1.Weifuchun Alleviates Gastric Precancerous Lesions by Inhibiting Pyroptosis via NF-κB/GSDME Pathway

		                        			ObjectiveTo explore the role and molecular mechanism of Weifuchun (WFC) in inhibiting inflammation and alleviating gastric precancerous lesions (GPL). MethodHuman gastric mucosal epithelial cells (GES-1) were stimulated with N-methyl-N′-nitro-N-nitrosoguanidine (MNNG) for the modeling of GPL (MC cells), with Caspase-3 inhibition by Z-DEVD-FMK. MC cells were divided into control (20% blank serum), WFC (15% and 20% WFC-containing serum), and caspase-3 inhibitor groups. The cell counting kit-8 (CCK-8) was used to examine the viability of GES-1 cells or MC cells. The Transwell assay and 5-acetylidene-2′-deoxyuridine (EdU) staining were employed to examine cell invasion and proliferation, respectively. Flow cytometry was employed to determine the level of reactive oxygen species. Real-time PCR was conducted to determine the mRNA levels of interleukin (IL)-1, IL-6, and tumor necrosis factor (TNF)-α. Gene Expression Omnibus (GEO) was used to analyze the role of pyroptosis in gastric cancer progression. Western blotting was employed to determine the protein levels of nuclear factor-κB (NF-κB) p65, gasdermin E (GSDME), and Caspase-3. Immunofluorescence staining was employed to detect the NF-κB p65 protein level and nuclear translocation. Hematoxylin-eosin staining was carried out to observe the pathological changes in the gastric mucosa before and after WFC treatment in the patients. ResultCompared with the control group, MC cells presented enhanced proliferation and invasion energy (P<0.01). Compared with the blank serum group, WFC-containing serum inhibited the proliferation and invasion of MC cells (P<0.01), down-regulated the mRNA levels of IL-1, IL-6, and TNF-α, and lowered the level of reactive oxygen species (P<0.05, P<0.01). The transcriptome data at different stages of gastric cancer showed that pyroptosis was involved in gastric cancer progression, and the GSDME level was significantly higher in GPL patients than in the normal group. Compared with the blank serum, WFC-containing serum lowered the level of NF-κB and inhibited the nuclear translocation of NF-κB (P<0.05), and it inhibited pyroptosis by suppressing the cleavage of Caspase-3 on GSDME (P<0.05, P<0.01). The analysis of patient specimens further demonstrated that WFC treatment down-regulated the NF-κB level and GSDME cleavage (P<0.01), inhibited pyroptosis, and alleviated gastric mucosal inflammation and intestinal epithelial metaplasia. ConclusionPyroptosis is involved in the progression of gastric cancer, and WFC inhibits pyroptosis via the NF-κB/GSDME pathway, thereby alleviating gastric mucosal inflammation in GPL. 
		                        		
		                        		
		                        		
		                        	
2.Comparison of Chinese version Braden-QD scale and Braden-Q scale in assessing the risk assessment of pressure injury for patients in pediatric intensive care unit

		                        			Objective:To compare the application value of the Braden-QD scale and the Braden-Q scale in assessing the risk of pressure injuries in pediatric intensive care unit (PICU) patients.Methods:A convenient sampling method was used to select 205 pediatric patients admitted to the First People’s Hospital of Lianyungang from December 2021 to September 2022 as the research subjects.The Braden-QD scale and the Braden-Q scale were used to evaluate the risk of pressure injuries.SPSS 25.0 and Python 2.7 softwares were used for statistical analysis. Inter-rater reliability of the scales was evaluated using intraclass correlation coefficient (ICC), internal consistency was evaluated using Cronbach's α, and the predictive power was compared using receiver operator characteristic curve (ROC), positive predictive value, negative predictive value, sensitivity, specificity and area under the ROC curve (AUC).Results:(1) A total of 24 (11.7%) out of 205 patients had pressure injury, predominantly in stage 1 (20 cases, 83.3%). (2)The ICC of Braden-QD scale and Braden-Q scale were 0.890 and 0.862, while the Cronbach's α were 0.891 and 0.710.(3) The AUC of the Braden-QD scale was 0.936. The optimal diagnostic cutoff value was 9.5, with a Youden index, sensitivity, specificity, positive predictive value and negative predictive value of 0.765, 0.875, 0.890, 0.512, 0.982, respectively.The AUC of the Braden-Q scale was 0.881. The optimal diagnostic cutoff value was 18.5, with a Youden index, sensitivity, specificity, positive predictive value, and negative predictive value of 0.626, 0.792, 0.834, 0.388, 0.968, respectively.Conclusion:Compared with the Braden-Q scale, the Braden-QD scale is more suitable for assessing the risk of pressure injuries in PICU children.
		                        		
		                        		
		                        		
		                        	
3.Strengthening research on prevention and treatment of chronic skin diseases — population medicine research program

		                        			Chronic skin diseases have complex pathogeneses and prolonged courses, and have long adverse impacts on the physical and mental health, as well as the normal life of patients. It is necessary to develop evidence-based strategies and measures for effective prevention and control of chronic skin diseases. However, related studies are limited in China. 4.Evaluation of eCura scoring system for treatment strategy selection after non-curative endoscopic resection of early gastric cancer

		                        			Objective:To evaluate the adjuvant role of the eCura scoring system in selecting appropriate treatment strategies after non-curative endoscopic submucosal dissection (ESD) of early gastric cancer (EGC) patients.Methods:The clinicopathological data of 110 EGC patients who underwent non-curative ESD at Fujian Provincial Hospital from January 2015 to June 2019 were retrospectively analyzed. According to the eCura score, patients were divided into three lymph node metastasis (LNM) risk groups: low-risk group (79 cases), middle-risk group (22 cases), and high-risk group (9 cases). The receiver operator characteristic (ROC) curve analysis was used to test the diagnostic efficacy of eCura scoring system in predicting LNM. Logistic regression analysis was used to explore the influence of risk stratification of eCura scoring system on LNM. Kaplan-Meier method was used to evaluate cancer survival rate, which was then compared with log-rank test.Results:Thirty-five patients underwent additional standard surgery after ESD, including 22 in the low-risk group, 8 in the middle-risk group, and 5 in the high-risk group. Among them, 5 cases had LNM, including 1 case in the low-risk group and the middle-risk group respectively and 3 cases in the high-risk group. The area under the ROC curve was 0.857 (95% CI: 0.697-0.952, P=0.001), and when the cut-off value of the eCura score was set at 3, the Yuden index reached the maximum value of 0.7, with the corresponding sensitivity and specificity of 80% and 90%, respectively. Logistic regression analysis showed that the probability of LNM in the middle-risk group was about 3.00 times (95% CI: 0.17-54.57, P=0.458) as high as that in the low-risk group, and the probability of LNM in the high-risk group was about 31.50 times (95% CI: 2.14-463.14, P=0.012) of that in the low-risk group. The follow-up time was 12 to 58 months, and the median follow-up time was 40 months. There were 10 cases of recurrence, including 4 cases in the low-risk group, 3 cases in the middle-risk group and 3 cases in the high-risk group, of which 2 cases in the low-risk group were from those of additional standard surgery after ESD, and the remaining 8 cases were from those who did not receive additional standard surgery after ESD. Kaplan-Meier survival curve analysis showed that the survival rate of patients with additional surgery in the low-risk group was similar to that of patients without ( P=0.319), and the survival rate of patients with additional surgery in the middle-risk group was also similar to that of patients without ( P=0.296). The survival rate of patients with additional surgery in the high-risk group was significantly higher than that of those without ( P=0.013). Conclusion:The eCura scoring system can assist the selection of treatment strategies after non-curative resection of EGC, and can accurately predict the risk of subsequent LNM and recurrence. Close follow-up may be an acceptable option for patients with low risk of LNM, and additional standard surgical treatment may be more conducive to improving the prognosis in patients with high risk of LNM.
		                        		
		                        		
		                        		
		                        	
5.Gene mutation types and clinical characteristics of children with G6PD deficiency in Zunyi area

		                        			Objective:To study the types of pathogenic gene mutations and their main clinical characteristics in children with glucose-6-phosphate dehydrogenase (G6PD) deficiency in Zunyi area.Methods:Children with clinical manifestations of "yellow staining" or "suspected yellow staining" who were admitted to Guizhou Children's Hospital, Affiliated Hospital of Zunyi Medical University, from September 13, 2018 to September 13, 2020 were selected for G6PD gene mutation detection by multicolor probe melting curve analysis, and the pathogenic gene mutation types and clinical characteristics of children with G6PD deficiency were analyzed.Results:The results of G6PD gene mutation detection showed that among the 1 740 children tested, 119 were positive for gene mutation, and the positive detection rate was 6.84%. The proportion of male infants was higher than that of female infants, and the difference was statistically significant (91 males and 28 females, χ 2 = 15.10, P < 0.001); infancy accounted for 63.87% (76/119), and early childhood accounted for 18.49% (22/119). A total of 11 known pathogenic gene mutation types and 1 unknown mutation were detected. Among the top 4 pathogenic gene mutations, the overall was c.1024 C>T, c.1376 G>T, c.1388 G>A and c.95 A>G, male was c.1376 G>T, c.1388 G>A, c.1024 C>T and c.95 A>G; female was c.1024 C>T, c.95 A>G, c.1388 G>A and c.519 C>T. Among the 119 children with G6PD gene mutation, 90 cases had varying degrees of jaundice, including 36 cases of severe and more severe jaundice (including 2 cases of extremely severe neonatal bilirubin encephalopathy), and 54 cases of mild to moderate jaundice; 37 cases had anemia of different degrees, including 6 cases of mild anemia, 12 cases of moderate anemia, and 19 cases of severe or more severe anemia (including 1 case of extremely severe anemia). Conclusions:There are 12 types of gene mutations in children with G6PD deficiency in Zunyi area, and the most common mutation types are c.1024 C>T, c.1376 G>T, c.1388 G>A and c.95 A>G. 6.Phenotypic and genomic characterization for Salmonella isolates recovered from meat products in Beijing wholesale markets, 2014-2015

Methods:A total of 336 Salmonella strains from meat products collected from wholesale markets in Beijing were tested for antimicrobial resistance to 25 antimicrobial compounds by micro-broth dilution method; whole genome data were sequenced, followed by the serotype and ST type prediction by Seqsero2 and SISTR software, and the drug resistance genes and virulence factors were also predicted with CARD and VFDB databases of Abricate software; Salmonella serotyping assay kit and serum agglutination method were used for serotype confirmation of some isolates with different genome prediction results. Results:The resistance rates to Nalidixic acid and Ampicillin were 62.5% (210/336) and 55.1% (185/336), respectively, and all isolates were susceptible to Tigecyclin, Cefoxitin and Carbapenem antimicrobial compounds; 207 isolates (61.6%, 207/336) were multi-drug resistant, some could even be resistant to ten categories of drugs at the same time, and the most common antimicrobial resistance spectrum was NAL-AMP-SAM. A total of 24 serotypes were detected with predominant serotypes of Enteritidis (34.5%, 116/336), Derby (17.3%, 58/336) and Indiana (10.4%, 35/336). A total of 27 ST types were detected, the dominant type was ST11; ST types were in good consistency with serotypes; The detection rates of resistant genes referred to aminoglycosides, fluoroquinolones, β-lactams, sulfonamides and tetracyclines are more than 48%, and the first two reached 100%. The prediction of drug resistance genes was consistent with the results of antimicrobial resistance phenotype. 8.Safety and long-term efficacy of endoscopic resection of large gastric stromal tumors: a multicenter retrospective study

		                        			Objective:To evaluate the safety and long-term efficacy of endoscopic resection of gastric stromal tumors with a diameter of >2-4 cm.Methods:The clinical data of 307 patients, who underwent endoscopic or surgical resection and pathologically confirmed to be gastric stromal tumors with a diameter ≤4 cm in Fujian Provincial Hospital, Jinshan Branch of Fujian Provincial Hospital or Fujian Geriatric Hospital from January 2014 to December 2019, were collected. The propensity score matching (1∶1) was performed for the cases with the tumor size of >2-4 cm.Then the incidence of adverse events related to the operation and clinical outcomes were compared between 41 patients in the endoscopic group and 41 patients in the surgical group.Results:Compared with the surgical group, the median operation time in the endoscopic group was significantly shorter (58.0 min VS 108.0 min, Z=-4.789, P<0.001), and the median hospitalization cost was significantly lower (22.7 thousand yuan VS 42.0 thousand yuan, Z=-7.164, P<0.001). There were no significant differences in postoperative fasting time or postoperative hospitalization time between the two groups ( P>0.05). Complications occurred in 7 cases (17.1%) in the endoscopy group, including 5 cases of postoperative acute infection, 1 case of postoperative perforation, and 1 case of postoperative bleeding; all 9 cases (22.0%) in the surgical group developed postoperative acute infection. There was no significant difference in the overall incidence of complications between the two groups ( χ2=0.311, P=0.577). Tumors in both groups were completely removed with negative resection margins. The follow-up time of the endoscopy group was 34.3±15.6 months, and that of the surgical group was 42.2±20.2 months. No recurrence or distant metastasis was observed during the follow-up period in the two groups. Conclusion:Endoscopic resection of large gastric stromal tumor (range>2-4 cm) is safe and effective in the long term, which can be used as one of the methods for gastrointestinal stromal tumors.

		                        			Objective It aims to investigate the relationships among the categories of Comprehensive Version for Stroke as described in the International Classiifcation of Functioning, Disability and Health (ICF) Core Set, and to provide new supports for Judicial Appraisal of functioning in stroke by ICF functioning mapping.Methods The variables of 59 categories of ICF assessment scale and the samples of 106 persons’ are selected and used in the least absolute shrinkage and selection operator (LASSO) for mining dependencies among those variables. The graphical modeling and analyzing with the software Gephi provides a visual map of the correlations among those classiifcations. Results 59 interconnected categories which organized into the functioning mapping. b340, b735, b175 and b152 are centrally positioned categories because of their high correlation.Conclusion Functioning mapping by graphical modeling can reveal complex relational structures embedded in functioning classiifcations, which provides the support for using ICF to appraisal stroke.
		                        		
		                        		
		                        		
		                        	
10.Survey on fungi contamination and natural occurrence of mycotoxins in 94 corn feed ingredients ;collected from China

		                        			Objective To investigate fungi contamination and the natural occurrence of mycotoxins in corn feed ingredients collected from China. Methods A total of 94 corn feed ingredient samples were collected from 8 Chinese provinces (i.e., Anhui, Hebei, Heilongjiang, Jilin, Jiangsu, Liaoning, Inner Mongolia, and Shandong) in February 2014. A tandem ultra-performance liquid chromatography-mass spectrometry method was used for simultaneous detection of twelve kinds of mycotoxins, including aflatoxin (AF), type A and type B tricothecenes, and zearalenone (ZEN). Contaminated fungi were also identified and counted. Results AF was detected in 36.2%(34/94) of samples;the concentration of AFB1 was the highest in the four AFs with the range:0.3~181.3μg/kg;and then followed by AFB2 (range:1.0-74.3μg/kg). There were 7 samples (7.5%) with AFB1 concentrations higher than the tolerance limit of 50 μg/kg. The concentration of type A tricothecenes in all samples was lower (0.1-10.5μg/kg). DON had the most serious contamination than other kind of type B tricothecenes (range: 0.7-606.6 μg/kg; median: 66.3 μg/kg). The DON concentration in all samples was below the tolerance limit of 1 000μg/kg. ZEN was detected in 76.6%(72/94) of samples (median: 36.9 μg/kg), with 3 samples having ZEN concentrations higher than the tolerance limit of 500μg/kg. The survey on fungi contamination showed that all samples were contaminated by fungi (range:5.0-1.4×105 CFU/g). There were 18 and 3 samples with quantities of fungi higher than the tolerance and forbidden limits, respectively. The Aspergillus, Penicillium, Fusarium, Trichoderma and Mucor genuses were the predominant fungi in corn feed ingredients, with detection rates of 71.3%(67), 60.6%(57), 71.3%(67), 27.7%(26), and 24.5%(23), respectively. The detection rate of Fusarium moniliforme, 73.4%(69/94) was higher than that of Aspergillus flavus, 41.5% (39/94). Conclusion In this survey, the corn feed ingredients were not seriously contaminated by AF and type A tricothecenes but mainly contaminated by type B trichothecenes, including DON and its derivatives, as well as ZEN.
