1.Significance and treatment strategies of pathologic complete response after neoadjuvant therapy for gastric cancer

Neoadjuvant therapy, as an important part of comprehensive treatment for locally advanced gastric cancer, has been recommended by various guidelines. Partial locally advanced gastric cancer patients can achieve pathologic complete response (pCR) after neoadjuvant therapy, thus achieving relatively good prognosis. However, there is still controversy over whether complete remission in local pathology can translate into survival benefits, whether pCR is equivalent to cure, and whether subsequent adjuvant therapy is needed. Therefore, how to predict patients who can achieve pathologic complete response after neoadjuvant therapy and identify truly cured patients is the direction of future exploration.
2.Correlation between hepatitis B virus infection and adverse pregnancy outcomes—a systematic review and meta-analysis

Objective:To systematically evaluate the effect of hepatitis B virus (HBV) infection on the risk of adverse pregnancy outcomes.Methods:We searched PubMed, Embase, Web of Science, and Cochrane databases. Two researchers independently screened the literature, extracted data, and evaluated the quality. Meta-analysis and cumulative meta-analysis were performed using R4.4.1 software. Fixed/random effects models were used to analyze heterogeneous and non-heterogeneous results. Heterogeneous modifiers were identified by subgroup analysis. Funnel plots and Peters' test were used to analyze potential publication bias.Results:A total of 48 studies involving 92 836 HBsAg-positive pregnant women and 7 123 292 HBsAg-negative pregnant women were included. In terms of adverse pregnancy outcomes, HBV infection was significantly correlated with the occurrence of gestational diabetes mellitus [odds ratio ( OR)=1.34, 95% confidence interval ( CI): 1.17-1.53] and intrahepatic cholestasis ( OR=2.48, 95% CI: 1.88-3.29), with statistically significant differences. In terms of adverse neonatal outcomes, HBV infection was significantly correlated with the occurrence of neonatal asphyxia ( OR=1.49, 95% CI: 1.20-1.86) and preterm birth ( OR=1.22, 95% CI: 1.12-1.33), with statistically significant differences. In addition, the cumulative meta-analysis demonstrated that the risk of gestational diabetes mellitus and preterm birth both tended to be stable in pregnant women with HBV infection following 2009 and 2010, respectively. The supplementary questions answered for repeated studies had limited significance. Conclusion:Intrahepatic cholestasis, gestational diabetes mellitus, neonatal asphyxia, and preterm birth occurrence risk can be raised with HBV infection in pregnant women.

Ischemic stroke has become the leading cause of mortality and disability among adults in China.After ce-rebral ischemia,various pathological and physiological pathways,including the increase in oxygen free radicals,inflam-matory response,calcium overload,and energy metabolism disorders,can disrupt cellular homeostasis,cause mitochon-drial dysfunction,and finally lead to cell apoptosis.By generating new functional mitochondria to replace damaged ones and restoring mitochondrial function,mitochondrial biogenesis(MB)is the primary mechanism for maintaining mitochon-drial homeostasis and plays a crucial role in mitochondrial quality control.In recent years,with the in-depth research on the mechanisms of ischemic stroke injury,MB has emerged as a potential therapeutic target.This article reviews the re-search advances in the role and mechanisms of MB in cerebral ischemic injury,in order to provide a theoretical reference for neuroprotective treatment in ischemic stroke.

6.Research progress of circRNA in breast cancer drug resistance

Breast cancer is the most common cancer among women worldwide, and it seriously threatens women′s life and health. circular RNA (circRNA) play a key role in the development and drug resistance of various cancers, including breast cancer. circRNA is an endogenous RNA molecule with a single-stranded closed structure, which has unique biological characteristics and function, and is a current research hotspot. This article will review circRNA as potential biomarkers to predict breast cancer drug resistance and therapeutic targets.
7.Dual-targeting prodrug nanotheranostics for NIR-Ⅱ fluorescence imaging-guided photo-immunotherapy of glioblastoma.

Glioblastoma (GBM) therapy is severely impaired by the blood-brain barrier (BBB) and invasive tumor growth in the central nervous system. To improve GBM therapy, we herein presented a dual-targeting nanotheranostic for second near-infrared (NIR-II) fluorescence imaging-guided photo-immunotherapy. Firstly, a NIR-Ⅱ fluorophore MRP bearing donor-acceptor-donor (D-A-D) backbone was synthesized. Then, the prodrug nanotheranostics were prepared by self-assembling MRP with a prodrug of JQ1 (JPC) and T7 ligand-modified PEG5k-DSPE. T7 can cross the BBB for tumor-targeted delivery of JPC and MRP. JQ1 could be restored from JPC at the tumor site for suppressing interferon gamma-inducible programmed death ligand 1 expression in the tumor cells. MRP could generate NIR-II fluorescence to navigate 808 nm laser, induce a photothermal effect to trigger in-situ antigen release at the tumor site, and ultimately elicit antitumor immunogenicity. Photo-immunotherapy with JPC and MRP dual-loaded nanoparticles remarkably inhibited GBM tumor growth in vivo. The dual-targeting nanotheranostic might represent a novel nanoplatform for precise photo-immunotherapy of GBM.
8.Metabolic engineering of yeasts for green and sustainable production of bioactive ginsenosides F2 and 3β,20S-Di-O-Glc-DM.

Both natural ginsenoside F2 and unnatural ginsenoside 3β,20S-Di-O-Glc-DM were reported to exhibit anti-tumor activity. Traditional approaches for producing them rely on direct extraction from Panax ginseng, enzymatic catalysis or chemical synthesis, all of which result in low yield and high cost. Metabolic engineering of microbes has been recognized as a green and sustainable biotechnology to produce natural and unnatural products. Hence we engineered the complete biosynthetic pathways of F2 and 3β,20S-Di-O-Glc-DM in Saccharomyces cerevisiae via the CRISPR/Cas9 system. The titers of F2 and 3β,20S-Di-O-Glc-DM were increased from 1.2 to 21.0 mg/L and from 82.0 to 346.1 mg/L at shake flask level, respectively, by multistep metabolic engineering strategies. Additionally, pharmacological evaluation showed that both F2 and 3β,20S-Di-O-Glc-DM exhibited anti-pancreatic cancer activity and the activity of 3β,20S-Di-O-Glc-DM was even better. Furthermore, the titer of 3β,20S-Di-O-Glc-DM reached 2.6 g/L by fed-batch fermentation in a 3 L bioreactor. To our knowledge, this is the first report on demonstrating the anti-pancreatic cancer activity of F2 and 3β,20S-Di-O-Glc-DM, and achieving their de novo biosynthesis by the engineered yeasts. Our work presents an alternative approach to produce F2 and 3β,20S-Di-O-Glc-DM from renewable biomass, which lays a foundation for drug research and development.
9.Synthetic biology of plants-derived medicinal natural products.

Medicinal natural products derived from plants are usually of low content and difficult to extract and isolate. Moreover, these compounds are structurally complex, making it difficult to obtain them by environmental unfriendly chemical synthesis. Biosynthesis of medicinal natural products through synthetic biology is a novel, environment-friendly and sustainable approach. Taking terpenoids (ginsenosides, paclitaxel, artemisinin, tanshinones), alkaloids (vincristine and morphine), and flavonoids (breviscapine) as examples, this review summarizes the advances of the biosynthetic pathways and synthetic biology strategies of plant-derived medicinal natural products. Moreover, we introduce the key technologies and methods of synthetic biology used in the research of medicinal natural products, and provide future prospects in this area.

10.Effects of hydromorphone on early postoperative recovery, postoperative analgesia and stress response in patients undergoing laparoscopic radical resection of colorectal cancer

Objective:To investigate the effects of hydromorphone on early postoperative recovery, postoperative analgesia and stress response in patients undergoing laparoscopic radical resection of colorectal cancer.Methods:120 patients with colorectal cancer who underwent laparoscopic radical resection of colorectal cancer in Taizhou Hospital of Zhejiang Province from June 2019 to June 2020 were included in this study. They were randomly assigned to undergo either postoperative patient-controlled intravenous analgesia with hydromorphone (observation group, n = 60) or postoperative patient-controlled intravenous analgesia with sugentanil (control group, n = 60). The changes of perioperative indexes, postoperative recovery, Visual Analogue Scale score at 3, 12 and 24 hours post-surgery, stress response [norepinephrine, interleukin-6 and cortisol] before and 3 days after surgery were compared between the two groups. Results:There were no significant differences in operative time and intraoperative blood loss between the two groups (both P > 0.05). The time to intestinal peristalsis, the time to getting out of bed, and the time to drainage tube removal in the observation group were (57.83 ± 8.98) hours, (43.12 ± 2.34) hours, and (121.38 ± 10.29) hours, which were significantly shorter than those in the control group [(65.21 ± 7.45) hours, (45.46 ± 2.19) hours and (150.28 ± 13.42) hours, t = 4.899, 5.656 and 13.238, all P < 0.05]. Visual Analogue Scale score in the observation group at 12 and 24 hours after surgery was (1.89 ± 0.27) points and (1.45 ± 0.23) points, respectively, which was significantly lower than that in the control group [(2.19 ± 0.24) points, (1.84 ± 0.20) points, t = 6.433, 9.911, both P < 0.05]. At 3 days after surgery, serum levels of norepinephrine, interleukin-6 and cortisol in the observation group were (185.49 ± 18.29) ng/L, (59.91 ± 6.89) ng/L, and (109.21 ± 15.46) μg/L, respectively, which were significantly lower than those in the control group [(235.41 ± 16.57) ng/L, (73.24 ± 7.68) ng/L, (128.39 ± 10.32) μg/L, t = 15.668, 10.008, 7.993, all P < 0.05]. Conclusion:Hydromorphone exhibits a good effect on laparoscopic radical resection of colorectal cancer because it can promote early postoperative recovery, has an obvious postoperative analgesic effect and little influence on stress response.
