.
Methods:
Through cluster sampling method, 3 603 primary school students age 9 to 15 and 2 791 secondary school students in Henan Province were investigated with Chinese version of the Adolescent Lifestyle Questionnaire(C-ALQ), General Self-efficacy Scale(GSES), Perceived Social Support Scale(PSSS), Resilience Scale for Chinese Adolescents(RSCA), Self-Esteem Scale(SES) during January to June in 2016.
Results:
The total score of health behavior among primary and middle school students was (80.30±16.36).There were significant differences in health behaviors among students with different gender(P<0.05), age(P<0.01), family income(P<0.01), body mass index(P<0.01), father’s education level (P<0.01) and mother’s education level(P<0.01). Health behaviors were significantly correlated with self-efficacy(r=0.26-0.42, P<0.01), social support(r=0.23-0.39, P<0.01), mental resilience(r=0.26-0.37, P<0.01), and self-esteem(r=-0.28--0.18, P<0.01). Gender, age, family monthly income, BMI, parents’ education level, self-efficacy, psychological resilience, social support and self-esteem was psychosocial determinants of healthy behaviors(P<0.05).
Conclusion
The engagement in health behaviors among primary and middle school students in Henan province is acceptable. Demographic and psychosocial determinants of health behaviors among primary and middle school students should be considered in health education curriculum planning.
7.Analysis of impact factors on prognosis of patients with gallbladder adenocarcinoma after surgery

Objective To study the impact factors on prognosis of patients with gallbladder adenocarcinoma after surgery.Methods The clinicopathological data of 1 285 patients who underwent surgery and were histologically confirmed to have gallbladder adenocarcinoma from 2004 to 2014 was extracted from the SEER database of US National Cancer Institute.Life table was used to calculate the survival rate.Kaplan-Meier was used to construct the survival curves.Univariate and Cox multivariate regression analysis were applied to evaluate the prognostic factors.The univariate analysis was evaluated by the log-rank x2 test.Results The median survival of 1 285 patients with gallbladder adenocarcinoma was 32 months.The 1-,3-and 5-year survival rates were 77.5%,46.8% and 36.5%,respectively.The 5-year survival rates of stage Ⅰ,Ⅱ,ⅢA,ⅢB,ⅣA,ⅣB were 72.7%,63.2%,24.6%,20.5%,0 and 5.1%,respectively.The median survival of stage ⅢA,ⅢB,ⅣA and ⅣB were 22 months,19 months,12 months and 16 months,respectively.The differences were statistically significant (P < O.05).Univariate analysis showed that sex,age,degree of differentiation,tumor size,T staging,N staging,number of lymph nodes detected,lymph node metastasis rate,location of lymph node metastasis,and American Joint Commission on cancer (AJCC) staging were significantly associated with prognosis (P < 0.05).There was no significant differences among race and marital status with prognosis (P > 0.05).Multivariate Cox regression analysis showed that sex (male),age (≥ 65 years),degree of differentiation,T staging,number of lymph nodes detected (< 4)and AJCC staging were independent risk factors affecting prognosis of patients with gallbladder adenocarcinoma (P < 0.05).Conclusions With increase in AJCC staging,the survival rates in patients with gallbladder adenocarcinoma after surgery decreased gradually.Parameters including sex,age,T staging,number of lymph nodes detected and AJCC staging were independent factors affecting prognosis of patients with gallbladder adenocarcinoma after surgery.
8.Optimization of Water Extraction Technology of Feng-Moutan Cortex by Orthogonal Test-Multiple-index Normalization Method

OBJECTIVE: To optimize water extraction technology of Feng-Moutan Cortex. METHODS: L9 (34) orthogonal design was used to optimize soaking time, solid-liquid ratio, extraction time and extraction times in water extraction technology of Feng-Moutan Cortex using comprehensive scores for the peak area percentage and extract yield of 8 ingredients in Feng-Moutan Cortex as gallic acid, paeoniflorin, catechin, paeoniflorin, benzoic acid, benzoyloxypaeoniflorin, benzoylpaeoniflorin and paeonol after normalization as indexes. Verification test was conducted. RESULTS: The optimal water extraction technology was that 20-fold water, soaking for 1 h, extracting twice, 2 h each time. Results of verification test showed that comprehensive scores of 3 times of tests were 65. 98, 68. 85 and 69. 25, respectively. RSDs of each index were lower than 5% (n=3). CONCLUSIONS: Established comprehensive scoring method can reflect the relative contents of effective components in Feng-Moutan Cortex comprehen- sively, so that optimized extraction technology is reasonable and feasible.
9.Prognostic factors for patients with pancreatic head carcinoma undergoing operation: a retrospective study on Surveillance, Epidemiology, and End Results using population-based data

Objective To study the survival of patients with pancreatic head carcinoma undergoing surgery,and to evaluate the clinicopathological factors associated with prognosis.Methods The data of 4 814 patients who underwent surgery for pancreatic head carcinoma from 2004 to 2009 were extracted from the US SEER database.The life table was used to calculate the survival rate at 5-years.The Kaplan-meier method was used to construct the survival curves of the patients.The univariate and Cox multivariate regression analysis were applied to evaluate the prognostic factors.and the univariate analysis was evaluated by the log-rank x2 test.Results The overall 5-year survival of 4 818 pancreatic head carcinoma patients was 16.3%.The median survival was 18.9 months,and the 5-year survival rates of the stage Ⅰ A,Ⅰ B,Ⅱ A,Ⅱ B,Ⅲ and Ⅳ patients were 38.8%,35.3%,21.8%,12.2%,9.1%,and 7.3%,respectively.The difference was statistically significant (x2 =287.702;P < 0.05).The factors including sex,age,years of diagnosis,pathological grading,tumor size,extent of invasion,lymph node metastasis,distant metastasis,and pathological pattern (all P < 0.05) influenced the survival rate on univariate analysis.There was no clear correlation between race,number of lymph node examined and prognosis.The factors including sex,age,years of diagnosis,pathological grading,tumor size (AJCC 8th),range,extent of invasion,lymph node metastasis,distant metastasis,and pathological pattern (all P < 0.05) were independent prognostic factors on multivariate analysis.Conclusions With the increase of TNM staging,the overall survival of patients with pancreatic head carcinoma undergoing surgery gradually decreased.Factors including sex,age,years of diagnosis,pathological grading,tumor size,extent of invasion,lymph node metastasis,distant metastasis,and pathological pattern were independent prognostic factors of overall survival.
10.The pathological types of pancreatic carcinoma: an analysis based on US SEER database

Objective To study the component ratios of pancreatic carcinoma,and prognosis of patients with the different pathological types.Methods The data of 92 011 pancreatic carcinoma patients who were diagnosed by the definite pathological method from 2004 to 2014 were extracted from the US SEER database.The life table was used to calculate the 1-,3-and 5-year survival rates.The Kaplan-Meier was used to construct the survival curves of the patients.The Cox univariate analysis was applied to evaluate the HR of the different pathological types,and the x2 test and independent sample t-test were used to evaluate the base line data.Results The overall 5-year survival of 92 011 pancreatic carcinoma patients was 7.6%.The median survival time was 8.8 months and the component ratios of the pancreatic ductal adenocarcinoma (PDAC),pancreatic epithelium cancer bedside the PDAC,neuroendocrine tumors,undifferentiated carcinoma,mesenchymal carcinoma and rare cancer unclassified were 85.78%,6.40%,6.67%,0.97%,0.10% and 0.08%,respectively.The differences were statistically significant between the baseline data such as age,gender,race and position (P < 0.05).The overall 5-year survivals of the PDAC,pancreaticepithelium cancer beside the PDAC,neuroendocrine tumors,undifferentiated carcinoma,mesenchymal carcinoma and rare cancer unclassified were 4.2%,13.4%,49.2%,5.0%,29.2% and 24.5%,respectively,and the median survival times were 5 months,7 months,58 months,2 months,26 months and 7 months respectively.The distant metastasis rate of neuroendocrine carcinoma was the lowest (35.0%).The undifferentiated carcinoma was the most prone to distant metastases (65.2%).Basically,half of the other types of pancreatic cancer had metastasis at the time of diagnosis.In pancreatic epithelium cancer beside the PDAC,the high to low 5-year survival rates were solid pseudopapillary carcinoma (87.3%),cystadenocarcinoma (36.8%),intraductal papillary mucinous neoplasm (36.5%),acinar cell carcinoma (20.0%),and pancreatic adenocarcinoma mixed with other subtypes (19.7%).The incidence of the other types of pancreatic carcinoma was less than 8%,such as mucinous carcinoma,squamous cell carcinoma,adenosquamous carcinoma and signet ring cell carcinoma.In neuroendocrine tumor,the 5-year survival rate of insulinoma (77.1%) was higher than those of neuroendocrine tumor (malignant) (NET,62.0%) and neuroendocrine carcinoma (NEC,46.5%).Conclusion The prognosis of pancreatic carcinoma was poor,and the pathological types had a significant impact on the prognosis of the patients.
